Compassion Fatigue Experience of Oncology Nurses (암 병동 간호사의 공감피로 경험)

  • This study was conducted to understand the meaning and nature of the compassion fatigue experiences among nurses working in oncology wards. The participants in this study were 11 nurses who worked for at least 6 months in oncology wards. The data were collected using in-depth interviews and analyzed using the phenomenological method by Giorgi. Six categories of the meaning of compassion fatigue were derived as follows; physically difficulty, emotion is control of patient's condition, collapsing in front of cancer patients, reminding of patient-related memory, finding a changed attitude, and searching for how to recover.

Simulation training applying SBAR for the improvement of nursing undergraduate students' interdisciplinary communication skills (SBAR 적용 시뮬레이션 교육이 간호학생의 의료팀간 의사소통능력 향상에 미치는 효과)

  • In this paper, we aim to evaluate the effect of simulation training with a focus on the application of SBAR for the improvement of communication skills of nursing students with physician. The results of the analysis of 25 videos recorded pre/post-simulation were as follows: In terms of the SBAR score, "the most recently measured vital signs" in stage B increased significantly (Z = -2.448, p =.014); the frequency of step-by-step progress in SBAR did not advance to the SBA or SBAR stage in the pre-simulation stage, but increased to 48% post-simulation. The frequencies of SBAR evaluation items mentioned in the post-simulation were the following order: the name of the patient (96%), nurse's name (80%), most recently measured oxygen saturation (76%), and main symptoms (60%). The results of the nurse's judgment (A), request for additional prescription or request for the doctor's direct patient visit (R) were not mentioned. Therefore, it is necessary to consider the application of SBAR in simulation training, which requires problem solving through cooperation with physicians, because it has a positive effect on education in nurse-physician communication.

The Relationship of Emotional Burnout on Hospital Nursing Workload (간호사의 업무 부담에 따른 정서적 소진)

  • This study examined the nurse's emotional exhaustion and influencing factors, and the relationship between emotional burnout and hospital nursing workload using multilevel logistic regression analysis. The study subjects were 3,083 nurses of 65 hospitals, who participated in the training conducted by the Healthcare Industry Trade Union in 2010 and responded to the questionnaire. First, 71.6% of nurses experienced emotional exhaustion, and the average score of emotional exhaustion was 33.53. Second, the 'non - nursing work experience' showed a significant effect on emotional exhaustion. Third, the 'sufficient nursing staff' variable increased the emotional exhaustion. Fourth, the nursing grades were correlated significantly with emotional exhaustion compared to those with more than four beds per nurse. Fifth, in the case of working in the internal ward, the shorter the clinical career, the higher the emotional exhaustion. In other words, higher emotional exhaustion was associated with more nursing work experience, more perceived insufficient nursing staff, more nurses per bed, the department of internal medicine, and a shorter clinical career.

Role accomplishment and job satisfaction of hospice nurse (호스피스간호사의 역할수행 정도와 직무만족도)

  • Purpose: The purpose of this study was to provide the basic information on the improvement plan by identifying the role accomplishment and job satisfaction of the hospice nurses and analyzing the factors that impact such role accomplishment and job satisfaction. Methods: The data was collected of 189 hospice nurses who have been working more than 6 months in 56 hospice programs registered in Korea Hospice Association and Korean Catholic Hospice Association from October to November, 2007 by structured questionnaire which was developed by Ryu(1979), based on the Riehl's nursing role and the research of Choe(2005) on the role of hospice nurse. Results: The role accomplishment and job satisfaction of the study subject were 3.53 point and 3.39 point, respectively. The roles of hospice nurses were accomplished by the order of advocator, nursing care provider, coordinator (cooperation), educator, quality manager, counsellor, administrator, and researcher. The job satisfaction of the hospice nurses was shown highest in the satisfaction of professional status, followed by the satisfaction of the interaction, satisfaction of the task itself, satisfaction of the administrative aspect, satisfaction of the autonomous, and the lowest in the satisfaction of the pay. The role accomplishment of the subjects according to their socio-demographic and job characteristics was found to have a significant difference by their age, marriage status, academic background, and position. It also has a significant difference by whether they play role of coordinator, whether they took whole responsibility as hospice, working experience as hospice/palliative nurse, and the level of hospice/palliative nursing training(p<.05). In the study of job satisfaction of the subjects according to their socio-demographic and job characteristics, the job satisfaction was found to have a significant difference by the increase of age, the number of duties consisting the hospice team, whether they play role of coordinator, whether they take whole responsibility as hospice, the level of hospice/palliative nursing training, and whether they wish to work for as long as possible(p<.05). The role accomplishment and the job satisfaction of the subjects showed a statistically significant positive correlation. (r=.541, p<.01) Conclusion: Raised saiary will be increased hospice nurse's job satisfaction. And we suggest a repetitive study using the identical tool to the equally extracted subjects with same representativeness of each hospice/palliative institute type. For the expanded role and enhanced professional standard of hospice nurses, we also suggest a study on the improvement plan to enhance the roles of researcher and administrator.

Nurses' Question and Explanation Strategies for Effective Communication with Cancer Patients (암환자와의 효과적인 의사소통을 위한 간호사의 질문 및 설명 전략)

  • Purpose: This study was conducted to identify communication principles for nurses in cancer units according to Grice's theory. Method: This study was a secondary analysis of qualitative research using conversational analysis. Results: The principles consisted of four maxims: Quality, quantity, relevance and manner and 14 strategies. For the quality maxim, 'asking what you believe to be true' and 'saying the facts you know' strategies were identified. In the quantity maxim, the strategies were 'asking single questions', 'asking open questions' and 'asking optimistic questions'. In the relevance maxim, 'asking about the same theme' and 'giving relevant explanations' strategies were identified. In manner maxim, the strategies were 'using clear expressions in asking questions', 'asking in an orderly way', 'verifying answers', 'using clear expressions in explanations', 'explaining in an orderly way' and 'verifying understanding'. Conclusion: It is expected that these strategies will help and increase the degree of cooperation for nurses in effectively communication with cancer patients.

Difference of Conflict Levels of Nurses and Nurse-aids against Doctors (의사와의 관계에서 간호사와 간호조무사의 갈등수준 비교)

  • 본 연구는 간호사와 간호조무사의 동기를 제고하고 효과적인 간호서비스 제공을 촉진하기 위해 의사와의 관계에서 간호사와 간호조무사가 느끼는 직무 및 대인관계의 갈등 정도와 원인을 파악하고자 수행하였다. 이를 위해 중소규모 병원에 근무하고 있는 간호사와 간호조무사 271명을 대상으로 직무 및 인간관계 갈등 정도, 원활한 의사소통 여부, 갈등 원인과 해결방법 등에 대해 설문조사를 실시하고 분석하였다. 분석 결과 의사와의 관계에서 간호사와 간호조무사가 느끼는 갈등의 내용과 정도는 대체로 비슷하였으나 의사와 간호직의 역할이 중복되거나 모호한 부분이 있다고 느낀다는 점에서는 간호사가 간호조무사 보다 더 많은 스트레스를 받고 있었다. 의사와 갈등이 발생했을 때 해결하는 방법으로는 간호사와 간호조무사 모두 적당히 피한다는 의견이 가장 많았다. 그 다음의 갈등해결 방법은 간호사와 간호조무사 간에 차이가 있었는데 간호사는 상대방에게 나의 의견을 따르도록 함, 노조를 통해 해결등의 순이었고, 간호조무사는 상사에게 해결하도록 함, 서로 조금씩 양보해서 타협함 등의 순이었다.

Mediating Effect of Social capital between Transformational leadership and Organizational Commitment of Nurses in Hospitals (병원간호사의 변혁적 리더십과 조직몰입의 관계에서 미치는 사회적 자본의 매개효과)

  • This study examined the mediating effects of social capital between transformational leadership and organizational commitment in hospitals nurses'. The survey was conducted with 244 nurses working in a general hospital. The data was collected with the transformational leadership, social capital and organizational commitment and analyzed using the SPSS/WIN 21.0 and Amos 18.0. A positive relationship was observed between transformational leadership, social capital, and organizational commitment; social capital had a partial mediating effect of a transformational leadership and organizational commitment. To increase the organizational commitment, mediating between the head nurse' transformational leadership and social capital is recommended.

A Case Study on Quality Improvement of the Food Services for Patients - Focused on back-up orders - (환자 급식서비스의 질 향상 사례 연구 - Back-up 처방 중심으로 -)

  • The purpose of this research which, surveyed target hospitals, was to evaluate job operations by surveying the influences of Quality Improvement (QI) activities in various divisions related to a decrease in their back-up orders. Statistical data analyses were completed using the SPSS 11.0 program. The results can be summarized as follows Before QI there were 147 cases of back-up orders ; after QI there were 83 cases, decrease of 64 cases. This was 44%, less than the projected goal of 50%. For each item, there was a decrease of 40 nurse cases, 9 patients and patron cases,9 test surgery deliverly cases and 5 doctor cases after QI. The registering of midnight meals was not shown after QI, due to the Order Communication System (OCS) settlement. After performing QI, the average manual operation per month was reduced from 840 minutes to 498 minutes, of which the difference was 342 minutes, and the average of 342 minutes per month could be used for the peculiar operation of each division. This QI activity provided a good opportunity for establishing cooperation among divisions in providing meals to patients through interactions among divisions. It was recognized that these interactions were effective only when medical services were achieved through organized cooperation among divisions. Among the 7 items included on the patient satisfaction questionnaire, “satisfaction with offered menus” (p<0.01) showed significantly higher scores before QI. However “satisfaction with meal times” (p<0.01) showed significantly lower scores before QI.

The health care experience of the clinical nurse in a university hospital (일 대학병원 임상간호사의 건강관리 경험)

  • This study analyzed the data using the phenomenological method proposed by Colaizzi (1978) to understand the meaning and reality of the health care experience of clinical nurses in a university hospital. The data were collected from 9 participants, who were nurses in a university hospital in the city of S from Oct 4, 2015 to Oct 30, 2015. The results showed that the health care experiences of the participants may be categorized into 5 categories, 'ambivalence toward the physical health care', 'pressure over physical exercise', 'the difficulty arising out of shift work', 'stress management', and 'rediscovery of self', and may be identified with a 14 theme cluster and 42 themes.

  • This study aimed to describe and understand the clinical nurses' experience of nursing care for delivery of married immigrant women and then provide useful information for making strategies to improve nursing service for the married immigrant women in delivery. Data were collected with three focus group interviews and a few followed individual interviews and analyzed with qualitative content analysis. As a result, the 35 subcategories and 14 categories were extracted and named according to the five main interview questions. Based on the findings, the programs for nurses providing nursing care for married immigrant women in delivery to improve their language and cultural competencies are recommended.
