A Study of Predictive Factors Affecting Health: Promoting Behaviors of North Korean Adolescent Refugees

  • Objectives: The present study aimed to analyze the factors that could affect the health-promoting behaviors of North Korean adolescent refugees residing in South Korea. Methods: Questions about their sociodemographic variables, subjective health status, healthy living habits, and health-promoting behaviors were asked. Results: Statistically significant differences were found in religion (t=2.30, p<0.05), having family members in South Korea (t=2.02, p<0.05), and subjective health status (t=4.96, p<0.01). Scores on health-responsible behaviors were higher with higher age (t=2.90, p<0.01) and for subjects without family or friends (t=2.43, p<0.05). Higher physical-activity behaviors were observed in males (t=3.32, p<0.01), in those with better subjective health status (t=3.46, p<0.05) and lower body mas index (t=3.48, p<0.05), and in smokers (t=3.17, p<0.01). Nutritional behaviors were higher in those who followed a religion (t=2.17, p<0.05). Spiritual growth behaviors were higher in those who followed a religion (t=4.21, p<0.001), had no family in South Korea (t=2.04, p<0.05), and had higher subjective health status (t=5.74, p<0.01). Scores on interpersonal relationships and stress-management behaviors were higher for those with higher subjective health status. A multiple regression analysis showed greater effects on health-promoting behaviors when subjective health status was better. Older people and non-smokers exhibited more health-responsible behaviors, while more physical-activity behaviors and spiritual growth activities were observed when subjective health status was better. Interpersonal relationship behaviors had positive effects on those with good subjective heath status and on non-smokers. Conclusions: Based on the results of the current study, an alternative was suggested for promoting health in North Korean adolescent refugees.

The Korean Prediction Model for Adolescents’ Future Smoking Intentions

  • Objectives: The purpose of this study was to develop a prediction model for future smoking intention among Korean adolescents aged 13 to 15 in order to identify the high risk group exposed to future smoking. Methods: The data was collected from a total of 5940 students who participated in a self-administrated questionnaire of a cross-sectional school-based survey, the 2004 Korea Global Youth Tobacco Survey. Chi-square tests and logistic regression analyses were carried out to identify the relevant determinants associated with intentions of adolescents’ future smoking. Receiver Operation Characteristic (ROC) assessment was applied to evaluate the explanation level of the developed prediction model. Results: 8.4% of male and 7.2% of female participants show their intentions of future smoking. Among non-smoking adolescents; who have past smoking experience [odds ratio (OR) 2.73; 95% confidence interval (CI) 1.92- 3.88]; who have intentions of smoking when close friends offer a cigarette (OR 31.47; 95% CI = 21.50 - 46.05); and who have friends that are mostly smokers (OR 5.27; 95% CI = 2.85 - 9.74) are more likely to be smokers in the future. The prediction model developed from this study consists of five determinants; past smoking experience; parents smoking status; friends smoking status; ownership of a product with a cigarette brand logo; and intentions of smoking from close friends’ cigarette offer. The area under the ROC curve was 0.8744 (95% CI=0.85 - 0.90) for current non-smokers. Conclusions: For efficiency, school-based smoking prevention programs need to be designed to target the high risk group exposed to future smoking through the prediction model developed by the study, instead of implementing the programs for all the students.

Effects of Oral Health Behavior and Mental Health on Metabolic Syndrome in Korean Adults

  • Background: Integrated management of general, mental, and oral health is necessary to improve an individual's quality of life. This study aimed to identify the effects of mental and oral health behaviors on metabolic syndrome. Methods: A total of 4,227 Korean adults aged 20 years or older were selected as study subjects using raw data from the first year (2019) of the 8th period of the Korea National Health and Nutrition Examination Survey (KNHANES). A complex sample chi-square test and a complex sample logistic regression analysis were performed using the PASW Statistics ver. 18.0 program. Results: The effect on metabolic syndrome was significantly higher in male than in 1.833 times in male, 2.914 times in 30~49 years old, and 3.855 times in 50~64 years old, and 3.929 times in people over 65 years old compared to 20~29 years old, and compared to those with a college degree or higher, those with a middle school degree or lower are 2.116 times, those with lower income levels are 1.507 times higher, those with middle-lower are 1.359 times higher, those with middle-high are 1.401 times. Compared to non-smokers, smokers were 1.570 times higher than non-smokers and compared to those without speech problem and chewing difficulty, they were 1.717 times and 1.397 times higher, respectively and 1.973 times higher in those with 0~1 brushing times per day. Mental health did not affect prevalence of metabolic syndrome. Conclusion: It is necessary to improve maintain a healthy lifestyle to lower the risk of metabolic syndrome. It is necessary to establish effective dental hygiene customized education and an efficient health management system at the national level that can induce improvement of oral health behavior for the prevention and management of metabolic syndrome.

Periodontal granulation tissue preservation in surgical periodontal disease treatment: a pilot prospective cohort study

  • Purpose: The aim of this study was to evaluate the clinical outcomes of periodontal granulation tissue preservation (PGTP) in access flap periodontal surgery. Methods: Twenty patients (stage III-IV periodontitis) with 42 deep periodontal pockets that did not resolve after non-surgical treatment were consecutively recruited. Access flap periodontal surgery was modified using PGTP. The clinical periodontal parameters were evaluated at 9 months. The differences in the amount of granulation tissue width (GTw) preserved were evaluated and the influence of smoking was analyzed. Results: GTw >1 mm was observed in 97.6% of interproximal defects, and the granulation tissue extended above the bone peak in 71.4% of defects. At 9 months, probing pocket depth reduction (4.33±1.43 mm) and clinical attachment gain (CAG; 4.10±1.75 mm) were statistically significant (P<0.001). The residual probing depth was 3.2±0.89 mm. When GTw extended above the interproximal bone peak (i.e., the interproximal supra-alveolar granulation tissue thickness [iSUPRA-GT] was greater than 0 mm), a significant CAG was recorded in the supra-alveolar component (1.67±1.32 mm, P<0.001). Interproximal gingival recession (iGR) was significant (P<0.05) only in smokers, with a reduction in the interdental papillary tissue height of 0.93±0.76 mm. In non-smokers, there was no increase in the iGR when the iSUPRA-GT was >0 mm. The clinical results in smokers were significantly worse. Conclusions: PGTP was used to modify access flap periodontal surgery by preserving affected tissues with the potential for recovery. The results show that preserving periodontal granulation tissue is an effective and conservative procedure in the surgical treatment of periodontal disease.

건강한 중년 남성에서 커피 음용 습관이 혈중 총 콜레스테롤 값에 미치는 영향 (The Effect of Coffee Consumption on Serum Total Cholesterol Level in Healthy Middle-Aged Men)

  • In present study, the authors investigated the possible effect of coffee consumption on serum cholesterol level in 1017 men between the ages of 40 and 59 years, who were randomly selected from the members of Seoul Cohort Study. Serum total cholesterol data was collected with other serologic indices (e.g. systolic blood pressure, diastolic blood pressure, hight, weight, etc.) through the program of biennial health check-up offered by Korean Medical Insurance Corporation (KMIC). The amount of coffee consumption was assessed by a self-administered questionnaire through mailing. Other confounding factors, such as age, body mass index, cigarette smoking, alcohol consumption, physical activity, and other dietary intake pattern were also determined by the questionnaire. The differences in means of serum total cholesterol in compared to non consumers were $-0.4{\pm}3.56mg/dl$ for those drinking less than 1 cup a day, $-0.6{\pm}3.60mg/dl$ for those drinking 1 cup a day, and $7.1{\pm}3.41mg/dl$ for those drinking more than 2 cups a day. Since smoking interacted the relationship between coffee consumption and serum total choleaterol, we re-analyzed those relationship in smokers and non-smokers separately Other atherogenic behaviors were well correlated with total cholesterol, so we adjusted the mean values of serum total cholesterol through multivariate model selection with age(r=0.12), total cigarette index (cigarette-years; r=0.10), Quetelet's index ($Kg/m^2$, r=0.16), daily calory expenditure (kcal/day, r=0.06), weekly meat and poultry consumption(g/week, r=0.05), weekly fish consumption (g/week, r=0.08), other caffeinated beverage intake (cups/week), and the amount of sugar and prim added to the coffee. Among those variables only age, Quetelet's index, fish consumption, and total cigarette index (in smokers) were remained in the models. After adjustment, the corresponing differences of total cholesterol in smokers were changed to $0.4{\pm}5.24mg/dl,\;-0.5{\pm}4.97mg/dl,\;and\;8.9{\pm}4.78mg/dl$, which were significantly different among themselves (P=0.011). In non-smokers, however, the differences were not statistically significant (P=0.76). Adjusted mean values of systolic blood pressure and diastolic blood pressure were also determined to evaluate the direct effect of coffee to cardiovascular system, but their means were not significantly different by coffee consumption(p=0.18 for SBP, p=0.48 for DBP). Asuming instant coffee in the most popular type of coffee in Korea, the association observed in our study between coffee and serum total cholesterol, especially in smokers, is very interesting finding for the connection between coffee and serum total cholesterol, because only 'boiled coffee' tend to show significant lipid raising effect rather than to other types of coffee, like filtered or espresso, in most of the western countries. We concluded that people who drink coffee more than 2 cups a day have significantly higher serum total cholesterol level than those who never drink coffee, especially in smokers.

Prevalence and Clinical Profile of EGFR Mutation In Non-Small-Cell Lung Carcinoma Patients in Southwest China

  • Aims: To investigate the distribution of epidermal growth factor receptor (EGFR) mutations, and explore any relationships with clinical characteristics in non-small-cell lung carcinoma (NSCLC) patients. Materials and Methods: EGFR mutations were assessed by ADx-ARMS in 261 NSCLC patients from West China Hospital of Sichuan University. Relationships between EGFR mutation and clinical characteristics were analyzed by SPSS. Results: The EGFR mutation rate was 48.7% (127/261), 19-del and L858R mutations occurred predominantly, accounting for 33.1% and 40.9%, respectively, in mutated cases. Moreover, 10.2% patients were found to carry double mutations. EGFR mutations occurred more frequently in women (57.5%) than in men (41.8%) (P=0.01), and were more frequent in non-smokers (61.2%) than in former or current smokers (31.2%) (P<0.00). In addition, they were more common in adenocarcinomas (52.8%) and adenosquamous carcinomas (42.8%) than in squamous cell carcinomas (14.8%) (p<0.00). However, only smoking history and pathological types, rather than gender, proved to be associated with EGFR mutations on multivariate logistic regression analysis. No significant differences in pathological stage and metastasis status were found between EGFR wild-type and mutated cases, although EGFR mutation type was related to pathological type (p=0.00) - 19-del, L858R and other mutation types respectively occurred in 34.2%, 42.5% and 23.3% of adenocarcinomas, but in 14.3%, 0% and 85.7% of non-adenocarcinomas. Conclusions: The EGFR mutation rate was 48.7% in NSCLCs in Southwest China, so that nearly 40% patients might benefit from targeted therapies. Smoking status and pathological types were independent predictors of EGFR mutation, while EGFR mutation type was related to only pathological type, rather than smoking status.

Research on the Relationship Between Serum Levels of Inflammatory Cytokines and Non-small Cell Lung Cancer

  • Aims: This study was conducted to evaluate the levels of TNF-${\alpha}$, IL-6, IL-8 and VEGF in serum of patients with non- small cell lung cancer, for assessing their possible diagnostic and prognostic roles. Methods: We enrolled 48 patients newly diagnosed with non-small cell lung cancer and 40 healthy controls. TNF- ${\alpha}$, IL-6 and IL-8 levels were measured in the serum of all the subjects with specific radioimmunoassay kits, while EGF was analyzed by sandwich enzyme immunoassay techniques. Results: A statistically significant difference was observed between lung cancer patients and the control group regarding the values of TNF-${\alpha}$, IL-6, IL-8 and VEGF in serum. Moreover, TNF-${\alpha}$, IL-8 and VEGF levels were higher in patients with advanced stages compared to early stages. In addition, higher serum levels of TNF-${\alpha}$, IL-6, IL-8 and VEGF were found in smokers than in non-smokers, both in patients and controls. Conclusion: Serum levels of TNF-${\alpha}$, IL-6, IL-8 and VEGF were all elevated in lung cancer patients, suggesting that inflammatory cytokines could be jointly used as a screening tool. Though TNF-${\alpha}$, IL-8 and VEGF levels were related to advanced disease, long-term survival studies of NSCLC patients should be performed to confirm whether they can act as biomarkers of advanced disease. In addition, smoking would be an important contributor to the processes of inflammation and lung cancer.

  • The purpose of this study was to compare the anthropometry, the serum lipid levels and the nutrient intakes of two groups according to their alcohol intake, smoking habits, regular exercise patterns, menopause status and obesity levels. The subjects consisted of 180 clinically normal adults (males 89, females 91) in the Youngdong area. There were significant differences in the triglyceride (TG) concentrations between the alcohol consumers (169.3 \pm 130.0 mg/dl) and the non-alcohol consumers (111.4 \pm 64.5 mg/dl), and the smokers (165.0 \pm 103.6 mg/dl) and the non-smokers (110.8 \pm 39.0 mg/dl). Although regular exercise did not affect the serum lipid levels, the dietary intakes of cholesterol, niacin and phosphate were significantly higher (p<0.05) in the male exercising group than in the male non-exercising group, and female exercising group consumed more polyunsaturated fatty acids (p<0.05) than the female non-exercising group. The waist/hip ratio. serum total cholesterol (198.4 \pm 36.3 mg/dl) and serum LDL-cholesterol (119.5 \pm 34.9 mg/dl) levels of the postmenopausal females were significantly higher than those of the premenopausal females (p<0.01). The obese males showed higher serum cholesterol, LDL-cholesterol, LDL/HDL, and relative cholesterol levels than those of the normal-weight males : and the obese females showed higher serum TG levels than that of the normal-weight females (p<0.01). These results suggested that drinking, smoking, menopause and obesity are risk factors for hyperlipidemia. Thus, moderation in alcohol consumption, non-smoking, regular exercise and the maintenance of normal weight are necessary to Prevent hyperlipidemia in middle-aged People.

일 지역 중.고등학생의 흡연, 음주와 안전생활 실천 (Smoking, Drinking and Safety Behavior Practices in Middle and High School Students)

  • Purpose: This study was done to identify the relationship between smoking, drinking and safety behavior in middle and high school students in one Korean province, North Cholla. Method: The study sample included 1,327 randomly selected middle and high school students from S-Gun area in Chonbuk Province. The data were collected using an anonymous questionnaire developed by the Ministry of Health and Welfare (2005). The data were analyzed using the SPSSWIN 15.0 program. Results: The smoking rate in the middle and high school students was 6.9%, and the drinking rate was 24.9%. The rate for seatbelt use was 49.3%, for drunk driving, 8.8%, and for riding in car driven by a drunk driver, 24.0%. The rates for seatbelt use, for drunk driving, and for riding in a drunk driver were higher in smokers and drinkers than in non-smokers and non-drinkers(p<.05). Conclusion: The smoking and drinking status of the adolescents was correlated with their safety behavior practices as they relate to seatbelt use, drunk driving, and for riding in car driven by a drunk driver. Therefore, an elementary school level program to curb student smoking and alcohol consumption is needed to prevent smoking and alcohol consumption in middle and high school students and contribute to improvement in their safety behavior practices.

대학생의 건강행위가 대인관계에 미치는 영향 -일개 D대학을 중심으로- (Determinant of the Personal Relationship of University Students' Health- Promoting Lifestyles - Focusing on D University-)

  • The aim of this study is to analyze the determinant factors of the personal relationships among university students. We collected data for 41 days, from 1 November to 10 December of, 2013. This study surveyed 221 university students at a university located in the metropolitan city of Busan. The findings were as follows. There were statistically significant differences in the relationships according to sex, smoking, drinking, and health status(p<0.05). Males showed this better than female. Smokers(drinkers) showed a greater difference than non-smokers(non-drinkers). Greater subjective health status was associated with more personal relationships. Students who managed their mental health and stress showed good personal relationships. From a multiple regression analysis, it was found that the determining factors as regards personal relationships were health status, current smoker, management of health, and gender health, all of which showed statistically significant differences(p<0.05). As a result, we suggest that we must provide information about health-promoting lifestyles at university to enhance personal relationships.
