The Japan-South Korea Comparative Study about a Developmental Handicapped Child's Education, and the Actual Condition of a Welfare Work (한.일 양국간 장애아 교육 및 복지 실태의 비교)

  • The comparative study of the administration policy that the government and the local self-governing body have enforced to a handicapped child and the family, was made in South Korea and Japan. As data, I utilized brochures distributed to a handicapped child's guardian in the government publication, health center, and hospital of both countries and the homepage that each organization manages. With the investigation of (1) the handicapped child's present condition, population and entering-school situation, (2) the organization which can consult about a juvenile entering-school problem, (3) the public service with which the parents of handicapped child, or a handicapped child are provided and (4) the feedback surveys of (3)’s services, the followings were proved. In (1), the handicapped child's population and their school attendance are not be specified by the South Korea side. In (2), a private consultation organization is mainly opened and be hard to say that use is simple from the little of a kind. In (3), there is almost nothing than rehabilitation education as the administration policy, which is universally held for the handicapped child. Besides they cannot receive freely the education. In (4), it became clear not to carry out. The improvement from the direction of both hard and soft aspects - the institution of basic education for the handicapped child who lives in all areas and the equal opportunity to all children - is called for urgently.

Mental Disabilities Mental Health Center Services Impact on Quality of Life (정신장애인의 정신건강증진센터 서비스 이용이 삶의 질에 미치는 영향)

  • A study of the factors influencing the quality of life and social support of people suffering with mental disabilities, who use mental health centers. The purpose of the study includes the general characteristics of subjects, characteristics of the mental health center, the mean score of social support and quality of life of people living with mental disabilities. The 132 questionnaires were administered by meeting directly with subjects and personal visits. 112 Questionnaires were returned(85% response rate). Data was collected from mentally disabled patients in Chungnam Mental Health Center. difference in quality of life by using t-test, one way ANOVA table, the relationship between social support, quality of life by pearson correlation coefficients, and relevant factors affecting quality of life by multiple regression with the SPSS 19.0 program. This study revealed that the group that participated regularly in the day time rehabilitation program showed better social support and positive attitude to a better quality of life than the other group that participated irregularly. This result is directly proportional to the participation frequency to the program during the recent 3-month duration. Another correlation with religion was found in the study due to the fact that a buddhist group showed positive significance in quality of life than non-religious groups. The results show positive correlation between the improvement of the quality of life of patients living with mental disabilities and the social support they receive.

A Study on the degree of satisfaction at Day Care Center's Service among elderly clients (일 주간보호센터 이용노인의 만족도에 관한 조사 연구)

  • The purpose of this study was to describe the degree of satisfaction at day care center's services and the degree of relationship between each service and overall satisfaction. The data collection was performed from July 1, 1999 to July 15, 1999. And total 199 elderly clients were interviewed in Koksung-kun. The results of the study is as follows: 1. The percentage of satisfactory response at transport service was 95.2%, food supply 94.3%, bath 93.2%, hair cutting 90.8%, physical therapy 90.2%, medical treatment 89.3%. And oriental medical treatment 82.0%. 2. For question of the degree of satifaction at day care service, the distribution of answer was 75.1% in exellent. 15.5% in good, 3.6% in moderate, 3.6% in poor, 2.1% in most poor. 3. The subjective satisfaction was not significantly different by sex, age, the presence or absence of religious, educational level, the presence or absence of family and economic level. 4. A significant association between the program satisfaction and the subjective satisfaction was observed: odds ratios were 26.9 in food supply, 26.4 in luncheon supply, 17.4 in bath, and 14.5 in hair cutting. The following is suggestion based on results of this study: A service program should be developed to fit conditions of rural elderies by specifically analyzing needs of the elderly.

A Study of Home Care Needs of Patients at Discharge from a General Hospital (가정간호 요구에 관한 연구 - 일 지역 종합병원 퇴원환자를 중심으로 -)

  • This study was conducted to provide basic policy of home care service centered on early discharging patients from general hospitals. This subjects for this study were 291 discharging patients from university hospital in Chon-ju area. The data were collected during the period from July 1, 1993 through July 16, 1993. The measurement tools were developed based on 9 categorized human responses patterns suggested by NANDA and modified by the research team. The collected data were processed with SPSS/PC + frequency, percentage and mean were used for analysis. From the study, the following summerized conclusions have been drown. A. For home care needs, the mean was $19.0\%$ of possible to total 100 and in a range of $6.2\~39.5\%$ 1. Exchanging Pattern: $17.8\%$ 2. Communication Pattern: $8.6\%$ 3. Relating Pattern : $15.4\%$ 4. Valuing Pattern: $13.4\%$ 5. Choosing Pattern : $6.2\%$ 6. Moving Pattern : $22.9\%$ 7. Perceiving Pattern: $16.5\%$ 8. Knowing Pattern : $30.8\%$ 9. Feeling Pattern : $39.5\%$ B. Response to home care services, 1. $85.6\%$ of subjects didn't hear about home care service. 2. Over $90.0\%$ subjects approved home care service. 3. $83.5\%$of subjects were willing to use home care service. 4. $85.9\%$ of subjects will follow to early discharge order. On the basis of this findings, further studies are required to compare home care needs between patients, their family and community health people groups. And also required to develop to information strategies for home care nursing service.

A Review on the Role and Functions of the Village Health Worker (마을건강원 역할 및 업무내용에 관한 고찰)

  • The objective of this study is to review the role and function of the village health worker in the wake of the recent social changes in Korea. The stud}' also aimed at' elucidating' the characteristics of the village health worker organizations developed through community participation in this country. The study methods employed were: a review of literature and other existing information related to VHW's activities ; the use of Questionnaire and interview with VHW's on their activities in one selected county in 1992. The major findings obtained are as follows: 1. So far the community health care activities have been carried out by medical colleges, foreign aid agencies and religious organizations with a view to improving health care for the population in rural area. 2. The VHWs employed by the local community health services received remuneratiorin cash or in kind. However, presently no remuneration is paid to VHWs employed by the government village health projects. The training for VHWs is organized by County Health Center once a year for 1-2days regardless of pre-employment or refresher. 3. The needs of care for maternal and child health and family planning has been decline due to the decrease of the target population, but on the other hand, the needs of care by VHW for prevention of infectious diseases, health of the aging, management of geriatric diseases, prevention of poisoning of pesticides and environmental sanitation has beer increased. 4. It was felt strongly that the training for VHWs should be strengthened by means of developing a problem-solving oriented educational program in order to improve practical skills of VHWs.

The Influencing Factors on the Degree of Nurse's Suffering Experience Caring for Terminal Cancer Patient (말기 암 환자를 간호하는 간호사의 고통경험 정도에 영향을 미치는 특성요인)

  • Purpose: The study was undertaken to examine the degree of nurse's suffering experience and to identify the influencing factors on nurses' suffering experience in Korea. Method: Data were collected using a questionnaire for 271 nurses working at 5 general hospitals in Daegu and Kyung-book province from Sep. 1, to Sep. 30, 2003. The questionnaire consists of 54 items, general characteristics(10) and nurse's suffering experience(44). All surveys were sorted and studied by frequency analysis, mean score, standard deviation, range, independent t-test, one way ANOVA, Pearson's correlation coefficient and Multiple regression. Result: The findings of this survey indicate 1) The degree of suffering experienced by nurses caring for terminal cancer patients was 2.96; 2) Demographic variables affecting the degree of nurses' suffering experience were age(F=5.62, p=.000), marital status(F=20.53, p=.000), religion(F=5.44, p=.020), career of clinical experience(F=6.96, p=.000), and feelings of end-life care(F=3.11, p=.016); 3) There were slight correlation between the subitem of nurse's suffering experience and general characteristics of subjects. For 'expanding self consciousness', age, career duration, and position; for 'forming empathy with family', age and career duration ; for 'spiritual sublimation', age, and career duration were affected variables. 4) As a result of the multiple regression analysis for predictable variables affecting nurses' suffering, it was found that 'career of clinical experience' was most significant(F=23.100, p=.000). The explanatory power of this regression formula was 17.6%. Conclusion: This study can provide the basic data useful towards improvement of nursing services for terminal cancer patients and the health of the nurse.

A Path Analysis on Morale among Staff of Long-term Care Insurance in National Health Insurance Corporation (노인장기요양인정조사원의 사기에 관한 경로분석)

  • Purpose: This study is designed to identify major factors that affect morale among staff of long-term care insurance in National Health Insurance Corporation. Methods: In order to collect the data, a survey was conducted by using the structured questionnaire based on 161 staff members of long-term care insurance of 50 long-term care operation centers, which were randomly selected by the table of random numbers in the whole centers of National Health Insurance Corporation from September 1 to 30, 2011. As for the data analysis, SPSS 18.0 was used to conduct the descriptive statistics, t-test, ANOVA, Pearson correlation coefficient, mutiple regression, and path analysis. Results: The average point of morale was 3.37, and had a negative correlation with the job stress, while it had a positive correlation with the social support, professional identity, and self-efficacy. Job stress, social support, and professional identity have a significant effect on morale among the independent variables. These variables have significant effects on morale, and also have a positive effect on self-efficacy. The findings show that self-efficacy mediates the process of morale. Conclusion: In this study, the factor which influences the morale was identified. It turned out that the morale could be improved by reinforcing the professional identity, managing the health status as well as rotating the working place. As such, it expects both the improvement of long-term care insurance services outcome and its quality through the morale management.

A Comparison of Characteristics between Home Health Care Needers and Non-needers in Rural Areas (농어촌지역의 가정간호 요구자와 비요구자의 특성 비교)

  • Purpose: The aims of this study were to describe general characteristics and needs of home health care, and to find the differences between home health care needers and non-needers. Method: In this study, 642 subjects participated who lived in Muan, Jollanam-do. Data were collected in August 2001 using a self-reported questionnaire. The questionnaire was a revised and simplified form of the Organization of Community Health System Program at the Seoul National University. Collected data were analyzed through Kruskal-Wallis test. t-test. and Chi-squire for cross-sectional analysis. Result: The average age of the subjects was 52.6 years and 33.3% of them aged over 65 years. Twenty six percent of them had chronic degenerative diseases. The percentages of hypertension patients and D.M. patients were 6.4% and 2.5%, respectively. The number of family members was 2.95 on the average, 2.19 in cases of families with the elderly and 3.33 in cases of families without the elderly. The rate of disability of the elderly was 10.5%. Marital status (p=.000), the number of family members (p=.000), education (p=.000), job (p=.000) and health insurance (p=.027) were significantly different between home care needers and non-needers. Home care needers had less living expenses (p=.001), more frequent hospital admissions (p=.004), higher chronic disease rate (p=.000) and more frequent visits to public health center (p=.027) than non-needers. Home care needers who wanted free service were twice as many as non-needers. Conclusion: Home care need was very high in rural areas and the needers had worse characteristics (low educational level. low income, no job and no family). Therefore, it is necessary to develop cheaper and more accessible services for home care needers in rural areas.

A Study on Job Characteristics, Job Motivation and Job Performance of Public Health Nurses (보건소 간호사의 직무특성, 직무동기, 업무수행 비교)

  • In Korea, visiting nursing services in pubic health centers have been in place since 1991. The purpose of this study is to compare the job characteristics, job motivation and job performance between visiting nurses and other public health nurses. 196 subjects were sent mailed questionnaires from Aug. to Sep., 1997. The response rate was 65.3%. Data were analyzed by a SPSSWIN program with a t-test, $X^2-test$, ANCOVA. The results were as follows : 1. The level of job ability in visiting nurses was higher than that of other public health nurses, but it was an insignificant difference(t=1.5975, p=.2078). Among job characteristics, skill variety was significantly higher in other public health nurses(t=8.2121, p=.0046); autonomy and feedback were significantly higher in visiting nurses(t=8.1877, p=.0047/t=7.7026, p=.0085). 2. There was no significant difference in the level of job motivation(t=.6485, p=.4216) and in job performance between the two groups (t=2.7393, p=.0995). In job performance, general nursing and organizational jobs were significantly higher in other public health nurses (t=4.8486, p=.1288/t=9.4237, p=.0024). 3. It was reconfirmed that there was, a significant difference in the level of skill variety, autonomy and feedback in analyzing age and career as covariates. 4. Significant positive relationship was found in job characteristics and job motivation(r=.5488, p=.0000). No significant relationship was found in job characteristics and job performance(r=.0486, p=.498). Through these results, it was supported that there were differences in some of the job characteristics and job performance between visiting nurses and other public health nurses. Therefore, it is requested to redesign job standards, training programs and management styles focusing on the job characteristics of visiting nurses.

A Survey on Health Service Activities of Public Health Nurses in Inchon & Kyung Gi Area (인천.경기지역 보건소 간호사의 보건사업 수행에 관한 조사 연구)

  • This survey was conducted in order to find out the degree of health service activities of Public Health Nurses & variables relevant to their activities. The subjects of this survey were 232 PHN in Health Centers, Inchon & Kyung Gi area. Data were collected by mean of questionairs from 30th, September to 30th, October, 1986. For the analysis, ANOVA was used. The results were as follows; 1. General characteristics of PHN: o An approximately four fifth of the subjects (82.3%) was between 20-39 years. o The average career length of the subjects as PHN was 4 years. o Their educational level was almost Nursing School (22.4%) Junior College of Nursing (53.4%). o The marital status of the subjects was already married (75.4%). 3. The degree of health service activities of PHN; o The total mean of health service activities was 2.147 (S.D: 1.477, range 11-5). o Among the 11 Health Service Categories; Health service planning (2.828) is the highest mean scored category. The lowest mean scored category was the other health services. o Among the specific activity items about the 11 health service categories, the highest scored activities were 'personal counselling & education' in well-baby care (2.504), family planning (2.530), Tb control (2.134), disease control (2.371), 'health history taking & health examination', in prenatal care, 'clinic care', in administrative service, 'record & report', in the other categories. o Almost categories (10), 'Home visiting' was the lowest scored activity. 3. The relevance for the health service activities by several variables were as follows; o The relevance for the health service activities by general characteristics of PHN was not shown significance at age, educational level, marital status, relegion status. o And at work place, & work department was shown high significance.
