파킨슨씨 병 환자와 정상인의 입각기시간과 상비율의 비교 (Comparison of the Total Stance Time And the Phase Ratio in Parkinson's Disease Patients And Normal Subjects)

  • The purpose of this study is to investigate the gait characteristics in Parkinson's disease patients. Specifically, the total stance time and the ratio of each stance phase (heel strike, mid-stance, propulsion) are analyzed from the foot-pressure measurement system which requires low cost and small space compared to the conventional gait analysis system. The gait characteristics were analyzed in 23 Parkinson's disease patients (before and after L-dopa medication), 34 elderly (sixties) normal subjects and 21 young (twenties) normal subjects. Bradykinesia global score (self-developed score of slowness of body movement) of patients before medication was determined to see the relationship between the score and the gait characteristics. The total stance time was greater in the erde. of patients, elderly, youngs (p<0.05). The phase ratio of heel strike and propulsion was smaller and that of mid-stance was greater in the order of patients, elderly, youngs (p<0.05). However, there was no significant difference in the above gait characteristics of patients before and after medication. There was a tendency, though statistically non-significant, that the total stance time is longer and the propulsion phase ratio is shorter in patients with greater Bradikinesia global scale, and this tendency was relieved after medication.

한국형 파킨슨 환자의 역학적 기능수행지수 개발 (Developing an Biomechanical Functional Performance Index for Parkinson's Disease Patients)

  • Objective: The study aimed to develop a functional performance index that evaluates the functional performance of Parkinson's patients, i.e., to integrate biomechanical measurements of walking, balance, muscle strength and tremor, and to use multiple linear regression with stepwise methods to identify the most suitable predictors for the progression of disease. Method: A total of 60 subjects were tested for sub-variables of four factors: walking, balance, isometric strength and hand tremors. Potential independet variables were extracted through correlation analysis of the sub-variables and dependent variables, Hoehn & Yahr scale. And then, a stepwise multiple regression analysis using the potential independent variables was performed to identify predictor of Hoehn & Yahr scale. Results: First, the results of the study showed that physical composition and gait had a relatively more correlated with the progression of the disease, compared to balance and hand tremor. Second, Parkinson's functional performance is characterized by dynamic pattern of walking, such as foot clearance and turning angle (TA) of walking, and a high-explained regression model is completed. Conclusion: The study emphasized the importance of walking variables and body composition in minor pathological features compared to Parkinson's patient's balancing ability and hand tremor. Specifically, it revealed that dynamic walking patterns functionally characterize patients. The results are worth considering when assessing functional performance related to the progression of the disease at the site.

Neuroprotective Effect of a Novel Herbmedicine, Hepad on SH-SY5Y Cells

  • Parkinson's disease is a neurodegenerative disease with a wide range of dopaminergic neuron cell death in the substantia nigra. Oxidative stress and neural degeneration are suggested to be involved in the pathogenesis of Parkinson's disease. In this study, we investigated whether a novel herbmedicine, Hepad protects against 1-methyl-4-phenylpyridnium [MPP(+)]-induced dopaminergic neurotoxicity in SH-SY5Y cells. We found that pretreatment with Hepad significantly increases the proliferation of SH-SY5Y cells (P<0.05) and reversed the loss of cell viability induced by $MPP^+$. Hepad may be a promising neuroprotective agent for the treatment of neurodegenerative disorders such as Parkinson's disease.

  • WPW 증후군은 발작성 상실성 빈맥을 유발할 수 있는데 매우 드물게 가족 내 동시 발생이 보고되고 있다. 그런데 최근 유전자 연구에 의해 가족 내 동시 발생된 WPW 증후군의 경우 PRKAG2 유전자의 변이가 보고된 바 있다. 저자들은 빈맥 등 특이할 만한 임상증상을 보이지 않았던 한가족에서 3명의 자녀 중 남매 간에 WPW 증후군의 심전도 소견을 동시에 나타낸 증례를 보고하면서 PRKAG2 유전자 검사 소견을 분석하였는데 이 유전자의 변이 소견이 환아와 가족 내에서 관찰되지 않아 본 증례의 원인으로 다른 유전자의 변이 가능성을 보고하는 바이다.

파킨슨병의 신경영상 기법을 이용한 침치료 임상 연구 논문에 대한 고찰 (Review of Studies on Clinical Trials of Acupuncture Treatment for Parkinson's Disease Using Neuroimaging Methods)

  • Objectives: The purpose of this study was to investigate the current state of clinical trials for acupuncture treatment of Parkinson's disease (PD) using neuroimaging methods. Methods: We searched for clinical trial studies of acupuncture treatment for Parkinson's disease that used neuroimaging methods in the MEDLINE (PubMed) database. The identified studies were then selected manually based on inclusion criteria. We subsequently analyzed the characteristics of the selected articles and reviewed the neural substrates of acupuncture treatment in PD. Results: Eight studies were included. The most frequently applied modality for PD was functional MRI. The most frequently selected acupoint for PD was GV34. Several studies showed that acupuncture treatment could improve the symptoms of PD. Through the analysis, we demonstrated that the neuroimaging method could capture the neural substrates associated with PD and that acupuncture treatment may affect the activation of brain areas that are impaired in PD in a different way than sham acupuncture. Conclusions: Applying a neuroimaging method could be helpful in clinical trials of acupuncture treatment for PD, and more efforts should be made in this area.

파킨슨병 환자 부양자의 부양부담에 영향을 미치는 요인 (Factors Affecting Caregiver Burden in Caregivers of Patients with Parkinson's Disease)

  • Purpose: Depression is a common sign of suffering among the patients with Parkinson's disease (PD). Frequent and severe neuropsychiatric symptoms lead to high levels of distress in patients and their caregivers, which results in the high levels of caregiver burden. The aim of this study was to determine the predictors of caregiver burden in caregivers of the patients with Parkinson's disease (PD). Methods: The study included 183 consecutive PD patients and their caregivers. Patients were assessed using the Hoehn and Yahr scale, Mini Mental State Examination (MMSE), Beck Depression Inventory (BDI), and Schwab and England Activities of Daily Living Scale. Caregivers' depressive symptoms were evaluated using the Caregiver Burden Inventor (CBI), BDI, and World Health Organization Quality of Life Scale. Results: All of the patients reported one or more neuropsychiatric symptoms. Patients' and caregivers' depressive symptoms, caregivers' age and education, time for caregiving, and quality of life were significantly associated with the increased caregiver burden. After controlling the level of education as a potential confounding variable, depression in both patients and caregivers, time for caregiving, and quality of life explained 45.6% of the variance in caregiver burden. Conclusion: Substantial attention needs to be given to the early identification of depression in PD patients and their caregivers to improve caregivers' quality of life and burden.

파킨슨병 환자의 경직에 대한 임상적 평가의 정량화 (Quantification of Clinical assessment of Rigidity in patients with Parkinson's Disease)

  • The purpose of this study was to quantify the clinical assessment of rigidity at wrist in patients with Parkinson's disease. The experimental system was designed that the effect of gravity was negated by restricting motion at the horizontal plane and inertia was predetermined from a biomechanical measurement. Forty five patients with Parkinson's disease participated in this study. Viscoelastic properties were calculated from the experimental data acquired during intermittent passive movement of wrist. Viscoelastic constants correlated well with the rigidity scores of UPDRS, i.e., Spearman's r=0.733 and 0.905 for spring and damping constants, respectively. The results suggest that viscoelastic properties can be used as quantitative measures of rigidity.

Evaluation of Gait Assistive Devices in Patients with Parkinson's Disease

  • Objective: There are no guidelines for choosing appropriate gait assistive devices. The aim of this study was to evaluate gait assistive devices in patients with Parkinson's disease. Method: We evaluated 15 individuals with Parkinson's disease who did or did not use one of two different devices including canes and two-wheeled walkers. Data were collected using the GAITRite system. Results: Participants in the group using canes and two-wheeled walkers had significantly increased double support time and decreased gait velocity, normalized gait velocity, and stride length compared with those who did not. Participants who used a two-wheeled walker had significantly decreased gait velocity, normalized gait velocity, and stride length compared with those who used a cane. Furthermore, participants who used a two-wheeled walker had significantly decreased coefficients of variation for step time, stride length, and swing time compared with those who used a cane. Conclusion: Our results indicated that the two-wheeled walker offered the most consistent advantages for decreasing the risk of falling.

양혈구풍탕가미방(養血驅風湯加味方)으로 호전된 진전 환자 4례에 대한 증례 보고 (Four Cases of Tremor Treated with Yanghyulgupungtang-gamibang)

  • Tremor is a rhythmic and involuntary muscular contraction characterized by oscillations of a part of the body. Tremor is a symptom of many disorders, including Parkinson's disease, essential tremor, orthostatic tremor, cerebellar disease, peripheral neuropathy and alcohol withdrawal. The appropriate treatment of tremor depends on accurate diagnosis of it's cause. Some tremors respond to treatment of underlying conditions. Senile tremor and Parkinson's tremor are more common in aged people. Yanghyulgupung-tang is effective herbal medication in blood deficiency-type senile tremor. Four cases of senile tremor and Parkinson's tremor are reported. All four patient were treated with Yanghyulgupungtang-gamibang and improvement was seen in all four.

파킨슨병 환자의 음성장애지수 및 음성관련 삶의 질 연구 (Voice Handicap Index and Voice-Related Quality of Life in Idiopathic Parkinson's Disease)

  • Objectives : The purpose of this study is to evaluate the voice handicaps of the idiopathic Parkinson's Diseases (PD) and their voice-related quality of life. Methods : Voice handicap index-10 (VHI-10) and Voice related Quality of Life were completed by 17 idiopathic PD patients, and Unified Parkinson's Disease Rating Scale (UPDRS) part I, II, III were assessed. The relations between VHI-10, VRQOL and UPDRS scores were analysed. Results : VHI-10 score of PD patients was $14.35{\pm}8.07$ and VRQOL total score of PD patients was $59.12{\pm}20.25$, social-emotional $59.93{\pm}20.50$, physical function $58.58{\pm}21.77$. There were significant relations between VHI-10, VRQOL score and UPDRS II (activities of daily living). Conclusions : These results suggest that voice impairments affect the daily living of PD patients and their quality of lives.
