Influence of Nurse Managers' Authentic Leadership on Nurses' Organizational Commitment and Job Satisfaction: Focused on the Mediating Effects of Empowerment (간호사가 인식한 간호관리자의 진정성 리더십이 조직몰입 및 직무만족에 미치는 영향: 임파워먼트의 매개효과를 중심으로)

  • Purpose: The aim of this study was to examine the mediating effect of empowerment in the relationship of nurse managers' authentic leadership, with nurses' organizational commitment and job satisfaction. Methods: The participants in this study were 273 registered nurses working in five University hospitals located in Seoul and Gyeonggi Province. The measurements included the Authentic Leadership Questionnaire, Condition of Work Effectiveness Questionnaire-II, Organizational Commitment Questionnaire and Korea-Minnesota Satisfaction Questionnaire. Data were analyzed using t-test, ANOVA, $Scheff{\acute{e}}$ test, Pearson correlation coefficients, simple and multiple regression techniques with the SPSS 18.0 program. Mediation analysis was performed according to the Baron and Kenny method and Sobel test. Results: There were significant correlations among authentic leadership, empowerment, organizational commitment and job satisfaction. Empowerment showed perfect mediating effects in the relationship between authentic leadership and organizational commitment. It had partial mediating effects in the relationship between authentic leadership and job satisfaction. Conclusion: In this study, nurse managers' authentic leadership had significant influences on nurses organizational commitment and job satisfaction via empowerment. Therefore, to enhance nurses' organizational commitment and job satisfaction, it is necessary to build effective strategies to enhance nurse manager's authentic leadership and to develop empowering education programs for nurses.

A study on the value orientation of nursing unit managers and nursing organization performances (간호단위 관리자의 가치지향성 리더쉽유형과 간호조직성과의 관계)

  • Purpose : This study is to confirm the relation of the value orientation of the nursing unit manager and the job satisfaction and organizational commitment of the nursing unit nurse. Method : We used the systematic questionnaires as a study tool. The contents were composed of 46 questions in total such as 4 questions of general feature, 23 value-oriented questions, 10 job satisfaction questions, and 9 organizational commitment questions. The period of data collection was from August to September 2000. Result : 1) The value of flexibility was 3.82 points (${\pm}.3788$), which is above average. The values of equalitarianism was 3.37 points (${\pm}.4422$), which is above average. 2) The leadership pattern according to the value orientation perceived by the head nurses in 6 clusters has been divided into 3 patterns of change-oriented leader, task-oriented and stability-oriented leader. 3) The nurses who worked with the change-oriented leader showed higher values of job satisfaction (F=5.941, p=O.003), and affective commitment (F=5.793, p=O.003) than those who worked with the stability-oriented leader. Conclusion : As revealed in this study, we think that we have to consider that the change-oriented leader can produce higher performance of an organization than the stability-oriented leader, and write the basic data for the educational courses of leadership development or workshops etc. in order for the nursing unit managers to play their roles for the effective ward-unit management.

Factors Influencing Organizational Commitment and Job Satisfaction of Nurses (간호사의 직무만족과 조직몰입에 영향을 미치는 요인)

  • Purpose: This study was to identify factors that influence the organizational commitment and job satisfaction of nurses. Method: The data were collected from 1st, August until 7th, August, 2005. The subjects were 238 nurses from a general hospital in Busan. Data were analyzed with descriptive statistics, t-test, one-way ANOVA, Pearson correlation coefficient, and stepwise multiple regression using SPSS program. Result: The organizational commitment and job satisfaction was significant positive correlations with flexibility, satisfaction, competence factors and significant negative correlations with empathy factors and centralization factor. The most powerful predictors of Nurses's organizational commitment and job satisfaction was managerial strategy of organizational characteristics. And, satisfaction factors of professional self-concept, affiliative-dominant culture and conservative- dominant culture and innovative-dominant culture were powerful predictor factors. The Other powerful predictors of nurses's job satisfaction were satisfaction factors of professional Self-concept, communication and centralization factor of organizational characteristics, affiliative-dominant culture and conservative-dominant culture, professional image factor of nurse image. Conclusion: This study revealed that important factors for nurses's organizational commitment and job satisfaction. Consequently, the manager should be make effective managerial strategy and encourage that nurse have more satisfy to professional self-concept. And the manager make constant efforts to create an affiliative culture in hospital.

Team Managers' Experience With a Team System in the Hospital (병원조직에서 간호팀장의 팀제 경험)

  • Purpose: The purpose of the study was to understand and describe the team manager's experience in relation to the team system in the hospital organization. Methods: In-depth individual interviews for collection of data were conducted from October to November 2011. Participants were chosen purposely among team managers who had worked as team managers under the team manager system in Seoul for more than three years. Content of all interviews was recorded and transcribed according to information collected by interviewers. Data were analyzed using Colaizzi's analysis. Results: Six categories were derived from the analysis: 1) Strengthening of team manager's authority; 2) Sharing of multifaceted leadership role; 3) Demonstrating people-oriented leadership; 4) Complete interpersonal relationship and communication; 5) Self-empowering with web-like plans and practices; and 6) Going through rigorous commitment for team performance. Conclusion: Results of the study demonstrate various positive factors in association with the team system in spite of the short period of time since its introduction in the hospital organization; however, some negative aspects of the team system need to be reevaluated.

Effect of Nurse Managers' Leadership Styles on Teamwork among Nurses (간호 관리자의 리더십 유형이 간호사의 팀워크에 미치는 영향)

  • The current study examined which leadership styles of nurse managers affect teamwork among nurses. The analytic sample consisted of 187 clinical or charge nurses, who were recruited from one university affiliated large hospital in Korea. The results from the multiple regression analysis showed that higher nurse managers' transformational, servant, and ethical leaderships were significantly associated with higher teamwork among nurses. Yet, nurse managers' transactional leadership was not related to teamwork among nurses. These findings indicate that it is effective for nurse managers to display transformational, servant, and ethical leadership rather than transactional leadership in order to enhance teamwork among nurses. This study suggests the needs to address not only transformational leadership but also servant and ethical leadership during leadership education for nurse managers.

  • The purposes of this study were 1) to assess the currunt documentation system 2) to identify the problems in communication regarding to documentation 3) to develop new documentation system 4) to suggest effective communication channel using new documentation system Research was conducted by direct observation, chart review, staffs interview and servey. Results were as follows: 1) nursing care plans were not used in ongoing care 2) documentation format was primarily narrative and charting was time consuming 3) documentation did not reflect the nursing process 4) patient records were not used as effective communication tool between case manager and part time nurse 5) difficult access to patient record for nurse manager created inefficiency in coordinating 6) documentation of patient education did not describe the precise contents of education, and the responses of the patients and evaluation To solve these problems, new documentation format was developed. With new formats nurses : 1) use standardized care plan which contains nursing diagnosis, ecpected outcome, time frame for evaluation, flow sheet for updating the plans 2) leave one copy of care plan at patient home for mutual agreement with patent and communication among nursing staffs 3) carry one copy of care plan for updating 4) document and evaluate the patient education using education check list keeping in patient's home 5) document nursing process in focus charting visit report 6) carry one copy of visit report 7) have one copy of visit report which was deligated to part time nurses 8) use documentation in direct communication with part time nurse 9) use beeper and memo to promote communication

Roles and Functions of the Rehabilitational Nurse Practitioner Expected by Nurses and Doctors in Rehabilitation Hospital (재활전문간호사에게 기대되는 역할과 기능)

  • Purpose: This study aims to identify the role and function of the RNP(rehabilitational nurse practitioner) expected by nurses and doctors. Method: This study was a survey. The data were collected 188 nurses and 21 doctors who worked for disabled patients in the rehabilitation hospital during months of June, 2004 and August, 2005. Results: 98.4% of nurse and 61.9% of doctors agreed at opening of RNP course. The major role of RNP expected by nurses were educator, counsellor and case manager. The major role of RNP expected by doctors were direct care, self care promoter & exercise and emotional care. There was a significant difference about the need for opening of RNP course and major role and function of RNP between the group of nurses and doctors. Conclusion: The results of this study showed that the need for opening of RNP was identified and the major role of RNP was educator, counsellor, case manager and direct care. So there is a need for further research about major role of RNP related to various setting including rehabilitation hospital, nursing home, home care etc.

A Study on Patients' Perception of Nurse's Behavior in Protecting Patient Privacy (간호사의 환자 프라이버시 보호행동에 대한 환자의 인식도)

  • Purpose: The purpose of this study was to investigate the patient's perception of the nurse's behavior in protecting patient privacy and to make suggestions for medical facilities to increase protection of patient privacy. Method: The research was a survey study The data on protection of privacy in primary nursing, for physical privacy, of patient information and in private conversations were collected in October 2005 from 187 patients in a university hospital. Frequencies, means, t-test, ANOVA, and $x^2$-test were used to analyze the data. The SPSS 13.0 for Windows program was used. Results: The mean score for patients' perception of nurse protection of their privacy was 3.33. There were significant differences in perception of protecting patients' privacy according to gender for private conversation, according to level of education for all but physical nursing and for number of admissions for total score and for primary nursing. Conclusion: The results of this study suggest the following: a) Institutional polices and nursing guideline should be clearly stated as to the nurses' duty to protect patient privacy. b) Medical facilities should be arranged in a way that allows for the protection of patients' medical information, and should focus on patient privacy. c) It is necessary of nurses to receive periodic in-service education on protecting patient privacy.

The Role Experience of Preceptor Nurses in Hospitals (프리셉터 간호사의 역할경험)

  • Purpose: The purpose of this study was to identify the role experience of preceptor nurses in general hospitals. Methods: Data were collected from 10 experienced preceptor nurses through in-depth interviews about their experiences. The main question was "How do you describe your experience as a preceptor in the process of role adaptation?" Qualitative data from field and transcribed notes were analyzed using Strauss & Corbin's grounded theory methodology. Results: The core category of experience in the process of role adaptation as a preceptor nurse was identified as 'being responsible to teach from beginning to end'. The participants identified five interactional strategies; 'abruptly having to take the role as a preceptor unwilling', 'difficulties in field training with variable situations', 'preceptor and new nurse with love and hate relationships', 'bring up preceptee as a child', 'redefining the concept of preceptor'. Conclusion: The results of this study provide useful information for understanding the role of preceptor nurses in general hospital who have to teach new nurses with patience and responsibility. It will be needed to establishment of preceptor's role and educational standards, and policy for their rewards by nurse manager. Additionally, making cooperative educational climate in nursing ward is important for teaching new nurse.

A Study on Link of Health and Welfare Service and Barrier′s Factors of Visiting Nurses and Social Welfare workers (방문간호사와 사회복지사의 보건복지서비스 연계 및 장애요인에 관한 연구)

  • Purpose: The purpose of this study was to explore the link between health and welfare service and barrier's factors by reviewing the connection between the public health center's visiting nurse and social welfare center's social workers Method: A survey by mail or a face-to-face interview of 151 visiting nurses in 25 public health centers and 48 social welfare workers in general social welfare centers in Seoul, was preformed from Feb. 12, 2001 to Mar. 15, 2001. The data were analyzed with frequency, percentage, mean value, paired t-test and independent t-test using SPSS/WIN 7.5 program. Result: 1. ‘The necessity and degree of cooperation with social welfare workers of visiting nurse’ scored average 4.49 and 3.19, and ‘The necessity and degree of cooperation with visiting nurse and social welfare workers’ scored average 4.81 and 3.15 on the five-point scale ; there was a significant difference between the two variable in visiting nurse and social welfare workers. 2. In barrier's factors which health and welfare service offer to, visiting nurses showed statistically significant higher score than social welfare staff; ‘job factor’, ‘resource factor’, ‘clients factor’, ‘individual ability factor’ Conclusion: In order to provide link system that hold clients in common in public health center and social welfare center, it is recommended a case management team should be constructed and educate visiting nurses for case manager.
