  • The loss of mandibular continuity due to trauma, neoplasm, or infection results in major esthetic and biologic compromise. The reconstruction of the mandibular bone defect still poses a challenge to oral and maxillofacial surgeons. There have been a number of variety graft materials. Among them, free block bone graft with rigid fixation has been widely used. However, cases using free block bone grafts may lead to a marked invasion of the donor site, mal-union, and absorption of the block bone. In this respect, particulate cortical bone using a titanium mesh tray can be an effective alternative option in order to achieve a proper bone contour and good oral rehabilitation. We have developed an intraoral approach for the mandibular reconstruction method using a titanium mesh tray with autogenous particulate cortical bone graft.

  • One case of an unusual form of carcinoma involving the submucosal gland and duct of tongue was reported and reviewed. According to Gerughty et al(1968) four distinct component parts were classified : ductal carcinoma in situ(involvement of the ductal epithelium by in situ carcinomatous changes), squamous cell carcinoma, and a mixed carcinoma(combination of glandular and squamous characteristics and occasionally consisted of large nests composed of "glassy" cell). This tumor was fond to be extremely aggressive and highly malignant. The histopathologic features and the clinical behavior of this tumor were sufficiently distinctive to warrant the designation adenosquamous carcinoma : exhibit concomitant glandular and squamous neoplasm. The mode of therapy was evaluated and the treatment of choice appears to be radical surgery. So, we has done the radical neck dissection and partial glossectomy. However, the limited number of cases indicated that collection and subsequent analysis of additional cases must be performed before any definitive conclusion can be drawn.

  • Undifferentiated pleomorphic sarcoma (UPS) in the mandible region is a rare malignant neoplasm. We encountered the case of a patient who had been diagnosed with deep benign fibrous histiocytoma (DBFH) prior to surgery, but the final biopsy revealed the presence of tumor tissues in the resected margin, which indicated a UPS. Therefore, radiotherapy was conducted post-operatively, but the tumor recurred approximately 3 years later and a second surgery was performed. The difference between the initial and final diagnoses was considered to be due to the characteristics of the tumor, as it involved both benign and malignant components. DBFH and UPS can occur in young women. DBFH can transform to a malignancy and UPS may also appear as a benign tumor during early biopsy. Hence, attention should be paid to diagnosis and treatment methods, and ongoing close observation is required.

Clear cell odontogenic carcinoma: a mini review

  • Clear cell odontogenic carcinoma (CCOC), a very rare neoplasm located mostly in the mandible, has been regarded as a benign tumor. However, due to the accumulation of case reports, CCOC has been reclassified as a malignant entity by the World Health Organization. Patients with CCOC present with regional swelling and periodontal indications with variable pain, often remaining misdiagnosed for a long period. CCOC has slow growth but aggressive behavior, requiring radical resection. Histologic analysis revealed the monophasic, biphasic, and ameloblastic types of CCOC with clear cells and a mixed combination of polygonal and palisading cells. At the molecular level, CCOC shows the expression of cytokeratin and epithelial membrane antigen, along with markers that assign CCOC to the sarcoma family. At the genetic level, Ewing sarcoma breakpoint region 1-activating transcription factor 1 fusion is regarded as the key feature for identification. Nevertheless, the scarcity of cases and dependence on histological data delay the development of an efficient therapy. Regarding the high recurrence rate and the potential of distant metastasis, further characterization of CCOC is necessary for an early and accurate diagnosis.

Odontogenic myxoma: a case report with recent image modalities

  • The odontogenic myxoma is an benign, slow growing neoplasm which is of ectomesenchymal origin. This neoplasm occurs almost exclusively in the jaw bones and comprises 0.2% to 17.7% of odontogenic tumors. The odontogenic myxoma may show a wide spectrum of radiographic appearances, unilocular, multilocular radiolucency and a distinct or diffuse border, making the differential diagnosis difficult. We present a case of the odontogenic myxoma in the maxilla with conventional and recent image modalities. Occlusal film revealed a medially extended multilocular lesion with intralesional fine and straight trabeculations from the scalloped margin and buccal expansion and thinning of cortical bone. Computed tomogram revealed lesion showed equivalent density to the muscles in the left maxillary sinus with partial cortical discontinuity of medial wall and the tennis-racket pattern with internal straight trabeculations. MRI revealed intermediate signal intensity on Tl weighted image and high signal intensity on T2 weighted image. In Gd enhanced MR image, the peripheral portions of the lesion were enhanced.

Effects of Oral Care Protocol on Oral Mucositis in Hematologic Malignancy Patients Receiving Chemotherapy (구강관리 프로토콜 적용이 항암화학요법을 받는 혈액암 환자의 구내염에 미치는 영향)

  • Purpose: This study was done to identify effects of oral care protocol on oral mucositis and oral care performance in hematologic malignancy patients receiving chemotherapy. Methods: The design of this study was a nonequivalent control group pretest-posttest design. Both groups were patients diagnosed with hematologic malignancies who were receiving chemotherapy-each group had 20 patients. In the experimental group, patients were given intensive education on oral care based on the oral care protocol, whereas in the controlled group, each patient was given an educational brochure. Before chemotherapy, and 3 days, 7 days, and 14 days after chemotherapy, oral mucositis status of two groups were assessed using the guide to physical assessment of the oral cavity. Oral care performance was examined before chemotherapy and 14 days later. Results: The experimental group with the oral care protocol showed a significant difference (F=18.15, p<.001) in the oral mucositis status, and also in oral care performance (t=-10.33, p<.001). Conclusion: Findings indicate that the application of the oral care protocol is an effective tool for lowering the occurrence of oral mucositis and enhancing oral care performance in hematologic malignancy patients receiving chemotherapy.

  • Adenoid cystic carcinoma is a distinctive neoplasm of the salivary gland and is the most common malignant tumor of submandibular gland. Although adenoid cystic carcinoma grows slowly, it shows aggressive tendencies with the ability to invade peripheral nerves. This ability enables an adenoid cystic carcinoma to extend along the nerves and spread long distances, resulting in high incidence of distant metastasis and recurrence. Therefore, radical resection is the treatment of choice for this carcinoma. We performed partial mandibulectomy, neck dissection and immediate reconstruction using a radial forearm free flap on a 54 year-old man with adenoid cystic carcinoma on the right submandibular gland. And as an adjunctive therapy, chemotherapy with cisplatin and 5-fluorouracil was used. We report our case along with a literature review.

Mandibular metastasis of cholangiocarcinoma: A case report

  • Tumors metastasizing from distant regions to the oral and maxillofacial region are uncommon, comprising only 1%-2% of all malignancies. Cholangiocarcinoma is a malignancy that arises from cholangiocytes, which are epithelial cells that line the bile ducts. These cancers are difficult to diagnose and have a poor prognosis. In this paper, we report a rare case of mandibular metastasis of cholangiocarcinoma diagnosed at the primary site and discuss the radiographic findings observed in this case.

Bilateral Warthin tumor of the parotid gland: A case report (이하선에 발생된 양측성 Warthin 종양의 증례)

  • The Warthin tumor is a benign neoplasm that occurs mostly in the parotid gland. The tumor frequently occurs in the tail of the parotid gland. A 75-year-old man was referred to Wonkwang dental hospital with a chief complaint of swelling on the right submandibular area. Numerous salivary stones were observed in the right submandibular gland on computed tomography (CT). And the two tumorous lesions were incidentally found in the parotid gland bilaterally. The tumorous lesions showed homogeneous enhancement on the CT and intermediate signal intensity on both T1- and T2-weighted magnetic resonance (MR) images. This tumorous lesions also showed contrast enhancement on fat suppressed T1 weighted MR images. We report common CT and MR features of this case of Warthin tumor in the parotid gland with literature review.

Conservative surgical approach to aggressive benign odontogenic neoplasm: a report of three cases

  • Aggressive benign odontogenic neoplasms have substantial potential to grow to an enormous size with resulting bone deformities, and they often invade adjacent tissues and spread beyond their normal clinical and radiographic margins; as such, they have a high rate of recurrence. Historically, management (conservative versus aggressive) on the basis of clinical, radiographic and/or histopathologic characteristics has been controversial. However, recent advances in the understanding of the biological features of these lesions may provide greater evidence of the benefits of conservative management. Three patients with different complaints and final histopathologic diagnoses were enrolled in the study. All three cases were treated by a single operator with similar conservative surgical procedures. During follow-up, the patients had uneventful secondary healing and bone regeneration, less packing time than previously reported, no clinical or radiographic evidence of recurrence and no apparent deformity. The aggressive behavior of these lesions requires long clinical and radiographic follow-up. Conservative surgical management may be an option to reduce recurrence and morbidity and increase the probability of uneventful secondary healing and bone regeneration.
