Nursing Needs of the Parents of Infants in Neonatal Intensive Care Unit (신생아집중치료실 입원 환아 부모의 간호요구)

  • Purpose: The purposes of this study were to identify the perceived nursing needs and the differences by general characteristics of parents of infants in a neonatal intensive care unit (NICU) and to provide useful evidences developing a new intervention for family-centered care in NICU. Methods: A convenience sample of 121 parents of infants in NICU was used and the participants asked to complete the Korean version of NICU Family Needs Inventory (NFNI) from April to May, 2014. Data were analyzed using descriptive statistics, t-test and ANOVA. Results: The participants reported high average score in the perceived nursing needs. In subscales, needs for assurance rated highest score and needs for information, proximity, support, and comfort were followed. Mothers reported higher nursing needs score than fathers except the subscale of support. Conclusion: The findings suggest that nurses in NICU actively provide information about treatment, nursing, results of laboratory tests, and current status to give reassurance to parents. Not only providing informational support of parents of NICU infants, but also finding methods to contact to parents is necessary. In addition, nurses need to provide optimized intervention in current healthcare system and hospital environment.

The Effects of Nursing Work Environment, Job Stress and Knowledge Sharing on Nursing Performance of Nurses in a Tertiary Hospital (일개 상급종합병원 간호사의 간호근무환경, 직무스트레스, 지식공유가 간호업무성과에 미치는 영향)

  • Purpose: The purpose of this study was to identify factors influencing nursing performance of clinical nurses. Methods: This was a descriptive study involving 172 nurses who worked for 6 months or longer at one general hospital in B City. The data analysis was conducted with t-test, ANOVA, Scheffé test, Pearson's correlation coefficients, and stepwise multiple regression. Results: Nursing performance of participants positively correlated with job stress (r=.16, p=.033). The factors affecting nursing performance were total clinical experience (β=.37 p<.001), salary (β=.19, p=.028), and job stress (β=.13, p=.045). These variables had a 27.2% variance to explain nursing performance. Conclusion: Based on these results, administrative and financial support are required for the development of a clinical career program, including the differentiation of salary systems of clinical nurses. There is also a need to develop and implement job stress management and coping programs.

A Comparison of the Rates of Hemolysis and Repeated Blood Sampling using Syringe needles versus Vacuum tube needles in the Emergency Department (응급실에서의 주사기 채혈과 진공관 채혈의 용혈과 재채혈 비교)

  • Purpose: This study was done to compare the rates of hemolysis and repeated sampling in blood samples obtained by a syringe needle versus a vacuum tube needle. Methods: A randomized, prospective study was used to evaluate the differences between the two blood sampling methods. The study group consisted of patients seen in the emergency department (ED) for blood sampling to determine electrolyte level. ED patients were randomly assigned to either the syringe group or the vacuum tube group. All blood samples were collected by experienced ED nurses and hemolysis was determined by experienced laboratory technologists. Data were analyzed using Fisher's exact test and binary logistic regression. Results: One hundred forty-five valid samples were collected (74 in the syringe group versus 71 in the vacuum tube group). 5 of 74 (6.8%) blood samples in the syringe group and 8 of 71 (11.3%) in the vacuum tube group hemolyzed. Repeated blood sampling occurred for 2 of 74 (2.7%) and 3 of 71 (4.2%) in each group respectively. There were no significant differences in rates of hemolysis and repeated sampling between two groups (B=1.97, p=.204; B=2.36, p=.345). Conclusion: Venipuncture with syringe needles can be recommended for ED nurses to obtain blood samples.

A Clinical Trial on the Method of Urine Sampling for Urinalysis (소변검사의 검체 채취 방법에 관한 임상실험연구)

  • Improving validity and reliability is the important components of clinical laboratory tests. And the quality control of the test should be started with the accurate collection of specimen. Urinalysis is one of the useful and common tests in diseases diagnosis and determining the process of medical treatment. Since urinalysis is requested routinely in hospital setting, the importance of the quality control for urine specimen is often ignored. To improve the validity of urinalysis, a clinical trial was done on the method of collecting urine specimen. The result was as follows : 1. The rate of presumtive UTI(urinary tract infection) was decreased in 21.6% with experiment method for collecting urine specimen. 2. The rate of presumtive UTI in female patients was decreased in 43.2% with the experiment method. 3. The rate of negative urine culture was decreased in 6.6% with the experiment method.

Current Status of Home Visit Programs: Activities and Barriers of Home Care Nursing Services (재가간호서비스 제공자의 업무 수행 현황과 장애요인)

  • Purpose: The purpose of this study was to examine the current status of home care nursing services provided by community health nurses and to identify barriers to the services. Methods: A cross-sectional survey was conducted with three types of community health care nurses. Participants were 257 nurses, 46 of whom were hospital based home care nurses, 176 were community based visiting nurses, and 35 were long term care insurance based visiting nurses. A structured questionnaire on 7 domains of home care nursing services with a 4-point Likert scale was used to measure activities and barriers to care. Data were analyzed using SPSS WIN 21.0 program. Results: Hospital based home care nurses showed a high level of service performance activity in the domain of clinical laboratory tests, medications and injections, therapeutic nursing, and education. Community based visiting nurses had a high level of service performance in the reference domain. Long term care insurance based visiting nurses showed a high level of performance in the service domains of fundamental nursing and counseling. Conclusion: The results show that although health care service provided by the three types of community health nurse overlapped, the focus of the service is differentiated. Therefore, these results suggest that existing home care services will need to be utilized efficiently in the development of a new nursing care service for patients living in the community after hospital discharge.

Development and Evaluation of a Simulation-based Education Course for Nursing Students (간호학생을 위한 시뮬레이션 기반교육과정 개발 및 평가)

  • Purpose: This study was conducted to develop a simulation-based education course and to evaluate the results after the application for second year nursing students. Methods: This study was a non-equivalent control pre-post design. Based on the clinical situation scenarios about patients with COPD and MI, a total of two simulation-based learning modules was developed. Pretest and posttest was conducted to evaluate the difference in critical thinking disposition, problem solving, and clinical competence between two groups of 102 students for the experimental group, 2007 and 90 students for the control group, 2006. The experimental group conducted a clinical performance evaluation in the final test, on December 10, 2007. Results: In the experimental group, knowledge related to learning objectives was significantly increased and core intervention was performed almost exactly, but the same result was not observed in domains of analysis of laboratory test, and nursing education for patients. Self-evaluated clinical competence and problem solving level were significantly more improved in the experimental group than control group, but critical thinking disposition level wasn't. Conclusion: In conclusion, a simulation-based education course needs to utilize self-directed learning module like computer-based learning through web contents and MicroSim and video productions for improving nursing students' critical thinking.

Topics for Evidence-Based Clinical Nursing Practice Guidelines in Korea (국내 근거기반 임상간호실무지침의 주제 선정)

  • Purpose: This study was to identify topics for evidence-based clinical nursing practice guidelines in Korea. Methods: Data were collected from 330 staff nurses from 10 general hospitals and 53 nurses in charge of nursing education in 110 hospitals with over 500 beds. Using open questions, the nurses identified activities which could not be verified, which lacked consistency among nurses, clinical units and/or hospitals, which were not based on the up-to-date knowledge and which needed reform. The data were analysed by content analysis using a qualitative methodology. Results: Collected data consisted of 1882 clinical topics, which were classified into 50 topics, 207 mid-categories, and 456 sub-categories. The most frequent topics in order of frequency were medications, central line management, intravenous injections, urinary catheterization, perioperative nursing care, skin tests, pressure ulcer care, blood transfusions, laboratory examination-culture, respiratory care which were performed routinely in clinical setting by staff nurses. Conclusion: The research findings indicate the urgent need to develop evidence-based clinical nursing practice guidelines related to these research findings. Further research is needed to identify topics related to health promotion, and symptom/management of health problem.

An Analysis of Productivity according to the Actual Conditions of Dental Laboratory Technician (치과기공사의 업무실태에 따른 생산성 분석 연구)

  • The factors which impact on the productivity of medical organization are technique, manager, raw materials, management style, manpower structure, job construction, and so on. Human resources is the most important factor among them, but the productivity of serice organization can not be measured easily. The purpose of this study was to verify the effect of laboratory status, the degree of job cognition, and stress on the productivity of dantal prosthetic products, and to fine out the factors which impact on the productivity. The results were as follows: 1. The job characteristice of crown bridge laboratory part and porcelain laboratory part similar to each other. Also partial denture laboratory part and complete denture laboratory part were similar to each other. But that of orthodontic laboratory part was different from other. 2. the degree of job cognition made significant difference statisfically according to job characteristics of each dental laboratorise. 3. The correlation coefficient between job cognition of dental laboratory technician and workshop environment was.23, but the correlation between job cognition job stress appeared reversely, its coefficient was-.17. 4. The productivity of technician engaged in dental laboratory was much heigher than that of technician engaged in dental clinical laboratory room, and each were different in the degree of job cognition. 5. the most important factors which impact one the performance productivity of dental laboratory technician were the fact whether he married or not and his status.

A Study on Model Development for Urban Community Nursing Center (일개 도시 지역사회 간호센터 모형개발을 위한 요구조사)

  • Objectives: The purpose of this study was to identify basic health needs of adult clients to develop a community nursing center model in Seoul. Methods: Data were collected using a questionnaire survey from 894 adults registered at a public health center who were health management members, and visiting 4 community nursing centers, University of Wisconsin, Milwaukee during the period from July 1999 to January 2000. The data were analyzed using frequency, percentile, t-test, and ANOVA. Results: 1) The University of Wisconsin- Milwaukee School of Nursing has a long tradition of developing 4 community nursing centers with innovative health care programs. CNCs integrate the ability to implement and test effective intervention strategies with education, research, and practices of nursing students and faculty. They were designed to enhance the health status and quality of life for urban communities through the development of productive, outcome focused, collaborative partnerships among UWM-Nursing faculties and staff, other health and human service providers, consumers, and policy makers. It links the financial resources between UWM and 9 voluntary agencies and 12 public funding organizations including federal. state, and local governments. 2) Of the total health management members, 37.4% were reported to have at least one type of chronic disease such as hypertension, diabetes, and arthritis. Ten percent of them reported having obesity, and 44.2% reported lack of exercise. The health status of the subjects was within normal range in laboratory tests. However, female subjects showed more significant differences in obesity and cholesterol levels than male subjects. The subjects, who were in their 50s, showed more significant differences in obesity, SGOT, SGPT, and cholesterol levels than the subjects in other age groups. Conclusion: A community nursing center needs to be developed, that has a link between the nursing college and the public health center, with partnerships and a multidisciplinary approach. Based on the study results, exercise programs for middle aged adults are considered necessary. In particular, specific exercise programs for pre-menopausal women needs to be implemented in the future to prevent them from developing osteoporosis.

Correlates of Fatigue with Physiological Factors in Hemodialysis Patients (혈액투석 환자의 투석 전.후 피로정도와 생리적 요인간의 관계)

  • Purpose : The purpose of this study was to identify the relationship between fatigue and physiological factors in patients on hemodialysis. Method : Data were collected between June 1 and 7, 2003 using a structured questionnaire which included general characteristics, Visual Analogue scale and laboratory result. Result : Fatigue score changes is the mean of predialysis fatigue score was 4.36 and postdialysis fatigue score 4.66. There were statistically significant differences in score of postdialysis to sex, education and admission. The relationship between predialysis fatigue and interdialytic weight gain showed a positive correlation. The relationship between postdialysis fatigue and Cr showed a negative correlation. Conclusion : Nursing Management program should be developed to fatigue factors in patients on hemodialysis.
