Interministerial GHS Activities and Implementation in Korea

  • To implement a globally harmonized system of classification and labeling of chemicals (GHS) in Korea, an interminsterial GHS working group involving 6 ministries established an expert working group composed of 7 experts from relevant organizations and one private consultant to prepare an officialKorean GHS version by March, 2005. As such, the translation and review of the official Korean GHS version, including annexes, started in October, 2004 and was completed on March 15, 2005. The official Korean GHS version has now been posted on the websites of the relevant ministries and organizations to solicit public opinions. The official Korean GHS version will be finalized after a public hearing scheduled forMay, 2005. Collaborative efforts as regards implementing and disseminating the GHS in Korea will be continued to avoid any confusion or duplication and for effective use of resources. The globally harmonized system of classifying and labeling chemicals (GHS) was originally adopted in 1992 at the United Nations Conference on Environment and Development (UNCED), as subsequently reflected in Agenda 21 chapter 19. The work was coordinated and managed under the auspices of the Interorganization Programme for the Sound Management of Chemicals(IOMC) Coordinating Group for the Harmonization of Chemical Classification Systems (UNCEGHS). The technical focal points for completing the work were the International Labour Organization (ILO); Organization for Economic Cooperation and Development (OECD); and United Nations Economic and Social Council's Subcommittee of Experts on the Transport of Dangerous Goods (UNSCETDG). The work was finalized in October 2002, and the World Summit on Sustainable Development in Johannesburg on 4 September 2002 encouraged countries to implement the new GHS as soon as possible with a view to having the system fully operational by 2008 (UN, 2003). Implementation has already started with pilot countries introducing the system to their national practices in different regions of the world. The GHS system will be kept dynamic, and regularly revised and made more efficient as experience is gained in its implementation. While national or regional governments are the primary audiences for this document, it also contains sufficient context and guidance for those in industry who will ultimately be implementing the national requirements that will be introduced (UN, 2003). The Japanese government published their official Japanese GHS version, the first in Asia, in April 2004 after starting work in January 2003 based on an interministerial chemical coordination committee involving 7 ministries, including the Ministry of Foreign Affairs, Ministry of Internal Affairs and Communications, Ministry of Health, Labour, and Welfare, Ministry of Agriculture, Forestry and Fisheries, Ministry of Economy, Trade and Industry, Ministry of Land, Infrastructure, and Transport, and Ministry of Environment (MOE, 2004). Accordingly, similar to the Japanese GHS efforts, this paper presents the interministerial efforts involved in publishing the official Korean GHS version.

재가급여 장기요양인정 노인들의 신체기능 상태 및 관련요인 (Physical Functioning and Related Factors in the Elderly People Admitted Long-term Home Care Insurance)

  • 본 연구는 재가급여 장기요양인정노인들을 대상으로 그들의 신체적 기능 상태를 평가하고 그에 관련된 요인을 검토하고자 실시하였다. 조사대상은 대전광역시에 거주하고 있으면서 재가급여 요양자로 등록되어 있는 65세 이상 노인 618명으로 하였다. 자료 수집은 2011년 12월 1일부터 2012년 3월 31일까지의 기간 동안에 표준화된 무기명식 면접조사용 설문지를 사용하여 면접조사를 실시하였다. 연구결과, 전체 조사대상자의 ADL에 대한 기능장애군의 분포는 45.6%로 나타났으며, IADL에 대한 기능장애군의 분포는 48.4%로 나타나 ADL보다 IADL에서의 기능장애 비율이 더 높았다. ADL과 IADL에서의 기능장애군에 속할 위험비는 여자보다 남자에서, 혼자 산다는 군보다 배우자나 자녀와 함께 산다는 군에서, 생활비조달을 자신이 한다는 군보다 자녀가 해 준다는 군에서, 의료보호군보다 건강보험군에서, 규칙적인 운동을 한다는 군보다 하지 않는다는 군에서, 외출을 자주 한다는 군보다 거의 하지 않는 다는 군에서, 취미활동을 자주 한다는 군보다 거의 하지 않는 다는 군에서, 주관적인 건강상태가 좋다는 군보다 좋지 않다는 군에서, 신체의 부자유가 없다는 군보다 있다는 군에서, 눈의 부자유가 없다는 군보다 있다는 군에서, 귀의 부자유가 없다는 군보다 있다는 군에서, 치아의 부자유가 없다는 군보다 있다는 군에서, 요실금이 없다는 군보다 있다는 군에서, 건망증이 없다는 군보다 있다는 군에서 유의하게 증가하였다. 결론적으로 재가급여인정노인들의 신체적 기능 상태는 ADL보다 IADL 기능이 더 감소되어 있었으며, ADL과 IADL 모두 인구사회학적 특성, 건강관련행위 특성 및 건강상태의 다양한 요인이 관련되어 있었다.

Exploratory Study of Dimensions of Health-related Quality of Life in the General Population of South Korea

  • Objectives: This study aimed to explore dimensions in addition to the 5 dimensions of the 5-level EQ-5D version (EQ-5D-5L) that could satisfactorily explain variation in health-related quality of life (HRQoL) in the general population of South Korea. Methods: Domains related to HRQoL were searched through a review of existing HRQoL instruments. Among the 28 potential dimensions, the 5 dimensions of the EQ-5D-5L and 7 additional dimensions (vision, hearing, communication, cognitive function, social relationships, vitality, and sleep) were included. A representative sample of 600 subjects was selected for the survey, which was administered through face-to-face interviews. Subjects were asked to report problems in 12 health dimensions at 5 levels, as well as their self-rated health status using the EuroQol visual analogue scale (EQ-VAS) and a 5-point Likert scale. Among subjects who reported no problems for any of the parameters in the EQ-5D-5L, we analyzed the frequencies of problems in the additional dimensions. A linear regression model with the EQ-VAS as the dependent variable was performed to identify additional significant dimensions. Results: Among respondents who reported full health on the EQ-5D-5L (n=365), 32% reported a problem for at least 1 additional dimension, and 14% reported worse than moderate self-rated health. Regression analysis revealed a $R^2$ of 0.228 for the original EQ-5D-5L dimensions, 0.200 for the new dimensions, and 0.263 for the 12 dimensions together. Among the added dimensions, vitality and sleep were significantly associated with EQ-VAS scores. Conclusions: This study identified significant dimensions for assessing self-rated health among members of the general public, in addition to the 5 dimensions of the EQ-5D-5L. These dimensions could be considered for inclusion in a new preference-based instrument or for developing a country-specific HRQoL instrument.

현행 환경영향평가 제도의 문제점과 개선방안 - 실무자적 관점에서 검토 - (A Study on the Issues and Improvement of the Existing Environmental Impact Assessment System - Evaluation in an operator Viewpoint -)

  • The Korean environmental impact assessment(EIA) system, and explored ways to improve it as a more efficient and viable institution relevant to the demand of our time and conditions in study. The first problem this study identified is found in the fact that the party to write up the assessment report is itself the business operator or the one who is planning to work out the business plan. This structure translates into placing an order with an agent for EIA report. The reporting job may br subcontracted to the agent at a cost far below the rate specified in the 'Standard for Estimate of Agency Fee for Environmental Impact Assessment.' This practice also causes the vicious circle of producing a report that is written to justify the project or business in question or it leads to rough-and ready and poor documentation to minimize the time required. Second, in order to achieve the goal of the plan or business, which is the target of EIA, the local residents tend to ve regarded as an obstacle. This means elimination of the local people from participating in the EIA or their opinion being frequently ignored. This is the seed of distrust and hostility that sometimes provoke disagreements or fierce conflicts. The first proposal to improve these problem is to improve the factors that cause poor documentation of the assessment report as well as improve the understanding of the EIA system. This study proposes the following measures for improvement. The agency cost for EIA should be paid by the business operator or a third party that can ensure faithful implementation of the payment. A system should be established to verify transparent estimation of the agency cost. In order to enhance the professional quality of EIA agents, there should be implementation of qualification test for industrial engineer of related engineers in addition to the current EIA Qualification Test. The second proposal for improvement is to improve the citizen participation process by instituting a legal framework to make clear the purpose of the briefing session for local residents, which is held as a procedure of EIA, and to ensure more positive publicity during the stage of listening to the opinion of the local community. For a smooth and rational communication process, a moderator and a communicator of opinion, as is the case in a public hearing, could be instituted to clearly get the purpose of the briefing session across to the residents and to help to carry out the explanation and Q & A sessions according to the categories of the opinion of the residents. At present, the notification of the public inspection of the draft of the assessment report and briefing session for the residents is made on the newspaper and internet network. But some people have difficulty with access to this method of announcement. A higher participation rate could be secured if a legal provision is added to specify putting up placards in specific places such as the entrance to the place for the briefing session for residents or the building of administrative agencies of the area concerned.

전북 서해안권 국가지질공원의 추진과정과 관련된 갈등 프레임 분석 (An Analysis of the Conflict Frames Related to the Process of the National Geopark in Jeonbuk Western Coast Area, Korea)

  • 본 연구의 목적은 국가지질공원 추진 과정에서 지역주민, 지질 전문가 및 지방자치단체 공무원 사이에서 나타나는 갈등 프레임의 양상을 규명하는 데 있다. 이를 위하여 각각 부안과 고창에서 실시된 국가지질공원 추진 관련 주민 공청회의 진행 과정을 참여자들의 사전 동의를 얻어 녹음한 후 텍스트 형태로 전사하였다. 이후 문헌연구를 통해 갈등 프레임을 분석하기 위한 기준 프레임을 개발하였으며, 이를 토대로 3명의 연구자에 의해 갈등 프레임을 분석하였다. 이렇게 분석된 갈등 프레임은 다시 언어네트워크 분석을 활용하여 지역별로 갈등 프레임 간의 구조적 특성을 분석하였으며, 그 결과는 다음과 같다. 먼저, 부안 지역에서는 'Sagree' 프레임, 'Snot' 프레임, 'Sdisagree' 프레임이 높은 위세중심성을 보였고, 언어네트워크의 중심에 위치한 'Snot' 프레임에 Cmexample 프레임, Gharm 프레임, Cmeconomy 프레임 등이 긴밀하게 연결되었다. 반면 고창에서는 'Aresource' 프레임, 'Cmexample' 프레임, 'Gharm' 프레임이 높은 위세중심성을 보였고, 언어네트워크의 중심에 위치한 'Aresource' 프레임에 'Cmexample' 프레임, 'Gharm' 프레임, 'Cmproblemsolution' 프레임 등이 긴밀하게 연결되었다. 이를 통해 부안 지역에서는 아직 이해당사자 사이의 갈등이 진행되고 있고, 고창 지역에서는 자신들의 자원을 자랑스럽게 인식하고 있음을 알 수 있었다. 그리고 부안의 이해당사자들은 갈등 해소에 있어서 경제적 이득에 초점을 둔 반면, 고창의 이해당사자들은 문제 해결에 초점을 두었다. 본 연구의 결과는 국가지질공원 추진에서 나타날 수 있는 갈등 관계를 해소하기 위한 중요한 정보를 제공했다는 점에서 의의가 있다.

고령자의 신체특성을 반영한 전시시설 환경디자인 체크리스트 개발에 관한 연구 (Study on the Development of Environmental Design Checklist at Exhibition Facility Based on Physical Characteristics of the Elderly)

  • 전 세계적으로 고령화시대에 접어들면서 고령자에게도 시대의 변화에 따라 지식과 기술을 습득하는 '평생교육'의 중요성이 대두되고 있다. 전시시설은 전시컨텐츠와 전시물의 공간적 구성에 의해 교육을 제공하는 평생학습 공간으로서의 시대적 역할이 요구됨에 따라 주 이용자층의 범위 역시 청소년, 청·장년층에서 고령자로 확대되어진다는 사실을 예견할 수 있다. 그러므로 고령자에게는 전시시설이 또 다른 학습공간이 될 수 있으며, 이러한 학습이 원활하게 이루어지기 위해서는 고령자의 신체특성을 고려한 물리적 환경지원이 필요하다. 본 연구는 고령자의 신체특성에 따른 전시시설 환경디자인 체크리스트를 개발하여 전시시설에서의 고령자의 원활한 전시관람 및 교육을 지원하는 환경계획의 틀을 마련하고자 한다. 고령자의 신체적 특성을 '감각(시력, 청력, 촉감)' 과 '운동(골격, 이동, 체력, 근력)'으로 구분하고 전시시설을 진입공간, 전시공간, 공공서비스공간, 교육활동공간, 이동공간으로 분류하여 각 공간에서의 환경디자인 체크리스트를 도출하였다. 기존 관련 법령, 매뉴얼, 선행연구를 종합하여 전시시설에서의 고령자 관람자를 위한 환경디자인 체크리스트를 새롭게 구성하였고, 후속연구를 위한 현장실측항목과 설문항목을 도출하였다.

한국 재활간호 현황과 전망 (Current Status and the Future Prospect of Rehabilitation Nursing in Korea)

  • The history of rehabilitation of disabilities in Korea began with the foreigners and missionaries who were interested in it after Korean War. In 1981, Disabled Persons Welfare Act was enacted and the 88 Paralympics brought the nations attention to the welfare and rehabilitation of persons with disabilities. Since then, the facilities and the services for the disabled persons have expanded rapidly and the rehabilitation treatment and nursing intervention are drawing more attention. Against this background, the survey on the current status of disabilities, welfare service, facilities, and rehabilitation nursing was conducted. The results of this survey are as follows. 1. According to the 2000 census of disabilities, the number of persons with disabilities in Korea is estimated at 1,449,500, or 3.09% of the entire Korean population, 0.74% up from 2.35% in 1995. 2. Disability Types in 2000 The 2000 census showed that the persons with disabilities numbered 1,449,496 out of the total population and 1,024,371 persons are registered for disability, making up 70.7% of the estimated disabled population. Among them, physically disabled persons accounted for the largest 41.7% (605,127) and mentally retarded persons stood at the smallest 9% (13,481). 3. Percentage of Disability Presence The survey showed that more than 90% of disability were acquired. However, 44.8% of mental disability and 61.4% of hearing/speaking disability were not acquired after birth. This means that these disabilities happened by congenital cause or birth accident. 4. Yearly Figure of Registered Disabled Persons In 1989, 218,601 persons registered for disability and, in 2000, the number increased by 4.7 times to 1,024,371. These figures are different from the actual number of disabled persons. According to the 1995 census, 1,053,486 were disabled persons but only 378,323registered for disability. And, in the 2000 census, 1,024,371 out of the 1,449,496 of disabled persons registered for disability. 5. Welfare Service for Persons with Disability 62.6% of the total disabled people are registered and physically disabled persons accounted for the highest percentage of 96.7%. 26.5% of non-registered disabled people said that they didnt know the registration procedure. The rest of them replied that they didnt think they were disabled or that registration didnt seem to give any benefits. 6. Welfare Policies for Disabled Persons The welfare benefits given to the disabled are as follows: Issuance of disabled sign for car drivers, Permission to use LPG fuel, Communication fee reduction, Tax exemption related to cars, Reduction of public facility fees, Household allowance, Tax reduction or exemption, Medical allowance and education subsidy for children, and Housing. 7. Current Condition of Welfare Facilities by Disability Type The welfare institutions for disabilities numbered 188 in total and they can accommodate 16,823 persons. Categories of these institutions are physical disability(37), visual disability(10), hearing/speaking disability(14), mental retardation(59), and sanatoriums(68). 8. Human Resource of Rehabilitation of Disabilities Advanced education programs include rehabilitation nursing in its curriculum and this was selected as the program of Korean Academic Society of Nursing in 1990. In November 1997, Korean Academic Society of Rehabilitation Nursing was launched and many academic meeting and seminars were held. This organization is also making efforts to develop the education program for qualified rehabilitation nursing professionals and to develop the standards of rehabilitation nursing practice. In the professionals of the rehabilitation, there are rehabilitation specialist, physical therapist, speech therapist, occupational therapist. It is needed to come up with the measures to supply stable human resources following the demand of disabled persons and to recognize the private certificates for rehabilitation professionals as official ones after reviewing the education and training programs of private institutions. 9. Rehabilitation Nursing 1) Rehabilitation nursing was taught as an independent subject in 11 undergraduate programs and 9 graduate programs. 2) Research on rehabilitation nursing in Korea were 24 experimental research and 11 non-experimental research. The intervention of experimental research were mostly education and exercise rehabilitation programs. 3) In the three rehabilitation hospitals, nursing is divided into two categories, direct nursing and education & counseling. Direct nursing includes tracheostomy or nasogastric tube care, urination and defication, skin care, pain control, complication prevention and care, prevention of injury from a fall, etc.

중재감정계약의 의의 및 법적 성질 (The definition and the nature of voluntary agreement for the arbitration which third party confirms factual bases of relationship of rights and duties, determines and supplements or modifies contents of the contract)

  • Arbitration Act does not have express provision about voluntary agreement for the arbitration which third party, that is, the expert confirms factual bases of party's relationship of rights and duties, determines contents of the contract, and supplements or modifies contracts, and then the parties obey the expert's decision, but it is more probable that the parties can agree to this kind of arbitration agreement as long as they freely make a contract within the scope of law. However, there is a split of authority on the scope of such arbitration agreement. Some scholars argue that the parties can only agree on the extent of the expert's confirmation about factual situations of party's relationship of rights and duties or contents of the contract. On the other hand, the other scholars argue that the parties can consent not only the expert's confirmation about factual situations of party's relationship of rights and duties or contents of contract, but also the expert's supplement or modification of contents of contract. Due to the expert's decision has effect on both parties and judges who give a judgment as a matter of law, this kind of arbitration agrement can contribute to prevent litigation. Also arbitration relieves court's burden, if such arbitration agreement was done on the important disputes. Considering that the arbitration agreement can function as a dispute resolution or a dispute prevention, it is desirable that legislators make the provision about this kind of arbitration and allow the application of arbitration Act in such arbitration agreement. Most scholars agree that the voluntary agreement for the arbitration as to third party's supplement or modification of contents of the contract can be included in the concept of a substantive law. However, it has not been concluded whether the voluntary agreement for the arbitration which follows the expert's confirmation about factual situations of party's relationship of rights and duties or contents of the contract has the nature of substantive law or procedural law. The dispute about the nature of such arbitration agreement have some shortcomings in the effect of second kind of voluntary arbitration and the applicability of procedural principles. Therefore, it will be more adequate that the focus is given to the original function of this kind of arbitration agreement and the applicability of procedural principles (the neutrality of arbitrator, the assurance of hearing of the parties) rather than the dispute regarding the nature of this kind of arbitration agreement. Considering that more attention is given to the substitutive dispute resolution these days, the function of arbitration as prevention to the litigation and resolution before the litigation should be emphasized. To do this, a legal dispute about such arbitration agreement has to be resolved. More important issues in this kind of arbitration agreement are to retain of the neutral expert and to positively inform the benefits of this institution to the public.

농촌 노인의 일.여가활동 유형에 따른 삶의 만족감 (Study on Life Satisfaction of Groups According to Farm Work and Leisure Activity in Rural Elderly)

  • This study aims to find out ways of improving life satisfaction of rural elderly by investigating their work and leisure activity, two significant axis of the life in later life. For this purpose, researchers and one assistant researcher visited house, senior citizen's center and health center, in order to have interviews with senior citizen above 65, who are currently working in the field. The summary of the research findings are as follows; First, the average size of the farm land which each senior citizen works on is $9,237.92m^2$ and they mostly work for economic reason. During farming season, they work on average 8.62 hour per day. As primary reasons for why farming work is difficult, "insufficient income" was picked. In fact, the average annual income senior citizens earn from farming is about 15.4 million won. 49.1% of sample senior citizens work both on watered farm and farm, and they are engaged in farming 48.82 years on average, showing that most of their lives have been spent on farming. In spite of these difficulties, most senior citizens showed high intention to continue farming work further. Second, senior citizens turn out to spend their leisure time mostly on public leisure activities such as watching TV, hearing radio and gatherings with families, relatives. However, individual activities such as meditation, yoga, drawing and calligraphy turn out to be low because they require foundations to enjoy. Third, senior citizen in rural areas were divided into four groups by farm work and leisure activity: group separated from work and leisure activity(27.09%), group centered to leisure activity(24.42%), group centered to work(23.75%), and group integrated work and leisure activity(23.75%). Fourth, the groups were different depending on demographic characteristics. Namely, male senior citizen, higher educational level, living in elderly couple households, and satisfied with their economic level belonged to the group integrated work and leisure activity. But senior citizen aged above 65 years old and working for their livelihood belonged to the group centered to work, and most of female and senior citizen aged above 80 years old belonged to the group separated from work and leisure activity. Finally, as the result of analysis of differences in life satisfaction according to farm work and leisure activity of senior citizen in rural areas, the group integrated work and leisure activity, and group centered to leisure activity were higher in life satisfaction than the group separated from work and leisure activity, and group centered to work. The above result indicates that life satisfaction of senior citizen depends upon leisure activity rather than work.

2010 당뇨병 환자를 위한 식품교환표 개정 (Korean Food Exchange Lists for Diabetes: Revised 2010)

  • A food exchange system for diabetes is a useful tool for meal planning and nutritional education. The first edition of the Korean food exchange lists was developed in 1988 and the second edition was revised in 1995. With recent changes in the food marketplace and eating patterns of Koreans, the third edition of food exchange lists was revised in 2010 by the Korean Diabetes Association, the Korean Nutrition Society, the Korean Society of Community Nutrition, the Korean Dietetic Association and the Korean Association of Diabetes Dietetic Educators through a joint research effort. The third edition is based on nutritional recommendations for people with diabetes and focuses on adding foods to implement personalized nutrition therapy considering individual preferences in diverse dietary environment. Foods were selected based on scientific evidence including the 2007 Korea National Health and Nutrition Examination Survey data analysis and survey responses from 53 diabetes dietetic educators. While a few foods were deleted, a number of foods were added, with 313 food items in food group lists and 339 food items in the appendix. Consistent with previous editions, the third edition of the food exchange lists included six food categories (grains, meat, vegetables, fats and oils, milk, and fruits). The milk group was subdivided into whole milk group and low fat milk. The standard nutrient content in one exchange from each food group was almost the same as the previous edition. Korea Food & Drug Administration's FANTASY (Food And Nutrient daTA SYstem) database was used to obtain nutrient values for each individual food and to determine the serving size most appropriate for matching reference nutrients values by each food group. The revised food exchange lists were subjected to a public hearing by experts. The third edition of the food exchange lists will be a helpful tool for educating people with diabetes to select the kinds and amounts of foods for glycemic control, which will eventually lead to preventing complications while maintaining the pleasure of eating.
