Development and effect of Simulation-based Educational Program for Communication to Prevent Patients from Safety Accident by Nurses working in the Public Medical Institutions (공공의료기관 간호사의 환자안전사고 대처를 위한 시뮬레이션 기반 의사소통 교육프로그램 개발 및 효과)

  • This study focused on the development of a simulation-based educational program for communication, targeted to prevent safety accidents of patients. Application of the program to nurses working in public medical institutions was assessed to identify the effects of the program in terms of communication skill, self-efficacy, critical thinking disposition, and problem solving ability of nurses, before and after completion of the education. Method: A total of 32 nurses, working in hospitals designated as public medical institutions, were selected as subject participants in the present study. Data were collected during the period August 19 to 20, 2019, and analyzed by SPSS 23.0. Results: Results obtained from the present study reveals significant efficacy of the educational program, in respect of improved communication skill, self-efficacy, and problem-solving ability. On completion of the educational program, we observed an increase in the critical thinking disposition of nurses, although it was statistically insignificant. Conclusions: We believe that the educational program developed in the present study can be exploited as a foundation for developing other programs intending to improve the communication skill of nurses employed in clinics of public medical institutions, targeted to prevent occurrence of safety accidents of patients.

Psychometric Evaluation of a Six Dimension Scale of Nursing Performance and Student Nurse Stress Index Using an Objective Structured Clinical Examination - Modules for Asthma and Type 1 Diabetes (객관구조화 임상시험을 활용한 간호수행능력의 Six Dimension Scale과 간호학생 스트레스 평가지수의 도구 평가-천식 및 1형 당뇨 모듈을 중심으로)

  • Purpose: The study purposes were to describe the process of developing the Korean versions of the Six Dimension Scale of Nursing Performance (Six-D) and Student Nurse Stress Index (SNSI) and psychometric evaluation of the two measurements. Methods: This was a methodology study using a descriptive cross-sectional design with 51 nursing students in 4th year of university. Internal consistency reliability was assessed using Cronbach alphas. Construct validity was determined by exploring correlations among Six-D, SNSI, objective structured clinical examination (OSCE), self-efficacy and grade point average (GPA). Results: Internal consistency reliability of Six-D and SNSI was acceptable with Cronbach's ${\alpha}$ of .95 and .82. Correlation analysis to determine construct validity revealed that Six-D presented positive correlations with OSCE (r=.109~.272) and self-efficacy (r=.005~.161) and negative correlation with GPA (r=-.246~-.394), although all were not statistically significant. SNSI presented all negative correlations with OSCE (r= -.007~-.238), self-efficacy (r=-.246~-.394), and GPA (r=-.092~-.426) and were mostly statistically significant except OSCE. Conclusion: Six-D needs more evidence to confirm validity to predict observed clinical competency and theoretical relationships with self-efficacy and GPA. However, SNSI presented trends of expected relationships with relevant variables. Therefore, further research is recommended in testing validity of Six-D with other student populations.

  • Objective: The aim of this study is developing an elderly automatic showering system by optimizing nozzle position and angle of water injection on ergonomics approach. Background: The elderly living in nursing home or hospital were increased by an aging population. Helping the elderly on showering is so hard. In addition, the existing showering/bathing systems are not effective because shower pattern of the elderly and washed range of nozzle were not considered. Method: Firstly, basic specification were determined by anthropometric approach. Secondly, position of nozzle and angle of water injection were determined through observation of elderly behavior on showering. And, finally, they were optimized by washing test and showering simulation. Results: On showering importance of body parts were able to analysis through observation of elderly behavior. The position of nozzle and angle of water injection was able to optimize by showering simulation. The automatic showering system was developed by considering their results. Conclusion: The most important technology of developing a showering system is the determining position of nozzle and angle of water injection, number of nozzle. It was developed by applying its results through user centered-research. Application: The user centered-research of developing products was able to apply directly to develop automatic bath, showering products etc. Further more it was available to apply senior friendly products.

Numerical Simulation of Pulsatile Flows around Micro-Stenosis for Blood Analog Fluids (혈액모사유체의 미세협착 주변 맥동유동 시뮬레이션)

  • Considering the role of viscosity in the hemorheology, the characteristics of non-Newtonian fluid are important in the pulsatile blood flows. Stenosis, with an abnormal narrowing of the vessel, contributes to block blood flows to downstream tissue and lead to plaque rupture. Therefore, systematic analysis of blood flow around stenosed vessels is crucial. In this study, non-Newtonian behaviors of blood analog fluids around the micro-stenosis with 60 % severity in diameter of $500{\mu}m$ was examined by using CFX under the pulsatile flow conditions with the period of 10 s. Viscosity information of two non-Newtonian fluids were obtained by fitting the value of normal blood and highly viscous blood. As the Newtonian fluid, the water at room temperature was used. During the pulsatile phase, wall shear stress (WSS) is highly oscillated. In addition, high viscous solution gives rise to increases the variation in the WSS around the micro-stenosis. Highly oscillating WSS enhance increasing tendency of plaque instability or rupture and damage of the tissue layer. These results, related to the influence on the damage to the endothelium or stenotic lesion, may help clinicians understand relevant mechanisms.

The Variation of Indoor Air Quality in Nursing Home

  • In this paper, we proposed a plan to maintain comfortable indoor air quality in nursing homes by suggesting ways to reduce items temporarily exceeding the reference values through real-time concentration variation analysis of indoor air quality. Five items including PM10, CO2, CO, VOC, and Radon are measured at nursing homes in spring (April) and autumn (September) was carried out and all of the measured items were analyzed to satisfy the criteria set by the Indoor Air Quality Control Act. As a result of the analysis of the real-time concentration change, the concentration of CO2 was close to the reference value based on the number of occupants in the sick room. Due to the disinfectant (alcohol) used to disinfect and the auxiliary tools (adhesive) used in the operation of the program such as making and coloring, it was analyzed to temporarily exceed the standard value in the hall. In conclusion, it is possible to provide pleasant indoor air quality and contribute to securing the nursing home's competitiveness if periodic ventilation, natural disinfectant and eco-friendly product are used in consideration of the thermal environment.

Impact of Inter-professional Attitude and Educational Burden on Clinical Nurses' Cardiopulmonary Resuscitation-related Self-efficacy Following Team-based Cardiopulmonary Resuscitation Simulation Training (팀 기반 심폐소생술 시뮬레이션 교육을 받은 임상간호사들의 전문직 간 태도 및 교육부담감이 심폐소생 관련 자기효능감에 미치는 영향)

  • Purpose: In-hospital cardiac arrest is rare, but often results in high mortality rates. Early and effective cardiopulmonary resuscitation (CPR) is crucial for survival and nurses are often the first responders. This study aimed to investigate how inter-professional attitudes and educational burdens affect self-efficacy related to CPR performance following team-based CPR simulation training. Methods: This retrospective observational study analyzed data from a satisfaction survey conducted after team-based CPR training sessions between January and November 2022. Of the 454 nurses surveyed, 238 were included in the study after excluding those with ambiguous responses. Multiple regression analysis was performed to assess factors influencing CPR self-efficacy. The factors examined included inter-professional attitudes and educational burden. Results: Higher levels of inter-professional attitudes, particularly regarding teamwork roles and responsibilities, lower educational burden, and a positive perception of CPR competence were all associated with improved CPR-related self-efficacy. Participants who reported higher engagement in teamwork, lower task load, and greater confidence in their CPR abilities demonstrated higher self-efficacy in performing CPR. Conclusion: Enhancing the competencies of nurses who may act as initial responders in CPR situations within or outside hospital settings can help save lives and support public health.

A Study on Fire and Evacuation simulation analysis for use of Disaster Vulnerable Personal Evacuation Device (재난약자 대피 도움장치 활용을 위한 화재 피난 시뮬레이션 분석 연구)

  • Purpose: In fire case, nursing hospitals are subject to considerable restrictions on evacuation due to the characteristics of occupants and vulnerable elements of buildings, it is important to make evacuation device for vulunerabale person, and need how to intend to increase the efficiency of evacuation by fire and evacuation simulation with helper Method: The smoke characteristics were analyzed by time through fire simulation, finally, the number of helpers according to the day and night was entered, and the evacuation completion time was compared and analyzed using the evacuation simulation. Result: It was found that the evacuation time was shortened by more than 20% when the evacuation assistance device was used for the vulnerable, and the evacuation time was delayed by almost 70% in case of a fire at night compared to the daytime. Conclusion: If the horizontal and vertical evacuation device are effectively utilized in actual fire situations, a strategy appropriate to the situation is deemed necessary. It is expected that evacuation efficiency will increase based on the use of horizontal evacuation evacuation device and vertical evacuation device by developing evacuation manuals

  • Known as an important criterion that evaluates performance of future high-speed backbone networks, burst data loss ratio is well-studied in Optical Burst Switching networks. Current literatures mostly focus on reduce burst loss ratio without considering the system stability and link utilization after reducing. In this paper, we propose a novel framework which comes from feedback theoretic to dynamically control burst loss ratio in OBS. The proposed scheme tries to track the pre-set values of burst loss ratio and increases the stability and link utilization degree. The simulation results show that measured burst loss ratio always tracks setup reference with small errors, wavelength channel utilization is increased up to 2% and the system stability is also improved.

The Supply and Demand Projection of Nurses in Korea (2010년까지의 간호사 인력 수요 및 공급 추계)

  • The study was conducted to project supply and demand of the nurses till year 2010 based on analysis of supply and demand of nurses up to year 1991. Results of the study will provide invaluable information for nurses manpower planning as well as overall health manpower planning for the 21th century. It is projected that nurses will be oversupplied based on the current prductivity which is undesirable situation if the quality of care is considered, and undersupplied based on the the medical law as well as optimal productivity. Thus, it is desirable to increase active supply of nurses. One of the ways of increasing active supply would be increasing the size of training and education. But, considering low employment rate of nurses which is about 59% better way of solving problems related to nurses shortage would be improvement in nurses' employment rate. According to simulation study done as part of this study, if nurses' employment rate goes up to 80%, there is no need for increasing the size of training to meet the demand at the level of medical law.

Changes in Fatigue and the Quality of Life of Cancer Patients Receiving Radiotherapy (방사선치료를 받는 암환자의 피로와 삶의 질 변화)

  • Quality of life(QOL) and fatigue in cancer patients receiving the radiotherapy was assessed. The subjects were 46 cancer patients who daily attended the radiotherapy department. Assessment was done on four occassions : the first assessment was done on the treatment simulation visit, the second one was four weeks after treatment started, the third one at the completion of treatment and the last assessment was done between six and eight week following treatment. The results are as follows : The fatigue scores of the patients at each stage of assessment ranged from 5.49 points to 7.67 points and highest score was recorded at the third assessment that is, at the completion of treatment. The fatigue points showed an increase from the 1st. to 3rd. stage. However, at the 4th. stage, fatigue points decreased to the level at the first stage of assessment. QOL were assessed in three areas namely, physical, emotional and social /functional. The QOL scores in the physical area showed the highest score, followed by social /functional and emotional areas. The QOL scores decreased gradually to the third. stage of assessment thereafter recovered to the level of the first. stage. Correlation between QOL and fatigue scores during the treatment indicated that the level of QOL decreased as the level of fatigue increased. In particular, fatigue persisted after completion of the treatment and showed a significantly negative correlation with QOL. The present study strongly suggests that a strategy to restore the emotional well being level of the patient should be devised in order to improve QOL and reduce fatigue of patients receiving radiotherapy.
