귀비탕(歸脾湯)으로 호전된 뇌경색을 동반한 하지정맥류 환자 치험 1례 (A Case of Varicose Veins of the Lower Limbs with Cerebral infarction Treated with Guibi-tang)

  • A varicose vein of the lower limbs is dilated, tortuous and elongated vein. Western medical treatment involves conservative management, sclerotherapy, and surgery. Currently, internal medicinal treatment is inadequate. Recently a patient was diagnosed with varicose veins of the lower limbs based on clinical symptoms. In this case, Guibi-tang was administered and clinical symptoms improved. This report suggests that oriental medicinal treatment is efficacious in treating varicose veins of the lower limbs. We think that further oriental medical studies on various surgical diseases are needed.

요추 척추관 협착증의 비수술적 치료에 대한 최근 국내·외 연구 동향 (Recent Domestic and International Trends on Non-Surgical Treatment of Lumbar Spinal Stenosis)

  • Objectives The present study examined the recent domestic and international trends of non-surgical treatment of lumbar spinal stenosis (LSS). Methods The studies on non-surgical treatment of LSS were investigated via searching Korean web databases and PubMed. As a result, 39 studies were analyzed according to the authors, the types of study, the relationship to surgical treatment and the method of treatment. Results The first authors' occupations were identified as 17 doctors, 12 Korean medicine doctors, six physiotherapists, three chiropractic doctors and one doctor of science. The most common type of study in Korea was the case report and most of the studies were retrospective. Overseas, there were many prospective or experimental studies including randomized controlled trials (RCT). In Korea, non-surgical treatment was performed independently of surgical treatment in most cases. Among non-surgical treatments, the most researched treatment was traditional Korean medicine (TKM), followed by injection therapy, exercise therapy, and physical therapy. Conclusions Through this review, we can determine the effectiveness of TKM and its research direction. TKM should be studied experimentally including RCT and it should be conducted not only on acupuncture, but also on other treatment methods such as acupotomy, chuna, and herbal medicine. It is also necessary to conduct studies on TKM before and after surgery to compare the effects of surgery and TKM together, as well the effects on non-surgical treatments.

  • Paralytic ileus is one of the common complications in stroke patients. In western medical treatment, surgery or conservative care is used for treating ileus. In this case, patient diagnosed as paralytic ileus had problems of constipation, or abdominal distension. Therefore the patient were treated with Crotonis Fructus(巴豆), and the patient showed improvement in X-ray findings and these symptoms. So we report this case.

  • 1. Objectives In general, arthropathy is treated with conservative method or only observed. Recently, I have effectively treated one patient with chronic arthropathy caused by loose body using constitutional method. So, I would like to report about this case. 2. Methods Consulting her Sungjeong(性情), Chehyungkisang(體形氣像) and QSCC II, I diagnosed her as Taeumin and treated her with Taeumjowi-tang(太陰調胃湯) plus jejo etc. 3. Results After treatment she felt her symtoms-pain, edema, stiffness of joint improved. 4. Conclusions This paper describes the process of management and changes of symptoms. I hope this paper useful to rehabilitatable treatment for arthropathy through Sasang Constitutional Medicine.

경근추나 치료 후 호전된 발음성 고관절 환자 치험례 (A Case Report on Snapping Hip Patient Treated by Chuna Manual Therapy for Meridian Sinew System)

  • 단순 방사선검사와 이학적 검사에서 특이 소견을 발견할 수 없는 발음성 고관절 환자에서 대전자의 움직임을 통해 고관절의 움직임 이상을 발견하고 경근추나를 시행하였다. 1. 환자분이 복와위에서 능동적 신전운동, 앙와위에서 능동적 굴곡운동을 하는 동안 환자의 대전자를 통해 고관절이 전내측 방향으로 움직임을 관찰하였다. 2. 고관절의 전내측 방향으로 움직임의 과도를 유발할 수 있는 대퇴근막장근과 슬괵근의 단축을 평가하고 경근이완기법을 적용하였다. 3. 슬곽근에 대항작용을 하고 고관절이 관골구내에서 안정성을 유지하기 위해 필요한 대둔근, 대퇴근막장근에 대항작용을 할 수 있는 후중둔근과 장요근의 약화를 관찰하고 경근강화기법을 시행하였다. 4. 13회의 치료 후 발음성고관절의 주증상인 통증과 탄발음이 거의 소실되는 양호한 결과를 얻었다. 5. 고관절의 움직임 이상이 발견된 발음성 고관절 환자에서 해당 경근의 단축과 약화가 발견될 경우 경근추나수기법을 이용해 볼 수 있을 것으로 판단된다.

베네딕트 증후군 환자의 동안신경마비 치험 1례 (A Case of the Oculomotor Nerve Palsy in Benedikt's Syndrome Patient)

  • The purpose of this case study is to present a case of oculolmotor nerve palsy due to midbrain infarction improved by acupuncture and herb medicine. Oculomotor nerve palsy is a disorder associated with dysfunction in the third cranial nerve, which causes eye movement disorder, diplopia and ptosis. The patient, who was diagnosed with Benedikt's Syndrome(Red Nucleus Syndrome), was given oriental medical treatment. Benetikt's Syndrome has the various symptoms of weakness on one side(contralateral) and eye movement disorder, ptosis, diplopia(ipsilateral) etc., but research on Benetikt's Syndrome or midbrain infarction is scant. Results of this study suggest a role for conservative therapy with herb medicine and acupuncture to treat oculomotor nerve palsy(eye movement disorder, ptosis, diplopia, etc., ipsilateral) and right motor weakness(contralateral) due to midbrain infarction. Further research into oriental medical treatment for such disorders will be forthcoming.

만성통증 치료에서 Deep Dry Needling의 모델들과 중재적 근육 및 신경자극 요법 (The Deep Dry Needling Techniques, and Interventional Muscle & Nerve Stimulation (IMS) for the Treatment of Chronic Pain)

  • Chronic pain can cause disability, mild to severe suffering and high medical costs. Some unfortunate patient do not improve despite administering conservative treatment and then the various interventional therapies, including oriental medical treatment and/or surgery, and they find themselves in search of a more effective pain relief. Deep dry needling is one of the newer treatment modalities for these patients. The last 10 years have seen a lot of progress in understanding the neural pathways and the type and extent of tissue involvement during chronic pain. This in turn has stimulated the development of new treatment techniques, and deep dry needling is one of them. So, the authors of this paper discuss the individual theories, the characteristics and future directions of several deep dry needling techniques, and we examine the new dry needling technique that has been recently developed in Korea.

망막분지정맥폐쇄로 유발된 황반부종 치험 1례 (A Case of Macular Edema Resulted from Branch Retinal Vein Occlusion)

  • Objective : This study was performed to prove the effect of the oriental medical treatments like acupuncture, moxibustion, herbal medicine on macular edema resulted from branch retinal vein occlusion which could lead to terrible result like permanent visual loss at worst. Methods : The patient was treated 62 times, from July 15th, 2009 through May 12th, 2010. We used fundus photography and Dr. Hann's standard test chart for 5m to examinate the patient's eyesight. We used acupuncture, electropuncture, moxibustion and herbal medicine to treat the patient. We considered visual field, scotoma, metamorphopsia and vitreous floater to measure the effect of treatment. Results : After about 10 months of treatment, the eyesight of Rt eye was improved from 0.8 to 0.9, Lt. eye from zero to 0.3. The visual field turbidity were absolutely cleared. The scotomas in Lt. eye were reduced from 5 to 2. The metamorphopsia were improved. The vitreous floaters in Rt. eye were decreased from 5~6 to 1~2. Conclusion : These results show that the oriental medical treatment can be conservative measure to the macular edema resulted from branch retinal vein occlusion.

요추관협착증의 한의학적 보존적 치료에 대한 임상적 고찰 (The Clinical Study of Lumbar Spinal Stenosis in Oriental Medical Hospital)

  • This clinical study was carried out 15 cases with lumbar spinal stenosis patients, who had been admitted and diagnosed by Computed Tomography from June, 1999 to May, 2000, in the department of acupuncture and moxibustion. Hanseo University Oriental Medical Nospital. The results obtained from this study were as follows; 1. We investigated 10 female and 5 male patients. The age distribution was from early 20's to 70's, among which 30's and 50's were most common. 2. The most common duration of symptom was less than a year(46.7%). 3. Regarding to the number of the involved levels, one was 12 cases(80%), two was 3 cases(20.0%). In the one level cases, L4~5 was the most common involved site. 4. Regarding to the clinical symptoms, low back pain was the most common, followed by lower extremity radiating pain, intermittent claudication, lower extremity paresthesia. 5. In orthopaedic examinations, positive sign in Milgram test was 80.0%, positive sign in straight leg raising test was 40.0%, and positive sign in Braggard test was 33.3%. Inneurologic examinations, sensory loss was seen in 60.0%, motor weakness and decreased deep tendon reflex were 26.7% in each. 6. According to clinical symptom scale proposed by Chae(1989), grade I1I was the most common (53.3%), followed by grade II, grade IV, grade I . 7. According to treatment outcome scale proposed by Chae(1989), Good was the most common (73.3%), followed by Excelleat, Fair and Poor.

뇌출혈로 항응고제 사용이 불가능한 심부 정맥혈전증에 대한 혈부축어탕 치험례 (The Effect of Hyulbuchuko-tang on a Case with Deep Vein Thrombosis (DVT) and Intracranial Hemorrhage (ICH))

  • Objectives: Deep vein thrombosis (DVT) is a common complication among stroke patients. The implication of DVT progressing into a fatal pulmonary embolism is one of the main reasons treatment cannot be delayed. However, when there is a contradiction for anticoagulants, such intracranial hemorrhage (ICH), it is difficult to determine the course of treatment. Our team reports a case with both acute DVT and ICH who improved with herbal medicine Hyulbuchuko-tang. Methods : A patient with a variety of thrombosis risk factors (atrial fibrillation, DVT, Cb-inf with intracranial hemorrhage due to thrombolytic complications) showed classic symptoms of DVT (pain, edema, discoloration), disorientation and chest discomfort. The patient was administered Hyulbuchuko-tang three times a day for 24 days without any anticoagulants. Conservative therapy including elastic stocking and leg elevation was co-administered. Laboratory tests and extremity vascular Doppler sonography were carried out 3 times during the treatment period. Results : After our treatment period, both popliteal vein DVT and calf vein DVT were not discovered by sonography, and thrombosis derived factors (eg. D-dimer, fibrinogen) decreased. There was no sign of edema or discoloration after treatment, and the patient no longer complained of leg pain, disorientation or chest discomfort. Conclusion : From these results, we suggest that there is a positive effect of Hyulbuchuko-tang on DVT. Hyulbuchuko-tang should be considered as a treatment option when western medical procedures are unavailable.
