Purpose: We aimed to describe and compare the cognitive function, anxiety, and quality of life for the demented elderly in day care centers or nursing homes. Methods: We selected 44 subjects from day care centers and 63 subjects from nursing homes by performing convenience sampling. The Korean version of Mini Mental Status Examination was used for measuring cognitive status; Rating Anxiety in Dementia, for assessing anxiety; Korean Version of Quality of Life-Alzheimer's Disease, for assessing the quality of life. The data were collected from May to June 2008 and analyzed by SPSS/WIN 14.0. Results: 1) The cognitive function of the subjects in the day care center was higher than that of the subjects in the nursing home. Anxiety in the subjects in the nursing home was higher than that in the subjects in the day care center. 2) The cognitive function and quality of life of the subjects in the day care center showed statistically significant correlation, 3) The anxiety and quality of life of the subjects in the nursing home showed statistically significant correlation. Conclusions: Nursing interventions that improve the quality of life of the demented elderly should be considered and applied according to the degree of cognitive function and anxiety.
The purpose of this study is to measure the resource use of the elderly in long-term care services and to examine the effects of patient and facility characteristics on their use of resources. The data were collected from 510 old people over sixty years of age, residing in five long-term care hospitals and two skilled nursing homes during the period between December 1, 2000 and February 28, 2001. For a full sample, when the first level of RUG(Resource Use Group)-III categories were employed as the proxy of patient severity, facility characteristics, such as location, size and ownership, have large effects on the resource use measured by service intensity, whereas patient characteristics such as severity have little or no effect. The resource use is significantly high if the facility: (1) is located in rural areas (gun): (2) has mare than 200 beds; (3) is a long-term care hospital; (4) is private; and (5) has a low percentage of medical aid patients. The analysis of the resource use in each RUG-III categories, for which ADL(Ability of Daily Living) were employed as the prosy of patient severity, shows a similar result. The loose relationship between the needs of residents and the resource use seems to be closely associated with the ineffective reimbursement system for providers. The current reimbursement system has no provision for quality improvement and reimburses facilities simply according to their types: fee-for-service for long-term care hospitals, and monthly-flat-rate or full-coverage-national-aid for skilled nursing facilities. It will be necessary to develop a more reasonable reimbursement system that takes patient's severity into account and gives incentives for long-term care providers to offer cost-effective services.
Purpose: The purpose of this study was to verify influencing factors on service quality provided by care helpers working for the elderly in long term care. Methods: The data were collected using self-reported questionnaire from 221 care helpers working in long-term care facility or home care service center. The data were analyzed using multiple regression with the SPSS/WIN 17.0 program. Results: There were significant differences in service quality depending on the health status, intimacy level, pleasure level, job education, and turnover intention. Factors influencing service quality were gender, intimacy level with elderly, type of working place and job satisfaction with $R^2$ value of 17.3%. The most influencing factor was intimacy level (${\beta}$=.249), followed by job satisfaction (${\beta}$=.208), gender (${\beta}$=.170), and type of working place (${\beta}$=.146). Conclusion: The results of this study indicate that the effort to improve the service quality of care helper should be focused on helping intimacy building between the elderly and the care helper. Also, a more effective way to improve service quality would be intervening for care helpers providing institutional care.

Revenue efficiency is the highest order of size are as follows. I8(98 people) \43,517,010, H6(86 people) \36,568,332, G8(78 people) \29,426,532, F8(68 people) \23,227,532, E8(58 people) \19,701,254, D8(48 people) \19,155,187, C6( 36) \14,389,109, B8(28 people) \9,920,031, A8(18 people) \3,721,031. It seems that its revenue efficiency even higher than the larger the scale. Meanwhile, The researchers focused on C6 (36 patients) model. Suggestion of this study are following; First, the arrangement can be provided based on needs of the elderly care facility staffing standards. Secondly, an elderly care facility selected wage guidelines. Thirdly, the elderly efficiency guidelines established by the size of a nursing facility. This study and other financial income factor are not the applicable limits.
The purpose of this study is to analyze physical characteristics, spatial composition, and using behavior of lounges in elderly skilled nursing facilities as spaces for activity programs and also to evaluate the spaces with the framework based on the indices for performing activity programs in elderly facilities. The results of the study were as follows: First, the lounge of an independent type ensured the privacy to perform programs, and so activities were managed more systematically. On the other hand, an expanded corridor type made the elderly feel difficult to pay attention to activities due to co-use of lounges and corridors. In a lounge of a hall type, the elderly had easy access to the place, but it also had weak home-like atmosphere because the space was used as a lobby entrance. Second, the facilities with western types of tables in whole lounges showed big changes in spatial composition, such as moving all the tables for any activities. It resulted in more preparation time and created obstacles in passages and space use. Third, in the evaluation of lounges based on the framework, most of lounges had accessibility of good quality, but they needed to improve home-like atmosphere and flexibility. To create home-like atmosphere, various spatial compositions and classifications in the lounge should be tried. Moreover, enough space and easy movable furniture can be considered for flexible spatial compositions.

All the subjects agreed to participate and filled out the survey questionnaire after signing the consent form. The instruments utilized in this study were the impairment of physio-sensory function, ADL IADL, cognitive function, and psycho-social function scale. This instrument was developed by modifying the scab developed by Gurland & Wilder (1984). Data was analyzed using the SPSS Win program. Physio-sensory functions (t=4.53. p<.001), ADL (t=3.61. p<.001), IADL (t=2.45, p=.014), cognitive functions (t=-2.63. p=.024) and psycho-social functions (t=3.69. p<.001) were significantly different between the two groups. The utilization of long-term care facility in the urban elderly was significantly higher than that in the rural elderly ($x^2=10.14$, p<.001). Conclusion: Considering these findings. the need for long-term care should be assessed by residence characteristics. Because of different utilization of long term care facility according to the elderly's needs, long-term care services should be considered the residence characteristics.
