Incidence and Related Factors of the Metabolic Syndrome in a Korean Medicine Hospital (한방병원 건강검진 수진자의 대사증후군 발생과 관련요인 연구)

  • The purpose of this study was to investigate incidence and related factors of the metabolic syndrome in a Korean medicine hospital. The 716 subjects were analyzed using biochemical data and survey who took medical examination in Daejeon Korean Medicine Hospital for general health check-up. This investigation was conducted from February in 2008 to July in 2010. The metabolic syndrome was diagnosed according to the definition by the NCEP ATP III. The abdominal obesity guidelines for waist circumference applied by the WHO Western Pacific Region, IASO and IOTF: The Asia-Pacific Perspective in 2000. Incidence of metabolic syndrome was 12% (14.6% in men, 8.2% in women). The groups that have two metabolic risk factors were 21.9% in men and 7.5% in women. The incidence increased with ageing. The mean of metabolic syndrome`s triglyceride was in hypertriglyceridemia, and that of their BMI in men was in primary obese and that of their AST, ALT, ${\gamma}$-GTP means were in abnormal liver function. Smokers in men have metabolic syndrome 10 times more than non-smokers in men. Exercisers that do the exercise once or twice a week in women have metabolic syndrome 0.2 times more than non-exerciser in women. Women that have family history of stroke, were associated with metabolic syndrome by $x^2$-test. Men that have family history of hypertension, have metabolic syndrome 4 times more than otherwise men. Men that have family history of diabetes mellitus, have metabolic syndrome 3 times more than otherwise men.

Body Cathexis and Actual Clothing Styles of Middle-aged Women According to Perception of Their Body (중년여성의 신체부위 인지유형에 따른 신체만족도와 착의 의복 스타일)

  • This study is designed for improving wearing styles for middle-aged women who are in unsatisfactory body types and also providing the basic information about the suitable clothing designs for them. This study focuses on what kinds of body types they perceive, what the differences are between the perception of their body types and the real ones, and how the actual clothing styles can be affected by those perceived body types. This study is based on the survey of the perception of body types, the clothing styles, and the satisfaction level of body cathexis, while measuring body size of each person responded. The data were analyzed by Mean, ANOVA, Duncan-test of SPSS Win. (ver 10.1)program. The results are as follows: 1. The perception of their body types can be categorized into three clusters, the obesity, the arm & leg obesity, the slender. 2. By and large, Middle-aged women perceive their body types exactly what they we. However, they show a relatively subjective view on the upper arm girth, ankle girth, and arm length 3. The abdomen is at the lowest satisfactory part in body cathexis for the entire groups responded, and there are some differences among the perception of their body types in whole body silhouette and trunk. 4. Middle-aged women who perceive themselves as a bit slender show a tendency of having tight and revealing clothing styles, while the obese who perceived themselves wouldn't take them as appropriate dresses.

The Effects of Body Composition, Blood Lipid & Lipid Metabolism on Bicycle Exercise of Various Intensities in Obesity Middle Aged Women (다양한 강도의 자전거운동이 비만중년여성의 신체구성, 혈중지질 및 지방대사에 미치는 영향)

  • In This study investigated the effects of various intensity cycling exercise on body composition, blood lipids and fat metabolism in obese middle-aged women. 17 people obesity middle - aged women aged 35 to 55 years were divided into 3 groups: 5 people in the low intensity group, 6 people in the medium intensity group, and 6 people in the high intensity group. The object of this study was to compare the effect of cycle exercise performed between 20 ~ 50 minutes per a day,1 3 day times per week on the body composition, lipid profile and lipid metabolism of obesity middle - aged women. There was not substantial difference in body construction between groups. in blood lipid changes, there was a substantial difference in glucose between groups, There was not substantial difference between groups in the change of fat metabolism. In addition, there was not substantial difference in the interaction effect between time and time × group. It is expected that the results of this study will be used as basic data for related field.

Upper Body Type Analysis for Middle-aged Women Aged 50-69 Years (50-69세의 중장년 성인여성을 위한 상반신 체형분석)

  • This study aims to provide upper body shape information by analyzing the measurement data of middle-aged women aged 50-69, including baby boomers, whose economic power and activity have improved compared to the previous generations. In order to provide accurate upper body shape information by analyzing the body type using the 8th Size Korea measurement data, body shapes were classified through factor and cluster analysis using 75 direct measurement items. Upper body type was classified according to the factors, and the associated characteristics were analyzed. As a result of the comparative analysis of the upper body measurements from the 4th to the 8th Size Korea measurement, it was found that in the height item, both the waist height and the hip height increased, making the overall height greater and the leg length longer. The body circumference items tended to increase, but were found to decrease significantly in the 8th Size Korea (2021) measurement. Middle-aged women were classified using five factors. Factor 1 was the upper body obesity factor, and Factor 2 was the trunk vertical factor. Factor 3 was the width of the back shoulder, Factor 4 was the vertical factor behind the back, and Factor 5 was the length factor of the front garment composition. Middle-aged women were classified into four body types through cluster analysis. Type 1 is relatively small and skinny, Type 2 has the most obese upper body and developed back shoulders, and Type 3 is skinny and has a long back and short front. In Type 4, the upper body was relatively long and the shoulders were narrow.

  • Purpose: The study examined obesity conditions and related factors in middle aged Korean women who had been diagnosed as obese based body mass index (BMI) or waist circumference (WC) measurement. Presently, BMI alone, WC alone, or BMI + WC were used as obesity measures. Methods: Subjects were 488 women 40~64 years of age living in one city, Korea. Structured questionnaires concerning general characteristics, health behavior practices, and chronic disease history were completed by each subject. Results: The proportions of obesity were 42.4% by BMI, 39.3% by WC, and 51.3% by the combination of both measurements. Chi-square test revealed significant associations of obesity with age, education level, menopausal status, frequency of excessive eating, and history of diabetes mellitus or hypertension. These associations were similar for the three measurement schemes. There was significant positive relationship between BMI and WC, whereas the relationship between BMI and WHR (waist-hip ratio) was not significant. Conclusions: The combination of BMI and WC measurement may be a valid way to evaluate the obesity status in middle-aged women. Developing an obesity management program that includes control of excessive eating, and consideration of menopausal status and history of chronic diseases would be prudent.

Evaluation of Nutritional Health Camp in Obese Elementary Students (일부 비만 초등학생을 위한 건강영양캠프의 효과 평가)

  • An evaluation of the influence of nutritional health camp for in obese elementary students was assessed. Anthropometric measurement, 24-hour recall of dietary intake, MMTIC(Murphy-Meisgeier Type Indicator for Children) and questionnaire response(nutritional knowledge, nutritional attitude, and self-satisfaction) was documented in 43 obese elementary students(29 boys and 14 girls). The mean age of the elementary students was 11.2 years. The mean height, body weight and obesity index was 144.7 cm, 55.4 kg and 41.0% in boys, and 136.1 cm, 45.2 kg, 40.0% in girls. The mean energy intake was 1,522.4 kcal in boys and 1,494.9 kcal in girls. The subjects taking less than estimated average requirement(EAR) for vitamin ${B_2}$, folate, vitamin C and calcium was over 70%. A considerable number of students(65.1%) showed U band(undetermined character). The students demonstrated a propensity towards increased average nutritional knowledge and self-satisfaction after attending the nutritional health camp. In addition, a significant increase in average nutritional attitude was observed(p<0.001). The results of this study suggest that nutritional health camp positively influenced the nutritional attitude of elementary students.

Comparison of the Effectiveness of Dietary Intervention between Calorie Restriction Diet and Food Selective Method according to Yin and Yang Constitution in Obese Woman (열량제한식과 체질식에 따른 비만 여성의 체중감량 효과)

  • This study was conducted to evaluate the effects of constitutional diets in comparison with general low calorie diets in obese women. The subjects were 52 adult obese women that participated in a calorie restriction diet program (control group, n=16) or a constitutional diet program (experimental group, n=36) for 8 weeks. The subjects in the experimental group were classified as negative (Yin) or positive (Yang) constitution. The obesity management program focused on the calorie restriction diet (300~400 kcal reduction per day) for the control group, whereas, for a constitutional diet without calorie restrictions was allowed for the experimental group. Daily intakes of nutrient and food were assessed using a 24 hr recall method. Body measurements and blood biochemical parameters were measured at baseline and after 8 weeks. After the intervention, weight, body fat, BMI, waist circumference and hip circumference decreased significantly in both the control and experimental group. Energy intake increased from 1,679.6 kcal to 1,810.6 kcal along with significant increase in protein, carbohydrate, fiber, calcium, phosphorus, iron, zinc, vitamin A, vitamin $B_1$, vitamin $B_2$, niacin and vitamin C in the experimental group. Calcium intake increased from 54.0% to 72.4% of DRI in the experimental group. Hemoglobin concentrations were significantly decreased in the control group, but were increased in the experimental group. In addition, blood glucose, serum total cholesterol and triglyceride were significantly decreased in the experimental group. In conclusion, consumption of a constitutional diet without calorie restrictions for 8 weeks was effective for weight reduction with improvement of blood glucose, serum cholesterol and triglyceride.

A Study on food Ecology According to Obesity Index of Elementary School Children in a High Socioeconomic Apartment Complex in Seoul (서울시내 고소득층 아파트단지 국민학교 어린이의 체격과 식생태에 관한 조사연구)

  • An ecological survey of food and nutrition was conducted in April and May, among 278 children, 6 to 12 years old, of the Youido elementary school, which offers no school lunch program, and is situated in Youido delta of Han River side of Seoul, known as the location of one of the socioeconomically high groups; Eighteen percent and 18.3% of subjects were proven to be overweight and obese, respectively. With regard to frequency of skipping breakfast, 28.4% of the subjects skipped breakfast often and 2.5% of subjects skipped breakfast every morning. Obese children showed good appetite and the time consumed for eating was shorter than those of normal or overweight children. Most serious anxiety among the mothers for those children was food habit of `eating rapidly' and `overeating'. Another striking observation was that 14.4% of the subjects ate only one side-dish with boiled rice, and such a pattern was more often found in the normal or slim group. Most of the subjects had their snacks after being back in home from school. Thirty-one percent of the children had snacks when they felt hungry, 69% had snacks for various reasons such as `habitually', `for unwinding', and `mother gives me snacks'. Thirty-eight percent of the boys and 19% of the girls had physical exercise everyday and overweight and obese children got exercise more frequently. But the time consumed for exercise was much more shorter than that of normal subjects. Most of the children liked fruits, meats, and biscuit, in contrast, lowest preferency was for vegetables, fermented fish products, and boiled rice mixed with other grains. Obese children experienced more kinds of food than normal or slim group. The study found that habits of overeating due to good appetite and eating rapidly were recognized as one of dominant reasons causing obesity among children. On the other hand, serious dietary problem of children of normal or slim group was eating less variety of food. Active nutrition education for both children and mothers will be recommended with joint participation of teachers.

The Effect of Metformin in Non-Obese Women with Polycystic Ovary Syndrome; Pilot Study (비만하지 않은 다낭성난소증후군 환자에서 메트포민 효용성의 예비 연구)

  • Objective: This pilot study was performed to investigate the effect of metformin on insulin resistance, hormone levels, and lipid profiles in non-obese patients with polycystic ovary syndrome. Methods: This study included 16 non-obese patients with polycystic ovary syndrome diagnosed at our hospital from June 2006 to September 2007. Blood samples were collected before and 6 months after metformin treatment for analysis of fasting serum glucose levels, fasting serum insulin levels, a glycemic response to 75 g oral glucose tolerance test (OGTT), and hormonal blood profile including FSH, LH, estradiol, testosterone, free testosterone, serum lipid profiles. Insulin resistance was estimated by calculating fasting glucose/insulin ratio (FGIR), 2 hr glucose/insulin ratio after 75 g glucose load. And we investigated insulin resistance and pancreatic beta cell function by calculating HOMA beta cell function and HOMA IR. Results: After the treatment of metformin, there was significant increase in 2 hr glucose/insulin ratio after 75 g glucose load (p=0.04) and decrease in HOMA IR (p=0.000). But serum lipid profiles did not change significantly. Also the metformin treatment induced a significant reduction in serum free testosterone and LH levels, and LH/FSH ratio (p=0.001, p=0.000, p=0.034). Conclusion: This pilot study showed that metformin might be effective in improving insulin sensitivity, ameliorating hyperandrogenemia in non-obese patients with polycystic ovary syndrome. Further investigations with larger number of patients and long-term observations are necessary to determine the role of metformin.

Comparing validity of using Body Mass Index, Waist to Hip Ratio, and Waist Circumference to Cardiovascular Risk Factors of Middle Aged Koreans (한국중년층에서 체질량지수, 허리-엉덩이둘레비, 및 허리둘레의 심장혈관계질환 위험인자 타당도 비교)

  • This study was conducted to compare the validity of obese index among body mass index(BMI), waist to hip ratio(WHR), and waist circumference(WC) and to determine which is the best in relation to cardiovascular risk factors of middle aged Korean(40-64yr).Data from the 1998 Korean Health and Nutrition Survey were used(N=3380). Anthropometric indices and cardiovascular risk factors were measured. Chi-square test, analysis of variance following duncan's multiple range test, partial correlation analysis, and Receiver Operator characteristic(ROC) curves were used in the analysis. There was a significant increasing trend in WHR, systolic blood pressure(SBP), high density lipoprotein cholesterol(HDL), and fasting blood sugar(FBS) with age categories of male and in BMI, WC, WHR, diastolic blood pressure(DBP), SBP, total cholesterol(TC), low density lipoprotein cholesterol(LDL), triglycerol(TG), and FBS with those of female. Specially female had the characteristics of upper body fat and systolic blood pressure risk(p<0.05). Proportions of subjects with lifestyle factors related to cardiovascular risk in overweight or upper body fat group were higher than that of normal group. Higher proportions of subjects were practiced exercise in upper body fat group of male than in other groups. Among 7 cardiovascular risk factors in partial correlation analysis, BMI had the highest correlation coefficient in 6 risk factors in male, whereas WC in 4 risk factors in female. Mean of each obese index according to cardiovascular risk groups except smoker was higher than that of normal(p<0.05). These trends were shown in upper body fat group and female. In ROC analysis of 12 risk factors and health conditions, the largest area under curve among obese indices for risk factors were BMI in male and WHR in female. The optimal cutoff values of each index(BMI: WHR: WC) for one or more cardiovascular risk factors were 23.13: 0.89: 85.35 in male and 23.57: 0.84: 78.35 in female. The results showed that cardiovascular risk factors were prevalent in middle aged Koreans within normal limits of obese indices like another Asians. For the identification of cardiovascular risk factors of middle aged Koreans, BMI for men and WHR for women are appropriate indices. But it is recommended that BMI, WHR, and WC, all three indices should be considered, when using these indices.
