Background: To implement medication management service in Korean nursing home (NH), medication review tool for residents in Korean long-term care facilities was developed. This prospective pilot study aimed to verify the applicability of this newly developed tool and to evaluate the drug related problems (DRPs) identified by pharmacists' medication review in NH setting. Methods: This study was carried out in two NHs in Korea. The elderly residents (65 or older) using 5 or more medications were eligible. Pharmacists conducted medication review and identified the DRPs and potentially inappropriate medications (PIMs) based on the newly developed tool. Results: Among 43 NH residents, 27 residents agreed to participate. The median age was 87 and about 55.6% of them were taking 10 or more medications. Pharmacists identified a total of 37 PIMs in 18 NH residents (66.7%) and 54 DRPs in 22 residents (81.5%). The most frequent PIM was general-PIM (26 cases) followed by PIM under specific diseases or conditions (7 cases). Out of 77 items contained in the tool, 15 items were detected in study participants. 'Continued use or regular daily use of hypnotics' was the most highly detected item (9 cases) followed by 'multiple prescriptions within each class of hypnotics/sedatives' (5 cases). Among 54 DRPs identified, pharmacist intervened 39 cases (72.2%) and interventions were accepted in 18 cases (46.1%). Conclusion: This pilot study demonstrated that newly developed tool is feasible for the nursing home residents. However, further studies with larger population are warranted.
Recent developments of smart phone and ICT, and explosive developments of wireless sensor area drive radical changes on traditional health care service. To accommodate the changes, many researchers have studied to expand traditional healthcare service areas including home care for independent living and public care for preventive and collaborative wellness area. This study proposes a research framework for healthcare information technology area based on Mettler and Raptis's(2012) work. Then, the study analyze the research trends in the area based on the framework. The area of monitoring patients health status at home using smart phone, providing innovative healthcare service by out-patients monitoring, and implementing preventive healthcare services are identified most active and emerging research agenda. It is expected that the research framework and implications of this study can assist future research efforts and practical utilization of healthcare information technologies.
This study was to investigate the effects of interpersonal relationships on job satisfaction of home-visiting care helpers and the category of interpersonal relationships was subdivided into elderly clients, guardians, colleagues and directors considering the person-to-person nature of home-visiting care helper's job characteristics. A group of 336 care helpers who engaged in home-visiting care service providing facilities in Daejeon City was selected as a research subject, and the data were analyzed with SPSS 22.0 program using reliability, frequency, correlation and hierarchical multiple regression. The results for the final analysis revealed that the effect of socio-demographic features was not significant while, among characteristics of job performances, work priority, turnover experience, and career choice reasons turned out to have significant impacts on job satisfaction. In particular, interpersonal relationships except the one with guardians were found to be the most influential variables and the effects of those relationships on job satisfaction were significant. The influencing power was stronger in relationships with director, elderly clients, and colleagues in order, and the job satisfaction level was higher as they were in better relationships with director, elderly clients, and colleagues. The results of this research shows that it is necessary to set an atmosphere where harmonious interpersonal relationships can be formed inside and outside of facilities in order to improve job satisfaction of home-visiting care helpers.

South Korea faces elderly care crisis with both low fertility rate and rapid ageing rate. Long-term care insurance in 2008 has taken care burden from family to nation. However, few options, lack of tailored services, and unstable community care plan caused blankness of care in some areas. Especially, increasing dementia rate has become an emergent social issue. Welfare technology could be a useful alternative to fill out the gap between demand and supply. Using FGI method on two groups(high-risk dementia group and MCI group), intimacy, accessibility and preventive way of dementia with welfare technology are analysed in this paper. Despites of a few differences of dementia progress, interests towards technology are expressed. More suggestions using TV and normal telephone to improve daily independence are presented as well. Amongst MCI group, help-service for technology-use at ADS and linkage ways of in-home services are also suggested.

It will be increasing and is projeted $23.0\%$ by 2030. The result indicated that utilizations rate by out-patient were 5.89 claims and utilizations rate by in-patient were 0.17 claims per person. The highest disease among respondents were disease of musculoskeletal system and connective tissue. A total of 310 elderlys were responded to analyze self-care ability and health behavior. The most important factors of long life were to have peaceful mind$(50.0\%)$. The common disease of acute and chronic disease was musculoskeletal system disease. $66.8\%$ of respondents went to hospital and local clinic when they got sick. The most needed health care service was home visiting service among public health center, representing $31.4\%$. The repondent's self-care ability and self-efficacy were relatively superiority. A total of 92 elderlys were conducted the intelligence test for the rate of dementia and their average age was 74.3. The result of Minimental State Scale indicated that 25% of respondents were suspected to be dementia. The followings are recommendations based on the survey result. 1) Concidering every conditions of self-care ability and health status for elderly. It is important to embody appopriate health care service. 2) Considering concrete method, it is necessary to establish health service, which match health status and self-care ability, and various planning for sepecial facilities for the elderly. 3) It is desiable to make actual programs for the elderly in each community level. 4) It must be develop the better use of volunteers and programs for prevention of dementia. Finally, Concerning the orgarnization of public health center, community health center need to be reorganized for health service for the elderly. It is important to develop and operate health promotion for the elderly, and it is necessary to form the foundation for the support of facilities equipments. This contribute to promote health status for the rural elderly.
The purpose of this study is to examine the job characteristics of care workers in elderly care voucher service emphasizing a social service quality management approach. The study sample was composed of randomly-selected 233 centers which dispatched care staffs to clients' home. Descriptive analyses were performed for examining the unique aspects of relationship-based labor of care staffs, and logistic regression analyses were performed for investigating the association between service quality structure and human right violation against staffs. As the first empirical study focusing on staff-side service quality factors, this study found out that human right violation against staffs was mainly influenced by record-keeping and document management capacity of center, risk protection under insurance, compliance of standard contract procedure, and regular supervision. These results suggest particular policy attention should be given to basic protection for and set-up of core activity boundaries of care workers as well as clients-centered rights both for preventing human right violation and improving overall social service quality.

The home nursing care system is an integral part of the health care delivery system in order to meet the various needs of consummer, in particular, early discharge patient from the hospital, patient with long term care needed and the elderly. To find out the cost of home nursing care services, the home nursing care records of patients registered by home nursing care units established in public hospital with 150beds during the period of 1996 - 1997 were analyzed. The subjects were 102patients, 45 of male patients and 57 of female patients, those who live in a rural area in Kymiggi - Do The results obtained are as follows : 1. The male patients accounted for 44.1% of the total, with 45cases : group aged 60 years and more was the largest group, accounting for 79.5%. 2. The most frequent disease revealed was the osteoporosis which constitute 35.3% of the total registered patients, followed, in order, by malignant tumor, cerebrovascular disease. 3. It revealed that the cost per visit for the male was 47,764won ; the female, 46,078 won per visit. Noteworthy the cost per visit was high in the older patient. It was clearly that the gender, years of age and the cost per visit were statistically significant at 0.01 level and 0.05 level. 4. The cost per visit for the non complicated disease was slightly higher than the complicated disease, but it is not statistically significant, the cost per visit by type of disease varied, the cost per visit for COPD was the highest, followed, in order, by in malignant tumor, cancer, diabetes, osteoporosis etc. 5. It revealed that home nursing care cost for a eligible disease for home nursing care was less than the cost for hospitalization of the same disease, therefore, we expect that the home nursing care is cost efficiency. In conclusion, the home nursing care costs are needed to analyze further in comparison with the hospitalization costs for a certain disease.
