Objective: We aimed to evaluate correlations between computed tomography (CT) parameters and pulmonary function test (PFT) parameters according to disease severity in patients with chronic obstructive pulmonary disease (COPD), and to determine whether CT parameters can be used to predict PFT indices. Materials and Methods: A total of 370 patients with COPD were grouped based on disease severity according to the Global Initiative for Chronic Obstructive Lung Disease (GOLD) I-IV criteria. Emphysema index (EI), air-trapping index, and airway parameters such as the square root of wall area of a hypothetical airway with an internal perimeter of 10 mm (Pi10) were measured using automatic segmentation software. Clinical characteristics including PFT results and quantitative CT parameters according to GOLD criteria were compared using ANOVA. The correlations between CT parameters and PFT indices, including the ratio of forced expiratory volume in one second to forced vital capacity (FEV1/FVC) and FEV1, were assessed. To evaluate whether CT parameters can be used to predict PFT indices, multiple linear regression analyses were performed for all patients, Group 1 (GOLD I and II), and Group 2 (GOLD III and IV). Results: Pulmonary function deteriorated with increase in disease severity according to the GOLD criteria (p < 0.001). Parenchymal attenuation parameters were significantly worse in patients with higher GOLD stages (P < 0.001), and Pi10 was highest for patients with GOLD III (4.41 ± 0.94 mm). Airway parameters were nonlinearly correlated with PFT results, and Pi10 demonstrated mild correlation with FEV1/FVC in patients with GOLD II and III (r = 0.16, p = 0.06 and r = 0.21, p = 0.04, respectively). Parenchymal attenuation parameters, airway parameters, EI, and Pi10 were identified as predictors of FEV1/FVC for the entire study sample and for Group 1 (R2 = 0.38 and 0.22, respectively; p < 0.001). However, only parenchymal attenuation parameter, EI, was identified as a predictor of FEV1/FVC for Group 2 (R2 = 0.37, p < 0.001). Similar results were obtained for FEV1. Conclusion: Airway and parenchymal attenuation parameters are independent predictors of pulmonary function in patients with mild COPD, whereas parenchymal attenuation parameters are dominant independent predictors of pulmonary function in patients with severe COPD.
Objectives: Ryodoraku is a physiological function test using electric current. Since it can evaluate the patient's overall condition as well as a function of individual organs, Ryodoraku can be assumed as an effective way to diagnose chronic obstructive pulmonary disease (COPD). Therefore, this study aimed to investigate the values of Ryodoraku for diagnosing COPD. Methods: We designated a COPD group, who were over 40 and satisfying COPD criteria, $FEV_1$/FVC<0.7. The Control group also consisted of members over 40 years old but without COPD and any other lung disease. We conducted a comparative study by checking the Ryodoraku score (RS), the average of Rt.&Lt. H1 (mean H1), the mean difference value between both H1s (${\mid}H1^R-H1^L{\mid}$) and the ratio of patients whose RS was under 40. Then we investigated the correlation between the mean H1 and the results of pulmonary function test (PFT). Results & Conclusions: In the COPD group, RS, mean H1 and ${\mid}H1^R-H1^L{\mid}$ were significantly lower and the ratio of 'RS<40' patients was meaningfully higher than in the Control group. On the other hand, PFT seems not to be replaced by Ryodoraku since there was no correlation between mean H1 and the results of PFT. Though more experiments and researches are needed to be further confirmed, Ryodoraku is expected to be an adequate way for prediagnosing and following up the COPD patients.
Objective: The aim of this study was to evaluate the effect of pentoxifylline (PF) on the conventional ICSI program undergone in severe asthenozoospermia. Method: Total 348 cycles of ICSI programs undertaken at CHA General Hospital from January, 1996 to September, 2000, were divided into two groups - injected with pentoxifylline-treated sperm (PFT, 204 cycles) or non-treated sperm (NPFT, 144 cycles) and the clinical results of PFT group were compared with those of NPFT. Results: PF-treatment on sperm increased their motility of normozoospermia and severe asthenozoospermia. Fertilization rate of PFT group was higher than those of ICSI programs undertaken using sperm of NPFT (70.6% vs. 62.9%, p<0.01). And, ET and clinical pregnancy rates of PFT were slightly higher than those of NPFT (93.1%, 44.2% vs. 90.3%, 36.2%). Conclusion: These results showed that treatment of pentoxifylline has a beneficial role on selection of viable sperm in severe asthenozoospermia.
Objective: This study was conducted to identify the clinical usefulness, validity, and reliability of the Spirokit, a device that combines the pulmonary function test (PFT) and respiratory muscle strength (RMS) test. Design: Cross-sectional study. Methods: Forty young adults (male: 23, female: 17) participated in a PFT and a RMS test. The concurrent validity for pulmonary function was assessed by comparing data obtained from MicroQuark and the Spirokit and the agreements between the MicroRPM and the Spirokit for RMS were compared. The test-retest reliability of the Spirokit was determined by comparing data obtained from the first and second sessions. The test and retest were performed at the same time after one day for the PFT and RMS test. Validity was estimated using intraclass correlation coefficients (ICCs), and by calculating 95% limits of agreement (LoA). To estimate interrater reliability, ICCs were calculated. Results: The Spirokit showed a high agreement intra class coefficient (ICC [2, 1]): 0.978-0.999, 95% limits of agreements (95% LOA): -0.798 to 0.847 with MicroQuark. It also showed a high level of concordance ICC (2, 1): 0.992 to 0.993, 95% LOA: -9.313 to 11.169 with MicroRPM. The test-retest reliability of the Spirokit was analyzed using ICC (2, 1), and showed a high level of reliability (ICC [2,1]=0.960 to 0.998). Standard error of measurement % (SEM%) was 0.12% to 3.39%, and minimum detectable change% (MDC%) was 0.02% to 3.79%, indicating high level of reliability. Conclusions: The Spirokit is a device with high validity and reliability that can be used to simultaneously measure PFT and RMS tests.

Objective: This study aims to assess the reliability and validity of the new hand-held spirometer as a potential substitute for traditional pulmonary function testing (PFT) devices. Design: Cross-sectional study. Methods: In this study, thirty healthy adults underwent spirometry using both the new hand-held spirometer and the MIR spirometer, which is a standard PFT device. Parameters including peak expiratory flow (PEF), forced expiratory volume in one second (FEV1), and forced vital capacity (FVC) were measured and analyzed for validity and reliability. Inter-rater reliability and validity were evaluated through 95% limits of agreement (LOA) and intraclass correlation coefficients (ICC). Statistical analyses, including the Bland-Altman plots and the ICC, were utilized to assess agreement between the two devices. Results: The new hand-held spirometer exhibited a good agreement with intra-class coefficient (ICC [2,1]) ranging 0.762 to 0.956 and 95% LOA of -1.94 to 1.80 when compared with MIR. The test-retest reliability of the hand-held spirometer analyzed using - ICC [2,1] demonstrated a good level of consistency (ICC [2,1] =0.849-0.934). Conclusions: In conclusion, the study aimed to assess the potential of the new hand-held spirometer as a viable alternative to traditional PFT devices, with a specific focus on its reliability and validity in spirometric measurements. The new hand-held spirometer exhibited good test-retest reliability across all measured variables, suggesting its potential as a valid and reliable tool for simultaneous PFT measurements.
폐 환기 검사는 직접 방사성 기체를 흡입한 후 이를 영상화 한다. 그러나 실제 폐 환기 검사 시 활용할 수 있는 흡입과 관련한 지표는 널리 알려진 바가 없다. 따라서 본 연구를 통해 흡입과 관련한 객관적인 지표로써 폐 환기 검사 계수율과 폐 기능검사 결과 값의 상관관계를 확인하고 그 유용성을 알아보고자 한다. 2010년 10월부터 2018년 9월까지 본원에서 폐 환기 검사와 폐 기능 검사를 모두 시행한 만성폐쇄성 폐질환 환자 36명을 대상으로 중증도(Mild, Moderate, Severe)별로 분류하였다. 폐 환기 검사는 Technegas Generator (Vita Medical Ltd, Australia)로 Technegas를 생성하여, 환자에게 흡입하게 하였다. 영상은 Forte (Philips Medical Systems, Netherlands) 장비를 사용하였으며, 폐 기능 검사는 Vmax Encore 22 (Carefusion Corporation, USA)를 사용하였다. 폐 환기 검사에서는 양측 폐에 관심 영역을 설정하여 계수율을 측정하였고, 이와 함께 폐 기능 검사의 결과 값 중 노력 폐활량(FVC), 1초 노력 호기량($FEV_1$), 일초율($FEV_1/FVC$) 사이의 상관관계를 비교 분석하였다. 만성폐쇄성폐질환 환자의 폐 환기 검사 계수율과 폐 기능 검사 결과 값의 상관관계를 분석한 결과, 중증도별 $FEV_1/FVC$와 계수율 사이의 상관계수는 Severe에서 -0.773, Moderate에서 -0.750, Mild에서 -0.437이였다. Severe, Moderate 결과 값은 통계적으로 유의하였으며(P<0.05), Mild는 유의하지 않았다(P=0.155). 반면에 FVC와 계수율 사이의 상관계수는 Mild에서만 0.882로 통계적으로 유의한 것을 확인하였다(P<0.05). 만성폐쇄성폐질환 환자를 대상으로 시행한 폐 환기 검사 시 계수율과 폐 기능 검사 결과 값의 상관관계를 확인할 수 있었다. 이를 통해 폐 환기 검사 시 호흡력에 대한 지표로써 폐 기능 검사 결과 값을 활용할 수 있을 것으로 생각된다.
Purpose: Human body have biological rhythmic pattern in a day, which is affected by internal and external environmental factors. We investigated whether respiratory function was fluctuated according to the influence of time-of-day (around at 9 am, 1 pm, and 6 pm) in health subjects, using pulmonary function test (PFT). Methods: Eighteen healthy volunteers (8 men, mean ages; $22.4{\pm}1.6$, mean heights; $166.61{\pm}9.60$, mean weight; $59.3{\pm}10.3$) were recruited. Pulmonary function test (PFT) was measured at three time points in day, around 9 am, 1 pm, and 6 pm in calm research room with condition of under 55dB noise level, using a spirometer (Vmax 229, SensorMecis, USA). Forced vital capacity (FVC), forced expiratory volume at one second (FEV1), FVC/FEV1, and peak expiratory flow (PEF) were acquired. Results: In comparison of raw value of PFT among three time points, subjects showed generally better respiratory function at 9 am, than at other points, although no significance was found. In comparison of distribution of ranking for respiratory function in each individual, only PEF showed significant difference. In general, distributional ratio of subjects who showed best performance of respiratory function in a day was high. Conclusion: These findings showed that circadian rhythm by diurnal pattern was not detected on respiratory function throughout all day. But, best performance on respiratory function was observed mostly in the morning, although statistical significance did not exist.
Furoxanthiols PFT and BPFT possessing thiomethyl or thiobenzyl groups in the furoxan ring were designed and synthesized as potential light-sensitive alkyne precursors on a gold surface. The synthesis of thiofuroxans PFT and BPFT was performed from the corresponding halofuroxans 1b and 2c, respectively, by the substitution with potassium thioacetate in ethyl acetate/ethanol or DMF, followed by basic hydrolysis as the key reactions. Electron-beammediated fragmentation of furoxans 1c and 2d in a mass spectrometer afforded the corresponding aryl alkyne fragments, with the evolution of NO in high preference. In the cases of thiofuroxans PFT and BPFT, carbon-sulfur bond cleavage was observed as a representative fragmentation, producing M-SH and M-SAc peaks, which competed with the release of NO. In the fragmentation of mono-aryl furoxan 1c, the release of molecule of NO was predominately observed to produce an M-NO fragment as a base peak by the formation of trimembered thiiranium or azirine intermediate.
Body index is known as it affects pulmonary function tests (PFT), so it has been used with predictive formula and nomogram in terms of sex, age, height, etc. Body indices as body weight, body mass index (BMI), and body surface area (BSA) might also affect PFT, so that we have analyzed the correlations between body indices and forced expiratory volume in one second ($FEV_1$), and have done multiple regression analysis to see how body indices affect $FEV_1$. We confirmed that $FEV_1$ had positive correlations with height (r=0.49, p<0.01), body weight (r=0.37, p<0.01), and BSA (r=0.47, p<0.01), inverse correlation with age (r=-0.45, p<0.01), but no correlation with BMI. We found that the 41.9% of $FEV_1$ was diverged from height, age and BSA. Therefore, BSA definitely needs to be considered with predictive formula and nomogram in PFT.

Objective: To compare correlations between pulmonary function test (PFT) results and different reconstruction algorithms and to suggest the optimal reconstruction protocol for computed tomography (CT) quantification of low lung attenuation areas and airways in healthy individuals. Materials and Methods: A total of 259 subjects with normal PFT and chest CT results were included. CT scans were reconstructed using filtered back projection, hybrid-iterative reconstruction, and model-based IR (MIR). For quantitative analysis, the emphysema index (EI) and wall area percentage (WA%) were determined. Subgroup analysis according to smoking history was also performed. Results: The EIs of all the reconstruction algorithms correlated significantly with the forced expiratory volume in one second (FEV1)/forced vital capacity (FVC) (all p < 0.001). The EI of MIR showed the strongest correlation with FEV1/FVC (r = -0.437). WA% showed a significant correlation with FEV1 in all the reconstruction algorithms (all p < 0.05) correlated significantly with FEV1/FVC for MIR only (p < 0.001). The WA% of MIR showed the strongest correlations with FEV1 (r = -0.205) and FEV1/FVC (r = -0.250). In subgroup analysis, the EI of MIR had the strongest correlation with PFT in both eversmoker and never-smoker subgroups, although there was no significant difference in the EI between the reconstruction algorithms. WA% of MIR showed a significantly thinner airway thickness than the other algorithms ($49.7{\pm}7.6$ in ever-smokers and $49.5{\pm}7.5$ in never-smokers, all p < 0.001), and also showed the strongest correlation with PFT in both ever-smoker and never-smoker subgroups. Conclusion: CT quantification of low lung attenuation areas and airways by means of MIR showed the strongest correlation with PFT results among the algorithms used, in normal subjects.
