  • Korean industry has achieved dramatic growth in a short time through continuous technical innovations. However, safety consciousness and management have not kept pace with industry growth. The characteristics of industrial accidents in socio-technical systems have become so complicated that it has been challenging for safety professionals to implement effective preventive measures because it is difficult to determine what is happening in the complex workplace. Thus far, they have focused on technical improvements and the requirements of OSH regulations to avoid legal responsibility. Accordingly, this study has used the systemic analysis method to explain the emergence of something unexpected in complex and multi-layered business structures, which is usually related to the variabilities of humans and organizations. This study chose a shipyard where a few fatal accidents had occurred because the shipbuilding industry includes numerous variabilities, including obtaining orders, manpower supply, procurement, etc. Systemic analysis progressed using FRAM based on two accident cases related to the truss platform used in LNG vessels. The outcome shows that each function within the system has its variabilities and has become coupled or dependent on other functions, increasing the possibility of accidents. This analysis method can provide OSH professionals with practical techniques for explaining what is happening in a complex socio-technical system and how to take proper measures.

노인일자리 전담기관 참여자의 위험사고 발생가능성과 대응 전략: 업무상 위험사고와 코로나-19 위험사고를 중심으로 (Possibility of Risk Accidents and Response Strategies for Participants in Agencies in Charge of Employment for the Elderly: Focusing on occupational risk accidents and COVID-19 risk accidents)

  • 본 연구는 노인일자리 전담기관인 시니어클럽에 활동하고 있는 참여 노인을 대상으로 업무상 위험사고와 코로나-19로 인한 위험사고 발생 가능성을 실증 분석하여 위험사고에 대한 인식 정도를 파악하였다. 연구 결과를 바탕으로 노인일자리 전담기관에서 발생할 수 있는 위험사고 개선방안 마련을 위한 기초자료 제공과 위험사고 예방을 위한 대응방안을 제시하는 것이 목적이다. 연구 대상과 자료수집은 부산광역시 소재 16개 시니어클럽 참여 노인으로 하였고, 수거된 430부 중 최종적으로 425부를 활용하였다. 본 연구 결과 첫째, 노인일자리 전담기관 참여 노인은 업무상 위험사고 발생 가능성과 코로나-19로 인한 위험요인 발생 가능성은 대체적으로 낮게 인식하고 있는 것으로 나타났다. 둘째, 인구사회학적 특성에 따른 업무상 위험사고 발생 가능성은 참여기간에서 통계적으로 유의미한 차이가 있는 것으로 나타났고, 코로나-19와 위험사고 발생 가능성은 참여노인 구분과 연령에서 통계적으로 유의미한 차이가 있는 것으로 나타났다. 본 연구 결과를 바탕으로 사업담당자는 안전사고 관리기록지 작성, 위험관리 매뉴얼 마련, 참여자의 개인적 특성을 반영한 위험관리 시스템 구축 등을 제언하였다.

적시기술지도 시범사업의 현황 및 성과분석 (A Study on the Present Condition and Performance Analysis for the Timely Technical Guidance Project)

  • More than 76% of occupational accidents occur in small and medium sized enterprises(SME) employing less than 50 workers, due to the lack of safety and health systems, and carelessness of workers and owners. Timely Technical Guidance(TTG) project of Korea Occupational Safety & Heath Agency(KOSHA) for SME was initiated to analyze and inform accident causes just after accidents occur. Improvement direction of TTG project is suggested through situation and performance analysis on TTG services conducted in 2012. KOSHA provided TTS services on 15,571 SMEs(suffering at least one accident in 2012), through which the working day losses from recurrence resulted in 19.0%p, 14.5%p and 31.3%p decreases in manufacturing, services and construction businesses, respectively.

Can We Reduce Workplace Fatalities by Half?

  • Singapore, an island republic of over 5 million inhabitants, has 3.1 million workers. Most are employed in the service, finance and tourist/transport industry. Significant numbers work in manufacturing, construction and heavy industry. Following a series of construction and shipyard accidents with multiple deaths in 2004, the government announced its intention to reduce workplace fatalities from 4.9 to 2.5 per 100,000 by 2015. There was strong political will to achieve this target. The strategic approaches were to build workplace safety and health (WSH) capabilities; implement legislative changes with enforcement; promote benefits of WSH and recognize best practices, and enhance partnership with stakeholders. The anticipated outcomes were to reduce workplace fatality and injury rates; have WSH as an integral part of business; and establish a progressive and pervasive WSH culture. With these measures, the workplace fatality rate declined from 4.9/100,000 in 2004, to 2.2/100,000 in 2010. However, other confounding factors could also account for this decline, and have to be considered. The next target, announced by Singapore's Prime Minister in 2008, is to further reduce the workplace fatality rate to 1.8/100,000 by 2018, and to have "one of the best workplace safety records in the world".

Workflow Interruptions and Failed Action Regulation in Surgery Personnel

  • Background: Workflow interruptions during surgery may cause a threat to patient's safety. Workflow interruptions were tested to predict failure in action regulation that in turn predicts near-accidents in surgery and related health care. Methods: One-hundred-and-thirty-three theater nurses and physicians from eight Swiss hospitals participated in a cross-sectional questionnaire survey. The study participation rate was 43%. Results: Structural equation modeling confirmed an indirect path from workflow interruptions through cognitive failure in action regulation on near-accidents (p < 0.05). The indirect path was stronger for workflow interruptions by malfunctions and task organizational blockages compared with workflow interruptions that were caused by persons. The indirect path remained meaningful when individual differences in conscientiousness and compliance with safety regulations were controlled. Conclusion: Task interruptions caused by malfunction and organizational constraints are likely to trigger errors in surgery. Work redesign is recommended to reduce workflow interruptions by malfunction and regulatory constraints.

Psychological Intervention for Post-traumatic Stress Disorder among Witnesses of a Fatal Industrial Accident in a Workers' Health Center

  • Post-traumatic stress disorder (PTSD) is a serious problem not only among workers who experience industrial accidents but also among workers who witness such accidents. Early intervention is needed to prevent prolonged psychological problems. There has been no study conducted regarding the psychological problems of and interventions for bystander workers in Korea. This study introduces the experience of intervention on psychological problems at the Busan Workers' Health Center workers who witnessed their colleagues' death. An investigation and an intervention were conducted according to the Korean Occupational Safety and Health Agency (KOSHA) Guide. In total, 21 individuals including indirect observers showed statistical differences on scores of the Impact Event Scale Revised and the Patient Health Questionnaire 9 after the intervention. Future interventions and research involving a larger sample size over a longer period are needed. The KOSHA Guide could be a useful tool for urgent psychological intervention in the event of major workplace disasters.

축열조 공사의 위험분석지수 비교에 관한 연구 (A Study on the Comparison of the Indices of Hazard Analyses for the Construction of Heat Accumulator)

  • This study is conducted to compare three methods of hazard analyses and to propose the control for the accidents arising under construction of heat accumulator of group energy projects(co-generation plants). The analysis of fatal accidents and the Analytic Hierarchical Process(AHP) using by the questionnaire survey for the workers engaged in the construction of heat accumulator and the quantitative questionnaire survey of expert group has been carried out. In order to analyze the fatal accidents, the 115 cases of accidents from 2001 to 2012 published by Korean Occupational Safety and Health Agency(KOSHA) and workers suffered from industrial accidents are collected and analyzed. The AHP is applied and analyzed for 66 persons of the construction site managers, safety engineers and construction engineers and the questionnaire survey of expert group is carried out and analyzed for the 51 persons of contractors and construction inspectors. The analyses of the types of accidents and the jobs of the suffered workers and the hazardous construction processes which are occurred accidents frequently are shown. The results of the analysis of fatal accidents and the AHP as well as the questionnaire of expert group are shown similar results.

중량물 취급시 재해 발생 현황 몇 원인에 관한 연구 (Causes and Condition of Industrial Accidents during Manual Materials Handling)

  • This study analyze industrial safety and health planning report for manufacture. The purpose of accident analysis is to obtain accurate and objective information about the causes of accidents in order to prevent accidents from reoccurring. This study investigated the causes and conditions of occupational accidents in Korean industry. As the most accidents are occurred in lifting material, the questionnaire analyses about injuries are operated for occur to deal with the Manual Materials Handing. The results show that there are some characteristics in the accidents of Korean industry since 1986: (1) accidents rates tend to have lower since 1983(1994-1.18%, 1995-0.99%, 1996-0.88%), (2) larger companies tend to have lower accident rates than the smaller ones, (3) inexperience is related to high accident rates(1995-51.26%, 1996-48.82%1, because of the careless education for freshman such as OJT which in order to prevent accident, and (4) the body sites most vulnerable to injury are the hands and fingers and increased the back(caused of Back pain). The results of questionnaire analysis show that operator(n= 104) interested in reinforce the back & belly(42.86%). These finding identified in this study can be used to prevent the industrial accidents, and used to improving directions based on analyzed result.

건설산업재해 원인분석 조사연구 (The Study of Field Survey for the Analysis of Occupational Accident Causes in Korean Construction Industry)

  • This study was conducted to analyse the exact causes of occupational accidents in Korean construction industry and to contribute to the accident prevention programs. The results and conclusions of the field survey are as follows : 1) The accidents caused by hazardous conditions are 96 percent, and those by unsafe acts are 97.3 percent. The accidents caused simultaneously by two categories are nore than 96 percent. 2) The injured workers who were employed less than 3 months are 71.1 percent. Safety training for newly recruited workers should be required. 3) More than 40 percent of all accidents were occurred in the morning and more than 30 percent were in summer. Required caution should be paid for the time and season. 4) Fracture, cut/laceration/puncture and multiple injuries in the lower extremeties, upper entremeties and head as well as many kinds of injuries by the fall from elevation mainly occurred. Safety. shoes, safety gloves, safety helmets, safety glasses, face shields and safity belts should be used 5) As the sources of injuries, each of building/structure and materials is one third of all sources, and machinery is a quarter. 6) The use of hazardous methods/procedures, defects of agencies and inadequate guarding of builing/structure, materials' and machinery mainly caused the accident types, such as s truck by, struck against, fall from elevation, and fall on same level. Such a hazardous conditiion should not be used and be correctet 7) The unsafe acts, such as improper use of hands or body parts, the operation or working at unsafe speed and improper use of equipment mainly caused accidents. Safety training for the control of such a unsafe acts should be strengthened.

Driving Conditions and Occupational Accident Management in Large Truck Collisions

  • Objective: Objective of this study is to provide characteristics of injury frequency and severity by driving condition in large truck-related traffic collisions. Background: Traffic accidents involving large trucks draw a lot of attention in accident prevention and management policies since they bring about severe human and financial damages. Method: In order to identify the major risk factors of accidents by driving condition, 255 recognized traffic accidents by large truck drivers were analyzed in terms of time of the day, road type, and shape of the road. Results: The driving conditions in the results are represented by the following form of combination, "Road Type (Non-expressway or Express) - Shape of Roads (Straight, Curved, Downhill, or Intersection) - Time of Accidents (Day or Night)". In the analysis of injury frequency, Non-expressway-Straight-Day condition was the most frequent one. Meanwhile, Expressway-Curved-Day, Non-expressway-Curved-Night and Non-expressway-Intersection-Night were evaluated as high level in view of injury severity. Also, Expressway-Straight-Night is the driving condition that is the highest in risk among the conditions that have to be managed as grade "High". Non-expressway-Straight-Night, Non-expressway-Downhill-Day, and Non-expressway-Curved-Day are also categorized as grade "High". Conclusion and Application: Safety managers in the fields require basic information on accident prevention that can be easily understood. The research findings will serve as a practical guideline for establishing preventive measures for traffic accidents.
