A Study on Performance and Achievement of Village Health Workers in Rural Primary Health Care Program (농촌 일차 보건사업에 있어서 마을건강원 업무량 및 업적에 관한 연구)

  • It is utmostly important to establish the efficient fitable way of peoples' active participation in primary health care especially in the areas where the public or governmental service input for the basic health care is insufficient like as in rural areas of Korea. In light of above reason, this study focused mainly on the evaluation of roles and activities of village health workers (VHWs) who were selected from grass- root level of village people in order to derive further motivation for active participation. This is believed to be a sort of feedback mechanisms. Actually, the authors collected the activity reports of VHWs who had been devoting themselves in the primary health care services of Jeomdong Area, of Yeoju Gun one of Korea University Community Health Action Programmes and survey record on the VHWs activity from correspondent people. 1 hose data were analyzed through computer programmed package. The activities performed by VHWs were limited to the performance in 1985 for conveniance. The summarized results were as follows; 1) General characteristics of VHWs. Among a total of 28 VHWs in the area, about 39.3g of them have been replaced up to the date since the implementation in 1983, because of moving out, occupational employment and of others. The age of majority (75.0%) lied between the range of 30-50, and educational background of 67.9% belonged to category of primary school graduation, about 50% of them experienced to be or were also entiled "chief of women club" of corresponding villages. 2) Work-load of VHWs. Each VHW was assigned for tasks of health care for average 55 households of 248 persons. They shared approximately 6 days a month for the activity in average and it covered 17 cases of basic health care in a month. A half of the VHWs performed home visits irregularly without solidified schedule. 3) Work performance analysis. Informations collected through VHWs were compared with data from official vital registration at local administration center "Myon Office" in 1985. VHWs collected 100.8 of new born, 116.2 of death, 58.3 of move in and 74.8 of move out in comparison with 100.0 of official registration each. Pregnant women of 79.8% of mothers among the total pregnancy of 94 which were confirmed as normally delivered or aborted cases by all means afterwards had been detected by VHWs as being pregnant and all of them received some of antenatal cares by VHWs. All(100%) of delivered women were detected by VHWs through home visits and they were cared postnatally. Whereas, according to the records of birth registration, the places of delivery were clinic in 33.7%, and mother's home in 66.3%, VHWs reported them to be clinic in 48.9%, midwifery in 20.2%. It was cleared that most of misinformation was caused by uncautious filling of birth registration at notification. Among the total of 717 eligible women under age 44 years, family planning status of 92.6% was reported by VHWs confirming practice of control to be 70.8% of reported fertile women. 4) Attitude of VHW on the roles and functions. Although 92.0% of VHWs expressed VHWs to be worthwhile, only 52.0% of them had dignity and satisfaction in their activity and 44.0% of them had passive attitude of working saying they followed direction regardlessly. Concerning difficulties in performance as a VHW, 60.7% of them pointed out lacking of medical and health related knowledge by themselves. Still, 64.0% of them thought visiting unfamilier house to be awful and 40.0% complained forms of activity to be difficult and hard. It was also revealed that 56.6% confessed lack of interest on community health service itself. Most of VHWs needed more educational training especially on clinical fields such as cares of gynecological diseases, hypertension, diabetes, and other chronic diseaes of the aged. Regular on-the-job basic trainings were said to be needed twice a year.

A Study of Test-Retest Reliability and Interrater Reliability of the Sensory Processing Scale for Children (SPS-C) (아동감각처리척도(Sensory Processing Scale for Children; SPS-C)의 검사-재검사 신뢰도와 검사자간 신뢰도 연구)

  • Objective : This study examined the test-retest reliability and interrater reliability of the Sensory Processing Scale for Children (SPS-C). Method : Senventy primary caregivers of children with sensory processing difficulties and 3 years old participated in the study. The subjects were recruited through child development centers, welfare centers, and acquaintances located in Seoul, Gyeonggi-do, Busan, and Gyeongsang-do. The test-retest reliability verification targeted 20 main caregivers of children with difficulty in sensory processing. Re-evaluation was performed within 7 to 14 days after the initial evaluation, and Pearson's correlation coefficient was used to confirm the relevance between the two time points, and the Intraclass correlation coefficient was used to confirm the degree of agreement. The interrater reliability verification was conducted with 18 primary caregivers and 18 subsidiary caregivers of children with sensory processing difficulties. Each caregiver evaluated the same child, and the Intraclass correlation coefficient was used to confirm the agreement between the two sets of caregivers. Results : The test-retest reliability was Pearson's correlation coefficient r=.914 and intraclass correlation coefficient ICC=.939, indicating a high level of relevance and agreement. The interrater reliability was an Intraclass correlation coefficient ICC=.727, which showed a moderate level of agreement, but the tactile area (ICC=.455) and proprioceptive area (ICC=.439) were not statistically significant and showed a low degree of agreement. Conclusion : Through this study, it was confirmed that the children's Sensory Processing Scale for Children (SPS-C) is a stable evaluation tool with test-retest reliability and interrater reliability verified, and it will be able to provide help in standardization studies for future clinical use.

Rapid HPLC Method for the Simultaneous Determination of Eight Urinary Metabolites of Toluene, Xylene and Styrene

  • Toluene, xylene and styrene are volatile organic solvents that are commonly used in mixtures in many industries. Because these solvents are metabolized and then excreted in urine, their urinary metabolites are thought to be biomarkers of occupational exposure to these solvents. Therefore, a simple, rapid, and yet reliable analytical method for determining the metabolites is required for accurate biological monitoring. In the present study, a simple and rapid HPLC-UV method was developed for the simultaneous determination of eight major metabolites of toluene, xylene and styrene: hippuric acid (HA), mandelic acid (MA), o-, m- and p-methylhippuric acids (o-, m- and p-MHAs), and o-, m- and p-cresols. A monolithic column was employed as the stationary phase and several conditions, including flow rate, composition of mobile phase and column temperature, were variables for the optimization of the chromatographic resolution. All eight metabolites were successfully resolved within 5 minutes in 10% aqueous ethanol containing 0.3% acetic acid and 1.6% $\beta$-cyclodextrin, using a flow rate gradient of 1.0 - 5.0 mL/min at 25 ${^{\circ}C}$. The performance of this method was validated by linearity, intra- and inter-day accuracy, and precision. The linearity was observed with correlation coefficients of 0.9998 for HA, 0.9999 for MA, 0.9989 for o-MHA, 0.9998 for m-MHA, 0.9991 for p-MHA, 0.9997 for o-cresol, 0.9998 for m-cresol, and 0.9986 for p-cresol. The intra- and inter-day precision of the method were less than 5.89% (CV) and the accuracy ranged from 92.95 to 106.62%. The validity was further confirmed by analysis of reference samples that were prepared by the inter-laboratory quality assurance program of the Korea Occupational Safety and Health Agency (KOSHA, Seoul, Korea). All measured concentrations of the analytes agreed with the certified values.

A Systematic Review and Meta-analysis of Sensory Integration Intervention Studies in Children with Cerebral Palsy (뇌성마비 아동의 감각통합 중재 연구에 대한 체계적 고찰과 메타분석)

  • The purpose of this study was to investigate evidence-based practice (EBP) for sensory integration (SI) intervention in children with cerebral palsy (CP) through a systematic review and meta analysis. The screening strategy was performed to select studies for analysis after that, a meta-analysis was implemented for calculating the effect size (ES) in group studies. Twenty-four studies were included for a systematic review and included seven case reports, three single-subject designs, and fourteen group experimental design studies(three randomized controlled trials, three two groups nonrandomized studies, and six one group nonrandomized studies). The ES of the experimental group studies was moderate size of 0.272. The results of the ES according to the dependent variables, the ES was the largest in the fine motor development. The effect size of the published papers was greater than the unpublished paper's and two groups nonrandomized studies' size effect was the largest in the design. The ES of the infants was larger than the children. The ES for a period of 8 weeks, the number of five times a week, and time in 90 minutes showed the biggest in SI program. Although the ES of SI intervention in children with CP showed moderate effect, accumulation of research well be needed.

The Intervention and Outcome Measurement Tools of Children With Attention Deficit Hyperactivity Disorder: A Systematic Review (주의력결핍과잉행동장애 아동의 중재방법과 측정도구에 관한 체계적 고찰 : 국제 기능.장애.건강 분류 모델의 분류기준에 근거하여)

  • Objective : The purpose of this study was to provide for the intervention and outcome measurement tools of children with Attention Deficit Hyperactivity Disorder (ADHD) through ICF model. Methods : The systematic review methods were used. Papers published in the journal between January, 2000 and July, 2014 were searched through MEDLINE/PubMed, Sciencedirect, Ovid. The main terms searched were 'ADHD, Children, intervention, outcome measure', and 8 papers were analyzed. Results : 1. The subjects of ADHD were pure ADHD (75.8%), ADHD with dyslexia (9.1%), ADHD with conduct disorder (5.8%), ADHD with tic disorder (3.8%), ADHD with DCD (3.0%), ADHD with emotional disorder (2.5%). 2. The nonpharmacologic intervention of ADHD were functioning and disability (80%) and contextual factors (20%). Most frequently used intervention were body function and structure (60%). 3. The outcome measurement tools of ADHD were functioning and disability (80.5%) and contextual factors (19.5%). Most frequently used outcome measurement tools were body function and structure (70.8%). Conclusion : This study can provide information on the intervention and outcome measurement tools of ADHD.

A Study on Ethical Values and Professionalism of Dental Hygienists (치과위생사의 윤리적 가치관과 전문 직업성에 대한 인식 조사 연구)

  • This study conducted a survey of 265 dental hygienists working in Seoul and Gyeonggi Province from January 10 to February 28, 2021 to understand the ethical values and professional occupational characteristics of clinical dental hygienists. The difference in ethical values and professional occupational perception between subjects was post-tested using SPSS WIN 20.0 program and Scheffe test using independent sample T-test and one-way ANOVA. According to the results of the study, ethical thinking in ethical values was statistically significant in dental hospitals with higher final education, more work experience, and ethical work experience (p<0.05). The higher the age, the higher the final education, the more ethical education experience at university, the higher the sense of mission, the fewer working days, the more satisfying the job, the higher the organizational compliance with professional organizations and public service (p<0.05). The results of this study confirm the necessity of regular ethics education such as continuing education for dental hygienists, and are expected to be used as basic data to maintain ethical values and professionalism in clinical practice.

A Convergence Study on Stress, Peer Relationships, and Depression according to Differences in Academic Performance and household Income of Youth in Coal Mine Areas (탄광지역 청소년의 학업성적 및 가구소득 차이와 스트레스, 교우관계 및 우울의 융복합 상관 연구)

  • This study investigated a differences in grade (G), academic performance (AP), and household income (HI) on adolescents' stress (S), peer relationship (PR), and depression (D) in the coal mine area. The study method was to measure S, PR and D according to different of G, AP, HI for 87 middle school students located in Taebaek and Samcheok Dogye area. As a result, there were significant differences in PR in the 2nd G, and significant differences were shown in PR and D according to HI and AP. Based on these results, it is necessary to expand the support program for PR of middle school students in the coal mine area, and to develop and support active health promotion programs in the community for stress relief and academic activities, especially for low HI.

Workplace influenza vaccination in private hospital setting: a cost-benefit analysis

  • Background: Influenza illness causes several disruptions to the workforce. The absenteeism that often ensues has economic implications for employers. This study aimed to estimate the cost-benefit of influenza vaccination in a healthcare setting from the employer's perspective. Methods: A cross-sectional questionnaire survey was conducted in a private hospital in 2018-2019 comparing voluntary vaccinated with non-vaccinated employees with influenza vaccine. The analyses were made based on self-reporting on absenteeism and presenteeism from Influenza-like illnesses (ILIs). The costs incurred, both direct and indirect costs, were included in the study. A cost-benefit analysis was performed by measuring the cost of the vaccination program. The costs of absenteeism and reduced productivity were calculated using 3 hypothesised levels of effectiveness in the following percentage of productivity of 30%, 50%, and 70%. The costs were also calculated based on four scenarios: with and without operating income and with and without replacement. The benefits of the influenza vaccination from the employer's perspective were analysed. The benefit to cost ratio was determined. Results: A total of four hundred and twenty-one respondents participated. The influenza vaccination rate was 63.0%. The rate of ILI of 38.1% was significantly lower among vaccinated. The ILI-related absenteeism reported was also significantly lower amongst vaccinated employees at 30% compared to 70% non-vaccinated. Employers could save up to USD 18.95 per vaccinated employee when only labour cost was included or 54.0% of cost savings. The cost-saving rose to USD 155.56 when the operating income per employee was also included. The benefit to cost ratio confirmed that the net cost-benefit gained from the vaccination was more than the net cost of vaccination. Conclusions: Influenza vaccination for working adults was cost-saving and cost-beneficial when translated into financial investments for the employer. A workplace vaccination demonstrates a significant cost-benefit strategy to be applied in any institutional setting.

The Effect of Job Stress and Depression on Job Satisfaction among Workers in Small and Medium Sized Enterprises (중소기업 근로자의 직무스트레스와 우울이 직무만족에 미치는 효과)

  • This study is to verify the mediating effect of depression between job stress and job satisfaction among workers in small and medium sized enterprises. The survey was conducted to workers in small and medium sized enterprises, and 117 cases were finally applied. As results, job stress has significantly negative effect on job satisfaction and positive effect on depression. Depression also has partial mediating effect on between job stress and job satisfaction. We finally suggest that customized management system should be provided for the workers in small and medium-sized enterprises, and occupational welfare system, including employee assistance program (EAP), must be launched for these workers.

Responsiveness Comparisons of Self-Report Versus Therapist-Scored Functional Capacity for Workers With Low Back Pain

  • The primary aim of this study was to compare responsiveness of self-report by worker and therapist-scored functional capacity instrument. Self-report and therapist-scored interval-level person measures and item difficulties were compared at admission and discharge. Therapist and worker ratings were collected on 230 clients from 27 rehabilitation sites using the newly developed Occupational Rehabilitation Data Base (ORDB) functional capacity instrument. ORDB comprises several subscales measuring relevant variables of "a return-to-work model" in work-related rehabilitation clinics. The functional capacity scale deals with 10 DOT job factors. The rating scale categories were 1-severely impaired, 2-moderately impaired, 3-mildly impaired, and 4-not impaired. Only data from clients with low back pain (n=98) with complete data (both admission and discharge scores) were used for the present study. Therapists and workers completed the functional capacity instrument at admission and discharge. Rasch analysis [1-parameter item response theory model (IRT)] was applied to calibrate item difficulty and person ability measure of therapist and workers ratings. Effect sizes for therapist and self-report ratings were slightly different, .69 and .30, respectively. Therapist and worker ratings were more consistent at discharge (r=.54) than at admission (r=.32). Workers have a tendency to be more severe in their ratings (show higher item difficulties) than therapists at admission and discharge. Therapists and workers report similar magnitudes of improvement following treatment program. These findings challenge the belief that injured workers may unreliable source for monitoring therapeutic outcomes. Self-report measures have the advantage of conserving therapist time for treatment (versus evaluation). While the therapist and self-report ratings are comparable at discharge, there is less consistency at admission. Comparable therapist-worker ratings may be achieved by controlling for rating severity using IRT methodologies.
