공공보건사업의 지역담당제 실시에 관한 보건기관 근무 공무원의 인식과 태도 (Recognition and Attitude to Implement at ion of Service Area Assigned System of Public Health Programs among the Health Officer)

  • 연구는 전라북도지역의 보건기관 공무원을 대상으로 지역담당제 실시에 관한 인식과 태도에 대한 기초자료를 얻기 위한 목적으로 시도하였다. 보건기관 근무 공무원들은 지역담당제에 대한 이해도가 매우 높았으며, 지역담당제 실시에 있어 그 실시의 필요성을 높게 인식하고 있으나, 그 실시시기는 단계적 실시의 주장이 높았으며, 제공서비스로는 방문간호와 만성질환관리에 높은 의견을 보였다. 건강증진사업에 국한할 시 건강교실이, 노인보건사업에 국한할 시 방문간호와 거동불편자, 독거노인 등이 높게 나타났으며, 지역담당제 구축을 위한 선결과제로는 재원확보, 인력 및 조직이 재정비가 가장 시급한 것으로 인식하고 있다. 제도개선사항으로는 근무여건 개선이 우선이었고, 지역담당제 구축을 위한 정보체계 확립이 부족하다고 인식하고 있으며, 팀별 지역담당제를 통한 보건사업 전달체계 확립을 위해 적정 전문인력의 배치를 가장 높게 들었다. 지역담당제 실시에 관한 이점으로 대상자에 대한 관리가 잘될 수 있고, 전문성 향상으로 환자에 대한 간호의 질이 향상될 것으로 인식하고 있다. 지역보건사업에서의 지역사회 주민의 요구증대 및 다양화에 따른 대응이 부족하며, 보건소의 사업의 내용개발에 있어서 충분히 개발되고 있지 못하다고 인식하고 있다. 그리고 새로운 보건사업의 확장에 있어서 가장 큰 문제로서 획일적(형식적)사업을 들고 있다. 이러한 연구결과 등을 토대로 하여 지역담당제 정착을 위한 검토방안과 추진전략에 대하여 아래와 같이 제언하고자 한다. 첫째, 지역담당제 실시는 단계적으로 담당지역을 선정하여 실시하고, 지역적 특성을 고려한 사업의 추진이 있어야하며, 현행의 보건사업 업무중에서 이들 사업이 지역담당중심으로 수행하는 것이 좋을 것인지 사업중심으로 수행하는 것이 효율적인 것인지를 사업대상 및 범위, 지리적 여건, 현재의 보건소 업무 수행체계를 근거로 검토되는 등 다양한 측면이 재고려되어야 한다. 둘째, 지역사회의 현실적인 요구의 수준과 보건소의 현재 위상을 중심으로 자원과 예산 소요를 추정하고 타당성과 현실성을 검토할 필요가 있으며, 이와 더불어 지역주민의 요구와 수요에 부응할 수 있도록 단계적으로 보건소 방문간호사 인력의 확충방안을 강구하고, 방문간호사 교육 훈련비 및 방문차량 구입비 등을 재원확보 및 지원을 하여야 하며, 보건소 사업에서 지역담당제에 관한 연구개발 및 시범운영이 있어야 할 것으로 판단된다. 셋째, 대상자 관리의 내실화와 지역사회 주민의 서비스 욕구에 대한 만족도를 제고시키기 위하여 사업내용을 충분히 개발해야 하며, 지역담당자의 근무여건을 개선하는 등 전문인력의 배치가 요구된다. 마지막으로 과거와는 달리 환경과 보건에 대한 일반인의 관심이 급격하게 확산되고 있는 상황 속에서 소수 전문가들의 실험적 시범사업으로는 대중적인 지지기반을 확대할 수 없기 때문에 지역담당제가 일반주민의 생활속에서 대중화된 사업으로 정착시킬 수 있어야 하며, 그동안 지역담당제에 대한 논의나 연구가 소수의 연구자 및 한정된 시범지역에 의해서만 연구된 점을 지양하고 폭 넓은 연구가 지속되어야 함을 제언한다.

유아 어머니의 자녀 성교육 실천정도 (Research on Sex Education of Young Children's Mothers)

  • Purpose: The present study is a descriptive research on the content of sex education conducted at home by the mothers of young children who are attending five kindergartens located in Busan, Korea. This study aims to provide basic materials for systematic sex education at home and in the school. Method: Based on the literature review and prior researches, the writer made a questionnaire consisting of 30 items regarding necessities, contents and difficulties of sex education. The 307 questionnaires were collected out of 480 which were distributed by the kindergarten teachers. Data was analysed with frequency and percentage using the SPSS Win 10.0 Program. The main subjects of the study were mothers in their thirties, who understood the necessity of sex education. However, they were lack of enough sexual knowledge in accordance with the transforming sexual culture and lack of suitable materials. They did not plan to educate their children according to their developmental stages, but just taught them verbally rather than using common materials for the situation. Result Regarding the origin of life that children often asks, mothers did not give a scientific and correct response to satisfy their curiosity about sex, saying that a baby was created after marrying a loved one. While more mothers than the prior research responded correctly to the queries of the path of delivery, some mothers did not. It may cause some children to suppress the questions, which may result a distorted sexual knowledge. That is because mothers understood that sex education was to deliver sexual knowledge, but not realized that sex education was a human education carried out throughout life. Mothers fell short of sexual knowledge, capability, and confidence, comparing with swift social changes and children's capacity. As a result, it is necessary to inspire sexual awareness into mothers and educate them through training. It is also needed to secure a training program similar to a teacher training program. Conclusion: It is essential to develop and to provide evaluation tools as well as proper materials in accordance with the children's developmental stage, so that organic management system among home, education institute and society must lead sex education Further, it is necessary to provide teaching methods and materials to mothers regularly, so that the school and home may conduct sex education together to cope with difficulties.

간호 관리 능력 개발을 위한 교육 연구 (Needs on Management Development Program for Head Nurse)

  • The objectives of this study are to identify the actual educational contents of management for head nurse and to propose the educational subjects according to identity the needs of head, charge, and staff nurses. The subjects were investigated the actual Management Development Programs and educational needs of head nurses and prospective nurse manager(charge nurse, staff nurse with a lot of clinical experiences) in general hospitals. The tools were composed of two questionnaires: One was developed from the literature review for making items to measure actual situation. The other was revised Katz's model for measurement of educational needs. The first respondents of actual situation were 27 general hospitals with over 400 beds in Seoul and the second respondents were 89 head nurses, 67 charge nurses and 136 nurses at 3 hospitals by convenient sampling out of 27 general hospitals. Data were collected by telephone interview, mail questionnaire and visiting from 7th of October through 30th of November in 1997. In data analysis, general characteristics of the respondents and actual status of Management Development Programs were analyzed by frequency and percentage. Educational needs according to general characteristics were analyzed by ANOVA The results were as follows: 1. Actual situation of Management Development Program 1) Seven hospitals(26%) had Management Development Program for prospective managers and 14 hospitals (52%) for head nurses. 2) Education Department existed in 14 hospitals (52%). 3) One hospital(4%) had top level managers took part in the Management Development. 4) Two hospitals selected head nurse, who had finished courses of Management Development. Eight hospitals(30%) assessed educational needs. The assessment tools consisted of making a question via questionnaire(75%), determining at department meeting(12%) and interview(13%). 5) Educational programs had 3 types: 10 lecture type, 7 discussion type and 4 role play type programs. 6) One hospital evaluated the change of learner's attitude. 7) Four hospitals scored educational point, but that was measured only by attending. 8) Actual Management Development Programs were as follows. parenthesis indicates the number of hospitals. The results of needs on Management Development subject 1) The educational needs of all respondents on 3 skill domains showed positive agreement to strongly positive agreement. 2) High priority(more than 4.5) items were 12 of 24 Human skill items(50%), 1 of 6 Technical skill items(16%), and 2 of 13 Conceptual skill items (15%). 3) Out of high priority items, 8 items were instituted. 4) All respondents showed high needs on 3 skill domains regardless of 3 positions (head nurse, charge nurse, and nurse). Educational needs of Human skill domain, according to position were 108. S, 108.7, 106.8 (mean score = 72) , needs of Technical skill domain were 26.5, 26.6, 26.I(mean score=18), and needs of Conceptual skill domains were 56.9,56.7, 55.1(mean score=39). 5) Needs of 3 skill domains according to clinical career showed significant difference. Out of respondents, nurses with career of over 16years showed lowest degree of needs in Human skill domains(F=4.47, P=.004) and Conceptual skill domain(F=2.93, P=.034). 6) Educational needs according to educational background were not significant difference. But out of respondents, nurses educated at 3-year junior college relatively showed lowest needs in all of the 3 skill domains. With the above-mentioned findings, further study is necessary for generalization of this study at hospitals with different bed size and location. Also it is needed to study about management skill of nurse and charge nurse, and effective educational method.

  • 최근 한국 내에서 서비스 디자인을 가장 활발히 연구하고 적용하는 분야 중 하나가 의료서비스 업계이다. 건강에 대한 관심의 고조로 의료서비스는 치료, 간호뿐 아니라 예방, 관리, 재활의 주체로 그 영역이 넓어지고 있다. 보건 의료 종사자는 서비스 공급자이며. 서비스를 제공받는 고객에게는 서비스 제공자의 전문적인 지식과 능력, 그리고 의료기술에 대한 신뢰가 가장 중요하다. 또한 그에 못지않게 의료기관의 시스템, 종사자들의 태도, 정보의 전달, 터치 포인트 등의 경험은 고객의 만족도를 좌우하는 주요한 요소가 되므로 이를 다루는 서비스 디자인은 더욱 주목 받고 있다. 특별히 다른 서비스업보다 고객의 예민한 상황과 감정을 다루어야 하는 의료서비스분야의 특성상 전문성이나 환경, 그리고 제품의 개선만으로는 충분한 만족감을 주기 어렵다. 그것은 감정 노동자라고 할 수 있는 서비스 제공자인 의료 종사자의 서비스 마인드와 태도의 변화가 함께할 때 실질적인 효과를 얻을 수 있기 때문이다. 따라서 이를 위한 교육이나 문제의식을 갖고 자체적으로 해결할 수 있는 시스템 등이 궁극적인 솔루션이라고 할 수 있다. 이 논문에서는 서울대학교병원에 적용된 서비스 제공자들을 위한 서비스 디자인 시스템과 교육 등을 포함한 몇 가지 방법을 소개하고 그 효과에 대해 다루고 있다.

DRG 지불제도에서 환자의 의료서비스 만족도와 제공량에 관한 연구 - 시범사업 전.후 제왕절개 분만 경험 산모를 대상으로 - (A Study on Patients' Satisfaction and Service Utilization in the DRG Based Payment System - Patients who Experienced Cesarean Section Before and After the Demonstration Program -)

  • Background : The objectives of this study were to examine patients' satisfaction with the DRG based payment method and its association with their awareness of the method, to examine patient reported changes in doctors' caring attitude, level of their out-of-pocket payments, providers' acceptance of patients' request for additional services after the program, and to examine changes in service utilization recorded in medical records. Method : One hundred-four patients who had cesarean sections before and after the demonstration program at two hospitals located in Seoul participated in the study. Patients were surveyed before discharges when their charges were finalized. Their medical records were reviewed as well to collect data for service utilization during hospital stays. The association between patients' satisfaction with the payment method and their awareness of the method was analyzed by ${\chi}^2$-test, and the significance of changes in providers' acceptance of patients' request for additional services and service utilization after the program were examined by ${\chi}^2$-test and t-test, respectively. Results : A large proportion of patients did not know of the DRG based payment method at the time of survey and a significantly larger proportion of patients who came to the hospitals with the knowledge satisfied with the method. About the same proportion of patients reported improvement and deterioration in doctors' caring attitude compare to the previous hospitalizations and a similar result was found concerning out-of-pocket payments. Providers' acceptance of patients' request for medication, PCA and painless delivery decreased significantly after the program whereas the acceptance for additional hospital days and laboratory and radiology tests did not. Length of stay, the numbers of days on antibiotics and antianemic medication, and the number of blood tests decreased significantly after the program, however, decreases in the rate of antianemic medication and the number of urine analyses were not statistically significant. Re-operation, in-hospital death, and complications were not observed before and after the program. Conclusion : The study findings indicated a need for better patient education and publicity about the newly introduced payment method to improve their satisfaction with the system. Other study findings concerning service utilization and quality of care indicators were consistent with the government funded evaluation studies.

임상간호원에 대한 연구조사 (A Study and Survey on Clinical Nurses concerning the General Items, the Motives of Determining their Profession, the Attitudes toward their Profession and the Desire and Expectation to their Profession and Society)

  • This study of 855 clinical nurses was conducted using a questionnaire that include tour different scales; the motives of determining their profession, the attitudes toward their profession, the general items, and desire and expectation to their profession and society. The data were analyzed by Chi-Square Test and Percentage. The results of this study included Hypothesis are as follows; The respondents were 855 (78.6%) among 1088 clinical nurses who were employed by General of Educational hospitals through the city of Seoul, Pusan, Taegu, Daejun, Kwangju, and Wonju. 1) a. In the Age Distribution, the majority of respondents were under the age of 30yrs(88.2%) and the minority were above 31yrs(11.2%), and the preponderance of the majority to minority(9:1) was noted. In compared with area, a group above 31yrs old in Seoul (6.9%) was lower than other area (16.3%). b. The types of Educational background were 16.3% in Voc.Tr. School, 66.5% in Diploma and 17.1% in Degree.146 clinical nurses were from the Degree course, and 142 (97.3%) CN among those of them were occupied around Seoul and 4(2.7%) around other area. c. In the Marital Status,71,5% were the unmarried and 28.5% were the married. And compared with the area was 20.4% in Seoul and 41.4% in other area. d. Most common Length of Clinical Experience after graduation was under tile 2yrs (55.4%), 3yrs(14.2%, and 4yrs (6.2%). In compared with area, Seoul (15.3%) was lower than other area (38.1%) above 5yrs of clinical experience, and the preponderance of tile other area to Seoul as 2.5: I was noted. 2) a. Hypothesis 1 was significant relation between the types of Educational Background of the CN and their motives for selection of Nursing, P-value was below 0.01. b. There was a significance on hypothesis 2 (P<0.01): that was relation between their motives for selection of clinical nursing field after their graduation and the area which they were employed. c. Hypothesis 4 was accepted as significant relation between the level of satisfaction of their clinical experience after their graduation and the types of educational back ground, P-value was below 0.01. d. There was a significance on hypothesis 5(P<0.01) that was relation between the CN's response about the orientation program and the area which they were employed. e. Hypothesis 6 was retained as significant relation between the area and inservice educational programme of their employed hospital was practising or not. P-value was 0.01. f. Hypothesis 7 was retained as significant relation between the area and the CN's response about the inservice educational programme of their employed. P-value was below 0.01. g. There was a significance on hypothesis 8 (P<0.0l) that was relation between the CN's experience on attending the professional meeting and the area. h. Hypothesis 10 was accepted as significant relation between the response about the present licence system and their educational background. p-value was below 0.01. i. There was a significance on hypothesis 11 (P fO.01) that was relation between the carrying out the regular and delivery vacation and the area. J. Hypothesis 12 was accepted as significant relation between the CN's consideration of the lack of leisure and their marital status. p-value was below 0.01' k. There was a significance on hypothesis 13 (P <0.01) that was relation between the CN's response about their salary and their marital status. l. Hypothesis 14 was significant relation between the most difficulties of CN during their working and the hospital which they were employed. p-value was below 0.01.

모자건강관리를 위한 위험요인별 감별평점분류기준 개발에 관한 연구 (A Study on the Risk Factors for Maternal and Child Health Care Program with Emphasis on Developing the Risk Score System)

  • For the flexible and rational distribution of limited existing health resources based on measurements of individual risk, the socalled Risk Approach is being proposed by the World Health Organization as a managerial tool in maternal and child health care program. This approach, in principle, puts us under the necessity of developing a technique by which we will be able to measure the degree of risk or to discriminate the future outcomes of pregnancy on the basis of prior information obtainable at prenatal care delivery settings. Numerous recent studies have focussed on the identification of relevant risk factors as the Prior infer mation and on defining the adverse outcomes of pregnancy to be dicriminated, and also have tried on how to develope scoring system of risk factors for the quantitative assessment of the factors as the determinant of pregnancy outcomes. Once the scoring system is established the technique of classifying the patients into with normal and with adverse outcomes will be easily de veloped. The scoring system should be developed to meet the following four basic requirements. 1) Easy to construct 2) Easy to use 3) To be theoretically sound 4) To be valid In searching for a feasible methodology which will meet these requirements, the author has attempted to apply the“Likelihood Method”, one of the well known principles in statistical analysis, to develop such scoring system according to the process as follows. Step 1. Classify the patients into four groups: Group $A_1$: With adverse outcomes on fetal (neonatal) side only. Group $A_2$: With adverse outcomes on maternal side only. Group $A_3$: With adverse outcome on both maternal and fetal (neonatal) sides. Group B: With normal outcomes. Step 2. Construct the marginal tabulation on the distribution of risk factors for each group. Step 3. For the calculation of risk score, take logarithmic transformation of relative proport-ions of the distribution and round them off to integers. Step 4. Test the validity of the score chart. h total of 2, 282 maternity records registered during the period of January 1, 1982-December 31, 1982 at Ewha Womans University Hospital were used for this study and the“Questionnaire for Maternity Record for Prenatal and Intrapartum High Risk Screening”developed by the Korean Institute for Population and Health was used to rearrange the information on the records into an easy analytic form. The findings of the study are summarized as follows. 1) The risk score chart constructed on the basis of“Likelihood Method”ispresented in Table 4 in the main text. 2) From the analysis of the risk score chart it was observed that a total of 24 risk factors could be identified as having significant predicting power for the discrimination of pregnancy outcomes into four groups as defined above. They are: (1) age (2) marital status (3) age at first pregnancy (4) medical insurance (5) number of pregnancies (6) history of Cesarean sections (7). number of living child (8) history of premature infants (9) history of over weighted new born (10) history of congenital anomalies (11) history of multiple pregnancies (12) history of abnormal presentation (13) history of obstetric abnormalities (14) past illness (15) hemoglobin level (16) blood pressure (17) heart status (18) general appearance (19) edema status (20) result of abdominal examination (21) cervix status (22) pelvis status (23) chief complaints (24) Reasons for examination 3) The validity of the score chart turned out to be as follows: a) Sensitivity: Group $A_1$: 0.75 Group $A_2$: 0.78 Group $A_3$: 0.92 All combined : 0.85 b) Specificity : 0.68 4) The diagnosabilities of the“score chart”for a set of hypothetical prevalence of adverse outcomes were calculated as follows (the sensitivity“for all combined”was used). 후기 학령기 아동의 성지식 요구 (The needs for sex knowledge in the late schooler)

  • The goal of this research was to provide the basic data of sexual education for late schooler by finding out what they want to know about sex, or to see, if any, its relevancy between female and male students and between each grade. The sample for this research was a total of 453 students in 4th, 5th and 6th grade from 12 different classes at two elementary schools which were located in C city and in B province. The children were requested to write down three points on what they want to know or to learn most about sex, and their answers were put through two analytical stages in order to classify and to examine. The followings are the conclusions from the data. 1. The female cluster took 44.2% out of the whole sample, and there were 151 students per each grade. The specific girl ratios for each grade were 42.4%, 50.3% and 39.7% respectively. 2. It was noted that a total of 1,195 questions were gathered from the students writings, 45% of the questions was raised by the female students. The 4th grade students raised 432questions (43.8% by the girl) while the 5th. and the 6th grade students raised 387 (53.2% by the girl) and 376 questions (42.3% by the girl) respectively. The average number of questions per students was 2.6 while the female students raised 2.8 questions which was more than the average 2.5 questions by male students. It was the 4th grade female student cluster, which raised the most question (3.0) while the 6th grade male students raised the least question (2.4) in average. 3. The questions raised by the children could be divided into seven categories of the knowledge need on sex. They were Reproduction (310 questions, 25.9%), Sexual Culture (230, 19.2%), Concept of sex (125, 18.0%), Changes of Puberty (172, 14.4%), Sexual Health (119, 10.0%), Anatomy and Physiology of Reproductive System (78, 6.5%) and Sexual Tendency and behavior (71, 5.9%). 4. 'Reproduction' was the most frequently raised questions not only by both sex groups but also by the 5th grade students. Both sexes in the 4th grade showed the highest interest in Sexual Culture while it was Changes in Puberty for the both sexes in the 6th grade. The knowledge need on sex of the children indicated what they understand sexuality. They apprehended sex as sex, gender and sexuality in an inclusive way. They showed a major interest in the biological sex and the sexual activities. It seems that the children managed to understand clearly the meaning of gender, furthermore, they even pointed out that the streotyped sex role, patrimonial system and the sexual discrimination were unreasonable. The students possessed not only the least but also the most negative understanding in regard of sexuality. Two suggestions were made from the above conclusions for the practical sex education as well as its research. 1. For the practice: The sex education for the elementary upper grade school student should be relevant with their cognitive characteristics, also it should be more specific on the reproductive organs of both sexes, the actual scenes of the pregnancy and child delivery procedures. The gender concept should be added to the biological sex education, which will lead them to understand the unreasonableness of today's male-superior phenomenon and correct them. It is also necessary to develop educational programs for this age group so as to help them to understand sex in the sense of sexuality as well. 2. For the research: It is not easy to draw out a through conclusion since this study was carried out as one-time data collection. Yet it is undeniably helpful for the sex education if we can understand what the children want to know about sex, how much they know about it by conducting deep-interview researches through a small number of sample.

  • 본 연구는 농촌지역의 보건의료수준이 의료자원의 양적, 질적 격차와 의료이용과 의료접근도 및 건강수준의 면 등에서 도시지역보다 낙후되어 있다는 사실을 각종 통계지표를 이용하여 논증하였다. 다음으로 이러한 격차를 빚은 농촌보건사업의 문제점을 파악하여 이에 대한 대처방안을 농촌보건사업의 조직, 인력, 시설 및 장비, 재원 및 그리고 관리라는 5가지 부문으로 나누어서 모색해 보았는데 구체적으로는 첫째, 농촌보건 인력의 자질향상과 적정배치방안의 수립, 둘째, 농촌보건인력의 생산성 증대, 셋째, 보건소 및 지소의 운영개선, 넷째, 취약지 민간병원의 운영 개선, 다섯째, 사회, 경제여건의 변화에 따른 새로운 보건사업의 개발, 여섯째, 통합적인 보건의료인력관리 전담기관의 설립 등의 정책대안을 제시하고 있다.

윤리적 쟁점을 중심으로 한 보건의료정책 변화의 고찰 (A Review on the Change of Health Policy Based on Ethical Issues)

  • Health policy is a historical product in the process of development, including the political and economic factors of the state as well as the social and cultural elements of the country. Bioethics began to debate the ethical questions that arise in the overall process of life's birth and death, and gradually evolved by presenting ethical directions for various social phenomena. Especially, according to the moral awakening of 'scientific medicine' which caused in some human problems in the rapidized scientific society from the late 19th century to the early 20th century, as a result of distress including the concept of various social relations, it is possible to say that it has reached the bioethics. Although health policy and bioethics are different in terms of starting and concept, they can be found in common with social, cultural, and political diversity in the times. In 2004, 'Bioethics Law' was enacted through the issue of research ethics in the life sciences. Therefore, in order to examine ethical aspects of current health policy direction and major issues, it can be divided into before and after enactment of 'Bioethics Law' in 2004. The authors would like to examine how the evolution of the ethical viewpoint on the health policy has changed in line with the enactment of the 'Bioethics Law' and how it is trying to solve it from an ethical point of view. Through the various events that took place in the 1990s and the 2000s, various discussions on bioethics were conducted in Korea. Prior to the enactment of the 'Bioethics Law,' ethical judgments of professions, distribution of healthcare resources, if the discussion focused on the ethical judgment of abortion, and the various events that appeared in the early 2000s became the beginning to inform that the ethical debate about the life, death, and dignity of human beings began in earnest in Korea with the enactment of the 'Bioethics Law.' Since then, 'Hospice and Palliative care Law' which was enacted in 2017, is based on the fact that the health policy of our country focuses on the treatment of the past diseases, health promotion, and delivery of health care services. It was an opportunity to let them know that even the quality problems were included. Therefore, considering the various circumstances, the ethical issue facing Korea's health care system in the future is the change of the demographic structure due to aging and what is to be considered as the beginning and the process of life in the overall process of life. It is the worry about how to die and when it sees as death. This has far exceeded the paradigm of traditional health care policies such as disease prevention and management and health promotion, and calls for innovative policy response at the national level that reflects the new paradigm, which in many cases creates a predictable ethical environment. And health policy should be shifted in the direction of future ethical review considering sustainability in the development process of future health care rather than coercive management.
