약화된 내측 관절낭을 가진 중증 무지 외반증에 대해 Internal Brace를 이용한 보강술의 수술적 치료 결과 (Surgical Outcomes of the Reinforcing Technique of a Weakened Medial Capsule in Severe Hallux Valgus Using Internal Brace)

  • Purpose: This study evaluated the clinical and radiological results after reinforcement of the weakened medial joint capsule using Internal Brace (Arthrex) for treating severe hallux valgus. Materials and Methods: This study reviewed 56 cases of 50 patients that were followed-up postoperatively for at least 12 months, from September 2017 until August 2018. An extended distal chevron osteotomy combined with a distal soft-tissue release was performed by a single surgeon to treat severe hallux valgus. Internal Brace was applied in 12 cases (group A) who had weakened medial joint capsules, and capsulorrhaphy was performed in 44 cases (group B), and these two groups were compared postoperatively for the clinical and radiological results. The postoperative complications were also investigated. Results: No significant differences at 1-year follow-up on the Manchester-Oxford Foot Questionnaire and the patients' satisfaction scores were found between the two groups (p=0.905 and p=0.668, respectively). For the radiology, the changes of the values between before surgery and at 1-year follow-up according to the group showed no significant differences in the hallux valgus angle, intermetatarsal angle, and the hallux interphalangeal angle (p=0.986, p=0.516, p=0.754, respectively). Recurrence of hallux valgus was reported in two cases in group A, and in three cases in group B. Transfer metatarsalgia occurred in 4 cases in group B. Conclusion: Based on these results, we recommend the capsule reinforcing technique using Internal Brace as a successful operative option for treating a weakened medial capsule in patients with severe hallux valgus.

수컷 시츄견에서 발생한 유레쓰랄 플러그의 수술적 관리 (Surgical Management of Urethral Plugs in a Male Shih-Tzu Dog)

  • 6년령의 중성화된 수컷 시츄가 본 병원에 혈뇨와 구토로 내원하였다. 혈액검사 결과 BUN, Creatin 수치가 증가한 상태였다. 방사선 상에서 방광 벽 배쪽으로 방사선 비투과성 물질이 붙어있었다. 또한 음경요도에 방사선비투과성 물질로 가득차 있었다 초음파 상에서는 신장에서 양측에서 유출물이 나타났으며, 방광벽은 얇아져 있었고, 음경골 뒤쪽으로 아쿠아틱 쉐도잉을 나타내는 물질이 있었다. 요도폐색으로 인해 배뇨가 확인되지 않아서 방광천자를 하였다. 요분석 결과 혈뇨소견이 나타났으며, 명확한 입자들은 확인되지 않았다. 요로수압추진술을 하였으나 방광까지 개통성을 확보하는 것을 실패하였기 때문에 방광절개술과 요도절개술을 요로수압추진술과 함께 진행하였다. 이 case report는 유레쓰랄 플러그를 가진 수컷 시츄 견에서 임상적 소견, 방사선학적 소견을 설명하였다. 유레쓰랄 플러그가 방광절개술과 요도절개술, 요로수압추진술을 통해 성공적으로 치료된 케이스이다. 이것은 유레쓰랄 플러그를 가진 강아지에서 한가지의 치료방법이 될 수가 있다.

Predicting Successful Conservative Surgery after Neoadjuvant Chemotherapy in Hormone Receptor-Positive, HER2-Negative Breast Cancer

  • Purpose: This study aimed to determine whether clinicopathological factors are potentially associated with successful breast-conserving surgery (BCS) after neoadjuvant chemotherapy (NAC) and develop a nomogram for predicting successful BCS candidates, focusing on those who are diagnosed with hormone receptor (HR)-positive, human epidermal growth factor receptor 2 (HER2)-negative tumors during the pre-NAC period. Methods: The training cohort included 239 patients with an HR-positive, HER2-negative tumor (${\geq}3cm$), and all of these patients had received NAC. Patients were excluded if they met any of the following criteria: diffuse, suspicious, malignant microcalcification (extent >4 cm); multicentric or multifocal breast cancer; inflammatory breast cancer; distant metastases at the time of diagnosis; excisional biopsy prior to NAC; and bilateral breast cancer. Multivariate logistic regression analysis was conducted to evaluate the possible predictors of BCS eligibility after NAC, and the regression model was used to develop the predicting nomogram. This nomogram was built using the training cohort (n=239) and was later validated with an independent validation cohort (n=123). Results: Small tumor size (p<0.001) at initial diagnosis, long distance from the nipple (p=0.002), high body mass index (p=0.001), and weak positivity for progesterone receptor (p=0.037) were found to be four independent predictors of an increased probability of BCS after NAC; further, these variables were used as covariates in developing the nomogram. For the training and validation cohorts, the areas under the receiver operating characteristic curve were 0.833 and 0.786, respectively; these values demonstrate the potential predictive power of this nomogram. Conclusion: This study established a new nomogram to predict successful BCS in patients with HR-positive, HER2-negative breast cancer. Given that chemotherapy is an option with unreliable outcomes for this subtype, this nomogram may be used to select patients for NAC followed by successful BCS.

부모-아동 상호작용치료가 양육기술과 아동의 문제행동에 미치는 영향: 메타분석 (Effects of Parent-Child Interaction Therapy on Parenting Skill and Children's Problem Behaviors: Meta-Analysis)

  • 목적 : 본 연구는 부모-아동 상호작용치료가 장애아동 부모의 양육기술과 아동의 문제행동에 미치는 영향을 메타분석으로 검증하여 중재 방법의 근거를 제공하고자 한다. 연구방법 : 본 연구는 2007년 1월부터 2018년 3월까지 출판된 연구 중 장애아동을 대상으로 부모-아동 상호작용 프로그램을 사용한 연구를 대상으로 하였다. 논문 검색은 PubMed, CINAHL, EBSCOhost, Science Direct를 이용하였고, 이차적으로 검색논문의 참고문헌을 추가 검색하였다. 총 7편의 연구가 선정기준에 적합하였고, 최종 논문에 대해 Jadad scale를 이용하여 질적 평가를 수행하였다. R-meta 분석프로그램을 사용하여 통계적 검정을 실시하였다. 결과 : 부모-아동 상호작용치료의 양육기술 효과크기는 큰 효과크기를 나타냈으며, 양육 스트레스 효과크기는 중간크기를 나타냈다. 아동의 문제행동 또한 중간 효과크기를 나타냈으며 모든 p값은 통계적으로 유의하였다(p<.0001). 결론 : 본 연구는 부모-아동 상호작용치료가 부모의 양육기술과, 양육 스트레스, 아동의 문제행동에 미치는 영향을 살펴보았다. 앞으로 아동작업치료에서 가족참여 중재에 대한 근거 마련을 위한 연구가 필요하다.

명제논리 기반 서울시 역세권 2030청년주택 운영기준의 개정효과 분석 (A Spatial Analysis Based on the Amendments in Seoul's 2030 Youth Housing Policy Using Propositional Logic)

  • 역세권 2030청년주택 정책은 역세권 개발이 서울시 내 임대주택 택지를 확보하기 위한 유일한 대안이라는 인식 가운데 기대를 받으며 출범했다. 그러나 임대주택 공급실적은 당초 계획에 미치지 못했고 용도지역체계에 혼란을 줄 수 있다는 비판이 지속적으로 제기되었다. 결국, 역세권 2030청년주택은 시행 후 3년 사이에 운영기준이 6차례 개정되었다. 특히 2018년 10월 이후에는 1년 이내에 큰 폭의 운영기준 개정이 두 차례 이어졌다. 이에 본 연구는 사업가능구역과 용도지역 변경 가능구역의 변화 양상을 분석하여, 지난 운영기준 개정의 방향성 평가와 정책적 시사성을 도출하고자 했다. 공간적 범위 및 구역의 도출에는 명제논리를 활용했다. 이를 위해, 가구(街區) 등 기초자료 정제 및 전처리, 명제별 진리 값 입력, 논리연산 과정을 수행하여 사업가능구역 및 용도지역 변경 가능구역 소속여부를 판별했다. 분석 결과, 1) 사업가능구역은 서울시 외곽의 역세권으로 고르게 분산되면서도 준주거지역과 상업지역으로 집중되는 경향이 있고, 2) 용도지역 변경 가능구역은 준주거지역 변경 가능구역을 중심으로 전반적인 증가추세가 두드러졌다.

Clinical Factors Influencing the Trial and Purchase of Bilateral Microphones with Contralateral Routing of Signal in Patients with Asymmetric Sensorineural Hearing Loss

  • Background and Objectives: Bilateral microphones with contralateral routing of signal (BiCROS) hearing aid is an option for hearing rehabilitation in individuals with asymmetric sensorineural hearing loss (ASNHL). The clinical factors influencing the trial and purchase of BiCROS were investigated. Subjects and Methods: We reviewed the medical records of 78 patients with ASNHL who were recommended to use BiCROS and analyzed the demographic and audiological factors influencing the trial and purchase of BiCROS. Results: Among the 78 patients, 52 (66.7%) availed of the free BiCROS trial and 21 (26.9%) purchased BiCROS. The mean pure tone audiometry (PTA) air conduction (AC) threshold of the better- and worse-hearing ears were 44.2±12.8 dB and 90.7±22.5 dB HL, respectively. The decision for trial or purchase of BiCROS was not influenced by age, sex, duration of hearing loss of the worse-hearing ear, or PTA AC threshold or speech discrimination score of both ears. The first and third quartiles of the PTA AC thresholds for the better-hearing ear of BiCROS buyers were 38.75 dB and 53.75 dB HL, respectively. The counterpart values for the worse-hearing ear were 72.50 dB and 118.75 dB HL, respectively. Conclusions: The clinical factors analyzed in this study were found to be irrelevant to the trial and purchase of BiCROS in patients with ASNHL. Nevertheless, the distribution range of the auditory thresholds of the subjects using BiCROS can be a useful basis for the counseling of patients with ASNHL and selection of candidates for BiCROS use.

Evaluation of forage production, feed value, and ensilability of proso millet (Panicum miliaceum L.)

  • Whole-plant corn (Zea may L.) and sorghum-sudangrass hybrid [Sorghum bicolor (L.) Moench] are major summer crops that can be fed as direct-cut or silage. Proso millet is a short-season growing crop with distinct agronomic characteristics that can be productive in marginal lands. However, information is limited about the potential production, feed value, and ensilability of proso millet forage. We evaluated proso millet as a silage crop in comparison with conventional silage crops. Proso millet was sown on June 8 and harvested on September 5 at soft-dough stage. Corn and sorghum-sudangrass hybrid were planted on May 10 and harvested on September 10 at the half milk-line and soft-dough stages, respectively. The fermentation was evaluated at 1, 2, 3, 5, 10, 15, 20, 30, and 45 days after ensiling. Although forage yield of proso millet was lower than corn and sorghum-sudangrass hybrid, its relative feed value was greater than sorghum-sudangrass hybrid. Concentrations of dry matter (DM), crude protein, and water-soluble carbohydrate decreased commonly in the ensiling forage crops. The DM loss was greater in proso millet than those in corn and sorghum-sudangrass hybrid. The in vitro dry matter digestibility declined in the forage crops as fermentation progressed. In the early stages of fermentation, pH dropped rapidly, which was stabilized in the later stages. Compared to corn and sorghum-sudangrass hybrid, the concentration of ammonia-nitrogen was greater in proso millet. The count of lactic acid bacteria reached the maximum level on day 10, with the values of 6.96, 7.77, and 6.95 Log10 CFU/g fresh weight for proso millet, corn, and sorghum-sudangrass hybrid, respectively. As ensiling progressed, the concentrations of lactic acid and acetic acid of the three crops increased and lactic acid proportion became higher in the order of sorghum-sudangrass hybrid, corn, and proso millet. Overall, the shorter, fast-growing proso millet comparing with corn and sorghum-sudangrass hybrid makes this forage crop an alternative option, particularly in areas where agricultural inputs are limited. However, additional research is needed to evaluate the efficacy of viable strategies such as chemical additives or microbial inoculants to minimize ammonia-nitrogen formation and DM loss during ensiling.

소아용 크라운 수복을 위한 3D 프린팅 레진과 레진 시멘트 간의 접착 강도 평가 (In Vitro Study on the Bond Strength Between 3D-Printed Resin and Resin Cement for Pediatric Crown Restoration)

  • 3D 프린터로 출력된 레진 크라운은 유치의 심미 수복을 위한 크라운으로 사용될 수 있으며, 접착에는 주로 레진 시멘트가 사용된다. 본 연구의 목적은 크라운 제작용 3D 프린팅 레진의 접착 능력을 평가하고, 이를 다른 레진 간접 수복용 재료들과 비교하는 것이었다. 3D 프린팅 레진인 Graphy (GP)와 NextDent (NXT), 레진 간접 수복용 재료인 MAZIC Duro (MZ)와 VIPI Block (VIPI)의 전단결합강도가 본 연구에서 비교되었다. GP, NXT, MZ, VIPI군의 평균 전단결합강도는 각각 23.29 ± 3.88, 26.14 ± 4.67, 25.41 ± 4.03, 18.79 ± 4.26 MPa였다. VIPI군을 제외한 GP, NXT, MZ군의 평균 전단결합강도 값에는 유의미한 차이가 없었다. 본 연구 결과 3D 프린팅 레진은 레진 시멘트와 임상적으로 허용 가능한 수준의 접착 강도를 나타냈으며, 이에 따라 유치 수복을 위한 크라운 재료로서 적합한 조건을 갖추고 있음을 확인하였다.

환경영향평가에서의 대기질 확산모델 적용방법 개선 연구(II) - AERMOD 모델 적용방법을 중심으로 - (A Study on Improvement of Air Quality Dispersion Model Application Method in Environmental Impact Assessment (II) - Focusing on AERMOD Model Application Method -)

  • 우리나라 환경영향평가서의 대기모델 적용 실태를 조사하기 위하여 2021년부터 2022년까지 협의가 완료되어 환경영향평가정보지원시스템(EIASS)에 공개된 환경영향평가서를 분석한 결과 AERMOD가 89.0%로 가장 많이 사용되었다. AERMET과 AERMOD의 버전 불일치는 25.3%로 나타났으며, 산업단지 및 도시개발사업의 발생하는 오염물질 배출량 산정시 적용한 가동시간과 모델에 적용한 가동시간의 불일치는 50.6%로 나타났다. 군산지역의 단순 및 복잡지형에서 다양한 버전의 AERMET과 AERMOD 모델을 면오염원과 점오염원에 적용한 결과, AERMOD 버전 12(15181) 이후부터는 동일한 값을 나타내었다. 배출량은 24시간 가동을 기준으로 산정하고, 모델에는 주간 8시간 가동을 적용하는 가변 배출계수 옵션을 사용할 때, 예측농도는 단순 및 복잡지형에서 면오염원은 32.06~74.27%, 점오염원은 14.85~43.13% 축소되는 것으로 나타났다. 따라서 AERMOD 모델의 가변 배출계수 적용 오류를 방지하기 위해서는 환경영향평가서에 배출량 산정과정을 명확히 제시하고, 모델링 입력자료 구성에 대한 구체적인 설명이 요구되며, 환경영향평가 전문 검토기관에서는 보다 철저한 검토가 필요하다.

Hate Speech Detection Using Modified Principal Component Analysis and Enhanced Convolution Neural Network on Twitter Dataset

  • Traditionally used for networking computers and communications, the Internet has been evolving from the beginning. Internet is the backbone for many things on the web including social media. The concept of social networking which started in the early 1990s has also been growing with the internet. Social Networking Sites (SNSs) sprung and stayed back to an important element of internet usage mainly due to the services or provisions they allow on the web. Twitter and Facebook have become the primary means by which most individuals keep in touch with others and carry on substantive conversations. These sites allow the posting of photos, videos and support audio and video storage on the sites which can be shared amongst users. Although an attractive option, these provisions have also culminated in issues for these sites like posting offensive material. Though not always, users of SNSs have their share in promoting hate by their words or speeches which is difficult to be curtailed after being uploaded in the media. Hence, this article outlines a process for extracting user reviews from the Twitter corpus in order to identify instances of hate speech. Through the use of MPCA (Modified Principal Component Analysis) and ECNN, we are able to identify instances of hate speech in the text (Enhanced Convolutional Neural Network). With the use of NLP, a fully autonomous system for assessing syntax and meaning can be established (NLP). There is a strong emphasis on pre-processing, feature extraction, and classification. Cleansing the text by removing extra spaces, punctuation, and stop words is what normalization is all about. In the process of extracting features, these features that have already been processed are used. During the feature extraction process, the MPCA algorithm is used. It takes a set of related features and pulls out the ones that tell us the most about the dataset we give itThe proposed categorization method is then put forth as a means of detecting instances of hate speech or abusive language. It is argued that ECNN is superior to other methods for identifying hateful content online. It can take in massive amounts of data and quickly return accurate results, especially for larger datasets. As a result, the proposed MPCA+ECNN algorithm improves not only the F-measure values, but also the accuracy, precision, and recall.
