A Case Report of Polycystic Ovary Syndrome Treated by Sajaehyangbu-hwan (사제향부환을 투여한 다낭성난소증후군 환자 증례보고)

  • Objectives: To report the effect of Sajaehyangbu-hwan on a polycystic ovary syndrome(PCOS) patient with anovulation. Methods: The patient with PCOS was treated by Sajaehyangbu-hwan twice a day. The patient measured basal body temperature(BBT) and took a urine luteinizing hormone test(LH test) for three months to predict ovulation. Results: Two ovulations were observed in BBT and LH test results within three months and the typical biphasic BBT pattern was established. Conclusions: Sajaehyangbu-hwan has an effect on establishing obvious biphasic pattern in BBT of the patient with PCOS and the anovulation caused by PCOS.

  • PCOS is the most common endocrine pathology in females of reproductive worldwide. The prevalence ranges between 5% and 15% depending on the diagnostic criteria applied.Its etiology involves both genetic and environmental factors. Typically, women with PCOS show clinical and biochemical hyperandrogenism, oligoanovulation, and micropolycystic morphology of the ovaries. Unani formulation containing Nankhwah, Badiyan and Wajturki possessing the properties of Mudirr-i-Bawl-o- Hayd, mujaff -iBalgham, Munaffis-i- balgham, Muhallil, Muqawwi-i-Jigar were used in the form of Joshanda 6gm BD Starting from 5 days prior to expected period date to 5 days during menses for 3 cycles, which led to regain regularity of menses, correcting the amount of flow and reducing the ovarian volume on US. Thus unani medications have the potential to treat ths symtoms of PCOS and improve the quality of life of women.

  • Objectives: The purpose of this systematic review was to overview and evaluate the efficacy of acupuncture treatment for women with polycystic ovary syndrome (PCOS). Methods: Relevant randomized controlled studies (RCTs) were identified by database searches in MEDLINE, EMBASE, and CENTRAL, up to Dec 2013, and by additional hand searches. Data were extracted regarding anovulation, hyperandrogenism, obesity indices. Meta-analyses were separatedly conducted for the symptoms of PCOS. The risk of bias was assessed. Results: Three studies which were included for analysis, but they showed severly heterogeneity therefore meta-analysis could not be performed. Outcomes for evaluating the efficacy of acupuncture treatment for PCOS were anovulation index (menstrual frequency), hyperandrogenism index (free testosterone) and obesity index (body-mass index). For menstrual frequency, acupuncture treatment consistently suggested an interventional benefit. Although other outcomes did not suggest any enough relevant evidence to interventional benefit for acupuncture treatment. Conclusions: Acupuncture treatment appeared to improve menstrual frequency in PCOS patients. Since a limited number of RCTs were available in the current literature and those studies were also clinically heterogeneous, further research is needed to gather evidence to support acupuncture therapy in PCOS.

  • In view of greater attention given to the incidence of Polycystic Ovarian Syndrome (PCOS) in women of reproductive age, particularly in urban and semi-urban population in India, research works in both the regimens of orthodox and complementary and alternative medicines have been rejuvenated in recent years. We report here relative efficacy of two potentized homeopathic remedies, Calcarea carbonica (Calc) and Lycopodium clavatum (Lyco) used traditionally for the removal of ovarian cysts. These drugs are most frequently used based on guiding symptoms of individual patients. Effects of either of these remedies on its ability of removing cysts, along with amelioration of certain other hormones and hormone-related parameters of PCOS, such as follicle stimulating hormone, luteinizing hormone, Estradiol, Testosterone (Free/Total), Dehydroepiandosterone, Prolactine, Progesterone (17- Hydroxyprogesterone), TSH including T3, T4, and Insulin were studied. The Insulin-related parameters like changes in fasting or postprandial glucose levels were also studied. The mentioned hormones play some- direct or indirect roles in causing irregular menstrual cycle and PCOS. The data collected at three fixation time points, namely, at 6, 12, and 18 months were considered. Results showed that out of 40 patients initially having PCOS, cysts were totally removed in 21 patients along with amelioration of other relevant symptoms. Both Calc and Lyco had amelioration of similar nature. Results of this study therefore validate safe and effective use of both Calc and Lyco in homeopathy, to patients with basic guiding symptoms for either drug, and can be recommended for patients with PCOS as they do not have any reported side-effects.

Expression of the genes for peroxisome proliferator-activated receptor-γ, cyclooxygenase-2, and proinflammatory cytokines in granulosa cells from women with polycystic ovary syndrome

  • Objective: To identify differences in the expression of the genes for peroxisome proliferator-activated receptor $(PPAR)-{\gamma}$, cyclooxygenase (COX)-2, and the proinflammatory cytokines interleukin (IL)-6 and tumor necrosis factor $(TNF)-{\alpha}$ in granulosa cells (GCs) from polycystic ovary syndrome (PCOS) patients and controls undergoing controlled ovarian stimulation. Methods: Nine patients with PCOS and six controls were enrolled in this study. On the day of oocyte retrieval, GCs were collected from pooled follicular fluid. Total mRNA was extracted from GCs. Reverse transcription was performed and gene expression levels were quantified by realtime quantitative polymerase chain reaction. Results: There were no significant differences in age, body mass index, and total gonadotropin dose, except for the ratio of luteinizing hormone to follicle-stimulating hormone between the PCOS and control groups. $PPAR-{\gamma}$ and COX-2 mRNA was significantly downregulated in the GCs of PCOS women compared with controls (p= 0.034 and p= 0.018, respectively), but the expression of IL-6 and $TNF-{\alpha}$ mRNA did not show significant differences. No significant correlation was detected between the expression of these mRNA sequences and clinical characteristics, including the number of retrieved oocytes, oocyte maturity, cleavage, or the good embryo rate. Positive correlations were found among the $PPAR-{\gamma}$, COX-2, IL-6, and $TNF-{\alpha}$ mRNA levels. Conclusion: Our data may provide novel clues regarding ovarian GC dysfunction in PCOS, and indirectly provide evidence that the effect of $PPAR-{\gamma}$ agonists in PCOS might result from alterations in the ovarian follicular environment. Further studies with a larger sample size are required to confirm these proposals.

Associations of serum fetuin-A and oxidative stress parameters with polycystic ovary syndrome

  • Objective: The aim of this study was to compare serum fetuin-A levels and oxidative stress markers, as indicators of insulin resistance, in women with polycystic ovary syndrome (PCOS) and in healthy controls. Methods: This prospective case-control study included 46 patients with PCOS and 48 age- and body mass index-matched control women. Levels of serum hormones, fetuin-A, and oxidative stress markers were measured in blood samples taken during the early follicular period from each participant. Results: Follicle-stimulating hormone (FSH), luteinising hormone (LH), total testosterone levels, and the LH/FSH ratio were found to be significantly higher in women with PCOS than in controls. Serum total antioxidant status, total oxidant status, and oxidative stress index parameters all indicated significantly higher levels of oxidative stress in PCOS patients than in controls. Serum fetuin-A levels, which were analyzed as an indicator of insulin resistance, were higher in the PCOS group than in the control group ($210.26{\pm}65.06{\mu}g/mL$ and $182.68{\pm}51.20{\mu}g/mL$, respectively; p= 0.024). Conclusion: The data obtained from the present study suggest that higher levels of both serum fetuin-A and oxidative stress markers might be related with PCOS.

A Review on Clinical Studies of Herbal Treatment for Infertility Caused by Polycystic Ovary Syndrome -Focusing on Chinese Clinical Trials- (다낭성 난소증후군의 난임치료에 대한 한약임상연구 경향 분석 -중국의 임상연구를 중심으로-)

  • Objectives: The purpose of this study is to review the trend of chinese clinical studies of herbal treatment for infertility caused by Polycystic Ovary Syndrome (PCOS). Methods: We searched the clinical studies published from 1979 to 2013 through CAJ (Chinese Academic Journal) with keywords of "PCOS and 不孕" or "多囊卵 巢綜合證 and 不孕". We analyzed the intervention type of clinical studies. In addition, We researched randomized controlled trials (RCTs) published from 2009 to 2013, assessed the quality of RCTs using the Jadad Scale and reanalyzed the intervention type of RCTs. Results: One hundred and seventeen clinical studies were finally selected. Clinical studies of herbal treatment for infertility caused by PCOS increased remarkably after the 2000s in China. Combined herbal medicine and western medicine was used the most frequently. RCTs published from 2009 to 2013 were fifty. Average of Jadad score of RCTs was 1.2. Combined herbal medicine and western medicine was also the most frequently used as intervention in RCTs. Conclusions: Clinical studies on effect of combined herbal medicine and western medicine for infertility caused by PCOS were mostly published in China. But Quality of studies was generally low. Further research on effect of herbal treatment for infertility caused by PCOS is needed.

The effect of genistein on insulin resistance, inflammatory factors, lipid profile, and histopathologic indices in rats with polycystic ovary syndrome

  • Objective: Polycystic ovary syndrome (PCOS) is characterized by hyperandrogenism, irregular menstruation, ovulatory dysfunction, and insulin resistance. Recent studies have reported the possible role of phytoestrogens in PCOS. This animal study aimed to evaluate the effects of genistein on insulin resistance, inflammatory factors, lipid profile, and histopathologic indices on PCOS. Methods: PCOS was induced by 1 mg/kg of letrozole in adult Sprague-Dawley rats. The rats then received normal saline (PCOS group), 150 mg/kg of metformin, or 20 mg/kg of genistein dissolved in 1% methylcellulose solution for 42 days. Body weight, the glycemic and lipid profile, and inflammatory, antioxidative, and histopathological parameters were assessed at the end of the intervention. Results: Treatment with genistein significantly alleviated the increased level of fasting blood insulin (p=0.16) and the homeostatic model assessment of insulin resistance (p=0.012). In addition, the genistein group had significantly lower levels of serum malondialdehyde (p=0.039) and tumor necrosis factor-alpha (p=0.003), and higher superoxide dismutase enzyme activity (p<0.001). Furthermore, the histopathological analysis indicated that genistein administration led to an increase in luteinization and the development of fewer cysts (p<0.05). Conclusion: Biochemical and histopathological analyses indicated that genistein administration to rats with PCOS induced significant remission in oxidative, inflammatory, and glycemic and histopathologic parameters.

Metabolic Syndrome and Insulin Resistance Syndrome among Infertile Women with Polycystic Ovary Syndrome: A Cross-Sectional Study from Central Vietnam

  • Background: Polycystic ovarian syndrome (PCOS) is one of the most common endocrinopathies among reproductive-age women. Its metabolic features often overlap with those associated with metabolic syndrome (MS) and insulin resistance syndrome (IRS). The objective of this study was to determine the prevalence and predictors of MS and IRS in infertile Vietnamese women with PCOS. Methods: A cross-sectional study was conducted at a tertiary fertility centre at Hue University Hospital from June 2016 to November 2017. A total of 441 infertile women diagnosed with PCOS based on the revised 2003 Rotterdam consensus criteria were enrolled. MS and IRS were defined based on the National Heart, Lung, and Blood Institute/American Heart Association Adult Treatment Panel III 2005 and American College of Endocrinology IRS 2003 criteria, respectively. Complete clinical and biochemical measurements of 318 women were available for analysis. Independent predictors of MS and IRS were identified using multivariate logistic regression. Results: The overall prevalence of MS and IRS in women with PCOS was 10.4% and 27.0%, respectively. We identified older age (>30 years) and obesity as independent predictors of MS and IRS. Elevated anti-$M{\ddot{u}}llerian$ hormone levels increased the risk of IRS, but not that of MS. Conclusion: MS and IRS are prevalent disorders among infertile Vietnamese women with PCOS. PCOS is not solely a reproductive problem. Screening and early intervention for MS and/or IRS based on anthropometric, metabolic, and reproductive hormone risk factors should be an integral part of fertility care.

  • MICE (Meetings, Incentive trip, Convention and Exhibition & Event) industry is a multifaceted industry with a variety of stakeholders including the public and private sectors, and PCOs (Professional Convention Organizers) play a mediating role in communicating opinions of the stakeholders. For enhancing their reliability and awareness, PCOs have their stakeholders' hyperlinks on the websites. This study attempted to analyze the hyperlink networks among PCOs (Professional Convention Organizers) and their stakeholders so as to understand the MICE industry ecosystem and find out whether the network structure of PCOs' shows different levels of hyperlink use with their stakeholders' websites based upon PCO's performance and size. The results showed the hyperlink network of MICE industry and that the bigger PCOs have higher centralities in the network.
