Abiraterone for Treatment of Metastatic Castration-resistant Prostate Cancer: a Systematic Review and Meta-analysis

  • Introduction: Although most prostate cancers initially respond to castration with luteinizing hormonereleasing analogues or bilateral orchiectomy, progression eventually occurs. Based on the exciting results of several randomized controlled trials (RCTs), it seems that patients with metastatic castration-resistant prostate cancer (mCRPC) might benefit more from treatment withabiraterone. Therefore we conducted a systematic review to evaluate the efficacy and toxicity of abiraterone in the treatment of mCRPC. Methods: Literature was searched from Embase, PubMed, Web of Science, and Cochrane Library up to July, 2013. Quality of the study was evaluated according to the Cochrane's risk of bias of randomized controlled trial (RCT) tool, then the Grading of Recommendations Assessment, Development and Evaluation (GRADE) System was used to rate the level of evidence. Stata 12.0 was used for statistical analysis. Summary data from RCTs comparing abiraterone plus prednisone versus placebo plus prednisone for mCRPC were meta-analyzed. Pooled hazard ratios (HRs) for overall survival (OS), radiographic progression-free survival (RPFS) and time to PSA progression (TTPP); Pooled risk ratios (RR) for PSA response rate, objective response rate and adverse event were calculated. Results: Ten trials were included in the systematic review; Data of 2,283 patients (1,343 abiraterone; 940 placebo) from two phase 3 trials: COU-AA-301 and COU-AA-302 were meta-analyzed. Compared with placebo, abiraterone significantly prolonged OS (HR, 0.74; 95% confidence interval [CI], 0.66 to 0.84), RPFS (HR, 0.59; 95% CI, 0.48 to 0.74) and time to PSA progression (HR, 0.55; 95% CI, 0.43 to 0.70); it also significantly increased PSA response rate (RR, 3.63; 95% CI, 1.72 to 7.65) and objective response rate (RR, 3.05; 95% CI, 1.51 to 6.15). This meta-analysis suggested that the adverse events caused by abiraterone are acceptable and can be controlled. Conclutios: Abiraterone significantly prolonged OS, RPFS and time to progression patients with mCRPC, regardless of prior chemotherapy or whether chemotherapy-na$\ddot{i}$ve, and no unexpected toxicity was evident. Abiraterone can serve as a new standard therapy for mCRPC.

  • The purpose of this study is to evaluate qualities of lightweight aggregate for structural concrete according to mixing proportions, pelletizer condition, sintering condition and to choose the suitable main and sub material. Main material used paper sludge ash(PSA) and sub material used clay, fly-ash and paper sludge. The aggregates are sintered after granulating at the various condition. As the result of test, quality difference of aggregate showed clear according to the mixing proportions and sintering conditions. It was possible to manufacture lightweight aggregate for structural concrete that dry specific gravity was ranged about 0.9 to 1.4 also the test results of the aggregates showed same physical properties compared with abroad product as 10% granules crushing value from 5 or 10% and absorption percentage from 10 to 20%.

포괄적 누적 충격 공통원인고장 모형 및 시스템 신뢰도 평가 (Comprehensive Cumulative Shock Common Cause Failure Models and Assessment of System Reliability)

  • This research proposes comprehensive models for analyzing common cause failures (CCF) due to cumulative shocks and to assess system reliability under the CCF. The proposed cumulative shock models are based on the binomial failure rate (BFR) model. Six kinds of models are proposed so as to explain diverse cumulative shock phenomena. The models are composed of the initial failure probability, shape parameter, and the total shock number. Some parameters of the proposed models can not be explicitly estimated, so we adopt the Expectation-maximization (EM) algorithm in order to obtain the maximum likelihood estimator (MLE) for the parameters. By estimating the parameters for the cumulative shock models, the system reliability with CCF can be assessed sequentially according to the number of cumulative shocks. The result can be utilizes in dynamic probabilistic safety assessment (PSA), aging studies, or risk management for nuclear power plants. Replacement or maintenance policies can also be developed based on the proposed model.

Initiating Events Study of the First Extraction Cycle Process in a Model Reprocessing Plant

  • Background: Definition and grouping of initiating events (IEs) are important basics for probabilistic safety assessment (PSA). An IE in a spent fuel reprocessing plant (SFRP) is an event that probably leads to the release of dangerous material to jeopardize workers, public and environment. The main difference between SFRPs and nuclear power plants (NPPs) is that hazard materials spread diffusely in a SFRP and radioactive material is just one kind of hazard material. Materials and Methods: Since the research on IEs for NPPs is in-depth around the world, there are several general methods to identify IEs: reference of lists in existence, review of experience feedback, qualitative analysis method, and deductive analysis method. While failure mode and effect analysis (FMEA) is an important qualitative analysis method, master logic diagram (MLD) method is the deductive analysis method. IE identification in SFRPs should be consulted with the experience of NPPs, however the differences between SFRPs and NPPs should be considered seriously. Results and Discussion: The plutonium uranium reduction extraction (Purex) process is adopted by the technics in a model reprocessing plant. The first extraction cycle (FEC) is the pivotal process in the Purex process. Whether the FEC can function safely and steadily would directly influence the production process of the whole plant-production quality. Important facilities of the FEC are installed in the equipment cells (ECs). In this work, IEs in the FEC process were identified and categorized by FMEA and MLD two methods, based on the fact that ECs are containments in the plant. Conclusion: The results show that only two ECs in the FEC do not need to be concerned particularly with safety problems, and criticality, fire and red oil explosion are IEs which should be emphatically analyzed. The results are accordant with the references.

수산화마그네슘의 합성과 솔비톨계 계면활성제를 이용한 표면개질 (Synthesis of Magnesium Hydroxide and Surface Modification by Sorbitol Surfactants)

  • 비이온계면활성제 가운데 솔비톨계 계면활성제인 Span을 이용해 수열합성법으로 수산화마그네슘을 합성하였다. 수산화마그네슘 합성의 전구체는 염화마그네슘과 수산화나트륨을 사용하였다. 비이온 계면활성제는 안정제와 분산제 그리고 표면 개질제로 적용하였다. 비이온 계면활성제를 첨가하였을 경우 수산화마그네슘 입자는 좀 더 작고 균일한 크기와 좋은 분산성을 나타내었으며, 소수성 성질을 나타내었다. 합성된 입자의 특성은 PSA, SEM, EDS, XRD 그리고 FT-IR을 통해 확인하였다. 기기 분석을 통해 개질 전과 후의 수산화마그네슘의 소수성, 분산성 특성을 비교하였다. 또한 실험조건에 따라 수산화마그네슘 입자의 표면 개질 특성 변화를 확인하였다.

제올라이트별 산소포화농도 측정 실험 (Measurement of Oxygen Saturation Concentration by Zeolite)

  • 인간의 생명 유지에 가장 필수적인 것은 공기이지만, 산업의 발달 인구의 증가 등으로 도시의 공기는 매연과 유해가스 등으로 오염되고 있다. 이런 이유 등으로 최근 환경문제 중 대기오염에 대한 문제가 크게 다루어지고 있다. 공기청정기와 함께 쾌적한 실내공기를 제공할 수 있는 또 하나의 기기는 산소발생기이다. 산소발생기의 주 원리는 합성 제올라이트를 이용하여 공기로부터 산소를 분리하는 데에 있다. 다양한 합성 제올라이트들은 그들이 갖고 있는 특성을 이용하여 실생활에 유용하게 적용되어지고 있다. 이런 제올라이트의 특성 중 질소를 흡착하는 성질을 이용하여 대기 중의 질소를 흡착 시킨 뒤 산소만을 발생시켜 산소발생기에 적용시킬 수 있다. 그리하여 우리는 5가지 종류의 천연제올라이트와 LTA 제올라이트, 3A, 4A, 5A(크기가 다른 두 가지)를 사용하여 각각의 질소흡착정도를 알아보고자 하였다. PSA 방식을 사용하여 질소흡착정도가 높을수록 산소포화농도가 높게 나오는 원리를 통해 5A(양이온이 칼슘으로 보충된 LTA제올라이트) 제올라이트의 질소흡착정도가 평균 6% 증가하여 가장 크다는 것을 알 수 있었고 또한 제올라이트의 크기에도 많은 영향을 받는다는 것을 알 수 있었다.

확률론적 평가를 이용한 원자력발전소 소내전력공급계통 신뢰도 감시 방법 (A Method to Monitor the Reliability of In-house Power Supply Systems in Nuclear Power Plants Based on Probabilistic Assessment)

  • This paper introduces a method to establish performance criteria of the in-house power supply system in nuclear power plants. The performance criteria of the system is presented in terms of the number of function failures and amount of the out-of-service time that can be allowed commensurate with the probabilistic safety assessment results of the nuclear power plants. To obtain the performance criteria such as reliability and availability, the functions of the system were analyzed and probabilistic assessment results were utilized. This method provides quantitative guidelines in selecting and monitoring system functions to determine an adequate level of maintenance quality in order to ensure the probabilistic goals for the safety of the nuclear power plants.

상동광산 광미를 콘크리트용 혼화재료로 사용하기 위한 품질특성 연구 (A Study on the Quality Properties for Using Tailings from the Sangdong Tungsten Mine as Admixture for Concrete.)

  • This study has focused on the possibility for recycling tailings from the Sangdong tungsten mine as admixture for concrete. The XRD(X-ray diffraction analysis) and PSA(Particle size analysis) were performed to find mineralogical characteristics. As a result of XRD analysis, the tailings were composed of quartz, feldspars and muscovites, and C-S-H and quartz were found in cement mortar. As a result of KSLT for cement mortar mixed with tailings, most of heavy metals were determined as below the guide line for waste material. In addition, the setting time and compressive strength of cement mortar mixed with tailings were investigated. It was indicated that the initial and final set were retarded according to increasing replacement. The compressive strength of mortar was decreased with increasing replacement.

모집단 분포를 이용한 핵의학 혈액검사의 통계적 품질관리의 유용성 (Availability of Statistical Quality Control of Nuclear Medicine Blood Test Using Population Distribution)

  • 정도관리(Quality Control)는 오차를 최소한으로 하여 검사의 신뢰도를 증진 시키는 것을 목적으로 하고 있으며 이러한 정도관리 활동을 통해 환자의 진단과 치료방침에 영향을 주는 핵의학 혈액 검사의 신뢰도가 확보되고 있다. 이에 연구자들은 신뢰도 향상의 추가적인 기법으로 건강검진 수검자를 대상으로 시행한 핵의학 혈액검사의 모집단 분포를 이용한 통계적 품질관리의 유용성을 평가해 보고자 한다. 2014년 1월부터 2014년 12월까지 서울아산병원 건강증진센터를 이용한 건강검진 수검자 41,341명을 대상으로 핵의학 혈액 검사 12종목에 대해 통계 분석을 시행 하였다. 통계분석은 핵의학 혈액 검사 12종목에 대한 결과 보고 치를 참고치 이내, 참고치 이상, 참고치 이하의 세 가지 그룹의 월간 백분율로 구분하여 모집단 분포의 평균값과 표준편차, 그리고 표준편차 지수(Standard Deviation Index: SDI)를 산출 하였고 그 결과 모집단 분포의 표준편차는 모든 그룹에서 대부분 ${\pm}2SD$ 이내의 결과를 보였다. 건강검진 수검자에 대한 핵의학 혈액검사 12종목(AFP, CEA, CA19-9, CA125, PSA, TSH, FT4, Anti-Tg-Ab, Anti-TPO-Ab, Calcitonin, 25-OH-VitD3, Insulin)의 모집단 분포를 분석한 결과 월간 백분율의 표준편차 지수(SDI)가 ${\pm}2.0$ 이상인 경우는, 1월과 3월 Insulin검사에서 각각 2.3, 2.4, 5월 Anti-Tg-Ab검사에서 2.2, 그리고 9월 CA19-9에서 2.2의 결과를 보였다. 이는 검사 Kit 시약의 이상 (동위원소 시약의 최대 결합률 저하), 검사반응 시간의 저하가 원인 이었다. 연구대상이 되는 전체의 속성을 포함하고 모집단 분포의 특성을 이용한 통계적 품질관리는 검사실에서 시행하고 있는 내부정도관리 프로그램을 보완하는 역할을 할 수 있으리라 생각된다.
