Prevalence and trends of pain associated with chronic diseases and personal out-of-pocket medical expenditures in Korea

  • Background: There have been few studies about pain using a big data. The purpose of this study was to identify the prevalence of pain, and trends of pain associated with chronic diseases and personal out-of-pocket medical expenditures over time. Methods: Subjects were 58,151 individuals, using the Korea Health Panel from 2009 to 2013. Chi-square and multinomial logistic regression were conducted to identify the prevalence and odds ratios (ORs) of pain. Repeated measures ANOVA was used to find the trend over these 5 years. Results: Prevalence of mild and severe pain was 28.1% and 1.7% respectively. The ORs of mild and severe pain were 1.6 and 1.4 in females compared with males. From 2009 to 2013, numbers of chronic diseases producing mild pain were 2.1, 2.4, 2.8, 2.9, and 3.1 and those producing severe pain were 3.0, 3.4, 3.9, 4.2, and 4.4, respectively. After applying the average South Korean inflation rate by year over 5 years, the annual, personal out-of-pocket medical expenditures (unit: ₩1,000) for mild pain were 322, 349, 379, 420, and 461, and those for severe pain were 331, 399, 504, 546, and 569, respectively (P < 0.0001). Conclusions: The pain prevalence was 29.8%. The numbers of chronic diseases and the personal out-of-pocket medical expenditures revealed increasing trends annually, especially in those with pain. Therefore, to eliminate and alleviate the pain, there needs to be further study for developing a systemic approach.

Hair Growth Promotion by δ-Opioid Receptor Activation

  • Literature has revealed that the delta opioid receptor (DOR) exhibited diverse pharmacological effects on neuron and skin. In the present study, we have investigated whether the activation of DOR has hair-growth promotion effects. Compared with other opioid receptor, DOR was highly expressed in epidermal component of hair follicle in human and rodents. The expression of DOR was high in the anagen phase, but it was low in the catagen and telogen phases during mouse hair cycle. Topical application of UFP-512, a specific DOR agonist, significantly accelerated the induction of the anagen in C3H mice. Topical application of UFP-512 also increased the hair length in hair organ cultures and promoted the proliferation and the migration of outer root sheath (ORS) cells. Similarly, pharmacological inhibition of DOR by naltrindole significantly inhibited the anagen transition process and decreased hair length in hair organ cultures. Thus, we further examined whether Wnt/β-catenin pathway was related to the effects of DOR on hair growth. We found that Wnt/β-catenin pathway was activated by UFP-512 and siRNA for β-catenin attenuated the UFP-512 induced proliferation and migration of ORS cells. Collectively, result established that DOR was involved in hair cycle regulation, and that DOR agonists such as UFP-512 should be developed for novel hair-loss treatment.

A Study on the Relationship between Liquid Electronic Cigarette Use and Allergic Rhinitis in South Korean Adolescents (청소년의 액상형 전자담배 흡연과 알레르기비염과의 관계에 관한 연구)

  • Background: The prevalence of allergic rhinitis in South Korean adolescents is increasing. Adolescents in South Korea currently use liquid electronic cigarettes more than heated tobacco products. Objectives: The purpose of this study was to assess the association between the use of liquid electronic cigarettes and allergic rhinitis diagnoses among South Korean adolescents. Methods: This study used data from the 17th Korea Youth Risk Behavior Survey in 2021. Out of a total of 59,066 students, data from 18,869 students who had experience with liquid electronic cigarettes were used in the final analyses. Diagnoses of allergic rhinitis by medical doctors were self-reported as outcome variables. Results: For females, when comparing 'daily users' and 'current users' with 'never users', the adjusted odds ratios (ORs) for 'allergic rhinitis' were 2.20 (95% confidence interval (CI), 1.00~4.84) and 1.49 (95% CI, 1.07~2.09), respectively. For males, when comparing 'daily users' and 'current users' with 'never users', the adjusted ORs for 'allergic rhinitis' were 1.42 (95% confidence interval (CI), 0.91~2.22) and 1.41 (95% CI, 1.08~1.84), respectively. Conclusions: The use of liquid electronic cigarettes was statistically significantly associated with increased odds of allergic rhinitis, especially for females. The results of this study will provide a useful scientific basis for establishing public health policy and using the educational data related to the use of liquid electronic cigarettes in the future.

Meta-Analysis of Risk Factors for Contamination of Environmental Waters by Legionella (환경수계 레지오넬라균 오염 지표의 메타분석)

  • To identify risk factors for Legionella contamination, water quality variables routinely measured in examination of natural and city waters were meta-analyzed for significance of correlation to Legionella incidences. For evaluation of abundance of Escherichia coli as a risk factor, which is currently used as an indicator of Legionella contamination in an official guideline in Korea, odds ratio (OR) of above-cutoff total coliform counts for Legionella presence/absence was used as the effect size in the meta-analysis. The OR was estimated as 1.05 (0.36-3.12, 95% CI), and the probability of having identical odds reached 0.92. Also, ORs from individual studies showed significant heterogeneity (P=0.008), which contributed to 63% of total variance of the ORs. In the case of heterotrophic plate count (HPC), the OR for Legionella presence/absence was 2.72 (2.04-3.63) with highly significant deviation from identical odds (P<0.0001). ORs from different studies were seemingly homogeneous ($Q_{df=8}$=12.7, P=0.12). Turbidity and concentrations of chlorine, iron ion and cupper ion were other routine variables that could be considered as risk factors. However, statistical measures from different studies were not uniform enough to develop an appropriate effect size while the number of studies reporting the variables was also small (3-5). In conclusion, HPC appeared to be appropriate as indicator of Legionella contamination, rather than fecal bacteria contamination. HPC may imply abundance of habitats (amoebas and biofilms) of Legionella in water. This result warrants further studies for standardizing protocols and cutoff values to infer Legionella risks from HPC.

Helicobacter pylori and Pancreatic Cancer Risk: a Meta-analysis Based on 2,049 Cases and 2,861 Controls

  • Aim: Helicobacter pylori (H. pylori) have been considered as a risk factor for many cancers. We conducted this meta-analysis to clarify the association between H. pylori infection and the risk of pancreatic cancer. Methods: We searched the Medicine/Pubmed and Embase databases, studies about the association between H. pylori infection and pancreatic cancer published up to Jan.2014 were included. Finally, a total of 9 studies were used for this a meta-analysis. The odds ratios (ORs) and 95% confidence interval (95%CI) of H. pylori infection on pancreatic cancer with respect to control groups were evaluated. Two authors independently assessed the methodological quality and extracted data. This meta-analysis was conducted using software, state (version 12.0) to investigate heterogeneity among individual studies and to summarize the studies. Using the fixed-effects or random-effects model, depending on the absence or presence of significant heterogeneity. Sensitivity analysis was performed to assess the influence of each individual study on the pooled ORs by omitting a single study each time. Publication bias was evaluated by funnel plot, using Egger's and Begg's tests. Results: There was no significant association between H. pylori infection and pancreatic cancer risk in the summary ORs,(OR=1.06, 95%CI: 0.74-1.37) through the random-effect method, but heterogeneity among studies was significant ($I^2$=58.9%), so we put the studies into two subgraphs (eastern and western). The results about western (OR=1.14 95%CI:0.89, 1.40) showed heterogeneity among the western countries of $I^2$=6.6%, with no significant association between Hp+ and pancreatic cancer, but the eastern countries (OR=0.62, 95%CI:0.49, 0.76), $I^2$=0, suggested that decreasing pancreas-cancer risk in subjects with Hp+ infection. Simultaneously, 7 studies examined CagA+ strains was (OR=0.84 95%CI:0.63, 1.04), $I^2$=36% with the random-effect method, subgraphs indicated that CagA+ could decrease the risk of pancreatic cancer in the eastern subjects (OR=0.66, 95%CI:0.52-0.80), but the association was not statistically significant in the western subjects (OR=0.95, 95%CI:0.73, 1.16). Conclusion: Hp+ and CagA+ infection are associated with a decreased risk of pancreatic cancer in eastern populations but have no significant associations in western countries.

Diabetes Mellitus and Site-specific Colorectal Cancer Risk in Korea: A Case-control Study

  • Objectives: Previous large-scale cohort studies conducted in Korea have found a positive association between diabetes mellitus (DM) and colorectal cancer (CRC) in men only, in contrast to studies of other populations that have found significant associations in both men and women. Methods: A total of 1070 CRC cases and 2775 controls were recruited from the National Cancer Center, Korea between August 2010 and June 2013. Self-reported DM history and the duration of DM were compared between cases and controls. Odds ratios (ORs) and 95% confidence intervals (CIs) were calculated by binary and polytomous logistic regression models. Results: DM was associated with an elevated risk of CRC in both men (OR, 1.47; 95% CI, 1.13 to 1.90) and women (OR, 1.92; 95% CI, 1.24 to 2.98). This association remained when we controlled for age, body mass index, alcohol consumption, and physical activity level. In sub-site analyses, DM was associated with distal colon cancer risk in both men (multivariate OR, 2.04; 95% CI, 1.39 to 3.00) and women (multivariate ORs, 1.99; 95% CI, 1.05 to 3.79), while DM was only associated with rectal cancer risk in women (multivariate OR, 2.05; 95% CI, 1.10 to 3.82). No significant association was found between DM and proximal colon cancer risk in either men (multivariate OR, 1.45; 95% CI, 0.88 to 2.41) or women (multivariate OR, 1.79; 95% CI, 0.78 to 4.08). Conclusions: Overall, DM was associated with an increased risk of CRC in Koreans. However, potential over-estimation of the ORs should be considered due to potential biases from the case-control design.

A Study of Decision Making Style according to Sasang Constitution (사상체질(四象體質)과 의사결정유형에 관한 연구(硏究))

  • 1. Objectives and Methods This study was performed to investigate the relationship between Sasang constitution and decision making style. Subjects were 69 men and 45 women. Decision making style score, physical measurements and results were measured and analyzed according to Sasang constitution. 2. Results Soeumin group had significantly high score in rational score compared with those of Soyangin. Soyangin group had significantly high score in intuitive score compared with those of Soeumin. Soeumin group had significantly high score in dependent score compared with those of Taeeumin. According to binary logistic regression analysis for decision making style score, Sasang constitution were significant risk factors and ORs of Taeeumin were significantly higer than those of Soyangin in rational score, ORs of Soyangin were significantly higer than those of Soeumin in intuitive score, and ORs of Soeumin were significantly higer than those of Taeeumin in dependent Score. 3. Conclusions Soyangin had significantly high score in intuitive score. Soeumin had significantly high score in dependent score. We found many evidences that Soyangin is intuitive style and Soeumin dependent style in the text of ${\ulcorner}$Dongyi Suse Bowon${\lrcorner}$ and other research. But as far as Taeeumin was concerned, the result of Taeeurnin’s was not coincident with other research. More cases and research were needed to confirm the personality and phychological type of Taeeumin. This study result will be an important method that classify Sasang Costitution and consultation of student career decision making and studying attitude.

Successful Factors of Smoking Cessation for Male Workers from Small and Medium Enterprises Registered with Local Tobacco Control Center (일개 지역금연지원센터에 등록된 중소규모 사업장 남성 근로자의 금연 성공 요인)

  • Objectives: The aim of the study is to identify the success factors of 6 months of smoking cessation targeting male workers in small and medium-sized businesses. Methods: The subjects of this study were smoking male workers who had got the visiting services at a tobacco control center to stop smoking from January 2018 to December 2020. Total number of the participants was 767. Results: The male workers in small and medium enterprises had a success rate of 20.2% for smoking cessation for 6 months. With multiple logistic regression, the factors causing the differences in the success rate for 6 months of the workers were as follows. - The odds ratios (ORs)(95% confidence interval, CI) of the age group of under 29 and 40-49 years were 0.10 (0.03-0.29) and 0.43 (0.24-0.76), respectively, compared to the 50 years and older group. The ORs (95% CI) of the daily average of smoking group of 11-20 and more than 21 were 0.52(0.34-0.80) and 0.46(0.24-0.90), respectively, when compared to the group of 1-10. The ORs (95% CI) of the supporting persons for quitting smoking group of with spouse and other family were 1.99(1.18-3.34), compared to the group with no one. Conclusions: This study showed that the chance of success for smoking cessation for 6 months is higher when the subjects have spouses and other family Supporting persons for quitting smoking. And it also shows that the less they smoke a day and the older they are, the more likely it is to succeed in quitting smoking.

Relationship between Hypertension and the Declining Renal Function in Korean Adults (한국 성인에서 고혈압과 신기능 저하와의 관련성)

  • This study examined the association between hypertension (HTN), estimated glomerular filtration rate (eGFR), and urine microalbumin/creatinine ratio (ACR) in Korean adults. Data for 8,922 adults (3,941 men and 4,981 women) aged ≥20 years from the Sixth Korean National Health and Nutrition Examination Survey VI (2013~2014) were analyzed. In men, after adjusting for the related variables, the odds ratios (ORs) of HTN [systolic blood pressure (SBP) ≥140 mmHg, diastolic blood pressure (DBP) ≥90 mmHg, or use of HTN medications] were significantly higher in the decreased eGFR group [eGFR <60 mL/min/1.73 ㎡, 1.98 (95% CI, 1.21~3.24)], elevated ACR group [ACR ≥30 mg/g, 2.03 (95% CI, 1.54~2.69)], and decreased eGFR plus elevated ACR group [eGFR <60 mL/min/1.73 ㎡ and ACR ≥30 mg/g, 6.03 (95% CI, 2.82~12.92)] than in the normal group (eGFR ≥60 mL/min/1.73 ㎡ and ACR <30 mg/g). In women, after adjusting for the related variables, the ORs of HTN were significantly higher in the decreased eGFR group (2.29, 95% CI, 1.27~4.13), elevated ACR group (2.22, 95% CI, 1.68~2.94), and decreased eGFR plus elevated ACR group (10.77, 95% CI, 3.89~29.82) than the normal group. In conclusion, HTN was associated with a decreased eGFR and elevated ACR in Korean men and women. In addition, the prevalence of HTN increased greatly when a decreased eGFR and elevated ACR occurred simultaneously.

Role of Gait Variability and Physical Fitness as a Predictor for Frailty Status in Older Women (여성노인의 허약 상태 예측을 위한 보행변동성 및 체력의 역할 검증)

  • This study examined the association of gait variability and physical fitness with frailty status in older women. In a cross-sectional design, 168 elderly women, aged 65 years and older (75.07±5.40 years), measured body composition, gait parameters gait variability, physical fitness variables, MMSE-DS and CES-D. Subjects were classified as robust, pre-frail, and frailty based on the Fried et al.(2001) criteria for frailty weight loss, exhaustion, low hand-grip strength, low gait speed, and physical inactivity. Logistic regression analyses were used to determine the odds ratio (ORs) and 95% confidence interval (CI) of frailty status for having gait variability and physical fitness levels. Compared to the robust group (OR=1), the frailty group had significantly higher ORs of having terminal double limb stance (OR=1.48, 95% CI=0.10-2.21, p=.049), step cadence (OR=2.06, 95%CI=1.20-3.43, p=.009) variability, and significantly lower ORs of having upper-strength (OR=0.49, 95%CI=0.31-0.77, p=.002) even after adjusting for age, education, comorbidity, K-IADL, MMSE-KC and CES-D score. The finding of this study suggested that terminal double limb stance, step cadence and upper body muscular strength were independent predictors of frailty.
