일부지역 치위생(학)과 학생들의 구강건강신념과 구강보건행태 관련요인 (Oral health beliefs and oral health behaviors related factors of dental hygiene students)

  • Objectives: The study was conducted to evaluate the effects of the students majoring in dental hygiene on their oral health beliefs and oral health behavior. Methods: The survey was conducted using selfadministered questionnaires targeting 619 female students majoring in dental hygiene at three Universities in k and J province. The stronger health oral belief means higher score in susceptibility, severity, benefit and salience, but the higher score in barrier means stronger recognition in obstacle. Results: There was significant correlation between susceptibility and experience of not-treated oral disease (OR [odds ratio] 2.40; 95% CI [confidence interval] 1.73-3.34)' and 'dental caries (OR 2.36; 95% CI 1.25-4.45)'. Benefit had significant correlation with 'visiting dental clinic (OR 1.58; 95% CI 1.14-2.19)'. Salience had significant correlation with 'experience of not-treated oral disease (OR 0.70; 95% CI 0.52-0.94)'. Barrier had significant correlation with 'visiting dental clinic (OR 0.73; 95% CI 0.56-0.95)' and 'removing oral plaque (OR 0.71; 95% CI 0.52-0.95)'. There was no significant correlation between oral health belief and 'using of oral hygiene device' or 'regular tooth brushing'. Conclusions: Diverse oral health behaviors were affected by susceptibility, benefit, salience and barrier in oral health belief. The programs for oral health education and preventing oral disease should be prepared to change oral health belief to promote the oral health systematically based on the results of this study.

QraycamTM 활용한 구강보건교육 피드백의 융합적 효과 (The Convergent Effects of Oral Health Education Feedback Using QraycamTM)

  • 본 연구는 $Qraycam^{TM}$을 활용한 구강보건교육 피드백의 효과를 알아보고자 하였다. 중학교 2학년 총 118명을 대상으로 4주간 실험을 진행한 후 다음과 같은 결과를 얻었다. $Qraycam^{TM}$을 활용한 치면세균막 검사에서는 이미지로 피드백을 받은 실험군의 ${\Delta}R30$, ${\Delta}R70$, ${\Delta}R120$값 모두 통계적으로 유의하게 감소하였다(p<0.001). SPS는 실험군과 대조군 모두 중재 후에 감소하였으나 실험군에서만 통계적으로 유의한 결과가 나타났다(p<0.001). 구강보건행태에서는 칫솔질 시간이 중재 후 실험군이 대조군에 비해 유의하게 높은 결과가 나왔다(p<0.001). 주관적 구강건강상태에서는 주관적 구강건강의 중요성이 대조군에 비하여 실험군에서 통계적으로 유의하게 높았다(p<0.001). 이상의 결과를 종합해 보면 $Qraycam^{TM}$이미지로 피드백을 받은 실험군에서 치면세균막 감소효과가 컸으며, 칫솔질 시간, 주관적 구강건강의 중요성이 높게 나타났으므로 $Qraycam^{TM}$을 구강보건교육에 시각적인 피드백 도구로 활용할 수 있을 것으로 보여진다.

교사의 구강보건교육 실천과 자기효능감이 구강보건교육 태도에 미치는 영향 (Effect of Oral Health Education Practice and Self-efficacy of Teacher on Oral Health Attitude)

  • 본 연구는 교사의 구강보건교육 실천과 자기효능감이 구강보건에 대한 인식에 미치는 영향을 파악하기 위해 주요 관련 변인인 지식, 태도, 자기효능감, 실천의 관계에 대한 몇 가지 인과모형을 설정하였다. 서울시에 있는 특수학교 교사 376명을 대상으로 설문조사를 실시하였으며, 217명의 응답자 가운데 198명을 대상으로 t-검정, 일원배치분산분석, 상관분석, 경로분석을 하였다. 1. 교사의 성별, 연령, 경력에 따라 교사의 구강보건지식, 구강보건교육 교육에 대한 자기효능감, 구강보건교육에 대한 태도 수준을 비교한 결과, 교사의 성별에 따른 유의한 차이는 나타나지 않았으나, 교사의 연령에 따라 구강보건교육에 대한 자기효능감(p = 0.001)과 태도(p = 0.013)에 유의한 차이가 나타났다. 또한 교사의 경력에 따라 구강보건지식, 구강보건교육에 대한 자기효능감과 태도는 유의한 차이가 나타나지 않았다. 2. 교사의 구강보건지식, 구강보건교육에 대한 태도, 자기효능감, 구강보건교육 실천에 대한 상관관계를 분석하였다. 구강보건교육 태도, 자기효능감, 실천 사이에는 통계적으로 유의한 정(+)의 상관관계가 나타났으나, 교사의 구강보건 지식와 실천 사이에는 유의한 상관관계가 나타나지 않았다. 3. 교사의 구강보건 지식이 직접적으로 교사의 구강보건교육 실천에 직접적인 정(+)의 영향을 미치며, 교사의 구강보건교육에 대한 태도의 매개에 의해 교사의 구강보건교육 실천에 영향을 미치는 부분매개 모형(모형1)의 검증 결과, 지식과 태도 사이의 경로(Estimate=0.183, C.R.=0.294, p=0.003)와 태도와 실천 사이의 경로(Estimate=0.398, C.R. =3.090, p=0.002)는 통계적으로 유의하였으나, 지식과 실천 사이의 경로는 유의하지 않았다. 이 경로를 제외한 지식, 태도, 실천 사이의 완전매개 모형(모형1-1)은 모든 적합도 지수의 기준을 총족하였으나($x^2$=0.081, df=1, p=0.776, GFI=1.000, AGFI=0.998, NNFI= 1.178, NFI=0.996, CFI=1.000), 실천의 분산은 지식과 태도에 의해 충분히 설명되지 않았다(SMC=0.05). 4. 교사의 구강보건교육 실천이 구강보건교육에 대한 태도에 직접적인 정(+)의 영향을 미치며, 구강보건교육에 대한 자기효능감의 매개에 의해 교사의 구강보건교육 태도의 매개에 의해 영향을 미치는 부분매개 모형(모형2)의 검증 결과, 실천과 태도 사이의 경로(Estimate=0.273, C.R.=5.734, p<0.001)와 교사효능감과 태도 사이의 경로(Estimate=0.242, C.R.=4.426, p<0.001)는 통계적으로 유의하였으나, 실천과 태도 사이의 경로는 유의하지 않았다. 이 경로를 제외한 실천, 교사효능감, 태도 사이의 완전매개 모형(모형2-1)은 모든 적합도 기준을 충족하였으나($x^2$=2.081, df=1, p=0.149, GFI=0.993, AGFI=0.958, RMSEA=0.074, NNFI=0,942, NFI=0.965, CFI=0.981), 태도의 분산은 실천과 교사효능감에 의해 충분히 설명되지 않았다(0.136). 5. 지식태도실천 모형(모형2-1)에 교사의 구강보건교육에 대한 자기효능감을 매개변인으로 추가한 지식태도 자기효능감실천 모형(모형3)은 적합하게 나타났으며 ($x^2$=3.971, d.f.=3, p=0.271, RMSEA=0.039, GFI=0.990, AGFI=0.967, NNFI=0.971, NFI=0.944, CFI=0.986), 실천의 분산 중 14.3%가 지식, 태도, 자기효능감에 의해 설명되었다.

일부 교정장치 장착환자의 구강보건지식 및 구강위생관리 실천도 (Oral health knowledge and oral hygiene care among some orthodontic patients)

  • Objectives : The purpose of this study was to examine the knowledge of orthodontic patients about oral health knowledge and their actual oral hygiene care, which affected the oral hygiene of orthodontic patients. It's specifically meant to provide information on the prevention of the possible side effects of orthodontic treatment and the promotion of the oral health of orthodontic patients. Methods : This study were 227 orthodontic patients of dental clinics specialized in orthodontics. The collected data were analyzed by the statistical package SPSS WIN 18.0. Results : 1.As for Oral health knowledge, they had the best knowledge about smoking and dental checkup among the subfactors of oral health knowledge. 2. As to actual oral hygiene care, what they did best was doing toothbrushing before sleeping and what they did worst was avoiding eating food injurious to teeth. 3. They scored highest in the right toothbrushing(3.85), the use of oral hygiene supplies(3.62), the prevention of periodontal diseases(3.13) in the subfactors of actual oral hygiene care. They scored lowest in dental checkup and diet(2.99). 4. There were significant differences among the patients in oral health knowledge according to age(F=2.95, p<.05). Those who received another treatment during orthodontic treatment had a better oral health knowledge than the others who didn't, and the gap between the two was statistically significant(t=2.26, p<.05). 5. There were differences among the patients in actual health hygiene care according to gender(t=2.71, p<.01), age(F=4.40, p<.01), educational experiences about oral hygiene care(t=3.06, p<.01) and experience of receiving another treatment during orthodontic treatment(t=2.56, p<.05). 6. There was a positive correlation between oral health knowledge and oral hygiene care(r=.261, p<.001). Conclusions :The above-mentioned findings suggest that more education of diet and toothbrushing should be provided for orthodontic patients to improve their oral health care.

노인의 삶의 질 향상을 위한 주관적 구강건강에 영향을 미치는 요인 (Influencing factors for perceived oral health for the improvement of quality of life in Korean elderly)

  • Objectives: The purpose of the study was to investigate the influencing factors of the perceived oral health for improvement of quality of life in Korean elderly. Methods: The subjects were 1,289 elderly over 65 years old from the sixth National Health and Nutrition Examination Survey 2013. The dependent variable was subjective oral health status. The independent variable was sociodemographic characteristics. Results: The perceived oral health of the unemployed elderly and those having chewing problems were 1.65-fold(95% CI=1.12=2.44) and 3.45-fold(95% CI=2.37-5.02), respectively than employed and chewable elderly. The perceived oral health of the former was 2.49-fold worse(95% CI=1.73-3.60) than the latter. Conclusions: The influencing factors of perceived oral health status included occupation, perceived health status and chewing problems. To improve the oral health-related quality of life in the elderly, continuous education and hands-on programs should be provided for the elderly in the long term care.

Analysis of Oral Pathogenic Microorganisms by Elderly's Systemic and Oral Health Status of the Elderly Over 65 Years

  • Korean society is experiencing rapid growth due to industrialization and urbanization, and the aging of population is emerging. Degradation of the masticatory muscle due to aging and reduction of the number of mastications causes dry mouth syndrome with decreased saliva in the elderly. Changes in the amount and composition of saliva cause various oral diseases, especially, periodontal disease is closely related to the microorganisms that form the plaque. In the elderly education, stimulation saliva was collected at the first visit and the last visit to observe changes in microorganisms in the oral cavity. For the collection of the probes, the paraffin wax was chewed for 5 minutes. The samples were immediately refrigerated in a 50 cc plastic tube and then stored at $-20^{\circ}C$ until the next use. The subjects of this study were the elderly people aged 65 years or older, and the study was conducted on 16 persons, 3 males and 13 females. The distribution of saliva microorganisms according to rolling brushing method showed significant difference in all three groups (Aa & Red complex, Orange complex and Green complex) before and after the education. In the group who answered that they had dentures, it was confirmed that the amount of microorganisms before and after education was significantly reduced in all three groups. There was a significant decrease in the amount of microorganisms in the Aa & Red complex and Orange complex group in the case of hypertension related to systemic disease. In conclusion, the higher the interest of oral health and general health of the subjects, the more effective the education.

  • Objectives: This study aimed to prepare basic data to improve the health-related quality of life of cancer survivors by confirming their oral and mental health statuses and identifying factors affecting their health-related quality of life. Methods: Of all participants in the 8th KNHANES (2019-2020), adults aged 19 years or older who responded 'yes' to the diagnosis of cancer and 404 cancer survivors who responded 'none' to the current cancer prevalence item were selected as the final research participants. Multiple regression analysis was conducted to confirm the effect of cancer survivors' oral and mental health on health-related quality of life. Results: Subjective oral health (p<0.01), chewing problems (p<0.05), subjective health (p<0.001), and depression (p<0.01) had an effect on health-related quality of life from multiple regression analyses. Conclusions: Therefore, oral and mental health promotion may improve health-related quality of life. Thus, it is necessary to recognize the importance of oral and mental health and implement preventive education and programs.

전신질환과 구강건강 상관성 인식에 관한 융합적 조사 (A Fusion Survey on the Investigation of Correlation between Systemic Diseases and Oral Health)

  • 전신질환과 구강건강의 상관성에 대해 대학생들이 어느 정도 인식하고 있는지를 파악하기 위해 광주광역시 지역 3개 대학교 약 485명을 대상으로 설문조사를 실시하였다. 연구결과, 여자가 남자보다 높았으며, 4학년에서 가장 높게 인식하고 있었다. 주관적 건강상태, 음주여부, 흡연경험 및 운동여부는 전신질환과 구강건강의 연관성을 인식하는데 있어서 유의한 차이를 보이지 않았다. 구강보건교육 경험이 있는 그룹에서 3.18로 인식하고 있었고, 교육내용에 대한 실천을 하는 그룹에서 3.21로 높게 인식하고 있었다. 두 군에서는 그룹 간 유의한 차이를 보였다. 다중회귀분석을 실시한 결과 전공계열 요인과 학년 요인이 전신질환과 구강건강의 연관성 인식도에 영향을 준 것으로 나타났다. 따라서 대학생들에게 구강 정기검진의 중요성과 필요성을 인식시킬 필요가 있으며, 질병의 발생은 한 가지에서 끝나는 것이 아니라 여러 가지 질병에 상관관계가 있다는 것을 인식하게 할 교육을 강화하여 시행할 필요가 있을 것이라 생각된다.

대구광역시 초등학교 구강보건실 시범운영의 효과평가 - 사업 4차년도 평가 분석 - (The 4th Year Appraisal for Effectiveness of School-based Oral Health Program in Daegu, Korea)

  • Objectives: The purpose of this study was to evaluate the long term effects of school-based oral health program on the decrease of dental caries among elementary school children. Methods: The subjects of this study were total 283 students of one elementary school in Deagu, Korea. They had received school-based oral health programs more than one year since 2004. This school-based oral health program included regular oral examination, fluoride mouthrinsing, pit-and-fissure sealing, APF gel application, tooth brushing instruction and chewing the xylitol tablets. The subjects' oral health status were examined and calibrated by a dentist every year from 2004 to 2007. Results: The change of DMFT index were as follows: (1)The subjects who entered the school at 2004 - 0.32(2004), 0.25(2005), 0.25(2006) and 0.38(2007), (2)the subjects who entered the school at 2005 - 0.18(2005), 0.31(2006) and 0.32(2007), and (3)the subjects who entered the school at 2006 - 0.19(2006) and 0.27(2007). Conclusions: This study partially showed the effects of school-based oral health programs on the increments of dental caries. This programs should be expanded widely among elementary schools in Korea.

일부 치위생과 학생들의 자기효능감과 구강보건행동 실천의지의 연관성 (The association between oral health behavior intention and self-efficacy of dental hygiene students)

  • Objectives : The purpose of this study is to provide basic data to develop health education program, by analyzing the relationship between the oral health behavior intention and self-efficacy in dental hygiene students. Methods : This research was based on self administrated survey conducted by 348 dental hygiene students. The survey was composed of five items of general characteristics, ten items of factor in self-efficacy and seven items of factor in oral health behavior intention. Results : According to general characteristics, students with high grades had higher oral health behavior intention(p<0.05) and students answered that the significance of oral health is very important had higher oral health behavior intention(p<0.05). The level of self-efficacy was separated by high-level(30%), medium-level(40%), and low-level (30%), and the higher self-efficacy was, the higher oral health behavior intention was. Conclusions : By improving dental hygiene student's self-efficacy, it was necessary to promote oral health behavior intention.
