Suppressive effect of Spirulina fusiformis in relation to lysosomal acid hydrolases, lipid peroxidation, antioxidant status, and inflammatory mediator TNF-alpha on experimental gouty arthritis in mice

  • The anti-inflammatory effect of Spirulina fusiformis on monosodium urate crystal-induced inflammation in mice has been investigated and compared with the non-steroidal anti-inflammatory drug Indomethacin. The paw volume, lysosomal enzyme activities, lipid peroxidation, anti-oxidant status and inflammatory mediator tumour necrosis factor-$\alpha$ were studied in control and monosodium urate crystal-induced mice after oral administration of Spirulina platensis in an experimental model for gouty arthritis. In the induced mice, the levels of lysosomal enzymes, inflammatory mediator tumour necrosis factor-$\alpha$, lipid peroxidation and the paw volume increased significantly, whereas the antioxidant status decreased when compared to control mice. $\beta$-glucuronidase and lactate dehydrogenase level were also found to be increased in untreated monosodium urate crystal-incubated polymorphonuclear leucocytes. After the oral administration of Spirulina fusiformis, the physical and biochemical changes observed in monosodium urate crystal-induced animals were significantly restored to near normal levels. The results clearly indicated the anti-inflammatory role of Spirulina fusiformis, a promising drug for gouty arthritis.

  • The methanol extract of Asparagus racemosus root was evaluated for its anti-inflammatory activity on carrageenin and serotonin-induced rat hind paw oedema models. The extract (200 and 400 mg/kg) showed maximum inhibition of oedema of 18.6% and 33.7% at 3 h with carrageenin and 22.2% and 40.5% at 5 h with serotonin-induced rat paw oedema respectively. The experimental models tested, where the effect produced by the extract was compared to that of phenylbutazone, a prototype non-steroidal anti-inflammatory drug.

  • Background: The aim of this study was to compare the effects of taping techniques on menstrual pain, body temperature, and menstrual symptoms in women of reproductive age. Methods: This study targeted 40 female students enrolled at G university with menstrual pain rated above 5 on the visual analog scale (VAS). The participants were randomly assigned to four groups: the Kinesio taping, spiral taping, non-steroidal anti-inflammatory drug, and control groups. The intervention was applied one day after the onset of menstruation, and menstrual pain, menstrual symptoms, and body temperature were measured before the intervention and 24 hours after the intervention application. We measured menstrual pain using the VAS. Additionally, we evaluated menstrual symptoms using the menstruation symptom scale and measured body temperature of the abdomen and feet using digital infrared thermal imaging. Results: After the intervention, all three experimental groups showed significant improvement in menstrual pain and menstrual symptoms compared to the control group, and there was no significant difference among the three groups. After applying Kinesio taping, there was a slight decrease in the temperature difference between the abdomen and feet, but no statistically significant difference was observed. Conclusion: The results of this study demonstrated that kisesio and spiral taping have similar effects as with anti-inflammatory medication in relieving menstrual pain and menstrual symptoms. Taping can be considered as an effective method to replace medications in order to alleviate menstrual pain.

Diclofenac 투여 후 시간경과에 따른 장손상과 장내세균전위의 변화 (The Changes in Intestinal Damage and Bacterial Translocation with Time after Administration of Diclofenac)

  • Non-steroidal anti-inflammatory drug (NSAID)-induced gut damage and bacterial translocation (BT) have not been studies well, especially from the perspective of time after administration of NSAIDs. We therefore examined these changes in animals. The study was performed on 5 groups of rat; a control group (group A) and diclofenac groups (groups B, C, E, and F). Rats in the diclofenac groups were orally administered diclofenac sodium before intestinal permeability (IP) measurement (group B, 1 h before measurement; group C, 10 h before; group D, 22 h before; and group E, 52 h before). The IP, stool pellet number, serum biochemical profile, enteric bacterial number, and BT in the mesenteric lymph nodes (MLNs), liver, spleen, kidney and heart were measured. The administration of diclofenac resulted in significantly increased IP, caused intestinal protein loss, decreased stool pellet number, caused enteric bacterial overgrowth and increased BT in multiple organs in groups A, B, C, and D. IF, intestinal protein loss, and the BT in the liver and the spleen in group E were decreased than those in group D. There were no differences in the other parameters between group D and E. In the recovery phase of the diclofenac-induced gut damage, enteric bacterial overgrowth and BT in the kidneys and the heart did not change while the BT in the reticuloendothelial systems such as in the MLNs and liver was decreased.

퀴놀론과 비스테로이드소염제 투여 후 발생한 급성 간질성 신염이 동반된 DRESS 증후군 (DRESS syndrome with acute interstitial nephritis caused by quinolone and non-steroidal anti-inflammatory drugs)

  • Drug reaction with eosinophilia and systemic symptoms (DRESS) syndrome is a rare and severe drug-induced hypersensitivity syndrome characterized by hematological abnormalities and multiorgan involvement. Liver involvement is the most common visceral manifestation. However, renal failure has been rarely described. The common culprit drugs are anticonvulsants and allopurinol. We experienced a patient with DRESS syndrome with acute interstitial nephritis caused by concomitant administration of quinolone and non-steroidal anti-inflammatory drugs (NSAIDs). A 41-year-old man presented with a diffuse erythematous rash and fever which developed after administration of quinolone and NSAIDs for a month due to prostatitis. He was diagnosed with DRESS syndrome. Skin rash, fever, eosinophilia, and elevations of liver enzymes improved with conservative treatment and discontinuation of the causative drugs. However, deterioration of his renal function occurred on day 8 of admission. The levels of blood urea nitrogen and serum creatinine increased and oliguria, proteinuria and urinary eosinophils were observed. Ultrasonography showed diffuse renal enlargement. The clinical features were compatible with acute interstitial nephritis. Despite intravenous rehydration and diuretics, renal function did not improve. After hemodialysis, his renal function recovered completely within 2 weeks without administration of systemic corticosteroid.

Synthesis of Mefenamic Acid Derivatives and Antioxidative and Anticoagulant Activities

  • Mefenamic acid has been widely used as clinical drug for anti-inflammatory and analgesic. This drug was known to non-steroidal anti-inflammatory drugs (NSAIDs) such as aspirin, ibuprofen and indomethacin. Although the drugs which comprise this group are of diverse chemical structures, they all share the antipyretic, analgesic and anti-inflammatory actions which are characteristic of aspirin. Action of this drugs is caused by inhibitory effect of biosynthesis of prostaglandin that are synthesized from arachidonic acid via the endoperoxide biosynthesis pathway, the initial step of which is catalysed by cyclooxygenase. Mefenamic acid has more potent inhibitory action of prostaglandin biosynthesis than aspirin. Therefore, mefenamic acid is expected to have anticoagulant activity as aspirin-like drugs. This study was carried out to investigate the sinthesis of mefenamic acid derivatives from mefenamic acid and aromatic compound of antioxidant and its antioxidative and anticoagulant activities. Synthesis of mefenamic acid derivatives was conformed by conjugation as using esterification method. Biological activities was examined using effect of anticoagulant on bleeding time and effect of antioxidant by TBA method. As a result, SJ-202 showed strong antioxidative activity and anticoagulant activity among tested 4 compounds and exhibited similar activity to aspirin at anticoagulant activity.

Nanodiamonds Conjugated with Nonsteroidal Anti-inflammatory Drugs for Transdermal Delivery

  • Most commercially available detonation nanodiamonds (DNDs) require further processing to qualify for use in biomedical applications, as they often contain many impurities and exhibit poor dispersibility in aqueous media. In this work, DNDs are modified to improve purity and impart a high colloidal stability to the particles. The dispersive and adsorption properties of modified DNDs are evaluated in terms of the suitability of DNDs as carriers for non-steroidal anti-inflammatory drugs (NSAIDs) in transdermal delivery. The study of adsorption on strongly positively and strongly negatively charged DNDs showed their high loading capacity for NSAIDs, and a pronounced relationship between the drugs and the particles' charges. Experiments on long-term desorption carried out with DND/NSAID complexes indicate that the nanoparticles exert a sustained effect on the drug release process.

비스테로이드성 항염증제의 약물독성 예방을 위한 Methocarbamol의 약물조합 (A Methocarbamol Combination to Prevent Toxicity of Non-steroidal Anti Inflammatory Drugs)

  • 비 스테로이드성 항염증제에 대한 개별적 독성증상을 예방하기 위해 로박스 플라티넘(Robax Platinum), 로박스아세트(Robaxacet), 나프록센(Naproxen) 그리고 재활치료를 선택적으로 처방하여 비 스테로이드 진통소염제(Non-Steroidal Anti-Inflammatory Drugs)에 대한 독성증상을 예방을 위한 효과 비교 연구를 진행하였다. 이 연구에서는 로박스 플라티넘(Robax platinum), 로박스아세트(Robaxacet), 나프록센(Naproxen)을 주한미군 처방전(Annex A-Over-The-Counter Prescription)을 기준으로 하여 처방하였고, 앨리스 리치의 통증 척도(Alice Rich's Pain Scale)를 사용하여 통증 경과를 비교하고 그 결과를 IBM SPSS statics 24 버전을 사용하여 데이를 계산했다. 결합된 메토카바몰(Methocarbamol, 500 mg), 아세트아미노펜(Acetaminophen, 325 mg), 이부프로펜(Ibuprofen, 200 mg) 정제는 결합된 메토카바몰(Methocarbamol, 500 mg) 정제와 함께 사용할 수 있다. 아세트아미노펜 (Acetaminophen, 325 mg)의 알약이 통증을 조절하는 방법으로 사용되었다. 메도카바몰 500mg, 아세트아미토펜 325 mg 및 이부프로펜 200 mg 정제의 약물 조합은 메도카바몰 500 mg 및 아세트아미노펜 325 mg 정제가 전반적인 통증 조절과 관련하여 물리적인 스트레칭 운동과 쌍을 이룬 것과 유사한 통증 완화효과를 나타내었다.

Acupotomy for Scapulohumeral Periarthritis

  • Background: Scapulohumeral periarthritis causes pain and stiffness, and limits movement but it is a treatable condition. This was a clinical study of acupotomy treatment for scapulohumeral periarthritis. Methods: There were 80 patients randomly assigned to the traditional Chinese Medicine group (acupotomy) and the Western medicine group (naproxen), with 40 cases in each group. All patients had adjunct physiotherapy exercises for 14 days. Patients received acupotomy treatment 3 times for 14 days (Day 0, 7 and 14) or naproxen (0.22 g capsule; a non-steroidal anti-inflammatory drug) 3 times a day, for 14 days. The visual analogue scale (VAS) scores, range of motion (ROM) values, and the Melle scale, together with the therapeutic standard of diseases and syndromes in traditional Chinese Medicine were used for diagnosis and evaluation. Results: There were significant differences in the VAS scores, ROM, Melle scores, cure rate and total effective rate in the group which took naproxen and the acupotomy group, before and after treatment (p < 0.01). There were significant differences in the changes in VAS, ROM and Melle scores between the 2 groups (p < 0.01), and the acupotomy group was better than the naproxen group. Conclusion: Traditional Chinese Medicine and Western medicine can improve functional activity and reduce the level of pain experienced by patients suffering from scapulohumeral periarthritis. However, improvement of functional activity of the shoulder joint following acupotomy treatment was more obvious than the use of a non-steroidal anti-inflammatory drug, and the cure rate, and total effective rate of acupotomy was better.
