Repair Integrity and Functional Outcomes after Arthroscopic Repair of Transtendinous Full-thickness Rotator Cuff Tears Minimum Two-year Follow-up

  • Background: To evaluate the clinical outcomes and associated repair integrity in patients treated with arthroscopic repair for a transtendinous rotator cuff tear followed by resection of the remnant rotator cuff tendon. Methods: Between July 2007 and July 2011, we retrospectively reviewed patients who were treated for transtendinous full-thickness tears in the tendinous portion of the rotator cuff by arthroscopic repair. Clinical outcomes were evaluated using the American Shoulder and Elbow Surgeons (ASES) score, the Shoulder Rating Scale of the University of California at Los Angeles (UCLA), the Constant-Murley score, a visual analogue scale (VAS) pain score, and range of motion (ROM). The repair integrity was determined by magnetic resonance imaging or ultrasonography. Results: There were 19 shoulders with transtendinous full-thickness tears in the tendinous portion of the rotator cuff. The ASES, UCLA, Constant-Murley, and VAS pain scores showed significant improvements in function and symptoms (all p<0.001). The active ROM for forward flexion and abduction was also significantly improved (p=0.002 and p<0.001, respectively). The postoperative radiological examination showed cuff integrity without a re-tear in 68.4% of patients. However, the UCLA, ASES, and Constant-Murley scores were not significantly different between healed and re-torn group (p=0.530, p=0.885, and p=0.262, respectively). Conclusions: Although repair of transtendinous rotator cuff tears followed by resection of the remnant rotator cuff tendon in the footprint has a relatively high re-tear rate, no significant difference was observed in the short-term clinical results between the re-tear and healed groups.

배열 정수 변화에 의한 순차 회전 배열 안테나의 특성 (Characteristics of Sequentially-Rotated Array Antenna by Varying Sequential Array Parameter)

  • 순차 회전 배열 안테나에서, 순차 배열 정수(배열소자 수, 회전정수)의 변화에 따른 안테나 이득, 축비 및 교차편파 특성을 각각 비교 분석하였다. 여기서 안테나 기본소자로는 중심주파수 11.85GHz인 좌원편파(LHCP), 프로브급전 트렁케이티드 마이크로스트립 안테나를 사용하였다. M=2, 3, 4, 6, 8인 경우에 대하여 P값을 $1{\sim}M$으로 변화시킨 순차 회전 배열 안테나 총 23개에 대한 시뮬레이션을 수행한 결과, 축비 대역폭은 배열소자 수 M에 상관없이 P=2인 경우가 가장 넓으며, 이득은 순차배열 정수(M, P)가 (2, 1), (3, 2), (4, 1), (6, 3), (8, 1)일 때 가장 크게 나타나고 있다. 특히 M=P인 경우 모든 안테나 특성이 나빠진다. 따라서 순차 회전 배열 안테나를 설계할 때, 많은 시뮬레이션 데이터들 중에서 최적의 순차배열 정수를 찾아 다음 설계하여야 한다.

  • To investigate the protein binding characteristics of cephalothin, the effects of ionic strength, pH and temperature on the binding of cephalothin to bovine serum albumin (BSA) were studied by UV difference spectrophotometric method. With increasing ionic strength at constant PH and temperature, association constant decreased, but the number of binding sites sites was about 2 constantly. It may be deduced that the binding process is not only due to electrostatic forces. And the increased association constant at high ionic strength is explained by conformational changes of BSA from complex to subunits. The pH effect on the affinity of interaction indicated that the binding affinity of drug is higher in the neutral region than in the alkaline region. And, at high pH value, the number of binding sites decreased from 2 to 1 because of the conformational changes of BSA in alkaline region. The decrease in binding affinity of BSA to drug with increasing temperature was characteristic of an exothermic reaction. And the negative sign of ${\Delta}G^{\circ}$ meant that the binding process occurs spontaneously under the experimental conditions. In cephalothin-BSA complex formation, since the net enthalpy change value and entropy change value are positive, it is assumed that hydrophobic bindings are predominant in this binding process.

  • We synthesized polypyrrole (PPy) by electrolysis of the pyrrole monomer solution containing support electrolyte, KCl and/or p-toluene sulfonic acid sodium salt (p-TS). The electrochemical behavior, was investigated using cyclic voltammetry and AC impedance. In the case of using electrolyte p-TS, the oxidation potential of the PPy was about -02 V vs Ag/AgCl reference electrode, while the potential was about 0 V for using electrolyte KCl. The falloff of the oxidation potential gave a sign of an improvement in the electron hopoing mechanism on the backbone. The AC impedance plot gave a hint of betterment of mass transport. PPy doped with p-TS improved in mass transport or diffusion. That was because the PPy doped with p-TS was more porous than PPy with KCl. We attained an effect of good kinetic parameters, in the case of PP-GOx enzyme electrodes doped with p-TS, which were determined by 58 mmol dm$\^$-3/ for apparent Michaelis constant and by 581 ㎂ for maximum current respectively.

  • $p-CH_3, p-H, p-Cl 및 p-NO_2 기로 치환된 염화벤젠술포닐의 가용매분해반응을 MeOH-MeCN혼합용매계에서 연구하였다. 반응속도는 p-NO_2 > p-CH_3 > p-H > p-Cl의 순위로서 비직선 Hammett관계를 나타내어 S_N1-S_N2경계반응메카니즘으로 반응함을 알았다. 반응성은 모든 경우 약 90∼95% 메탄올용액에서 최대였으나, fi = \frac{k1}{ki}로 정의되는 각 기질의 메탄올 단위체 선택성(monomer selectivity)은 80% 메탄올 용액에서 최대이며, 그 크기 순위는 p-NO_2 > p-Cl > p-H > p-CH_3$로서 전이상태 구조가 tight할 수록 fi값이 크며 메탄올 단위체를 예민하게 선별 감지함을 알 수 있었다

Humic Acid와 화학물질간의 결합상수 측정 (Measurement of Binding Constant between Chemical Compound and Humic Acid)

  • 수계에서 화학물질의 독성에 대한 humic acid(HA)의 영향을 예측하기 위하여 HA와 화학물질간의 결합상수($K_B$)를 equilibrium dialysis를 이용하여 cartap, diazinon, fenobucarb, pentachlorophenol, P,P'-DDT를 대상으로 측정하였다. $K_B$의 실험치 및 옥탄올/물 분배계수를 이용한 예측치로부터 대상 화합물 중 P,P'-DDT를 제외하고는 HA에 의해 수계 독성에 영향을 줄만한 농도의 감소는 없을 것으로 추정되었다. 실제 자연환경에서는 $K_B{\geq}10^5$ 또는 $K_{ow}{\geq}10^6$ (log $K_{ow}{\geq}6$)인 화학물질에 대해서만 HA에 의한 독성감소 효과를 기대할 수 있는 것으로 생각 되었다.

  • We have adopted the strain compensated PBH(planar buried heterostructure) - LD in which the MQW active layer consisted of 1.4% compressively strained GainAsP (E$_{g}$ = 0.905eV) wells and 0.7% tensile strained GaInAsP(E$_{g}$ = 1.107eV) barriers grown by metal organic vapor phase epitaxy (MOVPE). We hav einvestigated effects of number of wells and the structure of the separate confinement heterostructure (SCH) layer in the strain-compensated MQW-PBH-LD. The threshold current, the external quantum efficiency, the transparency current density J$_{o}$, and the gain constant .beta. have been evaluated for uncoated MQW-PBH-LD. As the number of wells increases, the internal quantum efficiency and the transparency current density decreases, whereas the gain contant increases. The small width of the SCH layer shows the large internal quantum efficiency. The small internal loss and the large gain constant have been obtained by inserting the large bandgap SCH layer.
