Helping Health Care Providers Recognize and Respond to Sensitive Issues

  • Sensitive issues are both common and problematic for health care providers because sensitive issues may interfere with the future provider-client relationship and effective care. Most current training for providers focuses on a particular issue, but this is inadequate because many issues may be sensitive, and which issues will be sensitive is unpredictable. We argue that issues become sensitive when they activate one or more of three common triggers, fear, stigma, and taboo. A cycle of negative internal and interpersonal responses to the sensitive issue often leads to unresolved health issues for clients and stress and feelings of inadequacy for providers. We recommend integrated pre-service and in-service skill building to help individual health care providers respond appropriately to a wide variety of sensitive issues. We also identify specific policies and procedures to strengthen organizational support for caregivers so that providers can address these sensitive issues effectively with their clients.

The Development of a Sliding Joint for Very Flexible Multibody Dynamics (탄성 대변형 다물체동역학을 위한 슬라이딩조인트 개발)

  • In this paper, a formulation for a spatial sliding joint, which a general multibody can move along a very flexible cable, is derived using absolute nodal coordinates and non-generalized coordinate. The large deformable motion of a spatial cable is presented using absolute nodal coordinate formulation, which is based on the finite element procedures and the general continuum mechanics theory to represent the elastic forces. And the non-generalized coordinate, which is neither related to the inertia forces nor external forces, is used to describe an arbitrary position along the centerline of a very flexible cable. In the constraint equation for the sliding joint, since three constraint equations are imposed and one non-generalized coordinate is introduced, one constraint equation is systematically eliminated. Therefore, there are two independent Lagrange multipliers in the final system equations of motion associated with the sliding joint. The development of this sliding joint is important to analyze many mechanical systems such as pulley systems and pantograph/catenary systems for high speed-trains.

  • 미국에서는 1998년 워싱턴주 시애틀에서 '만약 시애틀 전체 시민이 같은 책을 읽는다면 (if All of Seattle Read the Same Book)${\cdots}$'이라는 프로젝트로 시작한 '한 도시, 한 책' 운동이 2001년 일리노이주 시카고에서 '한 책, 한 시카고(One Book, One Chicago)'로 본격화되었다. 국내에서는 2003년 충남 서산에서의 '한 책' 시범 운영 후 김해와 순천, 부산, 청주, 대전, 용인, 부천 등 여러 도시에서 '같이 또 다르게' 진행되고 있다. 김해에서도 2007년 '책 읽는 도시-김해'를 선포하면서 시작한 "김해의 책"이 올해로 5년째를 맞는다. 그간의 활동을 정리하며 "김해의 책"이 갖는 비전과 사회적 책읽기로서의 가능성에 대해 전망해 본다.

Comparison of Benefit Estimation Models in Cost-Benefit Analysis: A Case of Chronic Hypertension Management Programs

  • Purpose: Cost-benefit analysis is one of the most commonly used economic evaluation methods, which helps to inform the economic value of a program to decision makers. However, the selection of a correct benefit estimation method remains critical for accurate cost-benefit analysis. This paper compared benefit estimations among three different benefit estimation models. Methods: Data from community-based chronic hypertension management programs in a city in South Korea were used. Three different benefit estimation methods were compared. The first was a standard deterministic estimation model; second, a repeated-measures deterministic estimation model; and third, a transitional probability estimation model. Results: The estimated net benefit of the three different methods were $1,273.01, $-3,749.42, and $-5,122.55 respectively. Conclusion: The transitional probability estimation model showed the most correct and realistic benefit estimation, as it traced possible paths of changing status between time points and it accounted for both positive and negative benefits.

Health Problems and Health Services Utilization of Infants Born Prematurely in the U.S.

  • Purposes: The purposes of this study are to describe the health problems experienced by VLBW premature infants and their health care services utilization during the first year of life Method: Eighteen mothers of VLBW premature infants completed a survey questionnaire, asking socioeconomic/demographic information, health/developmental problems experienced by their infants, and their use of health care services. Results: Of the 18 infants, 78% experienced respiratory problems such as cold/running nose and wheezing during the first year and 33% experienced gastrointestinal problems such as vomiting, diarrhea and constipation. Twelve (67%) infants visited the emergency department at least one time and 10 infants (56%) were hospitalized at least once during their first year of age. Interestingly, infants without chronic lung disease visited the emergency department more than infants without chronic lung disease (p=.213). Infants living in non-disadvantaged neighborhoods were hospitalized more than infants living in disadvantaged neighborhoods (p=.000). Conclusions: Health care providers should initiate educating mothers, particularly those living in disadvantaged neighborhoods, about post-NICU discharge health care needs of their VLBW premature infants while their infants were still in the NICU so that unnecessary visits to the emergency department and rehospitalizations can be possibly prevented.

  • From 1876 to 1883, British mathematician James Joseph Sylvester worked as the founding head of Mathematics Department at the Johns Hopkins University which has been known as America's first school of mathematical research. Sylvester established the American Journal of Mathematics, the first sustained mathematics research journal in the United States. It is natural that we think this is the most exciting and important period in American mathematics. But we found out that the International Congress of Mathematicians held at the World's Columbian Exposition in Chicago, August 21-26, 1893 was the real turning point in American's dedication to mathematical research. The University of Chicago was founded in 1890 by the American Baptist Education Society and John D. Rockefeller. The founding head of mathematics department Eliakim Hastings Moore was the one who produced many excellent American mathematics Ph.D's in early stage. Many of Moore's students contributed to build up real American mathematics research power in early 20 century The University also has a well-deserved reputation as the "teacher of teachers". Beginning with Sylvester, we analyze what E.H. Moore had done as a teacher and a head of the new department that produced many mathematical talents such as L.E. Dickson(1896), H. Slaught(1898), O. Veblen(1903), R.L. Moore(1905), G.D. Birkhoff(1907), T.H. Hilderbrants(1910), E.W. Chittenden(1912) who made the history of American mathematics. In this article, we study how Moore's vision, new system and new way of teaching influenced American mathematical society at early stage of the top class mathematical research. and the meaning that early University of Chicago case gave.

Development of One-to-One Shortest Path Algorithm Based on Link Flow Speeds on Urban Networks (도시부 가로망에서의 링크 통행속도 기반 One-to-One 최단시간 경로탐색 알고리즘 개발)

  • Finding shortest paths on time dependent networks is an important task for scheduling and routing plan and real-time navigation system in ITS. In this research, one-to-one time dependent shortest path algorithms based on link flow speeds on urban networks are proposed. For this work, first we select three general shortest path algorithms such as Graph growth algorithm with two queues, Dijkstra's algorithm with approximate buckets and Dijkstra's algorithm with double buckets. These algorithms were developed to compute shortest distance paths from one node to all nodes in a network and have proven to be fast and efficient algorithms in real networks. These algorithms are extended to compute a time dependent shortest path from an origin node to a destination node in real urban networks. Three extended algorithms are implemented on a data set from real urban networks to test and evaluate three algorithms. A data set consists of 4 urban street networks for Anaheim, CA, Baltimore, MD, Chicago, IL, and Philadelphia, PA. Based on the computational results, among the three algorithms for TDSP, the extended Dijkstra's algorithm with double buckets is recommended to solve one-to-one time dependent shortest path for urban street networks.

  • The gating kinetics of an inward-rectifier K$\^$+/ channel, ROMK2 (Kir1.lb), were described by a model having one open state and two closed states. The long closed state was abolished by EDTA, suggesting that it was due to block by divalent cations. These closures exhibit a biphasic voltage-dependence, implying that the divalent blockers can permeate the channel.(omitted)

  • Purpose: The aim of this study was to evaluate the efficiency of community visiting health service units using DEA and to compare the results with those of traditional evaluation methods. Methods: Data were collected using 2008 annual reports of 10 districts in one city. Input variables were number of staff and operational budget per year and output variables were number of managed household per nurse, rate of controlled hypertension, and diabetes mellitus. EMS Window version 3.1 was used to measure efficiency score and bootstrapping Chi-square test was applied to identify differences between efficient and non-efficient unit by organizational factors. Results: The average efficiency score of 10 community visiting health services was about 66%. Only two of the units operated program efficiently. The career of the staff was one among other factors associated with efficiency. The evaluation results from the traditional method and DEA were totally different. Conclusion: These results indicate that, evaluation methods have very important and significant effects on the evaluation results of community visiting health service units. The DEA method is recommend as an alterative method for evaluating community visiting health service.

  • The purpose of this study is to understand the value of the book, To Kill a Mockingbird by Harper Lee as the most favored 'One Book' and the goals of the 'One Book, One City' programs which selected this particular book. A total of 86 'One Book' programs from 2001 through 2014 and 138 The Big Read programs from 2007 through 2015 selected To Kill a Mockingbird as 'One Book' to read. With this book, many communities discussed the sympathy toward the dignity of human being with such subjects as courage, justice, and integrity, and presented its value through book discussions, various programs and events.
