Trend analysis of the number of nurses and evaluation of nursing staffs expansion policy in Korean hospitals (시계열 자료를 이용한 병원 간호 인력의 변화 추이 및 병원 간호사 확보를 위한 정책의 효과 평가)

  • Purpose : The purpose of this study was to analyze the trend of the number of nursing staffs and skill mix and to assess the effectiveness of hospital nurse expansion policies in Korea. Methods : The trend of the number of nursing staffs and skill mix were analyzed using time series data, which composed of yearly series data from 1975 to 2009. The impact of hospital nurse expansion policies was estimated by autoregressive integrated moving average(ARIMA) intervention model. Results : The number of general hospital and hospital nurses per 100 beds was decreased in late 1980s and late 1990s due to rapid growth of beds. As a result of the number of nurse aids per 100 beds decreased, skill mix became high in general hospital but nurse ratio among hospital nursing staffs was about 50%. Expansion of new nurse and revised differentiated inpatient fee were only effective in expansion of hospital nursing staffs. But they had no effect in general hospitals. Conclusion : In Korea, a few policies related to expansion of hospital nurses have an effect on increasing the number of hospital nurse. Nevertheless, level of hospital nursing staffs is inferior to that of general hospital.

  • Purpose: The research looks at differences between the 'General ward' and the 'Comprehensive nursing ward' in Seoul Medical Center, regarding the facility improvements and changes in nursing services. It investigates and analyzes spatial problems and improvement needs through a survey and conduct investigation of staffs. It is to propose the primary data for the architectural planning of the future ward with the comprehensive nursing service. Methods: Targeting the comprehensive nursing ward, changes of the physical environment and spatial problems are analyzed through a field survey, behavior investigation, present-condition investigation, and floor plan analysis. Results: The workforce is increased by approximately twice the amount of the nursing staffs in the comprehensive nursing ward, compared to the pre-general ward. When utilizing the general ward, various spatial problems arise due to the restrictions of the facility condition. Because Sub N.S is an important facility as a part of the nursing work function in the ward of the comprehensive nursing service, the opinions of staffs must be considered when selecting a location and composing a space. Implications: It can be used as a primary data for the comprehensive nursing ward when architectural planning of a new hospital.

Nursing staffs' Experience in Treatment and Managing Medical Problem in Long Term Care Settings (노인요양 시설 간호 인력의 의료적 처치 및 관리 경험)

  • Purpose: The purpose of this study is to describe the experience of medical treatment and management of nursing staffs in long term care settings. Methods: This study is qualitative research. The participants were seven nursing staff (five nurses and two nursing assistants) who had experience over six months in long term care facilities. Data were collected through in-depth interviews using semi-structured research questions. The collected data were analysed using qualitative thematic analysis method. Results: Four main themes were emerged as 'various medical treatments', 'difficulties about managing medical problems', 'lack of regulatory support' and 'strategies for solving medical problems'. Conclusion: The findings of this study provide the nursing staffs' difficulties and opinion about medical treatment and management in long term care settings. To provide more effective health service for the aged, the regulatory complement about medical treatment in long term care facilities should be considered.

  • The purpose of this study was to clarify residential satisfaction of the Living Environment according to responses of staffs in the Horsing Homes. For this, the data about residential satisfaction of the Living Environment of elderly with dementia were taken from the staffs in 2 nursing homes in Tokyo by the questionnaire. Additional information was collected by interviews and observation. It was impossible that the elderly with dementia answered their satisfaction and needs of spaces. For this reason, staffs who care the elderly with dementia during 24 hours and interact with the elderly with dementia were selected the subjects of this survey. Survey question asked for the general features of the staffs, satisfaction, dissatisfactory spaces and the reasons about dissatisfaction of the elderly with dementia. The data were analyzed using the frequency, percentage, crosstab, paired t-test with SPSS 11 program. The result of this study was as follows. First, most of the elderly with dementia were unsatisfied with their bedrooms and bathrooms in bedrooms. Second, the unsatisfied spaces among the living environment were bedroom, bathroom in bedroom, dining room, day corner, special activity room, common bathroom with mechanical bath, and dressing room. The reasons of the unsatisfied spaces were size, comfort, non-existence of the space, and noise in the space.

Factors Affecting Performance of the Infection Control of Carbapenem Resistant Enterobacteriaceae of Nursing Staffs in Long-term Care Hospitals (요양병원 간호인력의 카바페넴 내성 장내세균속균종 감염관리 수행 정도에 미치는 요인)

  • Purpose: This study aimed to identify factors affecting the carbapenem-resistant enterobacteriaceae (CRE) infection control performance of nursing staff, who closely contact patients with CRE in long-term care hospitals. Methods: A cross-sectional study design was used. A total of 135 nursing staffs working in seven long-term care hospitals in the southern and northern areas of the K province in Korea were included. We measured the CRE infection control general characteristics, knowledge, perception, and performance. Results: The main factors affecting the CRE infection control performance were education, knowledge, and perception. The model explained the 60.8% total variance in CRE infection control. Conclusion: Appropriate infection control strategies should be prepared to provide high quality nursing care and prevent the spread of CRE infection in long-term care hospitals. Establishing an efficient infection control system in long-term care hospitals is necessary.

The Development and Effects of a Tailored Simulation Learning Program for New Nursing Staffs in Intensive Care Units and Emergency Rooms (중환자실과 응급실 초보간호사를 위한 맞춤형 시뮬레이션 학습프로그램 개발 및 효과)

  • Purpose: The purpose of this study was to develop a tailored simulation learning (SL) program and to evaluate the effects of the program on the clinical competency, clinical decision-making competency, and communicative competency of new nursing staffs in intensive care units (ICU) and emergency rooms (ER). Methods: In this quasi-experimental study, fifteen nurses were assigned to the experimental group and fifteen to the control group. The experimental group was given the SL program of four sessions, whereas the lectures of four sessions were given to the control group. Data were analyzed using a $x^2$-test, Mann-Whitney U test, and Kruskal-Wallis test with an SPSS program. Analysis of covariance was used to treat the covariate of pre communicative competency between the experimental and control groups. Results: Based on the education needs of new nursing staffs in ICUs and ERs, three learning scenarios and one evaluating scenario were developed for the SL program. The score for clinical competency, clinical decision-making competency, and communicative competency were significantly higher in the experimental group compared with the control group. Conclusion: The SL program is an effective learning strategy for new nursing staffs in ICUs and ERs. These findings suggest that an SL program be offered as an alternative for new nurse orientation and continuing nurse education.

Current Status and Barriers to Health Care Services for Nursing Home Residents: Perspectives of Staffs in Korean Nursing Homes (노인요양시설 건강지원서비스 현황과 장애요인: 포커스 집단 면접)

  • Purpose: Focus group interviews were conducted to explore the current status and barriers of health care services in nursing home from the viewpoint of staffs taking care of nursing home residents. Methods: A qualitative thematic analysis using the focus group interview method was used. A total of 32 health care professionals (19 nurses and 13 social workers) from thirteen nursing homes in South Korea attended (5 focus groups) in 2014. Results: The two main themes were identified: 'minimal health care services that left personal care needs unmet' which has three subthemes of 'stereotyped and fragmented care by types of care providers', 'medically-oriented health care services' and 'health care services mixed with social or recreational programs'; and 'barriers to proper and timely care in nursing homes' with four subthemes including 'unmet care needs due to cognitive dysfunction or lack of expression', 'care guides or tools not suitable for long-term care facilities', 'health care needs that are beyond the facility's care boundary', and 'care delay due to lack of understanding on the older adult's status'. Conclusion: The findings from this study should help health care policy makers to recognize the factors that influence health care services and provide direction for nurses and other staffs involved in supporting health care services for nursing home residents.

A Study on Type of Clinical Experience of Adaptation of New Nursing Staffs in a General Hospital (일 종합병원 신규간호사의 실무적응 경험 유형)

  • This study designed to know subjective structure about clinical experience of adaptation of new nursing staffs in a general hospital and using Q-methodology. Using Q-methodology, 40 new nursing staffs sorted working a general hospital through personal interview. The statement positions on the sorting continuum were scored from +4(representing the highest level of agreement) through 0(at the neutral position on the continuum) through to -4(representing the highest level of disagreement). Factor analysis of the data was undertaken using PC QUANL program to determine if any patterns were discernible. The five types extracted all had eigenvalues greater than 1.0 and explained 62.38% of the variation in responses.: (1) New workload type, (2) Interest and responsibility type, (3) Improve the situation type, (4) Lack of knowledge and technology type, and (5) Excessive workload type. The significant differences found in the subjective structure of clinical experience of adaptation of new nursing staffs in ad general hospital. The findings of this study could contribute to the development of an effective programmes of education and personnel clinical practice strategies.

A Case Study on the Spatial Composition of the staffs at Nursing Homes for the Elderly with Dementia according to Ideal Distance between spaces - Focused on $K\;\cdot\;Y$ Nursing Home in Tokyo, Japan - (공간간의 이상적 거리를 통한 치매노인시설 직원의 공간구성 요구에 대한 사례 연구 - 일본 동경소재 노인전문 요양시설$K\;\cdot\;Y$시설을 대상으로 -)

  • The purpose of this study was to clarify spatial composition of the staffs at nursing homes for the elderly with dementia. For this, the data was taken from the staffs in 2 nursing homes in Tokyo by the questionnaire from December4 to 28 in 2004. Replicated MDS (Multi-dimensional Scaling) was employed for data analysis. The results of the study were following. Staffs perceived nursing home by four spatial compositions; facility spaces as like sanitation room, laundry, bathroom, washroom, washbowl, social interaction spaces as like training room, inner court/balcony, living room, staff related spaces as like room for staff, treatment room, fundamental living spaces as like bedroom, dining room. According to two-dimensional perceptual map, sanitation room was wanted near by laundry, treatment room was wanted near by room for staff, bedroom was wanted near by dining room, and training room, living room were wanted near by inner court/balcony. On the other hand, washbowl and washroom were desired somewhere near by every spaces. Therefore, simple shaped washbowls are desired to install in dining room, living room, hallways. And sanitation room and laundry were desired somewhere remote by dining room, bedroom, and living room. Responding upper results, it is recommended planning for visual and sense of smell isolation in case of having difficulty in floor structure and circulation.

Comparisons of the Nursing Workforce with Japan, and the U.S. (한국, 미국, 일본의 간호인력 양성 및 활동 현황 비교)

  • This study aims to derive implications on current problems in the nursing manpower management in Korea through the comparison with the nursing workforce and employmentrent in the United States and Japan. There are various issues in nursing policy, such as nursing shortage, quality of the nursing service, and the increased cost of management due to the high turnover rate of nursing staffs and it is urgent to seek various policy measures to resolve this. Although nursing shortage is a commom problem in the world, the U.S. and Japan were higher rate of employment than Korea in nursing staffs, which implicates the importance of the legislation of mandatory minimum staffing ratios, the establishment of policies such as the fees and charge policy and the nursing work environments. For quality nursing care and patient safety through the stable workforce of qualified nursing staffs, administrative mechanisms that support adequate nurse staffing and promote positive work conditions are needed, for which the improvement of legal system is required.
