배란 유도에 실패한 배란장애 환자의 한방치료 후 자연 임신 및 출산 1례에 대한 증례 보고 (A Clinical Study on 1 Case of Pregnancy in Ovulation Disorder Patient Resistant to Controlled Ovarian Hyperstimulation)

  • Objectives: The purpose of this study is to report the effects of Korean medical treatment in ovarian disorder patients resistant to controlled ovarian hyperstimulation. There are some case-reports about effectiveness of the Korean medical treatment to treat ovulation disorder and become pregnant, but cases are still deficient. Methods: The patient was diagnosed possibility of premature ovarian failure by serum hormone assay and underwent hyperstimulation to freeze oocyte. But ovary had no response to hyperstimulation. She was treated by Korean medical treatment, such as herbal medicine, acupuncture and moxibustion. Results: After the treatment, the patient recovered from irregular ovulation & menstrual cycle and became pregnant and gave birth. Conclusions: This case shows the Korean medical treatment might be effective in ovarian disorder patients resistant to hyperstimulation.

대영전(大營煎) 백서(白鼠)의 배란(排卵)과 난소(卵巢)에 미치는 영향(影響) (Effects of Daeyeongjeon on the Ovulation and Ovary in Rats)

  • Purpose : Deoyeongjeon(DYJ : 大營煎) is used in female infertility, caused by ovulation disorder. so this study is to examine what are the effects of the Deoyeongjeon(DYJ) on the vulation and Ovary in Rats Methods : 4weeks Female Sprague-Dawley 12 rats of weighting 160-l80g, were divided into three groups including the DYJ oral administration(4ml/kg) groups(4heads) and DYJ oral administration(8m/lkg) groups(4heads). then we observed changes in the serum concentrations of FSH, LH, and estradiol(E2) and the histological changes of ovary and the immunohistochemical staining for progesterone receptor in ovary of rats. Results : 1. Blood FSH level significantly increased in experimental group as compared with control group. 2. In blood LH level, experimental group showed no efficacy as compared with control group. 3. In blood estradiol(E2) level, experimental group showed no efficacy as compared with control group. 4. In histological observations of ovary, ovulation increased in experimental group as compared with control group, which showed no efficacy. 5. In observations of immunohistochemical staining for progesterone receptor in ovary, immunohistochemical staining score (ISS) of atretic follicles significantly showed a tendency to decrease in experimental group as compared with control group. Conclusion : DYJ influences the pituitary gland and ovary to increase the ovulation of rats.

가미귀비탕(加味歸脾湯)이 백서(白鼠)의 배란(排卵)에 미치는 영향(影響) (Effects of Gamiguibitang on the ovulation in rats)

  • Gamiguibitang(GMGBT) is used in amenorrhea and female infertility caused by ovulation disorder. An attempt was made to evaluate the influences of GMGBT on the serum concentrations of FSH, LH, estradiol(E2) and progesterone, the histological and optical changes of ovary of rats. The results of the study were as follows: 1. Blood FSH level significantly increased in experimental group controlled by four times quantity as compared with control group. 2. Blood LH level increased in experimental group controlled by four times quantity as compared with control group, which showed no efficacy. 3. Blood E2 level increased in experimental group controlled by four times quantity as compared with control group, which showed no efficacy. 4. Blood progesterone level significantly decreased in experimental group controlled by double quantity as compared with control group. 5. In optically observations of ovary, weight of ovary significantly increased in experimental group controlled by double quantity as compared with control group 6. In histological observations of ovary, ovulation significantly increased in experimental group controlled by both double and four times quantity as compared with control group. According to these results, it can be concluded that GMGBT influences the pituitary gland and ovary to increase the ovulation of rats.

Update on polycystic ovary syndrome

  • Polycystic ovary syndrome (PCOS) is a common disorder in reproductive-age women. In 2018, an international evidence-based guideline announced recommendations spanning a wide range of issues on the assessment and management of PCOS. From the 166 recommendations, the present study reviews those that are of particular clinical relevance for daily practice and introduces other relevant studies that have been published since the global guideline. The 2018 guideline increased the antral follicle count cutoff for the diagnosis of PCOS from 12 to 20 when using a high-frequency probe. Hirsutism was defined as having a score of ≥4-6 based on a lower percentile of 85%-90% or cluster analysis, which was lower than the traditionally used 95th percentile-based cutoff. The diagnosis of PCOS in adolescents is challenging, and irregular menstruation was defined carefully according to years from menarche. The use of ultrasonography for the diagnosis of PCOS was restricted to those 8 years after menarche. As medication for non-fertility indications, combined oral contraceptives are the first-line drug. Metformin, in addition to lifestyle modifications, should be considered for adult patients with a body mass index ≥25 kg/m2 for the management of weight and metabolic outcomes. An aromatase inhibitor is the recommended first-line medication for ovulation induction, a subsequent individual patient data meta-analysis also reported the same conclusion. Whether the new global guideline will be fully adopted by many specialists and change clinical practice is open to question. Further studies are needed to better understand and manage PCOS patients well.

젖소 공란우 과배란 처리 후 난소주기 재개에 관한 연구 (Resumption of Ovarian Cyclicity after Superovulation Treatment to Donor Cow in Holstein Donor Cows)

  • The objective of this study was to investigate the effects of abnormal ovarian cycles after superovulation treatment of Holstein Donor Cows. CIDRs were inserted into the vaginas of twenty two head of Holstein cows, regardless of estrous cycle. Superovulation was induced using folliclar stimulating hormone (FSH). For artificial insemination, donor cows were injected with $PGF_2{\alpha}$ and estrus was checked about 48 hours after the injection. Then they were treated with 4 straws of semen 3 times, with 12-hour intervals. Embryos were collected by a non-surgical method 7 days after the first artificial insemination. The cows were considered to have resumed ovarian cyclicity on the day of ovulation if followed by regular ovarian cycles. Seventy two point seven percentage of the cows(16/22) had normal resumption of ovarian cyclicity(resumption within 40 days after superovulation), and 27.3%(6/22) had delayed resumption(resumption did not occur until>40 days after superovulation). Delayed resumption Type II(first ovulation did not occur until ${\geq}40$ days after superovulation, i.e. delayed first ovulation 13.6%) were the most common types of delayed resumptions. The mean numbers of total ova from < 10 and $10{\leq}$ of corpora lutea(CL) was $7.8{\pm}1.8$ and $12.7{\pm}2.7$, respectively. The number of transferable embryos differed between < 10 and $10{\leq}$ CL was $5.4{\pm}1.3$ and $8.1{\pm}3.4$, respectively. Four point five percentage of the cows(1/22) did not resumption their ovarian cyclicity until 60 days after superovulation treatment. Diverse researches on the superovulation treatment method that is suitable for high-producing Holstein donor cows would contribute to preventing ovarian cyclicity disorder, as well as to the early multiplication of cows with superior genes by increasing the utilization value of donor cows.

  • The polycystic ovary syndrome is the most common endocrinological disorder of reproductive age women with a prevalence of 5 to 8 %. The most common diagnostic criteria used for polycystic ovary syndrome are oligo- or an-ovulation, clinical and/ or biochemical signs of hyperandrogenism and polycystic ovaries. Hyperandrogenism results in increased estrogen levels and lack of cyclic progesterone due to anovulation and persistent stimulation of the endometrium may lead to endometrial hyperplasia or adenocarcinoma development. In this mini review, we aimed to evaluate the possible relationship between polycystic ovary syndrome and endometrial cancer.

젖소에서 배란동기화법(OVSYNCH)의 이용 (Utilization of Ovulation-Synchronization Method in Dairy Cows)

  • 본 연구는 유우 사육 농가에서 배란동기화법(OVSYNCH)의 활용시 수태율에 미치는 영향을 검토하였다. 그 결과, 유우의 정상 번식우(15/23, 63%)와 번식 장애우(5/10, 50%)의 수태율 비교에서는 정상번식우가 수태율이 높게 나타났지만 유의차는 인정되지 않았다. 그러나 유우 경산우의 산유량에 따른 수태율 비교에서는 1일 산유량이 $30{\sim}35kg$ 이상인 유우군에서 배란동기화 처리시 $20{\sim}25kg$인 유우군에 비해 유의적으로 높은 수태율을 나타냈다(p<0.05). 또한 유우 경산우의 산차수에 따른 수태율 비교에서는 1산차의 시험우군이 3산차군에 비해 유의적으로 높은 수태율을 나타냈지만(p<0.05) 산차수 증가에 따른 수태율은 일정한 경향을 나타내지 않았다. 본 연구의 결과로부터, 배란동기화 방법을 유우의 다두사육 농가에서 실제로 이용할 경우 우유 생산이 가능한 정상번식우에서 효과적이었으며, 특히 1일 산유량이 많은 유우에서 높은 수태율을 얻을 수 있었다. 한편 앞으로의 연구를 통해 개선된 배란동기화법을 이용한다면 호르몬 불균형으로 초래된 번식 장애우의 번식 능력 회복과 함께 향상된 수태율을 얻을 수 있는 효과적인 방법으로 농가에서도 간편하게 이용될 수 있을 것으로 평가된다.
