일 대학병원 간호사의 간호정보역량과 셀프 리더십의 상관성 (Relationship of Nursing Informatics Competency and Self-leadership among Hospital Nurses)

  • Purpose: The objective of this study was to define nursing informatics competency and self-leadership perceived by hospital nurses and the relationship between those variables. Method: The subjects of this study were 215 nurses who had worked in one university hospital in P city. For this study the following study tools were used: the Staggers Nursing Computer Experience Questionnaire and self-leadership scale. Data was analyzed using mean, standard deviation, t-test, ANOVA, Scheffe and Pearson correlation coefficient. Results: The mean score of each item in informatics skill domain and informatics knowledge domain $2.93{\pm}.49$ and $3.40{\pm}.50$. The average score of self-leadership was $3.39{\pm}.53$. 'Self-leadership' was positively related with 'nursing informatics competency'. Conclusion: This study proved that there were significant positive correlation between nursing informatics competency and self-leadership. These results suggested that it would be necessary to develop regular educational program which promotes the nurses' self-leadership as well as nursing informatics competency in schools. And practical education program for nurses must be planned continuously for informatics competency in hospitals.

수술실간호사의 수술환자 안전관리에 대한 중요성인지도와 실천이행도 (Importance awareness and Compliance on Patient Safety for Nurses Working in Operating Rooms)

  • 본 연구의 목적은 수술실간호사의 수술환자 안전관리수준, 안전사고유형, 및 수술환자 안전관리에 대한 중요성인지도와 실천이행도를 파악하고 수술환자 안전관리 중요성인지도와 실천이행도의 관계를 분석하여 실천이행도 영향요인을 규명하는 것이다. 191명의 수술실간호사를 대상으로 구조화된 설문지를 이용하여 자료수집하였으며 기술통계, t-test, paired t-test, ANOVA, Pearson correlation coefficients, multiple regression으로 분석하였다. 수술실 간호사들은 수술환자 안전관리수준을 중등도(6.3)로 평가하였으며 수술환자 안전사고는 환자손상(39.8%), 계수(18.4%), 감염관리(17.5%) 영역에서 주로 발생하였다. 수술실간호사의 수술환자 안전관리에 대한 실천이행도는 중요성인지도 보다 유의하게 낮았으며(t=18.01, P<.001) 중요성인지도와 실천이행도 간에는 유의한 정적 상관관계가 나타났다(r=.56, p<.001). 안전교육경험, 동료수술간호사의 사고경험, 직위, 병원경력이 수술환자안전관리 실천이행도의 유의한 설명변수(13.9%)로 규명되었다(F=8.407, P<.001). 그러므로 각 병원은 수술환자안전관리 교육프로그램을 개발하여 교육을 제공하고, 안전관리지침서를 준수할 수 있도록 제도적인 지원을 하며 안전관리에 대한 개방적 의사소통을 활성화하는 노력이 필요하다.

외과적 손씻기 및 외과용 장갑의 천공율에 대한 연구 (A Study on the Surgical Hand Scrub and Surgical Glove Perforation)

  • Post - operative wound infections have been a serious problem in nursing care in the operating room and appear to be strongly related to the infection occurring during the performance of operation. The purpose of this study is to identify patterns in duration of surgical hand scrub (SHS), to evaluate the method of SHS and to examine the rate of glove perforation. Subjects for this study include 244 doctors and 169 nurses working in the operative theatre of a hospital in Seoul area. Test samples and related data were collected from this medical facility between April 1, through 15, and July 1, through 5, 1995 by the author and a staff member working in the operating room. For the study, data on the SHS of doctors and nurses were obtained at the time of operation and multiple batches of surgical gloves worn by the operating doctors were collected after each operation. The duration of SHS was measured with a stop watch and the method of SHS was evaluated according to Scoring Hand Scrub Criteria (SHS Criteria) and expressed as SHS scores. For the analysis of the data, t-test was used to compare the differences in the duration and the SHS scores of doctors and nurses, and Pearson's correlation coefficient was used to examine the relationship between the SHS duration and the SHS scores. The results of the study are summarized as follows. 1) The mean time spent in each SHS was 167 seconds in nurses, and 127 seconds in doctors. The data comparing nurses and doctors indicated that there were significant differences in Our ation of SH S between these two groups (t=5.58, p=.000). 2) The mean time spent in the first SHS was 145 seconds and that in the End SHS, 135 seconds, and there was not a significant difference in the duration of the SHS between doctors and nurses (t=1.44, P=.156). 3) The mean time spent in the SHS by OS (Orthopaedic surgery) doctors was 162 seconds, 150 seconds by NS(Neurologic surgery), 121 seconds by GS(General surgery), 94 seconds by OPH(Opthalmology) and DS(Dental surgery), 82 seconds by URO(Urology), 78 seconds by PS(Plastic surgery) and 40 seconds by ENT(Ear, Nose & Throat) These also showed a significant difference in the duration of the SHS among the medical specialities (t=4.8, P=.0001). 4) The average SHS score of the nurses was 15.2, while that of doctors was 13.1. The statistical analysis showed that t-value was 3.66, p was. 000. This indicates that the nurses actually clean their hands more thoroughly than the doctors do. 5) The average SHS score of NS doctors was 15.5, 15.3 for doctors for OPH,14.3 for OS,12.7 for GS, 12.0 for DS, 11.7 for URO, 10.1 for PS, 7.5 for ENT. Comparison of the average SHS scores from 8 specialties showed that there was a significant differences in the patterns of the SHS (F=5.08, P=.000) among medical specialties. 6) It appears that the operating personnel scrub the palms and dorsum of their hand relatively well, however, less thorough the nails and fingers. 7) The more the operating personnel spend their time in hand scrubbing, the more correctly they clean their hands(r=.6427, P<.001). 8) The overall frequencies of perforation in all post-operative gloves tested was 38 out of 389 gloves (10.3%). The perforation rate for PS was 13%, 12.1% for GS,8.8% for 05, and 3.3% for NS.

웹 기반 동영상 프로그램 선택속성이 수술실 신규간호사의 간호 수행능력에 미치는 영향 -학습몰입의 매개효과 중심으로- (The Effects of Web-based Video Program Selection Attributes of Confidence in Nursing Performance-Mediating Effects of Learning Flow)

  • 본 연구는 수술실 신규간호사의 웹 기반 동영상 프로그램 선택속성이 학습 몰입을 매개로 차후 간호 수행능력에 관한 확신에 미치는 영향을 검증하는 것이 목적이다. 연구대상은 인천 및 경기도 소재 대학병원의 수술실 근무 중인 간호사를 대상으로 하였으며, 250개의 표본을 분석에 활용하였다. 연구방법은 구조화된 설문지를 사용하여 2019년 7월부터 8월까지 수집되었다. 참가자들은 인천과 경기에 있는 대학병원 수술실 250명의 간호사이다. 수집 된 데이터는 IBM SPSS Statistics 25.0과 AMOS 24.0을 사용하여 분석하였으며, 구조모형을 통해 위에 적합도 지표를 기반으로 직접효과와 간접효과를 측정하였다. 연구 결과, 수술실 신규간호사는 수술과 1차적으로 직결되는 위생 안전요인과 환자 관리요인을 상대적으로 더욱 중요하게 인지하였다. 학습 몰입을 통하여 차후 간호 수행능력에 관한 확신이 있었고, 위생 안전요인의 경우 학습 몰입이 없어도 수술실 신규간호사가 중요성을 인지하여 차후 간호 수행능력에 관한 확신에 긍정적 영향이 미침을 검증하였다. 따라서 향후 시각적 효과에 더 비중을 둔 웹 기반 동영상 프로그램 구성을 통해 간호의 위생 안전요인뿐만 아니라 환자 관리요인 그리고 간호 관련 요인 모두가 환자의 생명과 회복에 핵심적 단계임을 신규간호사에게 적합하게 이해시킬 필요가 있다.

  • 본 연구는 수술 전 수술실 간호사의 지지간호중재가 자궁적출술 환자의 불안에 미치는 효과를 검정하기 위한 유사실험 연구이다. 2013년 5월 1일부터 6월 20일까지 불안, 혈압, 맥박에 대한 자료를 수집하였고, 통계 검정은 SPSS 20.0을 이용하여 $x^2-test$, Fisher's exact test, t-test를 수행하였다. 대조군 25명은 기존의 간호중재를 받았고 실험군 25명은 지지간호중재를 받은 자궁적출술 환자이다. 불안(p=.002), 이완기혈압(p=.012), 수축기 혈압(p=.045)은 대조군에 비해 실험군이 유의하게 낮았고 맥박수는 대조군과 실험군에서 유의한 차이가 없었다. 결과적으로 수술 전 지지간호중재가 환자의 불안을 감소시키는데 효과적인 것으로 확인되었다.

  • Purpose: Purpose of this study was to identify the effect of structured information on immediate preoperative anxiety and uncertainty for women undergoing total laparoscopic hysterectomy. Methods: Sixty women who were admitted for total laparoscopic hysterectomy were recruited at a university hospital in Gyeonggi-do from June to October 2014. Thirty women were assigned to either the experimental or the control group. Women in the experimental group were provided structured information, which consisted of visual and auditory materials about surgical preparation and process, practical experience on devices such as IV-PCA pump and Inspiro-meter and actual experience on route to go to the operating room. State-anxiety, uncertainty, and blood pressure and pulse rate as biological indicators were measured before and after the intervention to examine the effect. Results: Significant group differences were found on state anxiety, uncertainty, including ambiguity, inconsistency, and unpredictability at the holding area. There was a significant difference on pulse rate in the operating room between the two groups. Conclusion: Findings demonstrated that the structured information provided for women undergoing laparoscopic hysterectomy preoperatively was effective on immediate preoperative anxiety and uncertainty. Nurses may contribute to decreasing patients' anxiety and uncertainty by utilizing this structured information preoperatively.

수술실 간호사의 수술실내 배치전환 태도와 직무만족 (A Study on Nurses' Attitude of Job Transfer and Job Satisfaction in Operating Rooms)

  • Purpose: The aim of this study was to investigate the attitude of job transfer and job satisfaction in operating rooms, and to use this data towards efficient human resource management. Method: One hundred and seven nurses in operating rooms in a general hospital in Seoul, Korea were surveyed using a questionnaire. Data was collected and analyzed using SPSS/PC program. Results: The attitude toward the job transfer was generally favorable with a mean of 3.32 out of a 5 point scale in rating the need for job transfer. Among the five domains of attitudes toward job transfer, individual development scored the highest(mean, 3.75). The mean score of job satisfaction in the study participants was rated in a 5 point scale with a mean of 3.12. Among the seven domains of job satisfaction, networking(3.37) scored the highest, followed by the perspective of professional position(3.32). Therefore, it can be said that mutual interaction and professional position are the most important factors for job satisfaction. The correlation between the attitude toward job transfer and job satisfaction showed a positive relationship with a coefficient of 0.210, implying that job satisfaction was higher when the attitude toward transfer was more favorable. Conclusion: From the attitude toward job transfer, individual development and professional improvement was favorable, implying the need for such program. Therefore, it is recommended that an official policy should be provided to achieve these goals in perioperative nursing.

임상 간호사의 혈액매개감염 주의지침에 대한 수행정도 (A Study on Performance Level for Universal Precautions on Blood-Borne Infections among Nurses in Hospitals)

  • Purpose: This study was conducted to investigate the cognition level and the performance level of universal precautions on blood-borne infections and to analyze related-factors on the performance level for universal precautions among the nurses. Method: A total of 166 nurses participated in the survey. We performed multiple linear regression to assess the related-factors on the performance level for universal precautions. Result: The mean scores of cognition and performance level for universal precautions were 65.61(${\pm}8.74$) and 53.98(${\pm}8.42$) respectively. The mean score of the cognition level was significantly lower than the performance level in all items. The cognition level was associated with the working field. The performance level was highest for nurses working over 36 years, nurses in university hospitals, and nurses in the operating room. The cognitive level for universal precautions, education level, exposure to blood-borne infection, family's income, and marital status significantly predicted the performance level for universal precautions in the multiple linear regression model (Adjusted $R^2=0.42$). Conclusion: The cognition level for universal precautions is the strongest predictor on the performance level for universal precautions. An integrated approach should incorporate training experiences to improve cognition and perception of risk on universal precautions for preventing blood-borne infections.

정신적 병동 간호업무 개선에 관한 연구 (Studies on Improvement of the psychiatric Nursing)

  • Author studied on various psychiatric nursing problems applying with the test of questionnaire to the 376 nurses who are attending at 13 general hospitals and one mental hospital in Korea from Apr. 1, 1974 to July 30. 1974. The results obtained were ai follows: 1. 13.095 of nonpsychiatric registered nurses wanted the psychiatric service for their most attractive assignment and this was the 3rd in order next to the general surgical and operating room. Among the psychiatric nurses, the popularity toward psychiatric ward was 31. 0% and they were 26.0%at the private general hospitals and 6, 0% in national public hospitals. 2, The feelings or attitudes of disgust and apprehension on nursing care of the patients at-flirted with infectious diseases were the highest responses (38.5%) and these 1.ends were also appeared in 6.9% of psychiatric nurses. 3. 85.5% of nonpsychiatric registered nurses have had received course lectures on psychiatric nursing and nursing care training on the psychiatric ward at their school of nursing. 38.0% of psychiatric nurses had received post graduate psychiatric nursing and they were higher in national public hospitals (27.0% ) than in private general hospitals (11.0%). 4. The responses of satisfaction and security on their employment were almost similar patterns between nonpsychiatric registered nurses and psychiatric nurses. But among the psychiatric nurses. they were more satisfied at private general hospitals (33.0%) than that of national public hospitals (10.0%). 5. Almost the half of the nurses (50.8%) were employed by the hospitals without considering their past educational or clinical experience or career. Among the psychiatric nurses, who were employed by hospitals without considering their past experience or career were 35.0% in national public hospitals and 12.0% in private hospitals. On the contrary, the nurses who were employed by their wishes fore more higher (26.0%) in private general hospitals than national public hospitals (2.0%). 6. The nurses who thought their employment was fit for their aptitude were 48.6% in nonpsychiatric registered nurses and 51.0% in psychiatric nurses. Among the psychiatric nurses, this response was higher in private general hospitals (34.0%) than the national public hospitals (17.0%). 7. Responses on wort loadings of nurses showed almost same patterns between nonpsychiatric registered nurses and psychiatric nurses. But. among the psychiatric nurses who felt much heavier than the nurses of other part were 24.0% in national public hospitals and 8.0% in private general hospitals. 8. 92.6% of nurses felt that the psychiatric nurses should have post graduate training in psychiatric nursing prior to their assignment. 9. 96.0% of nurses agreed to the risk coverage on the payment for the nurses assigned to the psychiatric ward and the ward for infection diseases.

종합병원 간호사들의 피로수준 및 관련요인 (Fatigue Symptoms and Its Related Factors among General Hospital Nurses)

  • 본 연구는 종합병원 간호사들을 대상으로 특수부서 근무 간호사(수술실, 중환자실, 응급실)와 병동 근무 간호사의 피로수준을 비교하고 그의 관련요인을 파악하기 위해 2008년 8월에 대전광역시에 소재하고 있는 400병상 이상의 3개 종합병원에 근무하는 여성 간호사 393명을 대상으로 자기기입식설문조사를 실시하였다. 연구결과 평균 피로점수는 특수부서 근무 간호사가 병동 근무 간호사보다 높았다. 다중회귀분석결과 피로수준에 영향을 미치는 관련 요인으로는 특수부서 근무 간호사의 경우 주관적 건강 상태, 연령, 간호직 생활에 대한 만족도가 주요 설명변수로 선정되었으며, 병동 근무자의 경우는 주관적 건강 상태, 평소 수면 시간, 교대 근무, 간호직 생활에 대한 만족도가 주요설명변수로 선정되었다. 위와 같은 결과를 볼 때, 특수부서 근무 간호사는 병동근무간호사에 비해 피로수준이 높은 것을 알 수 있으며, 특수부서 근무간호사와 병동 근무간호사의 피로에 영향을 미치는 관련 요인에도 차이가 있음을 알 수 있다.
