The Therapeutic Efficacy of Acupuncture for Chemotherapy-Induced Peripheral Neuropathy: A Systematic Review and Meta-Analysis (항암화학요법 유발 말초신경병증에 대한 침치료의 효과 : 체계적 문헌고찰 및 메타 분석)

  • Objective: This study aimed to report the therapeutic effect of acupuncture on chemotherapy-induced peripheral neuropathy (CIPN). Methods: The articles were sourced from databases including PubMed, EMBASE, Cochrane Library, CNKI, CiNii, WHO ICTRP, JSOM, KMBASE, KISS, NDSL, and OASIS as of July 2019. The main search keywords were peripheral neuropathy and acupuncture, and only randomized controlled trials using acupuncture for therapeutic purposes were included. Cochrane's risk of bias was used to assess the risk of bias, and the Review Manager 5.3 program was used for meta-analysis. Results: Six studies with a total 394 participants were included. When combined treatment of acupuncture and usual care was compared with usual care alone, quality of life improved more significantly in the combination treatment group (SMD=-2.71, 95% CI: -5.01 to -0.41, P=0.02, I2=97%). The CIPN pain score was lower among the combination treatment group, but not to a significant degree (SMD=-2.55, 95% CI: -5.14 to 0.04, P<0.05, I2=98%). There were no severe side effects in any studies. Conclusion: Acupuncture combined with usual care may be considered to safely relieve CIPN pain and improve quality of life for cancer patients. However, as there are few randomized controlled trials studying the effect of acupuncture on CIPN, further well-designed research is needed.

Effect of Herbal Bath for Duration of Pediatric Fever : a Systematic Review and Meta-analysis (한약재 약욕요법이 소아 발열기간에 미치는 영향 : 체계적 문헌고찰 및 메타분석)

  • Objective The purpose of this study is to evaluate whether herbal bath shortens the duration of pediatric fever. Methods Databases, such as Medline, EMBASE, Cochrane library, OASIS, KISS, KMBASE, CNKI, Wanfang Med Online were used to search randomized controlled trials. Those trials that included children younger than 18-year-old were evaluated for the clinical efficacy of herbal bath in fever by analyzing 'body temperature (BT) drop start time' and 'time to return to normal BT'. Results 12 studies or 1306 participants were analyzed in the study. When applying herbal bath in combination with the controlled treatment, 'BT drop start time' was decreased by 0.11 hours, and 'time to return to normal BT' was decreased by 10.94 hours compared to the controlled group. When comparing herbal bath with tepid bath, 'time to return to normal BT' was reduced by 8.19 hours in herbal bath group. In regards to safety, 3 studies out of 12 studies mentioned about its adverse events: 2 studies did not have any adverse events, and a minor needle-shaped bleeding had occurred in one of the studies. Conclusion Use of herbal bath as a treatment of fever in children was associated with shortened duration of fever. However, cautious interpretation is necessary due to its limited safety data.

  • The purpose of this study is to introduce the improvement of current operational climate prediction system of KMA and to compare previous and improved that. Whereas the previous system is based on GloSea5GA3, the improved one is built on GloSea5GC2. GloSea5GC2 is a fully coupled global climate model with an atmosphere, ocean, sea-ice and land components through the coupler OASIS. This is comprised of component configurations Global Atmosphere 6.0 (GA6.0), Global Land 6.0 (GL6.0), Global Ocean 5.0 (GO5.0) and Global Sea Ice 6.0 (GSI6.0). The compositions have improved sea-ice parameters over the previous model. The model resolution is N216L85 (~60 km in mid-latitudes) in the atmosphere and ORCA0.25L75 ($0.25^{\circ}$ on a tri-polar grid) in the ocean. In this research, the predictability of each system is evaluated using by RMSE, Correlation and MSSS, and the variables are 500 hPa geopotential height (h500), 850 hPa temperature (t850) and Sea surface temperature (SST). A predictive performance shows that GloSea5GC2 is better than GloSea5GA3. For example, the RMSE of h500 of 1-month forecast is decreased from 23.89 gpm to 22.21 gpm in East Asia. For Nino3.4 area of SST, the improvements to GloSeaGC2 result in a decrease in RMSE, which become apparent over time. It can be concluded that GloSea5GC2 has a great performance for seasonal prediction.

The Effect of Banhasasim-tang in Cough: A Systematic Review and Meta-analysis (기침에 대한 반하사심탕의 효과 : 체계적인 문헌고찰과 메타분석)

  • Objective: This study confirms the safety and efficacy of Banhasasim-tang (BST) against cough through systematic review and meta-analysis. Methods: PubMed, Cochrane, ScienceDirect, Wiley online library, OASIS, KISS, NDSL, RISS, Science and Technology Society Village, DBpia, CNKI, J-STAGE, and CiNii were used as search engines. The search period was from the start of search engine support to June 30. 2020. And search language was not limited. Cochrane's risk of bias (RoB) tool was used to evaluate the quality of the studies. A meta-analysis of the total effective rate (TER), cough symptom score, and spirometry results was performed using the Review Manager program. Results: 6 studies were finally selected. The group using BST showed significant symptom relief, as the odds ratio (OR) value of TER was 5.56 times higher (95% CI: 3.50, 8.82, P<0.0001) compared to the control groups. The reported adverse effects did not occur in the intervention group. In addition, the cough symptom score and spirometry results showed that BST was more effective than Western medicine alone. Conclusion: BST can be expected to relieve cough, and it can be applied to chronic cough that is not treated with existing treatments. However, due to the low quality of the selected articles, additional high-quality RCTs will be required in the future.

Literature Review and Application Research on the Medicine in Removing Nebula and Improving Vision (퇴예명목(退翳明目) 약재에 관한 문헌적 고찰과 활용연구)

  • Objectives : This study was planned to help clinical treatment by examining the efficacy and frequency of use of medicines with removing nebula and improving vision action, centering on historical literature. Methods : Chinese literature centered on CNKI published after 2000 and ophthalmic books of the Beijing University of Chinese Medicine Library, regardless of the publication period. Domestic literature was searched using NDSL and OASIS, and literature published after 2000 was referenced. Results : According to traditional medical classics, as medicine with removing nebula and improving vision action, medicine with dispelling wind and clearing heat action accounted for the majority. However, it is desirable to use these medicine in the early stages of the disease. Conclusions : 1. Among the external contacts that induce corneal opacity, the wind-heat pattern was the most common. Among the visceral manifestation patterns that induce corneal opacity, liver or spleen was the most common. Cold-tempered medicine which is dispelling wind, clearing heat and bottling up the liver were most commonly used as therapeutic medicine. 2. As removing nebula and improving vision medicine, Cassiae Semen, Chrysanthmi Flos, Eriocauli Flos, and Buddlejae Flos have been widely used. 3. It has been investigated that removing nebula and improving vision medicine can be effectively used in the early stages of various diseases that cause corneal opacity.

Effects of Banhahubak-tang on Gastroesophageal Reflux Disease : A Systematic Review and Meta-Analysis (위식도역류질환에 대한 반하후박탕의 효과 : 체계적 문헌고찰과 메타분석)

  • Objectives : This study was designed to investigate the effect of Banhahubak-tang on gastroesophageal reflux disease(GERD) through a systematic review and meta-analysis of randomized controlled trials(RCTs). Methods : RCTs using Banhahubak-tang on GERD was searched in databases such as EMBASE, PubMed, MEDLINE, CENTRAL, CNKI, KISS, RISS, ScienceON, and OASIS. RCTs published up to October 8th, 2021 were included. Meta-analysis was performed by synthesizing outcome data, including Total Effectiveness Rate (TER), Reflux Symptom Index(RSI), Reflux Finding Score(RFS), and Incidence of Adverse Reactions. RevMan 5.4 software was used for data analysis. The Cochrane collaboration bias risk assessment scale was used to evaluate the methodological quality of the included studies. Results : Ten RCTs met the inclusion criteria. The total effective rate was the most commonly used outcome measure. The meta-analysis revealed that the TER in the experimental group was higher than that of the control group(N=2, RR:1.22, 95% CI:1.09 to 1.36, P=0.0004, I2=0%)(N=6, RR:1.22, 95% CI:1.14 to 1.32, P<0.00001, I2=0%)(N=8, RR:1.22, 95% CI: 1.14 to 1.30, P<0.00001, I2=0%). On the other hand, RSI(N=2, MD : -4.29, 95% CI: -4.71 to -3.86, I2=94%), RFS(N=2, MD : -3.28, 95% CI: -3.71 to -2.85, I2=96%), and Incidence of Adverse Reactions(N=5, RR: 0.32, 95% CI: 0.17 to 0.61, I2=0%) in the experimental group were lower than that of the control group. Conclusion : Treatment with Banhahubak-tang was found to be effective on GERD. However the results might be biased because of the poor quality and small sample size of the included RCTs.

Acupuncture for Premenstrual Syndrome: A Systematic Review and Meta-analysis (월경전증후군에 대한 침 치료의 효과 : 체계적 문헌고찰과 메타분석)

  • Objectives: This study aimed to assess the clinical evidence supporting the use of acupuncture (AC) for premenstrual syndrome (PMS). Methods: We searched randomized controlled trials (RCTs) that used AC for PMS in 9 databases (PubMed, EMBASE, Cochrane Library, CINAHL, CNKI, CiNii, SCIENCE ON, KoreaMed, OASIS) from their inception to July 2022. Results: We identified 1,168 studies, of which 15 RCTs met the inclusion criteria. The risk of bias in the included studies was relatively unclear or high. Meta-analysis of 3 RCTs showed that the AC group had a significantly higher effective rate than the Medroxyprogesterone group (RR 1.48, 95% CI (1.23, 1.78), p<0.0001). Meta-analysis of 2 RCTs showed that there was no statistically significant difference between the AC group and the Sham AC group in the effective rate (RR 4.72, 95% CI (0.88, 25.36), p=0.07). In a review of individual studies, the AC group was more effective than the control group in terms of the effective rate, symptom scale, quality of life, adverse events, and recurrence rate. Conclusions: The AC group was more effective than general treatments groups such as sham AC, western medicine, and dietary supplements, and there were no serious adverse events. However, the evidence on the effectiveness and safety of acupuncture for PMS was inconclusive due to the small number of included studies and low quality. Therefore, systematic reviews based on more rigorously designed randomized clinical trials are needed in the future to properly evaluate the effect of AC on PMS.

Usage of Korean Herbal Medicine Banhabaekchulcheonmatang-Analysis of Korean Clinical Studies (반하백출천마탕을 사용한 국내 임상연구 분석)

  • Objectives : To investigate the clinical usage of Banhabaekchulcheonmatang (BBCT), this study reviewed clinical studies conducted in Korea. Method : Literature search was performed on four Korean database; OASIS, RISS, KTKP and NDSL. Studies published before July 2023 were categorized and analyzed according to the target disease. Results : Fifty-four studies including 36 single case reports and 18 case series were included in this review and the total number of case patients was 79. There were 24 patients with peripheral vertigo such as benign paraoxysmal positional vertigo and Meniere's disease, 17 with central vertigo such as cerebellar infarction, 15 with vertigo accompanied by digestive symptoms, 13 with headache, and 10 with other diseases. Vertigo treatment accounted for more than 70% of the total case and there was no cases of hypertension and dementia. The BBCT formulation used was decoction in 69 patients (87.3%), powder in 7 patients (8.9%), and combination of decoction and powder in 3 patients (3.8%). Conclusion : Clinical research should be accumulated to expand the use of BBCT in the treatment of hypertension and dementia. In order to increase the use of insurance-covered extract preparation, it is necessary to make an effort to include the various diseases actually treated with BBCT in the indications.

Review on the Study of Sibjeondaebo-tang (Shiquan dabutang) Published in Korea from 2000 to 2013 (최근 십전대보탕의 실험적 연구 및 치험례에 대한 고찰(국내학술지를 중심으로))

  • Objectives The aim of this review is to analyse the study tendency in papers related with sibjeondaebotang (Shiquan dabutang) which are published in Korea from 2000 to 2013. Methods We searched the four electronic database (NSDL, RISS, Korean traditional knowledge portal, OASIS) and checked relevant Korean journals from 2000 to 2013. We classified the papers by publish date, speciality, study method, and field of study, and analysed the study tendency. Results 1. 4 papers were published annually on average. 2. After classifying papers by the speciality of journal, continuous study was followed not only in korean medicine, but also in many specialities like Dietetics, Biotechnology and Pharmacology. 3. In study methods, clinical case was 29%, in vivo was 26%, and in vitro was 26%. 4. After classifying papers by field of study, beneficial effect was 62%, toxicity was 14%, qualitative analysis was 9%, and adverse effect and pharmacology was 3%. 5. In beneficial effect, it is effective in antioxidation, treatment and prevention of neurologic disease, skin disease, gynecologic disease and so on. It has also an effect in enhancing the immune system, improving the dysfunction of organs, and it can also be used for anticancer and anti-metastatic purpose. Conclusions These results suggest that Sibjeondaebotang (Shiquan dabutang) can be used as cure medicine, not just as herbal tonic, but there are not sufficient evidence based papers, so there should be further studies in order to establish Sibjeondaebotang as a cure medicine.

A Review on Korean Traditional Medicine Research against Cervical Dysplasia - Focused on Studies of Cervical Cancer and Cervical Dysplasia- (자궁경부 이형증과 관련한 한의약 연구 고찰 - 자궁경부암 및 자궁경부 이형증 논문을 중심으로 -)

  • Purpose: This review is to figure out evidence that suggest effectiveness of Korean Medicine treatments against cervical dysplasia. Methods: Studies on cervical cancer and cervical dysplasia were searched through 6 databases: Korean Studies Information Service System(KISS), National Discovery for Science Leaders (NDSL), Korea Institute of Science and Technology Information (Korean TK), Oriental Medicine Advanced Searching Integrated System (OASIS), the Journal of Korean Medicine, and the Journal of Korean Obstetrics & Gynecology. After that, the articles were extracted with reference point of Korean Traditional Medicine. Results: 37 articles were included lastly according to selection criteria. 3 of them were case reports on cervical dysplasia, and 34 were in-vitro studies on Human Papilloma Virus (HPV) positive cancer cell. In case reports, acupuncture, moxibustion, medical herbs and pharmacoacupuncture were used for treatments of cervical dysplasia with about 3 months. Experimental studies on cervical cancer cell showed that several herbs function with clear heat, eliminate stasis (淸熱解毒, 化瘀消腫) have anti-cancer effects inducing apoptosis. Conclusions: The results of articles are not enough to use in practice. Therefore, we indicates more advanced research methodology as follows: development of Korean Medicine treatment protocol with oral and external, in-vivo experimental study, and evaluation immunity index.
