Early diagnosis of jaw osteomyelitis by easy digitalized panoramic analysis

  • Background: Osteomyelitis is an intraosseous inflammatory disease characterized by progressive inflammatory osteoclasia and ossification. The use of quantitative analysis to assist interpretation of osteomyelitis is increasingly being considered. The objective of this study was to perform early diagnosis of osteomyelitis on digital panoramic radiographs using basic functions provided by picture archiving and communication system (PACS), a program used to show radiographic images. Methods: This study targeted a total of 95 patients whose symptoms were confirmed as osteomyelitis under clinical, radiologic, pathological diagnosis over 11 years from 2008 to 2017. Five categorized patients were osteoradionecrosis, bisphosphonate-related osteonecrosis of jaw (BRONJ, suppurative and sclerosing type), and bacterial osteomyelitis (suppurative and sclerosing type), and the control group was 117 randomly sampled. The photographic density in a certain area of the digital panoramic radiograph was determined and compared using the "measure area rectangle," one of the basic PACS functions in INFINITT PACS® (INFINITT Healthcare, Seoul, South Korea). A conditional inference tree, one type of decision making tree, was generated with the program R for statistical analysis with SPSS®. Results: In the conditional inference tree generated from the obtained data, cases where the difference in average value exceeded 54.49 and the difference in minimum value was less than 54.49 and greater than 12.81 and the difference in minimum value exceeded 39 were considered suspicious of osteomyelitis. From these results, the disease could be correctly classified with a probability of 88.1%. There was no difference in photographic density value of BRONJ and bacterial osteomyelitis; therefore, it was not possible to classify BRONJ and bacterial osteomyelitis by quantitative analysis of panoramic radiographs based on existing research. Conclusions: This study demonstrates that it is feasible to measure photographic density using a basic function in PACS and apply the data to assist in the diagnosis of osteomyelitis.

  • Application of information system to hospital would bring innovative improvement on efficiency of business management and provide high quality services toward patients as well as the retrenchment of operating funds. PACS(Picture Archiving and Communication System) including X-ray film that manages the medical image information effectively, has drawn considerable attention to essential structural elements to the sophisticated information system for hospital. PACS system should be connected to the network after making a form of standard medical image file from different style of image information obtained from various medical instruments. In this paper, to solve this problem, we construct Mini-PACS that converts the form of Non-DICOM file to the form of standard file by designing the DICOM converter. This system is designed to be managed under Web environment. Comparing with the existed Mini-PACSs, consisting of the client and server module, our system is designed and implemented with integration of these functions in order to be strongly combine strongly between system.

  • The recent development of embodiment technology of the medical images makes most medical institutions introduce PACS in haste. However, while many older HIS and PACS systems are not yet capable of some of the integration, several new systems are moving rapidly in that direction. Typical PACS system architecture begins with the HIS since this is where the correct patient demographic information and in many cases the orders originate. So, PACS developed convenience of users and to satisfy user's demand because of financial limitations and administrator-oriented considerations in the process of development. Therefore, we have developed a CDA (Clinical Document Architecture) based PACS with HIS, by which we can search and refer to the patient's medical images and information with few restrictions of time and space for diagnosis and treatment. Target model of this research limited to 135 of hospital have 200 beds. We'll make more effort to develop the application which insures the better quality and information of medical images. Medical Image History manages the patient's image files and various medical informations like film chart in connection with time. This trial will contribute to the reduction of the financial loss caused by unnecessary devices and improve the quality in the medical services. The demand on the development of the program which refers to the medical data quickly and keeps them stable will be continued by the medical institute. This will satisfy the client's demand and improve the service to the patients in that the program will be modified from the standpoint of the users.

  • The purpose of this study was to survey a development of the wireless transmission system of medical images for ubiquitous medicine. There have been many changes in medical equipments and medical record medical treatment and medical record within hospital and PACS(Picture Archiving Communication System) which is picture management system for patients can be typical cases. It is difficult to use these automated medical systems unless they are within hospital and in case of rapid image reading in the emergency cases or in absence of doctor, it is difficult to perform it immediately. The present study implemented an image transmission system using CDMA connection so that images in the server can be viewed at any time and in any place. Remote wireless diagnosis based on medical images using PDA is applicable to medical areas that require mobility, and the use of PDA can be an ideal alternative for point of care. The use of PDA enables prompt and accurate access to digital medical images, which in turn reduces medical accidents and improves the quality of medical services through high productivity and efficiency of medical practitioners' works. It also enables quick response to patients' demands and high-quality medical services and, consequently, patients' high satisfaction.

Depending on PACS Operating System Differences Analysis of Usefulness of Lossless Compression Method in Medical Image Upload: SNR, CNR, Histogram Comparative Analysis (PACS운영 시스템 차이에 따른 의료 영상 업로드 시 무손실 압축 방식의 유용성 분석: SNR, CNR, Histogram 비교 분석을 중심으로)

  • This study focused on the fact that medical images that are issued at different hospitals may affect image quality on PACS when different software is used. A university hospital image was copied to the DICOM file and registered on the PACS of the university hospital B. The capacity and image quality of the software used in the university hospital were evaluated by SNR, CNR and histogram. As the compression ratio increased, SNR and CNR tended to decrease. Note that Lossless Compression decreased the data size by half compared to No Compression, but SNR and CNR did not change. As a result of the histogram analysis, the information loss due to the underflow phenomenon was conspicuous. When moving to another hospital, No compression or lossless compression method should be used. In conclusion, it is useful to use the lossless compression method, considering waiting time and economic efficiency in uploading.

Characteristic of Cross-sectional Area of Lumbar Paraspinal Muscle in Patients of Acute and Chronic LBP (20대와 40대의 급성 및 만성요통환자의 척추주위 근육에 관한 횡단면의 비교)

  • The purpose of this study was to compare acute and chronic LBP patient in twenties and forties, respectively by size measure paraspinal muscle (cross-sectional area; CSA). CSA of paraspinal muscle (psoas, multifidus, erector muscle) size was measured by free-handling technique of the picture archiving and communication system(PACS) using MRI at the level(lower end-plate of L4) in twenties(9 males, 10 females) and forties(9 males, 8 females) in acute and chronic LBP patient. The results of this study showed no significantly difference between acute and chronic LBP (p>0.05) in twenties patients. However, there was significant difference between acute and chronic LBP (p<0.05) in forties patients. Also, there was significant difference in paraspinal muscle CSA between chronic LBP patients in twenties and chronic LBP patients in forties (p<0.05). This study showed that paraspinal muscle atrophy was observed in forties with various cause, but Not chronic LBP patients in twenties. Accordingly it is required for chronic LBP patients in forties to minimize trunk muscle atrophy through immediate back muscle dynamic exercise and early functional activity.

Measurement Analytical Study of Computed Tomography of the Orbital Structure in Acute Blow-out Fracture (안와파열골절 급성기의 CT영상을 이용한 계측학적인 연구)

  • Purpose: Blow-out fracture is one of the most common fractures in facial trauma. It is diagnosed by Computed Tomography(CT) scan, which is considered as the most effective diagnostic tool. Since, the Picture Archiving Communication System(PACS) has been provided recently to many hospitals, doctors are more familiar with imaging software of PACS. Because this software has many useful measuring tools, doctors can measure orbital structure easily and make a plan for treatment with its data. Therefore, authors intended to analyze the data of orbital structure measured with PACS imaging software and evaluate its usefulness. Methods: The charts and CT images of 100 patients, which were 50 patients with medial wall fracture and 50 patients with floor fracture, were reviewed. Patients were selected by pre-determined criteria and their CT images were measured with image software of PACS. 'Extraocular muscle thickness', 'Defect ratio'(ratio of defect area to normal area) and 'Globe position index' were measured and analyzed statistically. Results: The thickness of inferior rectus muscle and medial rectus muscle was simultaneously increased in acute-stage of blow-out fracture. The medial rectus muscle was more thickened in medial wall fracture and inferior rectus was more thickened in floor fracture, respectively. In acute blow-out fracture, globe position is exophthalmic rather than enophthalmic. Especially in floor fracture, numerical value summed up thickness of all extraocular muscle is correlated to the defect ratio and globe position index. Conclusion: Clinicians can decide globe position or presume defect ratio in inferior wall fracture by measurement of CT image in acute blow-out fracture using PACS.

  • Digital gamma camera has many advantages over analog gamma camera. These include convenient quality control, easy calibration and operation, and possible image quantitation which results in improving diagnostic accuracies. The digital data can also be utilized for telemedicine and picture archiving and communication system. However, many hospitals still operate analog cameras and have difficult situation to replace them with digital cameras. We have studied a feasibility of digitalizing an analog gamma camera into a digital camera using Gamma-PF interface board. The physical characteristics that we have measured are spatial resolution, sensitivity, uniformity, and image contrast. The patient's data obtained for both analog and digital camera showed very similar image quality. The results suggest that it may be feasible to upgrade an analog camera into a digital gamma camera in clinical environments.

  • In recent years, two major streams in medical information systems are:1) system integration among OCS(Order Communication System), EMR(Electronic Medical Record), PACS(Picture Archiving and Communication System), and ERP(Enterprise Resource Planning) and 2) system integration through medical collaboration between East and West medical service providers. One of the characteristics which differentiate the Korean medical industry from the western medical industry is the East-West medical collaboration. In many respects there are many differences between East and West medical treatment. Although East and West medical treatment have developed from different medical philosophies and standards, we assume that the better medical care can be provided by integrating their medical procedures effectively. The two possible approaches to the integration of East and West medical information systems are suggested in this paper:One is loosely coupled model and the other is tightly coupled model. EMR improves the quality of medical record which reflects the quality of clinical practice. It provides more efficient and convenient way of input, retrieval, storage, communication and management of medical data. We abstracted the standard medical procedures from the two medical procedures performed in Daejeon Oriental Hospital and Hehwa Clinic at Daejeon University and also abstracted database schema by analyzing the characteristics of information needed in East-West medical collaboration. Our EMR is composed of two types of data:one is structured data and the other is unstructured data, which are formalized by SOAP(Subjective, Objective, Assessment, Plan) format. Currently the integrated system is implemented and operated successfully for six months.

  • Recently there has been attempts to develop hospital information system including order communicating system, patient tracing system, tumor registry system, office automation system and picture archiving and communication system(PACS). The authors devloped a practical system that was operated via order communication system of Yeungnam University Hospital. The system provided us the speed, accuracy, reliability, retention, economy and wide applicability through practical test. So we believed that this system would be one of standard computerized programs in radiation oncology practice and providing widely usable data for clinical statistics, medical record, tumor registry system and clinical researches.
