Silicone Stent Placement for Primary Tracheal Amyloidosis Accompanied by Cartilage Destruction

  • Primary tracheal amyloidosis (PTA) can lead to airway obstructions, and patients with severe PTA should undergo bronchoscopic interventions in order to maintain airway patency. Focal airway involvements with amyloidosis can only be treated with mechanical dilatation. However, the PTA with diffused airway involvements and concomitant cartilage destructions requires stent placement. Limited information regarding the usefulness of silicone stents in patients with PTA has been released. Therefore, we report a case of diffused PTA with tracheomalacia causing severe cartilage destruction, which is being successfully managed with bronchoscopic interventions and silicone stent placements.

  • 가스터빈 핵심부품은 모두 고온고압의 열악한 환경에서 사용되므로 정기적으로 열화상태를 파악하여 신품으로의 교체 또는 재생 정비해야 한다. 그러나 국내의 재생 정비기술의 낙후로 인해 재생 가능한 고가의 부품까지 폐기, 교체함으로써 발전원가 상승의 주 요인이 되고 있다. 본 연구에서는 발전용 GT 블레이드 재료인 IN738LC 및 GTD111 판상시편에 대하여 다양한 용접 방법과 변수로 PTA 육성용접하였으며, 각 변수에 따른 미세조직 변화를 관찰하고 모의결함 시편에 대해 기계적 물성을 평가하였다.

Recent Study of Overlay Welding on Welding methods (용접방법에 따른 오버레이 용접기술의 연구동향)

  • Recent developing tendency of overlay welding on welding methods are studied by searching of NDSL, ScienceDirect and KIPRIS. PTA, Laser cladding, SMAW, GTAW, GMAW, SAW and ESW are selected as welding methods. Development and improvement of various new overlay welding processes and technology are introduced, especially PTA and Laser cladding in abroad overlay welding technology are activated substantially.

  • Surface hardening of AC4C aluminium alloy with TiC powders was obtained by plasma transferred are (PTA) method, Bead appearance, microstrucutre and wear behavior were characterized in relation to the torch current, powder feed rate and wear condition. The width and the depth of bead were mainly increased with increasing torch current. The comparison of wear behavior between AC4C alloy and PTA treated AC4C alloy showed the remarkable enhancement of wear resistance.

A Study on the Abrasive Wear Properties of the PTA Overlay Layers using the Super Alloy Powder (초내열합금분말에 의한 PTA 오버레이부의 연삭 마모 특성 연구)

  • The Plasma Transferred Arc (PTA) overlay welding method is lately introduced as one of the most useful surface overlay method of the engine component. In this paper, the overlay welding on the SNCrW heat resisting alloy was conducted by the PTA overlay welding process using the super alloy powder. The characteristics of the overlay layers were investigated through the metallurgical and abrasive test. Experimental results showed that the overlay on the SNCrW heat resisting alloy surface was successfully made without hot cracking. The friction wear characteristics of the Co-base Stellite 6 overlayer were most superior. However the abrasive wear characteristics were most inferior in the Co-base Stellite 6 overlayer.

  • Merging of two supermassive black holes would generate gravitational waves that can be detected by the Pulsar Timing Array (PTA) in the nHz band. In order to assess the plausibility of GW detection with PTA and to develop the data analysis scheme, it is important to understand the underlying properties of black holes and black hole binaries. In this work, we present mass and redshift distributions of black hole mergers using the Horizon Run 5 (HR5) data and discuss their implications for GW detection. We find a general conjecture about the black hole merger tree is true with the Horizon Run 5. For example, a) relatively lighter black holes merge at higher redshifts and b) binary mergers do contribute to the formation of more massive black holes toward low redshifts. We also present our plan to use the black hole properties extracted from the HR5 data in order to generate simulated GW signals to be injected into actual PTA data analysis pipelines. Mass and distance obtained from the HR5 would be key ingredients to generate a more realistic PTA source data set.

Efficacy of arthrocentesis and lavage for treatment of post-traumatic arthritis in temporomandibular joints

  • Objectives: Joint injuries frequently lead to progressive joint degeneration that causes articular disc derangement, joint inflammation, and osteoarthritis. Such arthropathies that arise after trauma are defined as post-traumatic arthritis (PTA). Although PTA is well recognized in knee and elbow joints, PTA in the temporomandibular joint (TMJ) has not been clearly defined. Interestingly, patients experiencing head and neck trauma without direct jaw fracture have displayed TMJ disease symptoms; however, definitive diagnosis and treatment options are not available. This study will analyze clinical aspects of PTA in TMJ and their treatment outcomes after joint arthrocentesis and lavage. Materials and Methods: Twenty patients with history of trauma to the head and neck especially without jaw fracture were retrospectively studied. Those patients developed TMJ disease symptoms and were diagnosed by computed tomography or magnetic resonance imaging. To decrease TMJ discomfort, arthrocentesis and lavage with or without conservative therapy were applied, and efficacy was evaluated by amount of mouth opening and pain scale. Statistical differences between pre- and post-treatment values were evaluated by Wilcoxon signed-rank test. Results: Patient age varied widely between 20 and 80 years, and causes of trauma were diverse. Duration of disease onset was measured as 508 post-trauma days, and 85% of the patients sought clinic visit within 2 years after trauma. In addition, 85% of the patients showed TMJ disc derangement without reduction, and osteoarthritis was accompanied at the traumatized side or at both sides in 40% of the patients. After arthrocentesis or lavage, maximal mouth opening was significantly increased (28-44 mm on average, P<0.001) and pain scale was dramatically decreased (7.8-3.5 of 10, P<0.001); however, concomitant conservative therapy showed no difference in treatment outcome. Conclusion: The results of this study clarify the disease identity of PTA in TMJ and suggest early diagnosis and treatment options to manage PTA in TMJ.

  • Esterification of methacrylic acid with ethylene glycol was carried out over Heteropolyacids [HPA: $H_4SiW_{12}O_{40}$ (STA) and $H_3PW_{12}O_{40}$ (PTA)] supported on ZSM-5. For comparison, the same reaction was carried out over unsupported HPA, $H_2SO_4$, $BF_3$ and PTSA. Among the catalysts studied, HPA showed better activity compared to $H_2SO_4$, $BF_3$ and PTSA. Catalytic activity was compared with HPA supported ZSM-5 catalysts. Typical results indicated that 30 wt% PTA supported on ZSM-5 showed nearly the same activity as that of bulk PTA. It was found that the reaction follows first order kinetics with respect to methacrylic acid. The reaction products were identified by $^1H$-NMR and FT-IR.

Better Understanding of Direct Bone-Conduction Measurement: Comparison with Frequency-Specific Bone-Conduction Tones and Brainstem Responses

  • Background and Objectives: The present study aimed to compare thresholds of direct bone-conduction (BC direct) with those of behaviorally measured BC pure-tone audiometry (PTA) and objectively measured BC auditory brainstem response (ABR) to confirm the clinical feasibility of their relationships. Subjects and Methods: Young adults with normal hearing participated in the study to determine the thresholds from three measurements at four testing frequencies. In the BC direct, the vibrator of a bone-anchored hearing aid softband was placed on the right mastoid of each subject. In both PTA and ABR, a B71 bone oscillator was placed on the subject's right mastoid. While the subject's thresholds of BC direct and BC PTA were determined with a clinically routine 5-dB step procedure, BC ABR was conducted to determine the individual's hearing sensitivity by a peak V of the waveform using tone-burst and click stimuli. Results: The BC direct showed a different pattern between low and high frequencies. Precisely, its thresholds were 13.25 and 12.25 dB HL at 0.5 and 1 kHz, respectively, but 19 and 19.75 dB HL at 2 and 4 kHz, respectively. A significant positive correlation existed between BC direct and PTA at 1 kHz, which was also correlated with ABR. Conclusions: Based on the current data, the thresholds of BC direct were similar to BC PTA at low frequencies and BC ABR at high frequencies. The thresholds of BC direct might be predictable at approximately 5 dB higher (or lower) than that in PTA, although a large data set is required for standardization.
