AN EXPERIMENTAL STUDY OF THE EFFECT OF CO-60 IRRADIATION ON THE HEALING PROCESS OF EXTRACTION WOUNDS IN WHITE RATS (Co-60조사가 백서 발치창 치유과정에 미치는 영향에 관한 실험적 연구)

  • Because of the development of rampant caries, osteomyelitis and osteoradionecrosis that occur after radiation therapy of oral cancers, extraction of teeth at or near the malignant lesion has been done in the past. Few, however, have studied the radiation effect on the healing of extraction wounds. This study is concerned with the effect of Co-60 irradiation on the healing process of extraction wounds in rats. Fifty six, male, Spraque-Dawley rats are used. The right first molar of the mandible is extracted from all animals. They are divided into three experimental groups of 14 each and a control group of 14. Three experimental groups are irradiated respectively with 200 rad, 400 rad and 600 rad and a pair of rats in each group are killed on days 1, 3, 5, 7, 14, 21 and 28 after irradiation. Two animals from the control group are killed on the day when the experimental rats are killed. The irradiated hemimandibles are fixed in 10% neutral formalin, decalcified in 5% trichloroacetic acid, embedded in paraffin and sectioned. The sections are stained in hematoxylin and eosin, van Gieson, Masson's trichrome or silver nitrate. Results show that in general radiation effects on healing extraction wounds are dose dependent; i.e., the higher is the dose, the greater is the histologic changes observed: 1. Irradiation tends to retard blood clot organization and epithelial regeneration. 2. An increase in the number of giant cells and osteoclasts is noted after irradiation. 3. Formation of regenerating connective tissues around and within the extraction site is com- promised, and a clear reduction of primitive mesenchymal type connective cells is noted. 4. The healing process begins along the lateral aspect of the extraction socket in the control, while irregular histologic appearances of the brabecular pattern is present in the experimental rats.

Can parents prevent tooth loss related to dental avulsion? An assessment of knowledge related to permanent teeth

  • Purpose: Dental avulsion injuries have a poor prognosis that largely depends on the immediate steps taken to manage the avulsed tooth. A lack of knowledge about the initial management can lead to tooth loss, with further adverse implications for esthetics, phonetics, and overall growth and function. Hence, the present study aimed to assess parents' knowledge regarding dental avulsion and the variables associated with their knowledge of avulsion injuries. Methods: A series of closed-ended questions on parents' knowledge regarding avulsion, such as immediate management, storage media, handling, and urgency of visiting the dentist, was asked. Univariate associations between the outcomes were assessed using the Pearson chi-square test. The chisquare goodness-of-fit test was used to check whether the sample data were representative of the population. Results: In total, 211 mothers and 149 fathers were included, of whom 46.7% had experienced dental trauma during their own childhood. Sixty-one percent of mothers believed that they knew everything necessary about tooth avulsion and its management. A significant number of participants who thought that they had a good level of knowledge about avulsion chose water, tissue, or paper wrap to transport the tooth, and preferred tap water, alcohol, or antiseptic to clean the avulsed tooth. Conclusions: Both mothers and fathers had poor knowledge about tooth avulsion, indicating that there is an immediate need for educational programs focusing on this issue. Since a substantial proportion of participants believed incorrect information, it is vital to disseminate accurate information.

Periodontal status in patients with osteoporosis (골다공증 환자의 치주조직 상태)

  • The purpose of this study was to evaluate the relationship between osteoporosis and periodontal condition in postmenopausal women. Twenty-eight patients who have been treated at Chonnam national university hospital with osteoporosis(osteoporotic group, mean $age62.2{\pm}6.6$) and 21 normal postmenopausal women with periodontitis(control group, mean $age60.4{\pm}3.7$) were examined. Bone mineral density(BMD) of lumbar spine(L2-L4) was measured by dual energy X-ray absorptiomemtry(DEXA). Percentage to peaked bone mass in osteoporotic group was 70.9% which had lower BMD than in control group(p<0.05). The number of present teeth were 23.6 in osteoporotic group and 23.1 in control group. Percentage of bleeding on probing was 41% in osteoporotic group and 37% in control group. probing depth and attachment level were shown 3.18 mm, 3.63 mm in osteoporotic group and 2.85mm, 3.11mm in control group, respectively. Probing depth and attachment level were significantly greater in osteoporotic group than in control group(p<0.05). The significant negative correlation was found between BMD level and periodontal attachment level(p<0.001, ${\gamma}=-0.56$). These results suggest that osteoporosis may be associated with periodontal breakdown.

  • A cyst is a pathologic lesion characterized by a cavity filled with fluid, celluar products, air, or a combination of these. Dentigerous cysts were formed around the crown of unerupted teeth. The reduced enamel epithelium persists around the crown after it has formed. Proliferation of the epithelium in a fluid-filled sac may be induced by osmotic pressure. In the first decade the most frequent location is the premolar site. In each subsequent decade the largest number of cysts are in the mandibular third molar site, with the second most frequent site being the maxillary canine. The treatment of odontogenic cyst can be mostly classified into three types of cyst enucleation, marsupialization and decompression. We should consider age of patient, anatomic structure, location and size for choosing a treatment method. Advantage of cyst enucleation is fast healing, but a injury of a surrounding structure is highly. Marsupialization is conservative treatment that can reduce the damage of a adjacent structure, but it is only limited at superficial lesion. Decompression also is conservative treatment, but it has the difficulty of the oral hygiene and the troublesome of the lavage. We present the possibility that reduces the defect of decompression and cures the lesion efficiently. We report a male patient with the dentigerous cyst developed at left mandibular third molar in this study. We used the decompression for a treatment and created special appliance to treat the lesion efficiently. We report a case of the cyst treatment that is association with efficiency of decompression appliance.

Scanning Electron Microscopic Study of the Effect of EDTA on Demineralizing Diseased Root Surface (EDTA가 치근면 탈회에 미치는 효과에 대한 주사전자현미경적 연구)

  • The present study was performed to evaluate the effect of EDTA on diseased root surfaces with regard to the removal of the smear layer, exposure of dentinal tubule openings, and demineralization of the peritubular dentin. 20 periodontally involved single rooted teeth were used. After scaling and root planing, root conditioning was carried out with 3%, 17%, and 24% EDTA solution for 20 sec., 3 min., and 5 min. respectively. Then, the specimens were rinsed with tap water for 5 min. and processed for scanning electron microscopic observation. The following results were obtained. 1. In control group which was conditioned with cotton pellets soaked with saline for 3 min., the surface showed an amorphous irregular coating. The tubule orifices seemed to be com-pressed in the direction of the curette strokes. 2. In test group which was conditioned with EDTA solution, the number of opening dentinal tubules and the extent of the dentinal tubule opening increased with time irrespective of the concentration of EDTA solution. 3. In the group which was conditioned with EDTA solution for 20 sec., the smear layer was not completely removed. 4. If the time of root conditioning is above 3 min., collagen-like fibers covering the circum tubular dentin surface were shown even in the group which was conditioned with 3% EDTA solution. In conclusion, EDTA solution was a very effective root conditioning agent like low pH root conditioning agents which had various disadvantages. Therefore, using EDTA solution, more successful periodontal regeneration could be obtained.

  • Objectives : This study was performed to provide the basic data for oral health of infants, through the comparison of Streptococcus mutans activities(the representative bacteria of initial dental caries), in main nutritional foods for infants: breast milk, milk powder, and soya milk. Methods : The study targeted 30 infants and the saliva was sampled after the oral examination. The pH of sampled saliva was measured; and the sample of 'the saliva', 'the saliva and breast milk', and 'the saliva and formula milk' by researcher were produced; and the measurement was repeated 7 times. Results : As a result of examining the change of colony number of Streptococcus mutans by hour, the bacterial activity took in the source of nutrition took place in the first place; and in the group with dental caries, breast-feeding at night occurreds most frequently. Due to the combined activities, such as the increase in the contact time, the existence rate of dental caries appeared high. However, as the improvement of breast-feeding is closely related to the nutrition supply to babies, rather than the improvement of breast-feeding, the 12 hours after the culture in which Streptococcus mutans activities started was considered. Conclusions : In case of breast-feeding at night, it is necessary to remove the dental plaque in the mouth, before 12 hours has passed. Especially, using toothbrush and toothpaste to decrease the occurrence of dental caries, after the eruption of teeth, is recommended for oral administration.

RISK FACTORS FOR ORAL CANCER ; A CASE-CONTROL STUDY (구강암의 위험요인 분석을 위한 환자-대조군 연구)

  • The purpose of this study was to investigate the relationship between oral cancer and such factors as smoking and drinking pattern, oral health status, dietary intake pattern, socio-economic status. Oral cancer patients and other disease patients who visited Yonsei University Dental Hospital from May to September in 2000 were selected as the study subjects. The numbers of cases and controls were 41, 108, respectively. Two groups were matched with age and sex for case control study. Oral examination and questionnaires survey was performed by the dentist. To assess the strength of associations between oral cancer and other variables, chisquare tests were performed. The results were as follows : 1. The durations of smoking and alcohol drinking were not related significantly with oral cancer. But the doses of smoking and alcohol intake increased the risk of oral cancer significantly(OR=2.52, 4.11, p<0.05). 2. Denture wearing, the number of missing teeth and spicy and salty food, coffee, tea and fresh fruit intake frequency did not significantly increase the risk of oral cancer. But low education level, residency in rural area increased risk of oral cancer significantly(p<0.01).

EFFECT OF LASER IRRADIATION ON DENTIN SURFACE STRUCTURE AND SHEAR BOND STRENGTH OF LIGHT-CURED GLASS IONOMER. (상아질 표면 구조와 광중합형 글라스 아이오노머의 전단강도에 대한 레이저 조사의 효과)

  • The purpose of this study was to evaluate the possible efficacy of Nd-YAG laser as a dentin conditioner by observing the laser irradiation dentin surface under scanning electron micrograph and measuring shear bond strength of restored light-cured glass ionomer mold. Fifty intact premolars were prepared for shear bond strength tests. The teeth were randomly divided into five groups as follows; Group I. no treatment Group II. 10% poly acrylic acid, 20 sec Group III. laser treatment 2 w, 20 Hz, 2 sec Group IV. laser treatment 2 w, 20 Hz, 5 sec Group V. laser treatment 2 w, 20 Hz, 10 sec Samples of each group were restored with light-cured glass ionomer cement after dentin conditioning and then measuring the shear bond strength of each specimen were measured using universal testing machine. Additional ten premolars were prepared for SEM analysis The result from the this study can be summarized as follows. 1. Shear bond strength of polyacrylic acid-treated group (II) was significantly higher than other groups (p<0.05). 2. No statistically significant difference could be found between three laser-treated groups (III, IV, V) in shear bond strength(p>0.05) 3. According to the result of observation under SEM, Polyacrylic acid was shown to have removed the smear layer effectively and opened the dentinal tubules, whereas the laser has produced the irregular surface mainly composed of melted and fused structure. The microcracks found in laser-treated groups increased in number with irradiation time and formed the regular mesh-type in 10 sec-irradiation group. 4. The ultrastructural change of dentin surface created by laser irradiation was found to the improper for bonding of the glass ionomer restorative materials. And the lower shear bond strength of laser irradiated group might have been due to the failure to form the suit able dentin surface for the glass ionomer to penetrated into and form the proper micromechanical retention.

THE MOBILE ORAL HEALTH SURVEY OF THE DISABLED IN FACILITIES IN SEOUL (서울특별시장애인치과병원 이동검진기관 장애인들의 구강건강조사)

  • Mobile oral exam at the facilities for the disabled in Seoul is to improve the oral health of the disabled who can hardly approach dental clinic. We analyzed 1609 oral exam records at 24 facilities (11 special-education schools, 10 living facilities, 2 mental hospitals, 1 health care facility for the elderly) in 2011. The purpose of this study is to figure out the oral health state of the disabled in facilities in Seoul and to compare with the non-disabled in National Survey 2010 and 2008. 1. Special-education school : DMFT index of age 15 is 5.4 which is higher than 3.6 (DMFT index of age 15) of National Survey in 2010. 2. Living facility : DMFT index of ages 35~44 is 8.4 which is higher than 5.2 (DMFT index of ages 35 ~44) of National Survey in 2008. DT rate is higher (31.3% vs 19.2%) and MT rate is lower (7.6% vs 15.5%). 3. Mental hospital : DMFT index of ages 35~44 is 11.3 which is higher than 5.2 (DMFT indext of age 35~44) of National Survey in 2008. DT rate is significantly higher (50.7% vs 19.2%) and FT rate is lower (35.1% vs 65.3%). 4. Health care facility for the elderly : DMFT index of ages 65~74 is 7.1 which is slightly lower than 8.7 (DMFT index of ages 65~74) of National Survey in 2008. The number of existing natural teeth is similar (16 vs 18).

Role of Selenium in Alteration of Erythrocyte Parameters in Bovine Fluorosis

  • Signs of dental discolouration, difficulty in mastication, bony exostosis and debility were observed in cattle from Qingtongxia Ningxia, China where fluoride concentration in drinking water, soil, fodder, serum, bone, teeth, haircoat and urine were significantly higher than the corresponding health site. The problem of fluorosis in beef cattle is attributable to water containing toxic levels of fluoride. The objective of this paper was therefore to evaluate the influence of fluoride on erythrocyte parameters in cattle under high fluoride and low selenium conditions, as well as the protective efficacy of selenium exposure in feedstuff for bovine endemic fluorosis. Sixteen 6 to 7 year-old high fluoride beef cattle were randomly allotted into two groups each with eight cows: high fluoride control group, and supplemented with 0.25 mg/kg selenium per day for 83 days respectively. In addition, eight 6 to 7 year-old normal control beef cattle were selected from a non-high fluoride site. Blood samples were collected on day 0, 30 and 83 for erythrocyte parameters analysis and scanning electronic microscopy. The results indicated that erythrocytes, hemoglobin, packed cell volume values and $Na^+-K^+$ ATPase activity from affected cattle on the high fluoride site were significantly reduced during the period as compared with the corresponding samples of normal control cattle, a great number of echinocytes were present in peripheral blood, and subsequent anaemia. However, affected cattle exposed to selenium revealed increasable erythrocyte parameters, the extent of elevation in these values being dependent on the duration of supplementation with selenium. These findings suggest that fluoride exposure can cause erythrocyte damage, whereas selenium supplementation can antagonize fluoride-induced generation of free radicals and cumulative effects of lipid peroxidation in erythrocytes. Selenium supplementation may help to alleviate the possible hazards associated with bovine endemic fluorosis.
