Purpose: This study was designed to describe the experiences of dementia prevention program for older adults in nursing homes. Methods: Four focus group interviews (FGI) after the program were conducted with 35 participants. All interviews were audio-taped and transcribed verbatim. The transcribed data were analyzed by Colaizzi's phenomenological method. Results: Four themes emerged as a result of analysis. Participants without dementia reported "I don't have dementia yet." This statement probably reflects ignoring potential dementia. Motivation for participating the dementia prevention program were to reduce boredom, to enjoy exercising and communicating with others, and to follow group activities. Participants reported the positive effects of the program; (a) benefits from the repeating learned activities; (b) improvements of memory and emotional stability. A few participants reported their inability to follow the program due to physical and environmental limits. However, other participants reported their efforts to overcome physical limits and perform the activities regardless their limits. People who performed the activities with physical limits reported that their self-satisfaction was increased. Conclusion: Results of this study showed that participants without dementia were able to actively participate in dementia prevention programs and showed memory, emotional and behavioral improvements. Therefore, these findings can be used for developing customized dementia prevention programs in nursing homes.

Purpose: The purpose of this study was to empirically test a model of associations linking locations and competition among nursing homes (NHs), mediated by facility grade and registered nurse (RN) turnover, on activities of daily living (ADLs) in Korean NHs. Methods: This study used a cross-sectional design to identify causal factors on NH residents' ADLs. Data were collected from June 2017 to August 2017. A disproportionate stratified cluster sampling method of NHs across Korea was used to gain representation. The collected data consisted of location and the Herfindahl-Hirschman Index (HHI), RN turnover rate, facility grade, and NH residents' ADLs. Results: All pathways affecting ADLs were not significant, and the effect on facility grade was significant in RN turnover (β = -.59, p < .001). RN turnover associated negatively with facility grade. In other words, the higher the RN turnover, the worse the facility grade. Conclusion: This study is the first to examine the impact of location and HHIs, mediated by RN turnover rate and facility grade, on NH residents' ADLs. To improve residents' ADLs, subsequent studies are needed to identify the factors affecting ADLs utilizing other variables because this study did not identify factors that affect ADLs.
Purpose: This study was done to develop a postural-stability patient transfer technique for care helpers in nursing homes and to evaluate its effectiveness. Methods: Four types of patient transfer techniques (Lifting towards the head board of the bed, turning to the lateral position, sitting upright on the bed, transferring from wheel chair to bed) were practiced in accordance with the following three methods; Care helpers habitually used transfer methods (Method 1), patient transfer methods according to care helper standard textbooks (Method 2), and a method developed by the author ensuring postural-stability (Method 3). The care helpers' muscle activity and four joint angles were measured. The collected data were analyzed using the program SPSS Statistic 21.0. To differentiate the muscle activity and joint angle, the Friedman test was executed and the post-hoc analysis was conducted using the Wilcoxon Signed Rank test. Results: Muscle activity was significantly lower during Method 3 compared to Methods 1 and 2. In addition, the joint angle was significantly lower for the knee and shoulder joint angle while performing Method 3 compared to Methods 1 and 2. Discussion: Findings indicate that using postural-stability patient transfer techniques can contribute to the prevention of musculoskeletal disease which care helpers suffer from due to physically demanding patient care in nursing homes.
중국의 급속한 인구 고령화 상황에서 요양원은 노인요양을 제공하는 역할을 하고 있지만 요양원의 효율성에 대한 이해는 거의 없었다. 본 논문은 요양원의 효율성 향상전략을 제안할 목적으로 DEA 및 Malmquist 지수 분석을 활용하여 평가하였다. 효율성 평가지표는 병상의 수, 고정자산, 의료인원의 수를 투입변수로 하고, 자가간호 가능노인의 수, 부분간호 가능노인의 수, 와상노인의 수, 요양원의 수입을 산출변수로 하여, 계층화 분석결과 DEA-CCR에서는 베이징과 상하이가 조사기간 5개년동안 1.00의 결과를 나타냈고, DEA-BCC에서는 4개 지역(베이징, 장쑤, 산둥, 상하이)이 가장 높은 결과를 나타났다. Malmquist 지수(MPI)에서는 하이난이 가장 높은 것으로 나타났다. 효율성 평가 결과가 높은 성(省)의 요양원은 자원활용, 내부관리, 경영규모 등에서 효율적이고 기술 진보적인 반면, 효율성 평가가 낮은 지역의 요양원은 기술효율이 높아지는 특징을 보였다.
This study was done to investigate status of polypharmacy and inappropriate medications use of the elderly of in nursing homes. The subjects of this study were 270 elderly people in the nursing homes of G city and K city, In this study, the medications were classified by Anatomical Therapeutic Chemical (ATC) code, polypharmacy was defined as taking medications more than five, and inappropriate medications use were identified by Beers criteria. Data was analyzed by using descriptive statistics, t-test and one-way ANOVA. Total number of drug types in this study was the average $6.1{\pm}2.6$. The subjects with polypharmacy were 193(71.5%), and with inappropriate medications use were 138(51.1%). There was a significant difference in the polypharmacy according to the number of diseases(p<.001) and in the inappropriate medications use according to age(p=.018). Baesd on this study, polypharmacy and inappropriate medications use of elderly people were main problems that need to carefully assess for safe and correct medication usage in nursing home. Therefore, an ongoing medication monitoring system is necessary to minimize the adverse drug reactions of elderly.
Purpose: The purpose of this study was to verify the effects of the integrated dementia managing programme for the elderly with mild dementia in which the physiological, cognitive, emotional, sociological and spiritual aspects have considered. Methods: This study employed non-equivalent control group pre-post tests with the repeated measure design. The subjects of the study were 39 elderly with mild dementia (20 in the experimental group and 19 in the control group) who have admitted to two nursing homes in B city. The data were analyzed by repeated measures ANOVA. Results: There were significant interactions between two groups and times in self-efficacy (p=.011), cognitive function (p<.001), depression (p=.005), spiritual well-being (p=.002), and quality of life (p=.037). The integrated dementia managing programme of this study showed significant positive effects on the quality of life for the elderly with mild dementia. Conclusion: On considering the current trends of aging and increasing number of the elderly with dementia, this integrated programme would be highly recommended to be used in nursing homes for the elderly with mild dementia, and contribute to improving their quality of life, and saving the social and medical expenses as well.
The purpose of this study was to explore the level of the moral distress for nurses working in long-term care hospitals or nursing homes, and identify factors that influence the moral distress. Data were collected through self-reported questionnaires including the Korean version of Moral Distress Scale-Revised (KMDS-R), Jefferson Empathy Scale for Health professionals (K-JSE-HP), Moral Sensitivity Questionnaire (K-MSQ), and the Hospital Ethical Climate Survey (HECS). A total of 194 nurses from 11 long-term care hospitals or 27 nursing homes completed the structured questionnaires. Data were analyzed using IBM SPSS Statistics version 25. As results, the mean score for moral distress was $73.81{\pm}51.29$ in this study. The moral distress of nurses working at nursing homes was higher than that of nurses working in long-term care hospitals. Among the sub-factors of moral distress, the 'futile care' was the highest score and the 'limit to claim the ethical issue' was the lowest. The main factor affecting moral distress among nurses in this study was the ethical climate of organization. In this paper, we propose that in order to effectively reduce the moral distress of nurses working in a long-term care hospital or a nursing home, it is more impactful to address structural issues related to the caregiver workplace than to adjust individual factors.
Purpose: The purpose of this study was to identify patient safety awareness and emergency response ability and affecting factors perceived by nursing homes and home visiting caregivers. Methods: This study was a descriptive study that conveniently extracts nursing caregivers who care for elderly patients in S and G provinces, Korea. Data collection was done by structural questionnaires from April to May 2018. A total of 204 responses consisting of 103 nursing homes and 101 home visiting caregivers were used for data analysis in SPSS Win 22.0. Results: Patient safety awareness and emergency response ability of nursing homes caregivers with each $4.24{\pm}0.50$, $74.26{\pm}09.57$ was each higher than that of the home visiting caregivers with $3.68{\pm}0.49$, $68.02{\pm}12.12$ (p<.001). The affecting factors of the patient safety awareness were working place, safety education, and daily average working hours with 12 or more (F = 27.30, p<.001) and that of emergency response ability were number of patients per personnel with 9 or more and emergency situation experience (F=14.00, p<.001). Conclusion: These results suggest that it is necessary to develop a safety education program that can share indirectly experience emergency situations that occur on the job site.
Purpose: This study investigated the patient safety culture (PSC), the perception of importance on patient safety management (PIPSM) and the patient safety management activities (PSMA) of care workers in nursing homes. This was a descriptive study that attempted to provide basic data for the patient safety education program of care workers. Methods: Data were collected using questionnaires and interviews from July 1 to 31 in 2020. One hundred and seventy-four care workers participated in quantitative research. The collected data were analyzed by the SPSS/WIN 25.0 program using descriptive statistics, t-test, ANOVA, Bonferroni, and Pearson's correlation. The qualitative data were collected through semi-structured, audio-recorded interviews with six representatives and six care workers from six nursing homes. Content analysis was performed to analyze the data. Results: Positive correlations were observed between PSC and PIPSM, and between PIPSM and PIPSM. Care workers' experience in patient safety management was in the following six categories: "Safety accident risk factors", "Type of safety accidents", "How to prevent safety accidents", "Effective safety management education", "Emphasis on occupational ethics of care workers", and "Needs for standard protocol" Conclusion: These findings indicate that considering the care workers' age and facility size, nurses should enhance patient safety education for care workers and establish a management activity system.
Healing Environment is to cure the user as a whole by considering his social, psychological aspect, beyond the functional aspect of treatment. Various design elements of healing space have been developed so far, but they were mostly described with same rank. Even though every element is significant, facilities such as nursing homes tends to run with low budget, and they can't fulfill them all at the same time. It is important to find the sequence between design elements. The purpose of this study is to reorganize the design principles of healing environment by Maslow's hierarchy of needs(Physiological needs, Safety needs, Love/Affilitation needs, Self-Esteem needs and Self-Actualization needs), and analyze elderly nursing homes and examine the framework itself by application. Research method was consist of literature review on healing space design elements and hierarchy of needs, reconstruction of design elements with hierarchy, and application on 5 Korean nursing home cases. As the result, Maslow's hierarchy of needs and design elements were linked with good balance. Analysis of 5 nursing homes revealed that lower needs were short of environmental support compared to higher needs. Especially design elements related to Physiological needs should be improved. Also, new framework had strength in not only providing gasps on current situation in detail, but also suggesting elements for improvement with hierarchy and sequence. Through this research healing environment design elements and human needs were linked. Nursing home is the last environment for the elderly, which should help to heal their body and mind and also support them to accomplish self-actualization.
