A Comparison on the Characteristics of Benzene Exposure between the Coal chemical and Petrochemical refining method during Turnaround (석유 및 석탄화학의 대보수작업중 벤젠노출 특성 비교)

  • This study was conducted to investigate the benzene exposure levels in coal chemical and petrochemical refining industries during BTX turnaround (TA) processes where benzene was being produced. Three companies producing benzene were selected, one coal chemical and two petrochemical industries. TA processes were classified into three stages: shut down, maintenance, and start up. Data was analyzed by classifying the refining method into 2 groups (Petrochemistry, Coal chemistry) for 823 workers. Comparing the data from petrochemical industries with data from a coal chemical refining industry, while benzene concentration levels of long-term samples during TA were not statistically different (p> 0.05), those levels of short-term samples were significantly different (p< 0.001). About 4.79 % of data in petrochemical industries exceed the occupational exposure limits (OELs) of benzene, 1 ppm. In a coal refining methods, about 15.7% exceeded the benzene OELs. The benzene concentrations in maintenance and start up stage of TA for petrochemical refineries were higher than those in a coal chemical refinery (p <0.01). These findings suggest that the coal chemical refining site requires more stringent work practice controls compare to petrochemical refining sites during TA processes. Personal protective equipments including organic respirators should be used by TA workers to protect them from benzene overexposure.

A Survey on the Occupational Health Nursing Activities for primary care (산업간호사의 의료행위 직무지침서 활용 및 의료행위직무 수행실태)

  • The occupational health nursing guideline for primary care was developed by the Korean Academic Society of occupational health nursing and the organization for occupational health nurses (currently known as the Korean Association of Occupational Health Nurses) in 1993. Since then, there have been many changes in the health care environment and job performance of occupational health nurses. Appropriate revisions are necessary of the guidelinea based on this background. The purpose of this study was to describe the use of the occupational health nursing guideline for primary care and to analyze the characteristics of primary care activities by occupational health nurses. The questionnaire was mailed to 150 occupational health nurses(OHNs) with the response rate of 64%. The results can be summarized as follows; 1. 65.6% of OHNs have been using the guideline for primary care and 75.9% of them agreed that the guideline was be helpful for their job. 2. Common symptom care, emergency care and chronic illness care were more frequently implemented than occupational disease care by OHNs. In manufacturing industries, emergency care was more frequently implemented than chronic illness care in contrast to the service industries. 3. Most frequent common symptoms treated by OHNs were indigestion, diarrhea, abdominal pain, headache, and coughing. In the case of chronic illness, OHNs more frequently treated diseases of the gastro-intestinal system, skin and sensory organs, and the respiratory system. Emergency care for bruises, burn, and abrasions was more frequently provided. VDT syndrome was the most common occupational disease cared by OHNs in manufacturing and service industries. 4. OHNs prescribed the medicine for external application more frequently than internal medicine. Remedy for colds, analgesics, vitamins, and digestives were more frequently used. From these results, we suggest that the guideline should be revised to emphasize the activities consisting problem finding such as health assessment, physical examinations, monitoring and screening, and to renew the drug list in the range of over- the counter medication (OTC). In the future, the guideline will include the strategies for the role as the case manager.

The Lived Experience of Occupational Therapists in Public Health Centers Regarding Community Rehabilitation and Health Services: Phenomenological Study (보건소 작업치료사의 지역사회 재활 및 보건서비스 제공에 대한 업무 경험에 관한 현상학적 연구)

  • Objective : The purpose of this study was to examine the roles and practices of occupational therapists in public health centers based on their working experience of occupational therapists who are working in these facilities. Methods : We used Giorgi's descriptive phenomenological research method. Six occupational therapists were interviewed, and their recorded data were analyzed using Claizzi's thematic analysis. Results : As a result, 4 categories, 16 themes, and 24 elements were elicited. The major roles of occupational therapists in public health centers were the evaluation of clients, home-visit occupational therapy, management of dementia programs and community connection projects. Interventions that are needed to create a healthy community include case and program management, compensatory approaches, and healthy lifestyle interventions. The difficulties experienced by occupational therapists include their limited number job insecurity, performance-oriented administrative policy, and the lack of communication among professionals. Occupational therapists require education in health management and practice in public health centers to build their capacities. Conclusion : The results of this study could be used for guidelines for occupational therapists who work in community healthcare centers, as well as basic training for competent occupational therapists in the community.

An Analysis of Satisfaction with Job Ability Development Training among Dental Technicians (치과기공사의 직업능력개발훈련 만족도 분석)

  • Purpose: This study were to analyze satisfaction with job ability development training among Dental Technicians', examine the degree of their actual satisfaction with training courses and their status, and provide some basic data to help improve the quality level, satisfaction, and effective operation of job ability development training in the future. Methods: This study carried out self-administered questionnaire survey from January 15, 2011 to March 10 by having research subjects as 180 dental technicians who participated in the course of employees' Job Ability Development Training by 'Gil Occupational Training Institute,' which is being conducted as part of dental technicians' job ability development in Seoul region. Among these things, 160(88.8%) copies were collected. Except 19 copies with incomplete response, 141(78.3%) copies were used as the materials of final analysis. Results: First of all, as a result of grasping the whole satisfaction with training according to general characteristics, the significant difference was indicated in the whole satisfaction with training depending on gender & age, and voluntary participatory intention. Second, the satisfaction with contents of Job Ability Development Training was in order of helpful level in ability development with 4.10(${\pm}.70$), co-worker's recommendation and a plan for reparticipation with 4.08(${\pm}.77$), consistence with the content goal with 4.07(${\pm}.71$), and reflection of demand with 4.01(${\pm}.70$). Third, examining satisfaction with instructors of Job Ability Development Training, it is being indicated with a little higher score compared to the satisfaction with other items. Fourth, satisfaction with service of supporting learners in job ability development training was shown to be slightly low in all the items. Especially, the convenience of administrative procedure was indicated to be the lowest with 3.52(${\pm}.92$) Fifth, satisfaction with the facility of job ability development training was showing very low satisfaction compared to other satisfaction items. Conclusion: A continuous effort will be needed in order to be possibly strengthened dental laboratory technicians' professionalism and reinforced qualitative level in major ability through offering more educational opportunities and education in good quality.

An Example-based Korean Standard Industrial and Occupational Code Classification (예제기반 한국어 표준 산업/직업 코드 분류)

  • Coding of occupational and industrial codes is a major operation in census survey of Korean statistics bureau. The coding process has been done manually. Such manual work is very labor and cost intensive and it usually causes inconsistent results. This paper proposes an automatic coding system based on example-based learning. The system converts natural language input into corresponding numeric codes using code generation system trained by example-based teaming after applying manually built rules. As experimental results performed with training data consisted of 400,000 records and 260 manual rules, the proposed system showed about 76.69% and 99.68% accuracy for occupational code classification and industrial code classification, respectively.

  • Complex Regional Pain Syndrome(CRPS) is a chronic neruopathic pain syndrome, most frequently encountered after trauma, nerve injury, fracture, cerebral vascular accident(CVA), spinal injury, and surgery to a limb, and affected by diverse factors such as disease process, intervention method, and other biopsychosocial components. Occupational therapy as a multidisciplinary treatment approach may be potentially useful when simultaneously targeting such biological, psychological, and social components. Biological treatment can focus on increasing functional use of extremity for promoting independence. Psychological intervention can include relaxation/biofeedback training and cognitive-behavioral treatments, and social approach can focus on recreational therapy and vocational rehabilitation. Occupational therapy will encourage functional restoration, pain management, and regaining of psychological components with collaboration between different disciplines.

Influencing Factors of Empathic Competence and Occupational Stress of Nurses on Their Ego-resilience (간호사의 공감역량과 직무스트레스가 자아탄력성에 미치는 영향)

  • Purpose: This descriptive survey study assesses the extent of empathic competence, occupational stress, and ego-resilience of nurses, and confirms factors that affect ego-resilience in order to provide basic data for the competency development program for nurses. Methods: Data obtained from the questionnaire survey conducted with 193 nurses were analyzed by means of t-test, ANOVA, and Pearson correlation coefficient and Multiple Regression. Results: Empathic competence, in accordance with the general characteristics of nurses, displayed statistically significant differences in terms of final academic background. Further, occupational stress and ego-resilience displayed statistically significant differences related to the nurses' department of employment, age, and total clinical experiences, respectively. Empathic competence and ego-resilience have a significant positive correlation, and age and empathic competence were found to be the factors that impart influence on ego-resilience. Conclusion: The enhancement of empathic competence of nurses has affirmative effects on the satisfaction of patients with their nurses, and it also improves the ego-resilience of nurses. Therefore, it is necessary to develop and apply career training program aimed at enhancing the empathic competence of nurses as an in-service educational program.

Musculoskeletal Model for Assessing Firefighters' Internal Forces and Occupational Musculoskeletal Disorders During Self-Contained Breathing Apparatus Carriage

  • Background: Firefighters are required to carry self-contained breathing apparatus (SCBA), which increases the risk of musculoskeletal disorders. This study assessed the newly recruited firefighters' internal forces and potential musculoskeletal disorders when carrying SCBA. The effects of SCBA strap lengths were also evaluated. Methods: Kinematic parameters of twelve male subjects running in a control condition with no SCBA equipped and three varying-strapped SCBAs were measured using 3D inertial motion capture. Subsequently, motion data and predicted ground reaction force were inputted for subject-specific musculoskeletal modeling to estimate joint and muscle forces. Results: The knee was exposed to the highest internal force when carrying SCBA, followed by the rectus femoris and hip, while the shoulder had the lowest force compared to the no-SCBA condition. Our model also revealed that adjusting SCBA straps length was an efficient strategy to influence the force that occurred at the lumbar spine, hip, and knee regions. Grey relation analysis indicated that the deviation of the center of mass, step length, and knee flexion-extension angle could be used as the predictor of musculoskeletal disorders. Conclusion: The finding suggested that the training of the newly recruits focuses on the coordinated movement of muscle and joints in the lower limb. The strap lengths around 98-105 cm were also recommended. The findings are expected to provide injury interventions to enhance the occupational health and safety of the newly recruited firefighters.

Automated Phase Identification in Shingle Installation Operation Using Machine Learning

  • Roofers get exposed to increased risk of knee musculoskeletal disorders (MSDs) at different phases of a sloped shingle installation task. As different phases are associated with different risk levels, this study explored the application of machine learning for automated classification of seven phases in a shingle installation task using knee kinematics and roof slope information. An optical motion capture system was used to collect knee kinematics data from nine subjects who mimicked shingle installation on a slope-adjustable wooden platform. Four features were used in building a phase classification model. They were three knee joint rotation angles (i.e., flexion, abduction-adduction, and internal-external rotation) of the subjects, and the roof slope at which they operated. Three ensemble machine learning algorithms (i.e., random forests, decision trees, and k-nearest neighbors) were used for training and prediction. The simulations indicate that the k-nearest neighbor classifier provided the best performance, with an overall accuracy of 92.62%, demonstrating the considerable potential of machine learning methods in detecting shingle installation phases from workers knee joint rotation and roof slope information. This knowledge, with further investigation, may facilitate knee MSD risk identification among roofers and intervention development.

The Effect of Virtual Reality Rehabilitation Program on Upper function, Cognition and Activity of Daily of Living in the with Stroke Patients (가상현실재활프로그램이 뇌졸중 환자의 상지기능, 인지와 일상생활활동에 미치는 효과)

  • The purpose of this study was to investigate the effects of a virtual reality rehabilitation program on upper limb function, cognition, and activities of daily living in stroke patients. Among the 25 participants in the program, it was randomly classified 13 experimental group and 12 control group. Rehabilitation training was applied 3 times a week and general occupational therapy 2 times a week using virtual reality rehabilitation training equipment in the experimental group, and general occupational therapy was performed 5 times a week, 30 minutes per session in the control group. As a result of the virtual reality rehabilitation program, cognitive function increased by 3.39 points in the experiment group, The control group who received only general occupational therapy increased by 1.5 points. As for the upper limb function, the average of the experimental group subjected to the virtual reality rehabilitation program increased by 4.84 points The control group who received only general occupational therapy increased by 1.34 points. As for activities of daily living, the average of the experimental group that conducted the virtual reality rehabilitation program increased by 20.38 points, The control group who received only general occupational therapy increased by 7.61 points. This result show that the virtual reality program has an effect on upper limb function, cognition, and activities of daily living of stroke patients.
