장기기증 희망자의 의사결정 경험 (The Experience of Decision Making to Donate Organs)

  • Purpose: The aim of this study was to identify the process of experience toward decision making to donate organs. Method: Eleven subjects who registered at the Association for Organ Sharing in Korea were recruited. Using in-depth interviews and tape-recordings, data was collected from Jun. 2001 to Feb. 2002 and the contents of these were analyzed by Glaser's grounded theory analysis method. Result: Basic Social-Psychological Problem(BSPP) of subjects were 'attachment to body' and the core theme, that is Basic Socialization Progress(BSP) were discovered to 'find out what is the meaning of life'. Also it consisted of four steps: 1st 'hesitate', 2nd 'look into self', 3rd 'realize the tie up to self' and last 're-find the meaning of life'. Conclusion: These findings indicate the Korean's Basic Socialization Progress of decision making regarding organ donation, so possible development of promoting decision making is necessary strategies for people who is having his/her potential of organ donation in mind.

간호대학생의 죽음태도, 영적건강 및 장기기증 인식 간의 관련성에 대한 융합적 연구 (A Convergence Study on the relationships among Attitude toward Death, Spiritual Well-being and Awareness about Organ Donation of Nursing Students)

  • 본 연구는 간호대학생의 죽음에 대한 태도, 영적건강 및 장기기증에 대한 인식 간의 관련성을 파악하기 위한 융합연구이다. 대상은 일 대학 간호학과 학생 214명이었다. 자료는 구조화된 설문지를 이용하여 수집하였고, PASW 20.0 프로그램을 이용하여 분석하였다. 연구 결과 장기기증에 대한 인식은 죽음 태도와 부적 상관관계가 있었고, 영적건강과는 정적 상관관계가 있었다. 장기기증 인식에 영향을 미치는 예측요인은 죽음태도(${\beta}=-.159$)와 영적건강(${\beta}=.132$), 뇌사자의 조직기증에 대한 생각(${\beta}=.314$), 전공만족도(${\beta}=.134$)로 나타났으며, 모형 설명력은 20.5%이었다. 본 연구결과를 토대로 장기기증에 대한 긍정적 인식 함양을 통한 장기 이식률 향상을 위해 간호대학생의 죽음에 대한 태도와 영적건강 상태를 향상시킬 수 있는 교육 프로그램과 간호사 대상의 후속연구가 필요함을 확인하였다.

해외 주요 국가와 비교 고찰을 통한 인체조직기증 활성화 방안 (A Study on the Activation Measures of Human Tissue Donation in Korea through Comparing with Foreign Countries)

  • 현대 사회는 장기 및 인체조직기증에 대한 요구도가 높아지고 있으나, 우리나라에서 인체조직기증에 대하여 인식도와 기증률이 낮아 기증 활성화를 위한 현실적인 방안에 대한 연구가 필요할 것으로 사료된다. 본 연구는 국내외 관련 문헌고찰 및 나라별 이식 등록기구 홈페이지 자료를 참조하여, 인체조직기증과 관련한 국내 법률과 운영 제도 현황을 분석하였으며, 해외 주요 국가와 비교 고찰하였다. 본 연구를 통하여 국내 인체조직기증 활성화 방안으로 법률적 측면에서 이원화된 장기와 조직이식에 관한 법체계의 통합화, 제도 운영적 측면에서 과도한 압력이 없는 국가의 장기기증에 대한 추정적 동의 방식인 Opt-out 제도의 활성화와 사회 문화적 측면에서 대국민 홍보 활성화를 제시하였다. 그러나 추정적 동의 방식은 다양한 사회적 공론화 단계가 필요하며, 거부 의사를 위한 등록체계 운영사항에 대해 국내 실정에 맞는 이해가 있어야 할 것이다. 결론적으로 외국에 비하여 낮은 장기 및 인체조직기증을 활성화하기 위한 법적 제도 정비가 요구되며, 기증에 대한 긍정적 인식 전환, 대중 홍보 및 사후 기증자와 가족에 대한 지원 등이 필요할 것으로 사료된다.

장기기증에 대한 대학생들의 지식수준과 태도 (Level of Knowledge and Attitude about Organ Donation in An University Students)

  • Purpose. The purpose of this study was to investigate level of knowledge and attitude about organ donation in university and provide basic information on correct education and promotion. Method. The sample is consisted of 90 the students related to health and 90 general college students ; total 180 students in J city. The questionnaires were used to obtain the results. Data was collected from November 6th to 8th, 2013. For data analysis, the SPSS for window 12.0 K program was utilized to get descriptive statistics including the percentage, average, standard deviation and t-test. Results. Comparing the results between a series of health students and general college students, a series of health students showed high level of more knowledge and more positive attitude about organ donation than general college students. A series of health showed significant correlation but general college didn't show significant correlation. Conclusion. To educate correct knowledge draw positive attitude about organ donation is needed to improve the level of Knowledge and Attitude about organ donation.

호스피스 자원봉사자의 존엄사에 대한 태도요인 (Attitudes of Hospice Volunteers towards Death with Dignity)

  • The purpose of this study was analyzed the factors of influencing toward attitude to death with dignity to hospice volunteers. The data was collected for 21 days from 14 March to 3 April 2010. Among a total of 220 cases of the questionaries, only 195 cases were used. To data were analyzed by factor analysis, independent t-test, one-way anova and logistic regression using PASW statistics 18.0. The results were as follows; The attitudes towards death with dignity according to general characteristics was high in those with will to agreed to the passive euthanasia than those opposite to the attitude factors, namely, acceptive, the right to decide, negative, and dereliction of duty attitude factors. Significant variables for effects of death with dignity were gender, acceptive attitude factor and dereliction of duty attitude factors. Given that main provider of human organs is the brain-dead and we don't have enough organ donation, death with dignity should be linked with activating policy of organ donation, while solving donation shortage problem. This way, constructing social implementation and sharing consciousness on organ donation, would be diluting the bio-ethic controversies.

장기이식 코디네이터의 직무분석 (Task Performance and Analysis of Organ Transplantation Coordinators in Korea)

  • Purpose: Along the process of organ transplantation, coordinators perform complex professional roles, playing as a key person among recipients, donors, family members and medical staffs. The purpose was to analyze the tasks of organ transplantation coordinators using DACUM method and to establish the basis for standardizing them in accordance with relevant laws and systems. Methods: Participants were consisted of 78 transplantation coordinators working at the medical centers in Korea. The questionnaire was administered to analyze the criticality, difficulty and frequency of task elements. Results: The job of organ transplantation was classified into five duties, 13 tasks, and 84 task elements. The five duties were recipient management, donor management, organ donation activation management, organ transplantation administration, and professional capability development. On the four-point scale: donor management was the duty with the highest criticality (3.68), organ donation activation management was the duty of highest difficulty (2.96), and recipient management was the duty of the highest frequency (3.32). Conclusion: This study will be useful for developing an educational program and as a reference of nurse practitioner qualifying examination. It is necessary to develop a comprehensive educational program for transplantation coordinators in order to support them to take their complex roles successfully.

병원간호사의 간호윤리 가치관 및 뇌사자 장기기증에 대한 태도에 관한 연구 (Attitude on Nursing Ethical Values and on Brain Death Organ Donation of Hospital Nurses)

  • 뇌사자 장기기증에 대한 올바른 인식과 활성화를 위해서는 간호사들에게 확고한 가치관 확립과 체계적인 뇌사자 장기기증 및 장기이식에 대한 간호교육이 필요하다. 이를 위한 기초자료를 제공하기 위하여 222명의 병원간호사를 대상으로 2011년 2월 14일부터 17일간 구조화된 설문지를 이용하여 간호윤리 가치관 및 뇌사자 장기기증에 대한 태도를 조사하였다. 수집된 자료는 SPSS Program을 사용하여 분석하였으며 연구 결과는 다음과 같다. 첫째, 대상자의 간호윤리 가치관 점수는 평균 3.22${\pm}$0.38이었으며, 뇌사자 장기기증에 대한 태도 점수는 평균 3.34${\pm}$0.48이었다. 둘째, 일반적 특성에 따른 간호윤리 가치관은 연령, 종교, 경력, 간호직에 대한 생각에서 뇌사자 장기기증에 대한 태도는 결혼 여부, 간호직에 대한 생각, 뇌사시 장기기증 희망 여부에서 유의한 차이가 있었다. 셋째, 간호윤리 가치관과 뇌사자 장기기증에 대한 태도 간에는 상관관계(r=.160)가 거의 없는 것으로 나타났다.

간호학생의 생명의료윤리의식 영향 요인 (Affecting Factors of the Awareness of Biomedical Ethics in Nursing Students)

  • Purpose: This study was conducted to examine awareness of biomedical ethics, and to identify affecting factors of the awareness of biomedical ethics in nursing students. Methods: The subjects consisted of 266 nursing students their third and fourth years of study. The data were collected from October to December, 2015 by self-report using questionnaires. Data analysis was performed using SPSS/WIN 18.0, descriptive statistics, t-test, ANOVA, $Scheff{\acute{e}}$ test, Pearson correlation coefficient, and multiple regression analysis. Results: The mean score of the awareness of biomedical ethics was $2.81{\pm}0.22$, perception of death was $3.15{\pm}0.36$, and knowledge of brain death, organ donation, and organ transplant was $12.12{\pm}3.02$. The prediction factors of awareness of biomedical ethics were gender (${\beta}=.29$, p<.001), participation in religious activity (${\beta}=.23$, p=.015), and perception of death (${\beta}=.20$, p=.016). The explanation power was 17.1%. Conclusion: These results showed that education about biomedical ethics is necessary for nursing students, and the development of biomedical ethics educational programs should reflect affecting factors.

생체 간이식 공여자의 불확실성과 간 공여 영향 요인 (Uncertainty and Factors Affecting Organ Donation in Living Liver Donors)

  • As the patients who need to undergo liver transplant operation continues to grow. the number of livers that are donated can not keep pace with the demand. With the development of surgery skills, the necessity for operations from living donors is increasing. Nevertheless, satisfactory research has been conducted on the factors which generally affect the living donors. In this article. therefore. researchers focused on the factors which generally affect the donating liver donor in order to design a plan for recommending liver donation from living donors. The subjects were 91 living liver donors in C university hospital from October 1. 2000 to December 31. 2003. The results on the uncertainty of living donor, by test sheet. were analyzed with SAS program. The final results were as follows: 1. The uncertainty of the living donors was 51.54 marks per full credit 100. 2. The factor with the greatest effect on donation was the possibility of survival of the donor, followed by the admission period. marriage status and age. In recommending the living donation, the rate of donor survival after the operation was 5.2 times higher than death, 5.2 times higher when the admission period was under 20 days. 5.0 times higher when married. and 27.3 times higher when the family-related donation was very active at the age of 20s than in the 50s. These results suggest that all medical staffs should care for living donors with more interest and activity to give them the least complaints in admission and the lowest possibilities for complication. To enhance the survival rate and improve the surgical success rate. on-going monitoring should include regular health-checks. and continual efforts and education should be made to care for the health condition of the living donors after donation.

생체 신장 공여자의 공여 후 적응 경험 (Adaptation Experience of Living Kidney Donors after Donation)

  • Purpose: This study was done to explore adaptation experience of living kidney donors after donation. Specific aims were to identify challenges donors face in the process of adaptation following surgery and how they interact with recipients and other people. Methods: Grounded theory methodology was utilized. Participants were 13 living kidney donors at six months or more after donation. Data were collected by in-depth interviews with individual participants. Data were analyzed using constants comparative method with theoretical saturation. Results: A core category emerged as 'keeping the fences of my family in spite of vulnerability'. The adaptation process after donation was manifested in four phases: exploration, balance, maintenance, and acclimatization. Phenomenon was perception of vulnerability. Strategies to manage the vulnerability were assessing changes of body awareness, tailoring regimen to one's own body condition, coping with health problems, keeping restoration of health, and ruminating on the meaning of one's kidney donation. Consequences were reestablishing family well-being, realizing the values of one's kidney donation, and living with uncertainty. Conclusion: Findings of the study indicate that there is a need for health professionals to understand the vulnerability of living kidney donors and help their family system maintain a healthy and productive life. The results of this study can be used to develop phase-specific, patient-centered, and tailored interventions for living kidney donors.
