Lumbar Angiomatous Meningioma in a Dog (개에서 발생한 요추 혈관종성수막종 증례)

  • An 11-year-old castrated Schnauzer presented with paraplegia for the last two weeks, initially developed pelvic limb ataxia 6 months ago and left pelvic limb paresis 3 months ago. On neurological examination, paraplegia and postural reaction deficits were noted in the pelvic limbs with no deep pain sensation, and the dog showed exaggerated spinal reflexes and involuntary urination. Magnetic resonance (MR) images of thoracolumbar spinal cord were obtained with a 0.3 Tesla magnet. A mass with a broad base to the dorsal and left dural margins was identified at the level of L2, causing marked spinal cord compression. The mass showed isointensity to the spinal cord on T1-weighted (T1W) precontrast images, hyperintensity on T2-weighted images, marked homogeneous contrast enhancement with well-defined margins and the "dural tail" sign on T1W postcontrast images. An intradural-extramedullay tumor was considered, most likely, meningioma was highly suggestive. Left-sided hemilaminectomy and dorsal laminectomy were performed. An intradural mass partially adherent to dura mater was compressing the spinal cord, and the mass was completely removed with the attached dura mater. A histopathologic diagnosis of angiomatous subtype meningioma was made.

Accuracy and Reliability of The Spine-Pelvis Monitor to Record Three-Dimensional Characteristics of The Spine-Pelvic Motion

  • Objective: The aim of this study is to evaluate the accuracy and reliability of Spine-Pelvis Monitor(SPM) that was developed to measure 3-dimensional motion of spine and pelvis using tilt sensor and gyro sensor. Background: The main cause of low back pain is very much associated with the task using the low back and pelvis, but no measurement technique can quantify the both spine and pelvis. Method: For testing the SPM, 125 angles from three anatomical planes were measured three times in order to evaluate the accuracy and reliability. The accuracy of SPM in measuring dynamic motion was evaluated using digital motion analysis system. The motion pattern captured by two measuring methods was compared with each other. In result, the percentage error and Cronbach coefficient alpha were calculated to evaluate the accuracy and reliability. Results: The percentage error was 0.35% in flexion-extension on sagittal plane, 0.43% in lateral bending on coronal plane, and 0.40% in twisting on transverse plane. The Cronbach coefficient alpha was 1.00, 0.99 and 0.99 in sagittal, coronal and transvers plane, respectively. Conclusion: The SPM showed less than 1% error for static measurement, and showed reasonably similar pattern with the digital motion system. Application: The results of this study showed that the SPM can be the measuring method of spine pelvis motion that enhances the kinematic analysis of low back dynamics.

Factors Influencing Musculoskeletal Symptoms in Military Personnel during Basic Combat Training (육군 훈련병의 근골격계 증상 영향 요인)

  • Purpose: This study was done to examine physical, psycho-social, and individual factors influencing musculoskeletal symptoms among Korean military trainees. Methods: Using a correlation study design, military trainees who had completed almost of all the basic combat training (BCT) days were recruited from two military training units selected by convenience sampling. Data from 415 participants were analyzed. Results: Prevalence of musculoskeletal symptoms was 29.6% defined as a participant having pain or discomfort in one or more body parts during training hours for more than seven consecutive days. Back/pelvic (10.8%), knees (10.1%), shoulders (7.7%), feet/toes (5.6%), ankles (4.8%) were prone to musculoskeletal symptoms. Musculoskeletal symptoms appeared to be related to physical exertion during BCT, stress during BCT, social support from fellow trainees, or previous musculoskeletal injuries. In the logistic regression model, physical exertion during BCT (OR=2.27, 95% CI: 1.42~3.65), stress during BCT (OR=1.79, 95% CI: 1.15~2.78), and previous musculoskeletal injuries (OR=1.58, 95% CI: 1.01~2.47) were the significant factors affecting prevalence of musculoskeletal symptoms. Conclusion: Findings indicate that physical exertion and psycho-social stress should be managed to prevent musculoskeletal symptoms in military trainees with more attention being given to trainees having a history of musculoskeletal injuries.

Treatment Outcomes of Epithelial Ovarian Cancers Following Maximum Cytoreduction and Adjuvant Paclitaxel-Carboplatin Chemotherapy: Egyptian NCI Experience

  • Background: Epithelial ovarian cancer (EOC) is the commonest malignancy involving the ovaries. Maximum surgical cytoreduction (MCR) followed by adjuvant taxane-platinum chemotherapy are the standard of care treatments. Aims: To study treatment outcomes of EOC patients that were maximally cyto-reduced and received adjuvant paclitaxel-carboplatin (PC) chemotherapy. Materials and Methods: This retrospective cohort study included 174 patients with EOC treated at the Egyptian National Cancer Institute between 2006 and 2010. For inclusion, they should have had undergone MCR with no-gross residual followed by adjuvant PC chemotherapy. MCR was total abdominal hysterectomy/bilateral salpingo-oophorectomy [TAH/BSO] or unilateral salpingo-oophorectomy [USO] plus comprehensive staging. Results: The median age was 50 years. Most patients were married (97.1%), had offspring (92.5%), were postmenopausal (53.4%), presented with abdominal/pelvic pain and swelling (93.7%), had tumors involving both ovaries (45.4%) without extra-ovarian extension i.e. stage I (55.2%) of serous histology (79.9%) and grade II (87.4%). TAH/BSO was performed in 97.7% of cases. A total of 1,014 PC chemotherapy cycles were administered and were generally tolerable with 93.7% completing 6 cycles. Alopecia and numbness were the commonest adverse events. The median follow up period was 42 months. The 2-year rates for disease free survival (DFS) and overall survival (OS) were 70.7% and 94.8%, respectively. The respective 5-year rates were 52.6% and 81.3%. Advanced stage and high-grade were significantly associated with poor DFS and OS (p<0.001). Age >65 years was associated with poor OS (p =0.008). Using Cox-regression, stage was independent predictor of poor DFS and OS. Age was an independent predictor of poor OS.

Evaluation of the Interstitial Cystitis Symptom Index and Problem Index-Korean Version (한국판 간질성 방광염 증상지수/문제지수(ICSI/ICPI-K)의 평가)

  • Purpose: The O'Leary Symptom Index and Problem Index(ICSI/ICPI) have been used widespread in clinical studies and research. This research was aimed to determine the reliability and validity of the ICSI/ICPI-K that was translated into Korean. Methods: The research design was a methodological study to test the measurements. Subjects were 301 elderly women in Seoul, Chungnam and Gyungbuk. They were recruited through a formal announcement and they responded after informed consent. Data was analyzed with SPSS 15.0. The internal consistency was assessed by Cronbach's alpha coefficient and the criterion validity was assessed by Pearson correlation with the Geriatric Depression Scale(GDS) and inter-subscales. Results: The elderly women were on average 74.3 years old. As to the reliability of the scale, ICSI/ICPI-K had a Cronbach alpha of .832. In addition, the results showed that ICSI/ICPI-K was valid through the Pearson correlation. That is, the ICSI-K and ICPI-K had a significant relationship with GDS(r=.295; r=.374, respectively) and strong correlations of subscale to subscale(ICSI-K & ICPI-K: r=.676). The most common symptom of the lower urinary tract by the ICSI/ICPI-K was nocturia 249(83.3%) followed by frequency 187(62.8%), urgency 148(49.8%). and pelvic pain 71(23.9%) in the elderly women. Conclusion: We can conclude that the ICSI/ICPI-K is reliable and valid in Korean elderly women. It needs to be evaluated in the clinical area.

Tc-99m HMPAO White Blood Cell Scintigraphy of an Enterovesical Fistula Complicating Crohn's Disease (장방광루를 가진 크론병 환자의 Tc-99m HMPAO 백혈구 스캔)

  • Computed tomography (CT) seems to be the best imaging modality to diagnose an enterovesical fistula, but is not always able to demonstrate enterovesical fistula itself. In this case report, we present Tc-99m HMPAO white blood cell (WBC) scintigraphic findings of an enterovesical fistula complicating Crohn's disease. A 22 year-old male presented with a one-month history of urinary symptoms such as dysuria, hematuria, and frequency. The patient had intermittent right lower quadrant pain, diarrhea and hematochezia. Enterovesical fistula was highly suggestive in pelvic CT which showed air density in the urinary bladder, but cystoscopy failed to find an opening of the fistula. Tc-99m HMPAO WBC scintigraphy for evaluation of inflammatory bowel disease incidentally demonstrated enterovesical fistular tract. Crohn's disease was later confirmed by histologic examination of the surgical specimen. In our patient, Tc-99m HMPAO WBC imaging was helpful in determining the location of the fistula as well as assessing the disease activity and extent of the Crohn's disease.

The Experimental Study on Anti-thrombotic and Anti-inflammatory Effect of ChungyeolHaedogHwaeoTang(CHHT) (청열해독화어탕(淸熱解毒化瘀湯)의 항혈전(抗血栓) 및 항염(抗炎)효과에 관한 실험적(實驗的) 연구(硏究))

  • This study was performed to evaluate anti-thrombotic activities and anti-inflammatory effects of ChungyeolHaedogHwaeoTang water extract(CHHT). The results were summarized as follows. In experiment of anti-thrombotic effect; 1. CHHT inhibited human platelet aggregation induced by ADP and epinephrine as compared with the control group, and inhibited pulmonary embolism induced by collagen and epinephrine (inhibitory rate is 37.5%). 2. CHHT increased platelet number significantly, and also CHHT shortened PT and APTT significantly as compared with the control group in thrombus model induced by dextran. In experiment of anti-inflammatory effect; 3. CHHT inhibited $IL-1{\beta}$, IL-6, TNF-${\alpha}$, COX-2 and NOS-II mRNA expression as compared with the control group in a concentration-dependent degree, and inhibited NO production significantly at 50, 100 ${\mu}g/ml$, and also inhibited ROS production in a concentration-dependent degree as compared with the control of group in RAW 264.7 cell line. 4. CHHT inhibited $IL-1{\beta}$, IL-6 and TNF-${\alpha}$ production significantly in serum of acute inflammation-induced mice, and decreased $IL-1{\beta}$, IL-6 and TNF-${\alpha}$ production in spleen tissue, and also decreased $IL-1{\beta}$, and IL-6 production in liver tissue, but increased TNF-${\alpha}$ production in liver tissue of acute inflammation-induced Balb/c mice. 5. CHHT increased survival rate from the 3rd day in ICR mice with lethal endotoxemia induced by LPS. These results suggest that CHHT can be useful in treating diverse female diseases caused by thrombosis and inflammation such as menstrual pain, menstrual disorder, leukorrhea, vaginitis, cervicitis, pelvic inflammatory disease and so on.

  • Purpose: This is a following study of preceding study about application and improvement of acupuncture benefits in OB & GY disease that was performed in 2007. And this study was performed to complement and compare with a preceding study. Methods: The questionnaire used in preceding survey was distributed to general korean medical doctors who have mainly worked in oriental medical clinic. And results of this survey was compared with results of preceding survey. Results: Results of two surveys were almost similar. 1. The common acupuncture benefits items that postpartum pain syndrome(K29) should be added as suitable the sick and wounded name were Intra-articular acupuncture(關節內; LI15, SI10, GB30), Intervertebral acupuncture (脊椎間; GV14, 16, 6, 4, 12, 11, 9, 8, 3), Puncture each adjoining acu-points in one insertion (透刺; SP6-GB39). Conclusion: Based on this study, Survey to more KMDs should be performed. And the academic and clinical verifications that supports this results should be supplied.

Traumatic Diaphragmatic Hernia (외상성 횡경막 허니아)

  • The records of 10 patients with traumatic diaphragmatic hernia seen from November 1977 through July 1987 were reviewed. All the patients had a transdiaphragmatic evisceration of abdominal contents into the thorax. We treated 7 male and 3 female patients ranging in age from 3 to 62 years. In 8 patients, diaphragmatic hernia followed blunt trauma and in 2 patients, stab wounds to the chest. The herniation occurred on the right side in 3 patients and on the left side in 7. All the patients sustained additional injuries: rib fractures [7 patients], additional limb, pelvic and vertebral fractures [6], closed head injury [2], lung laceration [1], liver laceration [1], renal contusion [1], ureteral rupture [1], and splenic rupture [1]. Organs herniated through the diaphragmatic rent included the omentum [6 patients], stomach [4], liver [4], colon [3], small intestine [1], and spleen [1]. For right-sided injuries, the liver was herniated in all 3 patients and the colon, in 1. in the initial or latent phase, dyspnea, diminished breath sounds, bowel sounds in the chest were noted in 4 patients, and in the obstructive phase, nausea, vomiting, and abdominal pain were found in all 3 patients. Two patients had a diagnostic chest radiograph with findings of bowel gas patterns, and an additional 8 had abnormal but nondiagnostic studies. Hemothorax, pleural effusion or abnormal diaphragmatic contour were common abnormal findings. Three patients were operated on during the initial or acute phase [immediately after injury], 4 patients were operated on during the latent or intermediate phase [3 to 210 days], and 3 patients were operated on during the obstructive phase [10 to 290 days]. Six patients underwent thoracotomy, 2 required thoracoabdominal incision, and 2 had combined thoracotomy and laparotomy. Primary suture was used to repair the diaphragmatic hernia in 9 cases. One patient required plastic repair by a Teflon felt. Empyema was the main complication in 2 patients. In 1 patient, the empyema was treated by closed thoracostomy and in 1, by decortication and open drainage. There were no deaths.

  • Neglected adult Monteggia fracture could induce the pain, instability and malformation of elbow. Especially, compared with the chronic Monteggia fracture of child, that of adult is difficult to treat and could concur with valgus instability and deformity, limitation of range of motion and tardy ulnar nerve palsy. But recently, the chronic Monteggia fracture of adult could be treated by the 3.5 mm compression plate (DCP) or 3.5 mm pelvic reconstruction plate, so that the result improved more and more. The treatment of choice of the chronic Monteggia fracture of adult is the corrective osteotomy and reduction of radial head or resection of radial head. We experienced two patients who had neglected Monteggia fracture over 1 year 6 months and 25 years respectively and we want to report the result of surgical treatment of chronic Monteggia fracture of adult.
