아말감의 구강내 부식 및 인공 부식에 관한 연구 (A STUDY ON IN VIVO AND IN VITRO AMALGAM CORROSION)

  • The objective of this study was to analyze the in vitro and in vivo corrosion products of low and high copper amalgams. The four different types of amalgam alloy used in this study were Fine cut, Caulk spherical, Dispersalloy, and Tytin. After each amalgam alloy and Hg were triturated according to the directions of the manufacturer by means of the mechanical amalgamator(Amalgam mixer. Shinhung Co. Korea), the triturated mass was inserted into a cylindrical metal mold which was 12mm in diameter and 10mm in height. The mass was condensed by 150Kg/cm compressive force. The specimen was removed from the mold and aged at room temperature for about seven days. The standard surface preparation was routinely carried out by emery paper polishing under running water. In vitro amalgam specimens were potentiostatically polarized ten times in a normal saline solution at $37^{\circ}C$(potentiostat : HA-301. Hukuto Denko Corp. Japan). Each specimen was subjected to anodic polarization scan within the potential range -1700mV to+400mV(SCE). After corrosion tests, anodic polarization curves and corrosion potentials were obtained. The amount of component elements dissolved from amalgams into solution was measured three times by ICP AES(Inductive Coupled Plasma Atomic Emission Spectrometry: Plasma 40. Perkim Elmer Co. U.S.A.). The four different types of amalgam were filled in occlusal and buccal class I cavities of four human 3rd molars. After about five years the restorations were carefully removed after tooth extraction to preserve the structural details including the deteriorated margins. The occlusal surface, amalgam-tooth interface and the fractured surface of in vivo amalgam corrosion products were analyzed. In vivo and in vitro amalgam specimens were examined and analyzed metallographically by SEM(Scanning Electron Microscope: JSM 840. Jeol Co. Japan) and EDAX(Energy Dispersive Micro X-ray Analyser: JSM 840. Jeol Co. Japan). 1. The following results are obtained from in vitro corrosion tests. 1) Corrosion potentials of all amalgams became more noble after ten times passing through the in vitro corrosion test compared to first time. 2) After times through the test, released Cu concentration in saline solution was almost equal but highest in Fine cut. Ag and Hg ion concentration was highest in Caulk spherical and Sn was highest in Dispersalloy. 3) Analyses of surface corrosion products in vitro reveal the following results. a)The corroded surface of Caulk spherical has Na-Sn-Cl containing clusters of $5{\mu}m$ needle-like crystals and oval shapes of Sn-Cl phase, polyhedral Sn oxide phase. b)In Fine cut, there appeared to be a large Sn containing phase, surrounded by many Cu-Sn phases of $1{\mu}m$ granular shapes. c)Dispersalloy was covered by a thick reticular layer which contained Zn-Cl phase. d)In Tytin, a very thin, corroded layer had formed with irregularly growing Sn-Cl phases that looked like a stack of plates. 2. The following results are obtained by an analysis of in vivo amalgam corrosion products. 1) Occlusal surfaces of all amalgams were covered by thick amorphous layers containing Ca-P elements which were abraded by occlusal force. 2) In tooth-amalgam interface, Ca-P containing products were examined in all amalgams but were most clearly seen in low copper amalgams. 3) Sn oxide appeared as a polyhedral shape in internal space in Caulk spherical and Fine cut. 4) Apical pyramidal shaped Sn oxide and curved plate-like Sn-Cl phases resulted in Dispersalloy. 5) In Tytin, Sn oxide and Sn hydroxide were not seen but polyhedral Ag-Hg phase crystal appeared in internal space which assumed a ${\beta}_l$ phase.

광 전달 쐐기를 이용한 중합 방법이 레진 충전물의 미세 누출과 미세 경도에 미치는 효과 (THE INFLUENCE OF CURING WITH LIGHT TRANSMITTING WEDGE INSERTED INTO THE COMPOSITE RESIN BODY ON THE DEGREE OF MICROLEAKAGE AND MICROHARDNESS)

  • The purpose of this study was to test the hypothesis that the more thorough cure of lightactivated composite resin could be obtained if a new way of delivering the light source deep through the resin body is developed when compared to the conventional bulk cure or incremental cure. Using cylindrical resin blocks and natural teeth, various curing conditions were tested for their effects on the degree of microleakage and microhardness. Data were analyzed statistically using One-Way ANOVA and Scheff's Multiple Range test. The results of the present study were as follows: 1. The increment of reduction in microhardness with depth was shown to be higher in group I and II than in group II and V. 2. At the level of lower inner surface and the bottom surface, significantly lower microhardness values were obtained in group I and II when compared to group III, IV and V. Group IV showed the lowest value among three groups(III, IV, V) representing the only statistically significant difference from group V(p<05). 3. Differences between groups in the degree of microleakage detected were shown to be statistically significant (p<05) with the exception of group IV and V at both occlusal and gingival margins. 4. Based upon the above-mentioned results, commonly cited recommendation of incremental curing is strongly supported and the light-transmitting wedge insertion method can be considered as a effective and feasible clinical procedure for the better curing of composite resin. However, more studies under a variety of conditions should be completed before this method is applied actually to the clinical setting.

시멘트 공간이 캐드캠 시스템으로 제작한 지르코니아 수복물의 변연과 내부 적합도에 미치는 효과 (Effect of cement space on the marginal and internal fit of zirconia restorations fabricated by CAD/CAM system)

  • Purpose: The aim of this study was to evaluate the marginal and internal gap of zirconia copings fabricated by CAD/CAM system and to research the effect of cement space on the fit of zirconia prosthesis. Methods: Zirconia copings were designed using 3shape software and then milled from presintered zirconia blocks by Kavo dental milling machine. The cement space was set at $0{\mu}m$ around the margins for all groups, and additional cement space appling 1 mm above the finish lines of tooth. The samples were classified into 4 groups according to the cement space setting(Cs-0, Cs-40, Cs-80, Cs-120). Replica technique used to investigate marginal and internal gap. The silicone film thickness was recorded using a optical microscope with image software analysis system at 75${\times}$magnification. The assessment performed on the six points in bucco-lingual and mesio-distal sections(4 groups, 10 copings per group, 12 measuring points per coping). The data were statistically analyzed with one-way ANOVA followed by pair-wise Tukey's post hoc tests(${\alpha}$<0.05). Results: Better marginal and internal fit values were exhibited by $80{\mu}m$ cement space. The results showed that different cement space values had statistically significant effect on the marginal and internal gap of zirconia copings(p<0.001). The gap of margin and axial wall area was smaller than that the gap of occlusal area in all groups(p<0.001). Conclusion : Effect of different cement spaces on the marginal and internal gap of zirconia copings.

제 5급 와동에서의 단일용기 상아질 접착제와 자가 산부식 접착제의 상아질에 대한 침투도 평가 (A CONFOCAL MICROSCOPIC STUDY ON DENTINAL INFILTRATION OF ONE-BOTTLE ADHESIVE SYSTEMS AND SELF-ETCHING PRIMING SYSTEM BONDED TO CLASS V CAVITIES)

  • Objective : The purpose of this study was to evaluate the resin infiltration into dentin of one-bottle adhesive systems and self-etching primer bonded to Class V cavities using confocal laser scanning microscope(CLSM). Material and Methods : Forty Class V cavities were prepared from freshly extracted caries-free Human teeth. These teeth were divided into two groups based on the presence of cervical abrasion: Group I, cervical abrasion : Group II, wedge-shaped cavity preparation. Resin-dentin interfaces were produced with two one-bottle dentin bonding systems-ONE COAT BOND(OCB; Coltene$^R$) and Syntac$^R$SPrint$^{TM}$(SS; VIVADENT)-, one self-etching priming system-CLEARFIL$^{TM}$ SE BOND (SB : KURARAY)- and one multi-step dentin bonding system-Scotchbond$^{TM}$Multi-Purpose (SBMP, 3M Dental Products)-as control according to manufacturers' instructions. Cavities were restored with Spectrum$^{R}$(Dentsply). Specimens were immersed in saline for 24 hours and sectioned longitudinally with a low-speed diamond disc. The resin-dentin interfaces were microscopically observed using CLSM. The quality of resin-infiltrated dentin layers were evaluated by five dentists using 0~4 scale. Results : Confocal laser scanning microscopal investigations using primer labeled with rhodamine B showed that the penetration of the primer occurred along the cavity margins. Statistical analysis using one-way ANOVA followed by Duncan's Multiple Range test revealed that the primer penetration of the group 2(wedge-shaped cavity preparation) was more effective than group 1(cervical abrasion) and that of the gingival interfaces was more effective than the occlusal interfaces. In the one-bottle dentin bonding systems, the resin penetration score of OCB was compatible to SBMP, but those of SS and self-etching priming system, SB were lower than SBMP.

복합레진 수복 시 재접착 술식이 미세누출에 미치는 영향 (INFLUENCE OF REBONDING PROCEDURES ON MICROLEAKAGE OF COMPOSITE RESIN RESTORATIONS)

  • 복합레진 수복시, 일반적으로 연마, 산부식, 수세 및 건조 단계를 거쳐 저점도 레진을 적용하는 재접착 술식을 통해 예상되는 수축간극을 봉쇄한다. 그러나, 이 과정은 재접착제 적용 이전에 물이나 잔사가 간극을 매워버릴 수 있어 그 효과가 의심된다. 본 실험에서는 연마 단계 이전, 즉 복합레진을 중합한 직후에 재접착제를 도포한다면 변연누출을 더 줄일 수 있을 것이라는 가설을 검증하고자 하였다. 실험을 위해 35개의 발거한 대구치에서 협면과 설면에 교합면 변연은 법랑질에, 치은측 변연은 상아질에 위치하는 5급 와동을 형성하였다. 와동은 Z250 (3M ESPE, USA)로 충전하였고, 접착제로는 $Adper^{TM}$ Single Bond 2 (3M ESPE)을 사용하였다. 재접착제로는 Biscover LV (Bisco, USA) 레진 전색제와 ScotchBond Multi-purpose system (3M ESPE)의 접착제, 및 점도가 낮은 실험용 접착제를 연마 전 또는 일련의 연마 및 산부식 후 수복물의 변연에 적용하였다. 2% 메틸렌 블루 용액에 네 시간 동안 침적한 뒤 교합측 및 치은측 변연에서의 색소 침투 깊이를 광학 입체 현미경으로 측정하였다. 재접착제의 점도와 변연미세누출의 상관관계도 평가하였다. 재접착 술식, 재접착제, 및 변연의 위치, 그 각각에서는 통계적으로 유의한 차이를 관찰할 수 없었다. 그러나, 재접착 술식을 시행하지 않은 대조군의 경우에는 세 가지 재접착제를 사용한 재접착군에 비해 치은측 변연에서 미세누출이 통계적으로 유의하게 크게 나타났다(p < 0.05). 대조군과 재접착군의 미세누출의 차이는 교합측 변연에서는 관찰되지 않았다. 연마 및 산부식 후 치은변연에 재접착제를 적용한 경우에서 약한 음의 상관관계(r = -0.326, p = 0.041)를 보인 경우를 제외하고는, 재접착제의 점도와 미세누출은 유의한 상관관계를 보이지 않았다.

부하순환 하에서 제V급 복합레진 수복물의 미세변연누출에 대한 재접착제의 효과에 관한 연구 (THE EFFECT OF REBONDING IN MICROLEAKAGE OF CLASS V RESTORATIONS UNDER LOAD CYCLING)

  • 저점도의 레진을 복합레진 수복물 위에 도포하는 것은 미세누출방지와 변연 완전성(integrity) 유지를 위한 한 방법이다. 하지만 장기간 관찰하면, 수복물은 저작압에 의한 영향을 받게 되는데, 저점도의 레진으로 수복물 변연부를 재접착(rebonding)하였을 때 저작압에 의한 응력에 견디면서 계속하여 수복물에 긍정적인 영향을 주는지에 대한 연구는 미흡한 실정이다. 본 연구는 부하순환(load cycling) 하에서 제V급 수복물의 미세변연누출에 대한 재접착제(rebonding agent)의 효과를 알아보고자 하였다. 40개의 건전한 소구치의 협면에 제V급 와동을 형성하고 제조사의 지시대로 광중합복합레진으로 수복하였다. 20개씩 무작위로 나누어 표면전색을 하지 않는 I군과 표면을 산부식 후 재접착한 II군으로 설정하였다. 열순환 후 각 군에서 10개씩의 하위군(A군=No load cycling군, B군=Load cycling군)으로 나누어 이중 B군의 시편에 1 Hz의 속도로 수직력이 4-100N까지 100,000회의 부하순환을 가하였다. methylene blue 용액에 침윤시켜 변연의 미세누출정도를 평가하여 다음과 같은 결과를 얻었다. 1. 교합변연에서는 재접착과 무관하게 유의한 차이가 없었다(IA-IIA군, IB-IIB군)(p>0.05). 2. 치은변연의 경우는 부하순환을 하지 않은 군의 경우 재접착을 한 군(IIA군)이 재접착을 하지 않은 군(IA군)에 비해 더 적은 미세누출결과를 보였다(p<0.05). 3. 치은 변연에서 부하순환을 시행한 군의 경우에는 재접착을 한 군(IIB군)과 재접착을 하지 않은 군(IB군)과 유의차가 없는 비슷한 미세누출의 결과를 보였다(p>0.05).

S-PRG 필러를 포함하는 치면열구전색제의 전단결합강도 및 미세누출 (Shear Bond Strength and Microleakage of a New Self-etch Sealant Containing S-PRG filler)

  • 본 연구의 목적은 S-PRG 필러를 포함하는 치면열구전색제(BeautiSealant)의 법랑질 전단결합강도 및 미세누출을 평가함에 있다. 45개의 소구치를 실험대상으로 사용하였으며, 자가부식 프라이머 도포 후 BeautiSealant를 적용한 군(SEPBS), 인산 부식 후 BeautiSealant를 적용한 군(AEBS), 인산 부식 후 $Clinpro^{TM}$ sealant를 적용한 군(AECP)으로 나누어 실험하였다. 전단결합강도 측정을 위해 치면열구전색제 블록을 법랑질 표면에 적용하였고, 미세누출을 평가하기 위해 소구치의 소와열구에 치면열구전색제를 도포하였다. 전단결합강도와 관련하여 SEPBS군은 AEBS, AECP군에 비해 유의하게 낮은 값을 보였으나(p < 0.05), AEBS군과 AECP군 사이에는 통계적으로 유의한 차이가 존재하지 않았다. 미세누출과 관련하여 3군 사이에 통계적으로 유의한 차이가 존재하지 않았다.

다이아몬드 기구로 삭제한 와동에서 2단계 접착제의 미세누출 비교 (MICROLEAKAGE OF 2-STEP ADHESIVE SYSTEMS IN DIAMOND-PREPARED CAVITY)

  • 이 연구는 거친 입자와 미세한 입자의 다이아몬드 포인트로 형성한 5급 와동에서 서로 다른 2단계 접착시스템 사용시 법랑질과 상아질 변연에서의 미세누출 차이를 상호 비교하기 위하여 시행하였다. 40개의 발거한 대구치의 협면 치경부에 각각 $106-205\;{\mu}m$의 거친 다이아몬드 포인트 (EX-41)와 $53-63\;{\mu}m$의 미세한 다이아몬드 포인트 (TF-21F)를 이용하여 20개씩 5급 와동을 형성한 후 사용된 접착시스템과 복합레진에 따라 다음과 같이 4개의 군으로 분류하였다; 1군은 EX-41 포인트로 형성한 와동에 Single Bond와 Z 250을 사용한 군, 2군은 TF-21F 포인트로 형성한 와동에 Single Bond와 Z 250을 사용한 군, 3군은 EX-41 포인트로 형성한 와동에 Clearfil SE Bond와 Clearfil AP-X를 사용한 군, 4군은 TF-21F 포인트로 형성한 와동에 Clearfil SE Bond와 Clearfil AP-X를 사용한 군으로 분류하였다. 각 군은 $5^{\circ}C$$55^{\circ}C$의 증류수에서 500회 열 순환한 후 2% methylene blue용액에 1일 동안 침적하였다. 각 군의 치아를 협설로 절단하여 광학 입체현미경 하에서 법랑질과 상아질 변연에서의 색소침투 정도를 관찰하여 미세누출 점수를 평가하고, 각 군 간의 유의성을 검정하여 다음과 같은 결과를 얻었다. 이 연구를 종합하면, 5급 와동에서 Single Bond는 이 연구에 사용한 다이아몬드 포인트의 종류에 따른 변연 미세누출 차이를 보이지 않았으나 Clearfil SE Bond는 거친 다이아몬드 포인트 보다는 미세한 다이아몬드 포인트를 사용하는 것이 법랑질과 상아질 변연 모두에서 낮은 미세누출을 보였다.

재습윤제의 적용이 상아질 접착에 미치는 효과 (EFFECT OF A REWETTING AGENT ON DENTIN ADHESION)

  • This study compared the dentin adhesion to composite resin according to air-dry, blot dry, application of rewetting agent on air-dry or blot dry dentin surface by microleakag test and SEM observation. For microleakage test, class V cavity preparations with dentinal margins were prepared on both buccal and lingual surfaces of 40 extracted human molars. For SEM observation, occlusal dentin of 20 extracted human molars were exposed. After etched the dentin, prepared teeth were randomly divided into four groups; D group: air dry for 10-15 sec., B group: blot dry with moist cotton pellet, D-R group: air dry and rewet with Aqua-Prep F for 20 sec., B-R group: blot dry and rewet with Aqua-Prep F for 20 sec. Treated cavities and surfaces were filled or constructed using One-Step adhesives and Aelitefil composite resins. Specimens were stored in distilled water for 24 hours. For microleakage test, the specimens were thermocycled and soaked into 2% methylene blue. The specimens were sectioned longitudinally and evaluated for microleakage under steromicroscope. The data were statistically analysed by Kuskal-Wallis Test, Mann-Whitney and Wilcoxon signed ranked tests. For SEM observation, the specimens were bisectioned mesiodiatally. After decalcified and deproteinized, specimens were observed under SEM. The results of this study were as follows; 1. The microleakges on dentinal margin were the highest in D group compared with B group, D-R group, and B-R group(p<0.05). But there was no significant difference between B group, D-R group and B-R group. 2. D group showed gap and a few resin tags between dentin and composite resin. 3 B group, D-R group, and B-R groups showed close adaptation between dentin and composite resin. It showed that resin rags in B group were numerous and long, in D-R group were few and short, in B-R group were numerous and short or long. 4. Adhesive layer showed in D-R group ($10{\;}\mu\textrm{m}$) and B-R group ($3{\;}\mu\textrm{m}$) In conclusion, use of rewetting agent to dry dentin was efficient to dentin adhesion, also it did not provide reverse effect on blot dry dentin.

CAD/CAM으로 제작된 세라믹 인레이의 변연 및 내면 적합성 (MARGINAL AND INTERNAL FIT OF CAD/CAM-MANUFACTURED CERAMIC INLAY)

  • CAD/CAM-fabricated ceramic restorations nowadays are used as alternatives of amlagam and posterior composite resin restorations, especially in the cases of inlay restorations. But the reported results on marginal and internal fit of CAD/CAM-fabricated ceramic inlay have showed considerable difference. In this study, to evaluate the marginal and internal fit of CEREC2-fabricated ceramic inlay restoration and to compare with the fit of gold inlay and amalgam restoration, standardized Class II MO cavities were prepared in forty extracted caries-free human premolars. The teeth with prepared cavities were divided into 4 groups of ten teeth each. In group 1, CEREC2-fabricated ceramic inlays were treated with Scotchbond Multi-Purpose Plus(SMP plus) and cemented with Scotchbond Resin Cement. In group 2, casted gold inlays were cemented in the same method as in group 1. In group 3, casted gold inlays were cemented with zinc-phosphate cement. And in group 4, the prepared cavities were restored with amalgam. Restored teeth were thermocycled, stored in 1% methylene blue for 24 hours, and sectioned faciolingually and mesiodistally using EXAKT. Sectioned surfaces were observed with stereomicroscope and the gaps were measured at 9 points of mesiodistally sectioned surface and 7 points of faciolingually sectioned surface. The measured data were treated by Kruskal-Wallis one way ANOVA and Student-Newman-Keuls test. 1. The differences among measured gaps at each points were statistically significant for 4 experimental groups (P<0.05). 2. There were statistically significant differences in the measured gaps at each points between group 1 and group 2, group 1 and group 3, group 1 and group 4, group 2 and group 4, and group 3 and group 4 (P<0.05). 3. There were not statistically significant differences in the measured gaps at each points between group 2 and group 3 (P>0.05). 4. In the cases of inlay restorations(group 1, group 2, group 3), the gaps at internal line angle(distopulpal, axiogingival, faciopulpal, linguopulpal line angle) had a tendency to increase. In the cases of amalgam restorations(group 4), the gaps at occlusal margin, gingival margin and axiogingival line angle were greater than those at the other parts of cavities. 5. In CEREC2-fabricated ceramic inlays which were treated with Scotchbond Multi-Purpose Plus and cemented with Scotchbond Resin Cement, the mean gaps were $111{\mu}m$ at cavity margins, $168{\mu}m$ at vertical walls of cavities, $225{\mu}m$ at internal line angles and $123{\mu}m$ at cavity floors.
