THE EFFECT OF LINGUAL FRENECTOMY ON PHONATION & TONGUE MOVEMENT (설소대성형술이 발음 및 혀의 운동에 미치는 영향에 관한 연구)

  • This sutdy aimed at examining the effect of lingual frenectomy on phonation & tongue movement. Almost the patient visiting to department of oral & maxillofacial surgery for the treatment of tongue tie always complain the speech problem. Many operation was performed according to this problem. But the objective evaluation of the speech change have been deficient. The experimental group was 25 adult males. Fourteen Korean consonants & after Korean vowels was combined and seventy sound was made for speech analysis. Before & after lingual frenectomy, the speech of the above mentioned group was recorded and then analysed by the Speech Workstation computer software. And before & after operation, the lingual frenum & tongue protrusion amount vas measured. The results were as follows : 1. The pre-operative length of lingual frenum was inverse proportion with the pre-operative length of the protrusive tongue. 2. The average difference between pre & post-operative length of the protrusive tongue was about 23 mm. 3. In the comparison of consonant continuing time change, fricative consonant(r, s, h) was increased post-operatively. 4. In the comparison of the vowel frequency formant change, the "i"and "u" sound vas reliably changed. 5. There was no reliable speech changes on the other sounds.

Locoregional Recurrence of a Tongue Cancer Patient with 10 Year Follow-up (10년 관찰기간 중 다수의 국소 재발을 보인 설암 환자)

  • Locoregional recurrence of tongue cancer is higher than that of other sites of the oral cavity. Locoregional control has shown improvement over the past 20 years, however, a high rate of recurrence and second primary tumor occurrence is still frequently reported. Leukoplakia is a clinical term, which describes a whitish lesion of the oral cavity. Clinicopathologic features may range from hyperkeratosis to malignancy. Because of its diverse pathologic characteristics, management of this lesion for diagnosis, treatment planning, establishment of a clear surgical margin, and periodic follow-up is difficult. We report on a case of successfully treated tongue cancer which developed from leukoplakia over 10 years. Periodic follow-up strategy and surgical planning are most important to management of locoregional recurrence.

Acupuncture Application Case Report for Symptom Relief of Oral Leukoplakia (구강 백반증의 증상 완화에 대한 침치료 적용 사례)

  • Oral leukoplakia is the most common premalignant lesion of the oral mucosa. This case report presents the effect of acupuncture on Oral leukoplakia. The 67 years old male patient who has suffered for 2 years was treated with Right side Saam acupucture Damjeonggyeok(BL66, GB43, LI1, GB44) without other treatment. Acupuncture was performed for 15 minutes. The effect of acupuncture was evaluated as photographic change and visual analog scale(VAS) of subjective symptoms in oral cavity. After 87th acupunture treatment during 226 days, Leukoplakias were decreased at the right side of ventral tongue and the left border of the tongue and the lower lip. Furthermore, ulcer, erosion at the left border of the tongue were disappeared and VAS of Burning pain in the tongue decreased from 8 to 2. Although acupuncture is unusual treatment for Oral Leukoplakia, this case report suggested the effectiveness of using Damjeonggyeok on Oral Leukoplakia according to Saam acupuncture method.

Oral hygiene form and the subjective health analysis of the elderly (노인의 구강위생행태와 주관적 구강건강 형태에 대한 분석)

  • Purpose: This study was conducted to prepare the fundamental data on oral hygiene of the elderly and to assess the behavior of oral hygiene, subjective oral health, oral health problem among the elderly, and to contribute to successful enhancement of life in their declining years. Methods: The author surveyed the behavior of oral hygiene, subjective oral health, oral health problem to 192 elderly at a health center of Busan using structured self- administered questionnaire from Nov. 2nd 2009 to Feb. 10th 2010. Collected data were analysed by SPSS WIN 18.0 statistical program for frequency, percentage production, ${\chi}^2$ (Chi-square) verification, t-test, One-way ANOVA, Logistic Regression. Results: The distribution of total subjects by oral health behaviors were 56.8% for "have ever trained how to tooth brushing", 76.6% for "correctly performed tooth brushing by himself ", 71.8% for "tooth brushing a day is two times and less", 87.5% for "it doesn't tooth brushing in meals and after" and 53.6% for "it doesn't use the facility for oral health", respectively. The distribution of total subjects by the state of perceived oral health were 51.0% for "feel the oral discomfort", 51.6% for "feel the comfortable chewing on starchy food", respectively. The mean point of perceived oral health by general characteristics of total subjects were statistically significant difference in sex, type of medical insurance, smoke, exercise, and chronic disease. The mean point of perceived oral health by oral health behaviors of total subjects were statistically significant difference in educational experience on tooth brushing, recognition for tooth brushing by himself, tooth brushing a day, and tongue cleaning. The risk factors of perceived oral health were type of medical insurance and chronic disease in general characteristics, tooth brushing in meals and after and tooth brushing a day, and tongue cleaning in oral health behaviors. Conclusion: The author recommend to the prevention of chronic disease, carry out the tooth brushing in meals and after, three times and over tooth brushing a day and tongue cleaning for oral health. and the author consider that it need to prepare the active countermeasure to oral health such as reeducation for oral health and supply to the tongue cleaner.

  • Benign symmetric lipomatosis(BSL) is a lesion characterized by symmetrical and diffuse growth of adipose tissue. It is commonly found in the posterior neck or upper trunk, manifesting itself as an entity known as Madelung disease or Lanoid-Bensaude syndrome. Lipomatosis affecting the tongue is very unusual. The etiology of BSL is unknown, but It can be associated with alcoholism, liver dysfunction, hypothyroidism. Because of its diffuse histologic feature, complete resection of the lesion is often difficult. Seventy one years old male patient, whose chief complaint was multiple nodules of tongue was referred to our department. The lesion was diagnosed histopathologically as BSL and subsequently removed surgically. So we report this rare case of BSL with literature review.

Relationship between oral health status and subjective oral malodor in public health students (보건계열 학생들의 구강건강상태와 주관적인 구취자각과의 관련성)

  • Objectives: The purpose of this study is to analyze relationship between oral health status and subjective oral malodor awareness of public health students. Furthermore, this study aimed to recognize the importance of prevention and treatment of internal and external factors and to contribute to the correct oral health management behavior. Methods: The study subjects were 500 students from 5 health departments of 2 universities located in Jeollanamdo who participated in self-administered survey from September 1 to 15, 2014. For statistical analysis, SPSS 21.0 for Windows was used. Descriptive analysis and a Chi-square test were conducted to investigate the effects of general characteristics, health behavior, oral health behavior, and oral health status on subjective oral malodor awareness. Finally, to investigate the relationship between oral health status and subjective oral malodor awareness logistic regression analysis was performed. Results: Subjective oral malodor awareness was significantly higher in the group requiring dental care with the score of 1.63 (95% CI 1.00-2.65) compared to the group not requiring dental care. Subjective oral malodor awareness was significantly higher in the severe coated tongue group with the score of 5.31 (95% CI 1.45-19.40) and significantly higher in the moderate coated tongue group with the score of 2.56 (95% CI 1.61-4.08). Subjective oral malodor awareness was significantly higher in the often mouth breathing group with the score of 2.13 (95% CI 1.02-4.47) and significantly higher in the sometimes mouth breathing group with the score of 2.66 (95% CI 1.65-4.29). Conclusions: In order to prevent oral malodor, it is emphasized that regular dental checkups, proper brushing after the meal, and brushing of the tongue are necessary to remove the coated tongue. In addition, the use of supplementary oral care products is considered to be a meaningful oral health behavior.

  • Introduction. In patients with mandibular prognathism, Bilateral Sagittal Split Ramus Osteotomy(BSSRO) combined with orthodontic treatment reduces oral volume and influences tongue and other surrounding tissues. Purpose of this study was to analyze post-operative tongue position and airway dimension, as well as mandibular changes in vertical, horizontal, and angular dimensions. Materials and methods. Height of dorsum of tongue, width of airway, gonial angle and lower facial height of mandibular prognathic patients who visited Kangdong Sacred Heart Hospital from Jan. 2001 to Dec. 2006 were anaylzed via pre-operative and post-operative cephalograms. T-test was used to compare pre-operative and post-operative measurements. Also, correlations among pre-operative measurements of the patients were analyzed. Results and conclusion. A significant correlation was shown between ANS-Xi-PM area and location of dorsum of tongue in pre-operative patients. A significant superior movement of tongue and decrease of airway width was observed in post-operative patients. Also the upper gonial angle decreased significantly.

Characteristics of Oral Tongue and Base of the Tongue Cancer: A Hospital Cancer Registry Based Analysis

  • Background: Tongue cancer is one of the leading sites of cancer in our population. Aim: To evaluate the socio-demographic profiles and stages at diagnosis of oral tongue (OT) and base of tongue (BT) cancers, and identify any possible variations in characteristics. Materials and Methods: A retrospective analysis was conducted on tongue cancer cases, divided into OT and BT, registered at the hospital cancer registry of North-East India during January 2010 to May 2013. Cases were analyzed for age, gender, residential status and different levels of education for patients, the stage at diagnosis and presence of distant metastasis. Results: A total of 1,113 cases of tongue cancers were registered, 846(76.1%) of BT and 267(23.9%) of OT. While 33.9% of BT cancer patients were above 65 years of age, the figure for OT cancers was 18.4%, stages III and IV accounting for 90.8% and 77%, respectively. The relative risk for distant metastasis in OT cancers was 3.3 (95% CI 1.08-10.1, p=0.03). Conclusions: In the subsites of tongue cancers in our population, the majority arose from the base of tongue, these tending to occur in older individuals and presenting at late stage.

Size and Retention of Tongue Bulb for Tongue Retaining Device (혀 유지구의 크기와 유지력 평가)

  • In several treatment modalities for snoring and obstructive sleep apnea (OSA), oral appliances mainly including mandibular advancement appliance (MAA) and tongue retaining device (TRD) are recognized as a non-invasive, reversible alternative with favorable results. Tongue bulb is a major component of TRD which prevents the tongue from approaching the posterior wall of the pharynx and can be combined with MAA. Determination of tongue bulb size for the patient is important for therapeutic effect, but frequently needs time-consuming work. For effective fabrication and standardization of tongue bulbs, this study aimed to categorize tongue bulb size for healthy young men and to examine its relation with maximum retention force and with physical parameters including tongue-related variables. 36 non-snoring, asymptomatic young men with normal occlusion were voluntarily participated in this study (mean age: $24.47{\pm}2.58$ years). Experimental procedures consisted of prefabrication of tongue bulb set (20 types with a width of 27-36mm and thickness of 8 and 10 mm), determination of tongue bulb size and the maximum retention force for each subject, and measurement of physical parameters including body mass index (BMI), neck circumference and width, thickness and length of tongue. This study showed that there was significant difference of retention force among the bulb size-related groups both in upright and supine position (p<0.05) and that retention force increased with bulb size. Correlation of tongue bulb size with physical parameters was not clearly verified and there was no significant difference in retention force between upright and supine positions. Based on our results, it can be suggested that retention force relates with tongue bulb size, ultimately with tongue volume. A further study needs to be performed in the patients with snoring and OSA.

The effect of a full mouth disinfection on oral malodor in chronic periodontitis patients (Full mouth disinfection이 치주질환자에서의 구취에 미치는 영향)

  • Halitosis, defined as an unpleasant oral odor, is a commonly experienced condition with a variety of etiological factors and may cause a significant social or psychological handicap to those suffering from it, In most cases, halitosis originates within the oral cavity itself and patients with periodontal disease often suffer from oral malodor, The most common cause of this disease is related to microbiota which reside on the tongue and in the periodontal pocket, This study was undertaken to examine the effect of full mouth disinfection including tongue scraping on oral malodor in a group of patients with chronic periodontitis, The relationship between halitosis and oral health status was also investigated, The volatile sulfur compounds (VSC) scores were significantly correlated with Plaque Index, Bleeding Index, pocket depth, and tongue coating score, The organoleptic ratings were significantly associated with Plaque Index, Bleeding Index. and tongue coating score, The VSC scores and organoleptic ratings correlated strongly with each other. Full mouth disinfection resulted in a significant reduction in the VSC scores. organoleptic ratings, and self-perception of malodor up to 12 weeks, This study indicates that in patients with chronic periodontitis. a full mouth disinfection including tongue scraping has a significant effect in the treatment of oral malodor.
