The treatment of peri-implantitis using various types of lasers (다양한 Laser를 이용한 Peri-Implantitis의 치료)

  • Peri-implantitis is the inflammatory process, such as edema, bleeding, pus, of the mucosa surrounding dental implants. As the symptoms become severe, the surrounding bone is absorbed causing the implant surface to be exposed. Clinicians treat periimplantitis in various ways since a gold standard for the treatment of peri-implantitis has not been established. Various treatment methods include mechanical, chemical surface treatment and surgical excision, and recently decontamination of the implant surface using various types of lasers has been proposed. Thus, this study reviews the types of lasers and its effects that can be used for the treatment of peri-implantitis.

The Clinical Observation on 1 Case of Patient with Herpes Zoster Infecting Ophthalmic Branch of Trigeminal Nerve (안구 대상포진환자 1례에 관한 증례보고)

  • The herpes zoster infecting ophthalmic branch of trigeminal Nerve that is similar to migraine at first stage symptom has been treated with oriental medication at Dept. of Internal Medicine, Semyung University Oriental Hospital. The fIrst symptom of roster is burning pain, tingling or extreme sensitivity in one area of the skin, usually limited to one side of the body. This may be present for one to three days before a red rash appears at that site. There may also be a fever or headache. The rash soon turns into groups of blisters. The blisters start out clear but then pus or dark blood collects in the blisters before they crust over (scab) and begin to disappear. The pain may last longer. In this case, the severe pain was present for five days, the blisters and scabsdisappeared entirely on the seventeenth day, but postherpetic neuralgia, the most common complication and is observed most frequently in the ophthalmic branch of trigeminal nerve, was not prevented entirely. We have observed this case and report to help treatment on this disease at oriental medicine clinic.

Modified drainage of submasseteric space abscess

  • Once a submasseteric space infection is diagnosed, the key to resolving the infection is via surgical intervention to evacuate the pus. Although it is possible and occasionally practical to drain the submasseteric space via an intraoral approach, an extraoral approach may sometimes be required. Surgeons have encountered complications such as facial nerve damage during extraoral incision and drainage procedures, and they have felt that extraoral dissection was very difficult. As such, an easier and simpler technique is needed. Our department recently modified various drainage techniques for submasseteric space abscesses. Damage to the marginal branch of the facial nerve did not occur, and this technique was very simple and rapid, such that a novice physician could perform this procedure. This modified technique was possible with trismus and under local anesthesia. After intraorally checking the position of the drain, the intraoral wound is closed with an absorbable suture and the drain is fixed to the extraoral skin. When a masseteric space infection is diagnosed, multiple space involvement is ruled out, and dependent drainage is required, this modified drainage technique can be useful.

Roentgenographic examination was Caine, premlars and molars involved in maxillary sinus in left side and Radiopaque in same Antrm. This underwent caldwell-Lue approach the Extracted Caine, premolars and molars and Curettage maxillary sinus walls and Closed primary sutured under Diagnosis of Odontogenic maxillary Sinusitis and Therapeutic principles. Patients had healed Completely one year after operation and no complaints and no Oro-Antral fistula.

Antibiotic sensitivity and resistance of bacteria from odontogenic maxillofacial abscesses

  • Objectives: This study investigated the types and antibiotic sensitivity of bacteria in odontogenic abscesses. Materials and Methods: Pus specimens from 1,772 patients were collected from affected areas during incision and drainage, and bacterial cultures and antibiotic sensitivity tests were performed. The number of antibiotic-resistant bacteria was analyzed relative to the total number of bacteria that were tested for antibiotic susceptibility. Results: Bacterial cultures from 1,772 patients showed a total of 2,489 bacterial species, 2,101 gram-positive and 388 gram-negative. For penicillin G susceptibility tests, 2 out of 31 Staphylococcus aureus strains tested showed sensitivity and 29 showed resistance. For ampicillin susceptibility tests, all 11 S. aureus strains tested showed resistance. In ampicillin susceptibility tests, 46 out of 50 Klebsiella pneumoniae subsp. pneumoniae strains tested showed resistance. Conclusion: When treating odontogenic maxillofacial abscesses, it is appropriate to use antibiotics other than penicillin G and ampicillin as the first-line treatment.

Non-Cooperative Game Joint Hidden Markov Model for Spectrum Allocation in Cognitive Radio Networks

  • Spectrum allocation is a key operation in cognitive radio networks (CRNs), where secondary users (SUs) are usually selfish - to achieve itself utility maximization. In view of this context, much prior lit literature proposed spectrum allocation base on non-cooperative game models. However, the most of them proposed non-cooperative game models based on complete information of CRNs. In practical, primary users (PUs) in a dynamic wireless environment with noise uncertainty, shadowing, and fading is difficult to attain a complete information about them. In this paper, we propose a non-cooperative game joint hidden markov model scheme for spectrum allocation in CRNs. Firstly, we propose a new hidden markov model for SUs to predict the sensing results of competitors. Then, we introduce the proposed hidden markov model into the non-cooperative game. That is, it predicts the sensing results of competitors before the non-cooperative game. The simulation results show that the proposed scheme improves the energy efficiency of networks and utilization of SUs.

Empyema Occurred after Completion of Antituberculous Chemotherapy (항결핵치료 종료후 발생한 농흉)

  • A 38 years old man had been treated as a pulmonary tuberculosis by the positive result of acid fast stain of bronchial washing from the focal infiltrative lesion at left lower lobe. On radiologic examination after one year treatment, there was an aggravation of lesion at left lower lobe with moderate amount of pleural effusion at the same side. After 11 weeks, follow up chest film disclosed bilateral pleural effusion. The pleural fluid of both side was pus in gross appearance with low pH, high LDH, low glucose and high protein. Pleurodectomy was performed to remove the loculated empyema with the thickened pleura of right thorax. This pleuro-pulmonary lesion can be easily misdiagnosed as a tuberculous lesion if it is not taken into consideration as a possible diagnosis.

  • Craniovertebral junction (CVJ) tuberculosis is a rare disease, and potentially fatal due to the risk of atlantoaxial dislocation. The disease usually accompanies a retropharyngeal cold abscess, which can cause subsequential otolaryngologic symptoms such asdysphagia, odynophagia, or airway obstruction. Such a patient must be handled with great care to avoid a disaster. The disease is diagnosed with microbiological or other laboratory tests on the pus collected through puncture and aspiration, which will need otolaryngologic skills. For treatment, otolaryngologists play an important role by doing incision and drainage of the retropharyngeal abscess, or by attending the transoral vertebra surgery as partners with spine surgeons who will manage the CVJ lesions and ensure the craniocervical stability.

Antilipidperoxidative activity of Astragalus membranaceus (황기의 지질과산화 억제작용)

  • The root of Astragalus membranaceus (Leguminosae) has been used in traditional chinese prescriptions for strengthning the superficial resistance, promoting pus discharge and tissue regeneration, diuretis and alleviating edema. Lipid peroxidation has been suggested as a major cause of atherosclerosis, cancer, liver disease, and the aging process. In order to investigate the anti-lipid peroxidation activity, Astragali Radix was extracted with $80\%$ MeOH and fractionated with hexane, $CH_{2}Cl_2$, EtOAc, BuOH and Water. The antilipid peroxidation activities of them were determined by human erythrocyte ghost and $CCl_4$-induced lipid peroxidation. $CH_{2}Cl_2$ and EtOAc fractions, especially, isoflavonoids, 7,2'-dihydroxy-3',4'-dimethoxyisoflavan-7-O-${\beta}$-D-glucoside and calyco sin-7-O-${\beta}$-D-glucoside from EtOAc fraction showed anti-lipid peroxidation activities.

A rare case of bovine tuberculosis caused by Mycobacterium bovis in a domestic rabbit

  • A 12-month-old domestic rabbit died suddenly without specific clinical signs. Gross examination revealed irregular yellowish hepatic nodules with pus in the submandibular muscles, lungs, uterus, and small intestines. Histopathological examination of the liver showed granulomatous inflammation with acid-fast-positive bacteria. Mycobacterium bovis SB1040 was identified by polymerase chain reaction and spoligotyping, and Pasteurella multocida was isolated from the multiple lesions. This report demonstrates the pathological features of rare bovine tuberculosis (bTB) in a domestic rabbit, the first case in the Republic of Korea. To ensure public safety, we recommend routine monitoring of rabbits to control the incidence of bTB.
