결합조직형성 법랑아세포종 (DESMOPLASTIC AMELOBLASTOMA)

  • ameloblastoma는 일반적으로 경조직을 형성하지 않는 진성 종양으로 알려져 있으나 최근에 저자는 종양내에 골형성을 수반한 경우의 ameloblasoma를 경험하고 이것을 보고하였다. 이 드물게 나타나는 ameloblastoma는 desmoplastic ameloblastoma로 명명되고 있으며 방사선 사진상으로 fibro-osseous lesion 과 감별을 요한다.

  • We report a case of tenosynovial giant cell tumor with severe bone erosion in the right fifth finger of a 46-year-old man. Throughout this case review, we describe the imaging findings of tenosynovial giant cell tumor with severe bone erosion and review the literatures regarding osseous lesions caused by tenosynovial giant cell tumor and their significance related to the differential diagnosis and patient treatment.

  • Technetium-labeled phosphate bone scan was shown to detect bone fractures and bone metastasis in early stage than general radiographs. Therefore, bone scan has become one of the most frequently performed nuclear medicine imaging examination. However, non-osseous radiopharmaceutical uptake on the bone scan are unusual findings. We report a case of diffuse splenic absorption of Tc-99m dicarboxypropane diphosphonate in patients who undergo liver transplantation.

백서 두개골 결손부에서 동결건조골과 gel/putty 형 탈회골기질의 골재생효과 (The effect of the freeze dried bone allograft and gel/putty type demineralized bone matrix on osseous regeneration in the rat calvarial defects)

  • Purpose: This study was aimed to evaluate the effect of the Freeze Dried Bone Allograft and Demineralized Bone Matrix on osseous regeneration in the rat calvarial defects. Methods: Eight mm critical-sized calvarial defects were created in the 80 male Sprague-Dawley rats. The animals were divided into 4 groups of 20 animals each. The defects were treated with Freeze Dried Bone Allograft($SureOss^{TM}$), Demineralized Bone Matrix($ExFuse^{TM}$ Gel, $ExFuse^{TM}$ Putty), or were left untreated for sham-surgery control and were evaluated by histologic and histomorphometric parameters following a 2 and 8 week healing intervals. Statistical analysis was done between each groups and time intervals with ANOVA and paired t-test. Results: Defect closure, New bone area, Augmented area in the $SureOss^{TM}$, $ExFuse^{TM}$ Gel, $ExFuse^{TM}$ Putty groups were significantly greater than in the sham-surgery control group at each healing interval(P < 0.05). In the New bone area and Defect closure, there were no significant difference between experimental groups. Augmented area in the $ExFuse^{TM}$ Gel, $ExFuse^{TM}$ Putty groups were significantly greater than $SureOss^{TM}$ group at 2weeks(P < 0.05), however there was no significant difference at 8 weeks. Conclusions: All of $SureOss^{TM}$, $ExFuse^{TM}$ Gel, $ExFuse^{TM}$ Putty groups showed significant new bone formation and augmentation in the calvarial defect model.

치근단 수출 시 형성된 골결손 부위를 가진 치아의 발치 후 즉시 식립 임플란트에서 골 접촉률에 대한 흡수성 차폐막의 효과 (Effect of resorbable membrane on immediate placement of implant in extraction socket during periradicular surgery)

  • Purpose: The guided bone regeneration (GBR) technique is widely used in periradicular surgery. However, there is still some controversy regarding the effectiveness of GBR in promoting bone healing after periradicular surgery. The purpose of this study was to evaluate the resorbable membrane on the osteointegration of immediate implants in sites with periradicular lesion that had been removed by periradicular surgery. Materials and methods: Six roots of lower second premolars and 15 roots of lower third and fourth premolars of dogs were used as control and experimental teeth, respectively. Periradicular lesions were induced only in the experimental teeth. Twelve weeks later, the control and experimental teeth were extracted and implants were placed immediately. Periradicular lesions were removed with osteotomy, curettage and saline irrigation. Resorbable membranes were used in experimental group 1 but not in experimental group 2. After 12 week of healing period, the implants were clinically not mobile and showed no signs of infection. Data obtained by histomorphometric analysis were analyzed by Kruskal-Wallis test. Results: The control group showed a significantly higher bone to implant contact (BIC) ($74.14{\pm}16.18$) than experimental group 1 ($40.28{\pm}15.96$) and 2 ($48.70{\pm}17.75$)(p<0.05). However, there was no significant difference between experimental group 1 and 2. Conclusion: Although BIC in experimental groups were lower than in control group, immediate implant can be successfully placed at extraction socket with periradicular lesion and osseous defect. However, the use of resorbable membrane in bony defect created during periradicular surgery was questioned.

  • Deep circumflex iliac artery (DCIA) flap can be harvested as a composite free flap and is often used to adequately reconstruct wide mandibular defects. However, the harvesting of this DCIA flap can result in severe osseous defect of the donor site causing a morphologic defect in the iliac crest. To reconstruct this defect of the iliac donor site, several innovative techniques using bioactive ceramic spacers, autogenous rib bone, polylactic acid mesh, or titanium plates have been introduced. Nonetheless, these methods have not been widely used due to high cost, secondary donor site morbidity, difficulty of use, and postoperative dissatisfaction. We used two titanium plates to reconstruct the donor iliac site defect at the harvesting time of primary DCIA flap surgery in the 30-year old female with an ameloblastoma in the left mandible. Postoperatively, both iliac sites were relatively balanced and there were few complications. At the 2 years follow-up, there were no specific abnormal radiographic findings and the patient was very satisfied with her esthetic iliac contouring. In our report, we evaluate the effect of two titanium plates on the reconstruction of the iliac donor site in the aspects of esthetics and usefulness. This technique has many advantages, such as reduced cost, simplicity, decreased postoperative pain or discomfort, and improved bilateral balance of both anterior iliac crest contours, especially in young female patients.

두개골에 발생한 고립성 형질세포종 - 증례보고 - (Solitary Plasmacytoma of the Skull - A Case Report -)

  • Solitary plasmacytomas are rare and account for 5-10% of all plasma cell disorders. These tumors are categorized as solitary plasmacytomas of bone(osseous) or extramedullary plasmacytomas(non-osseous). About a half of solitary plasmacytomas of bone occur in the spine but rarely in the skull. We report a case of solitary plasmacytoma of the skull presented with a painless palpable left parietal calvarial mass in an otherwise asymptomatic 38- year-old man. Skull radiographs showed a large radiolucent lesion with well defined non-sclerotic margins. Computed tomograph scan demonstrated a markedly enhancing mass extending from the epidural to the subcutaneous space. The patient underwent surgery and tumor was completely excised. Pathological examination showed tumor to be a plasmacytoma synthesizing IgG. Postoperatively, the patient received radiotherapy. There was no evidence of systemic involvement on postoperative laboratory wokups. Our recommended treatment is a complete surgical excision combined with postoperative radiation therapy. The patient should be follwed carefully for more than 10 years because of either local recurrence or possible progression to multiple myeloma.

Intracranial Metaplastic Meningioma : Clinical and Radio-logical Characteristics of 11 Cases

  • Objective : Metaplastic meningioma is an extremely rare subtype of World Health Organization (WHO) grade I meningioma. It has distinctive histological subtypes according to its own mesenchymal components. Owing to its scarcity, clinical or radiological features of a metaplastic meningioma are poorly described. Methods : Between 2004 and 2018, we analyzed total 1814 cases surgically proven meningioma for 15 years. Among them, metaplastic meningioma was diagnosed in 11 cases. Magnetic resonance images were taken for all patients, and computed tomography scan was taken for 10 patients. Results : WHO grade I meningiomas were 1376 cases (75.9%), 354 cases (19.5%) in WHO grade II, and 84 cases (4.6%) in WHO grade III meningiomas. Metaplastic meningioma was 11 cases as 0.8% of WHO grade I meningioma and 0.6% of entire meningiomas for 15 years. Among the entire 11 metaplastic meningiomas, five tumors (45%) were diagnosed as a lipomatous subtype with rich fat components, four (36%) as an osseous subtype with extensive bone formation and two (18%) as a xanthomatous subtype. There was no cartilaginous subtype metaplastic meningioma in our study. Lipomatous and osseous metaplastic meningioma have peculiar radiological characteristics according to mesenchymal components. Conclusion : We investigated a rare metaplastic meningioma subtype based on our 15-year surgical experience with meningiomas. Further investigation will be necessary for the clear clarification of tumor nature of this rare tumor.

치은부에 발생한 거대세포육아종에 의한 골연화증 (TUMOR INDUCED OSTEOMALACIA : ASSOCIATED WITH GIANT CELL GRANULOMA ON THE GINGIVA)

  • 비내분비성 골종양이나 연조직종양에서 유래되는 구루병이나 골연화증은 매우 드문질환으로 1947 년 이래로 14 명의 환자만이 보고되고 있다. 이 질환의 본태는 아직 밝혀지지 않았으나 종양에서 생성하는 인산염뇨유발성 물질이 신세뇨관에 영향을 주고 저인산염혈증을 일으키며 골조직의 석회화에 장애를 일으키는 것으로 생각된다. 본 증례의 환자는 약 4 년 전부터 지속된 심한 전신동통과 근육위축, 현저한 신장의 감소를 주소로 내원한 41 세 남자로 상악우측 치은부의 종양에 의한 전신적 골연화증의 진단하에 종양의 외과적적출을 시행하였으며 술후 환자의 혈청과 뇨의 생화학적결과는 12 일 후에 정상으로 회복되었고 임상증상은 수술후 6 주에 회복되었으며 4 달후에 촬영한 치과 방사선상 놀랄만한 골재생과 새로 형성된 치조백선의 형성이 관찰되었다. 적출된 종물은 거대세포육아종의 조직소견을 보였으며 종물의 적출에 의하여 골연화증의 치유소견이 관찰 되었기에 종양에 의한 골연화증의 회유한 증례로 사료되어 이에 보고하는 바이다.

덱사매타존이 백서 발치와의 치유과정에서 미치는 영향 (The Effects of Dexamethasone on Healing of Tooth Extraction Socket of the Rat)

  • The purpose of this study was to evaluate the effect of dexamethasone on the healing aspect of gingiva and alveolar bone after extraction. Extracted socket of 24 Sprague-Dawley rat was used. To extract easily and minimize injury, ${\beta}-APN$ 0.2g/kg/day soluted in mineral water was administrated for 5 days before extraction in both group. Ampicillin 1.5ml/kg i.m.,q.d, was administered for preventing infection after teeth extraction in both group, and dexamethasone 0.2mg/kg/day was injected for 3 days in experimental group.3 rats on each day was sacrificed on 1, 3, 7, 15 days after extraction. Histologic examination and the activity of osteoclasts by tartrate resistant acid phosphatase was observed. The results were as follows : 1. The Overall healing pattern was similar with both the experimental and control group, but in experimental group osseous healing was delayed. 2. The activity of osteoclasts was increased to day 3 and then decreased after day 3 in the experimental group. In comparison to the control group, the experimental group showed increased appearance to day 7 and then decreased appearance following day. 3. Regarding to the change of osseous tissue, the activity of osteoblasts was shown at day 7,but osteoclastic activity of the experimental group was less than that of the control group. The osteoclastic activity was statistically significant between two groups except day 7(p<0.05, p<0.01). In conclusion, the effects of dexamethasone for healing of extraction socket were considered as limiting the activity of osteoclasts, and the healing of extraction socket was delayed.
