Purpose: In Korea, the prevalence of dementia patients has increased, which makes the care burden of nurses important. The purpose of this study is to identify factors affecting the care burden of nurses caring elderly patients with dementia. Methods: A cross-sectional design was conducted using a convenience sample of 127 nurses from two hospitals and a nursing home in Korea. Participants completed questionnaires on knowledge of and attitudes toward dementia, social support, self-esteem, dementia problematic behavior (DPB), and professional caregiver burden index. The data were analyzed by using the t-test, ANOVA, Pearson correlation, and multiple regression with the SPSS/windows version 21.0 program. Results: The influencing factors for nurse burden include day shift, DPB, self-esteem, social support, which explain 28.0% of care burden of nurses. Conclusion: To reduce the burden of the nurses, there needs to be an administrative system that focuses on enhancing their self-esteem and social support. Active institutional support may be necessary for the nurses taking care of elderly patients with dementia.

Objectives : Shift work disorder occurs when you have difficulties adjusting to a work schedule that takes place during a time which most people sleep. Some people may have diverse psychosomatic symptoms, such as sleep problems, depression, anxiety, and headaches even after the shift work schedule ends. The aims of this study are to compare difference of psychosomatic symptoms between rotating shift and daytime working nurse groups. Methods : Volunteer nurses working in a general hospital were recruited in a general hospital. We collected sociodemographic data. We used questionnaires for headache type, headache frequency, and VAS(Visual Analog Scale) for headache intensity, BDI(Beck depression Inventory) and GSAQ(Global Sleep Assessment Questionnaire). We used Mann-Whitney test and Chi-square test for hypothesis testing. Results : Data collected from 84 women nurses. Rotating shift(N=37) working nurses showed younger, more unmarried, and shorter work periods than daytime working nurses (N=47). Also rotating shift group showed more problems of headache, insomnia, excessive daytime sleepiness, depression and anxiety than daytime group. The above results were statistically significant. Conclusions : The rotating shift work women nurses produced more headache, insomnia, excessive daytime sleepiness, depression, and anxiety than daytime working ones.

The purpose of this study was to examine the relationship between sleep disorders, job satisfaction, health promotion behavior, quality of life, turnover intention. And also to find the predicting factors on turnover intention of shift and non-shift nurses. A descriptive study design was used. Study subjects were 239 nurses worked as a shift (167) and non-shift (72) in two general hospitals in P city. Turnover Intent, Sleep disorders, Job satisfaction, practicing health promotion profile and quality of life scales were used to collect the data. Data were analyzed by descriptive statistics and Pearson's correlation coefficient for find the relationship between study variables. Stepwise multiple regressions used to find predicting factors of turnover intention with other variables. The shift group showed lower Job satisfaction, practice of health promotion behavior and intention of turnover than non-shift nurses. The most important predictive factors of turnover intention in of shift group was job satisfaction (β =-. 477, p <.001) and non-shift group was health promotion behavior (β =-. 295, p = .040) than other factors. Findings showed that turnover intention highly influenced by job satisfaction than health promoting behavior and quality of life. This study suggests organizational efforts to provide sufficient staffing and nurse managersshould make more concentration to allot work schedule in order to avoid over load shift nurses and promote quality of client care.
The human papilloma virus (HPV) is the main aetiological agent for cervical cancer, one of the most frequent cancers observed in women throughout the world. There are effective programs for reducing the incidence of cervical cancer with HPV vaccination. The objective of this study was to discuss the applicability of the HPV vaccination and the role of nurses in prevention of cervical cancer. Use of bivalent and quadrivalent vaccines has been initiated against the types of HPV which are the primary cause of cancer. The quadrivalent HPV vaccination has entered into the routine vaccination schedule in many European countries for use in children and adolescents between 9-15 years of age and for women between 16-26 years of age, whereas it has been proposed that the bivalent vaccination should be given to girls between 9-18 years of age. While cervical cancer is among the cancers that can be prevented, it is essential to continue screening tests while introducing vaccination in a systematic manner for protection. On this subject, among the most important roles of nurses is to implement the screening programs by fulfilling the caregiving, training and consultancy roles for the society and especially, for high risk groups and to increase the awareness of the people.

This study was designed to evaluate the current orientation and further to develop an effective orientation curriculum of new nurses by analyzing questionraire taken from 45 numbers of all nurses who have worked for 18 months below at one hospital located in C city on July, 1995. The results obtained are summarized as follows. 1. The subjects showed the high necessity of "emergency nursing", "cardiopulmunary resuscitation", "disinfection and infection" and "interpersonal relationship" in order, but relatively low necessity of "doctor supports" and "medical insurance affair" for the practical orientation. Therefore, the orientation should provide prepondently an education Which they really need, on the basis of their education experiences and/or requirements. 2. Practical training, individual teaching and performance will be an effective orientation more than theorical education. The educator should be selected from unit based persons being capable of providing technical education to trainee. Also, in order to develop the teaching method and to improve the corresponding ability, a special program is required for educators. 3. It will be desirable that task training is given at least one month before working at their unit. In addition, orientation schedule should be made to concentrate trainee on their task training fully. 4. The subjects showed that half of them had spent four to six months for work adaptation. A meeting of new nurses may be helpful not only to acclimate themselves to new circumstances. but also to provide an exchange of views and an emotional relationship. Furthermore, unit based staffs should exerts efforts to maintain the educational circumstances and warm concern for new nurse's adaptation.
Purpose: The purpose of this study was to identify the factors affecting shiftwork tolerance among nurses and provide basic data, ultimately, in formulating a plan for improving their shiftwork tolerance. Methods: The participants were 317 nurses who were working at three general hospitals in Daejeon and Seoul. Data analyzed t-test, ANOVA, correlation analysis, and regression analysis using SPSS 20.0 program. Results: As for the sub-areas of shiftwork tolerance for the respondents, more than a half of nurses working at shifts suffered from insomnia, felt drowsy, complained of chronic fatigue, and were at the anxious and depressed state. Shiftwork tolerance differed by the daily intake of coffee and exercise. Shiftwork tolerance was significantly positively correlated with sleep-wake habits, hardiness, and flexibility and negatively correlated with a languidity. Regression analysis revealed that shiftwork tolerance was affected by age, sleep-wake habits, languidity, flexibility, and hardiness. Conclusion: To put the results, nurses had shiftwork tolerance affected by sleep-wake habits, languidity, and hardiness. It is therefore necessary to make a scheme for allowing nurses to improve hardiness and flexibility and decrease the languidity with the objective of improving their shiftwork tolerance.

The aim of this study was to investigate the level of depression among female hospital nurses and the influence of rotating shift work and quality of sleep on depression. A total of 295 nurses in Seoul were recruited and the data were analyzed using descriptive statistics and multiple regression analysis. The number of depressed nurses was 71.2%(CES-D total score ≥ 16) and factors influenced on nurses' depression were quality of sleep, rotating shift work, and work experience in nursing(1-2.9 years, more than 10 years). Based on the result of this study, nurse managers and hospital organization should use appropriate interventions to improve quality of sleep and negative effects of rotating shift work for nurses.
Purpose: The purpose of this study was to identify factors affecting clinical nurses' presenteeism. Methods: A descriptive cross-sectional design was used in which participants completed self-report questionnaires that consisted of measures for nurses' job satisfaction, job stress, professionalism and presenteeism. 267 nurses working in general hospital participated in this study. Logistic regression analysis was used to estimate predictors of the presenteeism in clinical nurses. Results: 250 nurses experienced one or more health problems last one month. The group who experienced presenteeism during the last one year had significant differences in professionalism and perceived productivity. Predictors of sickness presenteeism in clinical nurses included work experience, total health problem and last 1 year absenteeism. Last 1 year absenteeism and total health problem were strongly related to presenteeism in clinical nurses. Conclusion: Major findings of this study indicated that in dealing with nurses' presenteeism, not only managing nurses' job stress and job satisfaction but also providing flexible work schedule and increasing staffing level as an organizational approach are necessary to be considered. Further repeated and expanded research is needed to explore the multidimensional aspects of nurses' presenteeism including a broad range of work setting and the influence of Korean nurses' organizational culture on presenteeism.

Purpose: This study was carried out to deveop nursing costs and to substantiate the application of appropriate activity-based nursing costs for the current clinical nurse specialists. Method: The study sample was a group of 8 home health care team of the tirtiary Hospital in Korea. The data was collected from September 2003 through December 2004. The statistical analysis was done by SPSS PC 11.0 program and calculated mean and the standard deviation. In Lee(2003)'s nursing activity analysis, nursing activities were classified into two major classification, 19 domains and 70 activities. Each activities was calculated using its work validity, physical effort, psychological effort, stress as a intensity, and the necessary time. The simple work costs was calculated the work wage per minute and the necessary time. The work load intensity was calculated using the work wage per minute and the necessary time work load intensity/100. Results: In this study, the work wage was 283 won per minute. The work validity ranged from 2.71-4.00. The highest simple work cost/work load intensity cost was 12,735won/47,374won for research activity, 10,700won/27,499won for bedsore care, and 9,727won/35,114won for deathbed care. The lowest simple work cost/work load intensity cost was 2,123won/2,038won for intramuscular injection, 2,210won/2,166won for hypodermic injection, and 2,210won/1,547won for a application of medicine(or ointment or cream). Conclusions: It revealed that the nursing cost should be considered validity, physical effort, psychological effort, stress as a intensity, and necessary time. Therefore, It is necessary to calculate nursing cost systematically based on activities.
To assess the adequacy of medical manpower and medical fee for the newborn nursery care, the author visited 20 out of 24 hospitals with the pediatric training program in Youngnam area between July 29 and August 14, 1991. Total number of newborn, both normal and sick, admission and discharge in 1-30 June 1991 was obtained from the logbook of nursery. Head nurse and staff pediatrician of the nursery were interviewed to get the current staffing for the nursery and their subjective opinion on the adequacy of nursery manpower and the difficulties in recruiting manpower. Average medical fee charged for the maternity and normal newborn nursery care was obtained from the division of self-audit of medical insurance claim of each hospital. Average minimum requirement of nursing care time for one normal newborn per day was 179.5 (${\pm}58.6$) minutes; 202.3(${\pm}50.7$) minutes for the university hospitals and 164.2(${\pm}60.5$) minutes for the general hospitals. The ratio of minimum requirement of nursing care time and available nursing time was 1.42 on the average. Taking the additional requirement of nursing care for the sick newborns into consideration, the ratio was 2.06. The numbers of R. N. and A. N. in the nurserys of study hospitals were 31%, and 17%, respectively, of the nursing manpower for the nursery recommended by the American Academy of Pediatrics. These findings indicate that the nursing manpower in newborn nursery is in severe shortage. Ninety percent of the head nurses and 85% of the staff pediatrician stated that the newborn nursery is short of R.N. and 75% of them said that the nurse's aide is also short. Major reason for not recruiting R.N. was the financial constraint of hospital. For the recruitment of nurse's aide, short supply was the second most important reason next to the financial constraint. However, limit of quarter in T.O. was the mar reason for the national university hospitals. Average total medical fee for the maternity and newborn nursery cares of a normal vaginal delivery who stayed two nights and three days at hospital was 219,430won. Out of the total medical fee, 20,323won(9.3%) was for the newborn nursery care. In case of C-section delivery who stayed six nights and seven days, total medical fee was 732,578won and out of the total fee 76,937won (12.0%) was for the newborn care. Cost for a newborn care per day by cost accounting was 16,141won for the tertiary care hospitals and 14,576won for the all other hopitals. The ratio of cost and the fee schedule of the medical insurance for a newborn care per day was 5.0 for the tertiary care hospitals and 4.9 for the all other hospitals. Considering the current wage level of the medical personnel, capital investment for the hospital facilities and equipments, and the cost for hospital maintenance, it is hard to expect adequate quality care in the newborn nursery under the current medical insurance fee schedule.
