대학생의 흡연 유형 및 흡연과 심리사회적 요인의 관계에 관한 연구 (A Study on the Types of Smoking and the Relationship of Psychosocial Factors to Smoking of College Students)

  • For the purpose of examining the relationship between smoking, perceived stress and vulnerability factors, and furthermore suggesting interventions to reduce excessive smoking and prevent nicotine dependency effectively, the questionnaires of perceived stress(stressful experiences), ways of stress coping(passive and active copings), smoking motives, social influence, age, amounts of smoking, and nicotine dependency were administered to smokers among medical students in Chosun university(n=186). 1) As a result of classifying smokers by the types of smoking, the most was a type of smokers to reduce negative emotions and the next in order were addictive smokers, smoker for stimulation, smoker for pleasure, etc. 2) The effects of motive, age, and active coping style were significant, but the effects of perceived stress, passive coping style, and social influence were not significant in average amount of daily smoking. 3) The effects of motive, social influence, and age were significant, but the effects of perceived stress, passive and active coping styles were not significant in the degree of nicotine dependency. To summarize, it is concluded that there were a lot of people who smoke to solve negative emotions, but the direct effects of perceived stress and passive coping style on both average amounts of daily smoking and the degree of nicotine dependency were not significant. On the other hand, motive, age, active coping style, and social influence were proved to be more important variables than the others in explaining college students' smoking.

Assessment of Nicotine Dependence among Smokers in a Selected Rural Population in Kerala, India

  • Objectives: An attempt was made to understand the nicotine dependence of smokers selected for an ongoing smoking cessation intervention programme in rural Kerala, India. Methods: Data were collected from resident males in the age group of 18 to 60 years from 4 randomly allocated community development blocks of rural Thiruvananthapuram district (2 intervention and 2 control groups). Trained accredited social health activist workers were utilised to collect data from all groups through face to face interview. Nicotine dependence among participants was assessed by means of the six-item Fagerstrom Test for Nicotine Dependence (FTND) translated into the local language. The internal consistency of FTND was computed using Cronbach's alpha coefficient. Criterion validity (concurrent) was assessed by correlations of nicotine dependence scores with age at initiation of smoking and cumulative smoking volume in pack-years. Results: Among the 928 smokers identified, 474 subjects were in the intervention area (mean age = 44.6 years, SD = 9.66 years) and 454 in the control area (mean age = 44.5 years, SD = 10.30 years). The overall FTND score among current daily smokers was 5.04 (SD: 5.05). FTND scores in the control and intervention areas were 4.75 (SD: 2.57) and 4.92 (SD: 2.51) respectively. The FTND scores increased with age and decreased with higher literacy and socioeconomic status. The average FTND score was high among smokers using both bidi and cigarettes (mean 6.10, SD 2.17). Internal consistency analysis yielded a Cronbach's alpha coefficient of 0.70 in a subsample of 150 subjects, a moderate result. The association of the scale was strongest, with the number of pack-years smoked (rho = 0.677, p < 0.001). Conclusion: A moderate level of nicotine dependence was observed among smokers in the current study. Tobacco cessation strategies could be made more cost effective and productive if a baseline assessment of nicotine dependence is completed before any intervention.

  • Objectives: This study aims to investigate the psychological factors associated with abstention following a smoking cessation program, and to suggest useful information for those who want to stop . smoking and health practitioners who help them Methods: Participants were 73 smokers (65 males, 8 females) that participated in a hospitalized smoking cessation program, whose mean age was 44.89 (SD=9.61). Participants completed questionnaires and psychological tests including: Demographic sheet, Eysenck's Personality Questionnaire, Multidimensional Coping Scale, Fagerstrom Test for Nicotine Dependence, and Jerusalem and Schwarzer's Self-Efficacy Scale. To identify whether the subjects abstained or not and to encourage them to abstain, the researcher called them on the telephone once a week for three months. After three months, they were contacted every other week till six months passed since they had left the smoking cessation program Tthey were THEN contacted once a month for another six months. The data was analyzed by using ANCOVAs with SPSS 10.0 for Windows. Results: 42 (57.5%) out of 73 abstained for one month and 26 (35.6%) abstained for one year. People who failed to abstain within a month showed a higher psychoticism and introversion personality trait than those who abstained for one month, while people who abstained for one month were coping actively in most situations, showed a higher self-efficacy and lower nicotine dependence than those who failed to abstain within a month, and people who failed to abstain within a year showed a higher psychoticism than those who abstained for one year. While people who abstained for one year were coping actively, obstinate, and interpreting positively most situations, they showed a higher self-efficacy than those who failed to abstain within a year. Conclusion: These findings reiterate the roles of personality, self-esteem, nicotine dependence in smoking and suggest the roles for smoking cessation. And it was found the roles of coping styles ,in smoking cessation. It might help smokers who want to stop smoking and health practitioners who help them

흡연갈망의 신경해부학적 특이성:기능자기공명영상연구 (fMRI Investigation on Cue-induced Smoking Craving:A Case Report)

  • Object:Nicotine dependence is the most common substance abuse disorder. One of the characteristics of nicotine dependence is craving. Regional activation of the brain induced by craving for nicotine was evaluated by using functional magnetic resonance imaging to investigate neuroanatomical site of smoking craving. Method:A smoker who satisfied DSM-IV criteria for nicotine dependence and a non smoker was studied. MRI data were acquired on a 1.5T Magnetom Vision Plus with a head volume coil. Two sets of visual stimuli were presented to subjects in a random manner. One was the film scenes of inducing smoking craving and the other was neutral stimuli not related to smoking. There were two fMRI sessions before and after smoking or sham smoking. Data were analyzed using SPM99. Results:fMRI showed significant activated area in anterior cingulate and medial frontal lobes in the smoker during smoking craving. Right dorsolateral prefrontal cortex and parietal lobes were activated in the control during visual stimulation before smoking. After smoking, there was no brain activation during visual stimulation in both of smoker and non smoker. Conclusion:Metabolic activity of the anterior cingulate and medial frontal lobes increased during craving for smoking. This result suggests that fMRI may be a valuable tool in the identification of neurobiological process of craving.

Ciga-X inhibits nicotine-induced human lung fibroblasts cytotoxicity and craving for cigarettes

  • Cigarette smoking contributes to lung cancer, cardiovascular diseases, oral diseases, etc. In desire to reduce their risk of disease, many cigarette smokers have tried to quit smoking. Sensory aspects of cigarette smoke are important for providing smoking satisfaction. Previously it was reported that citric acid aerosol significantly reduced craving for cigarettes and enhances smoking reduction and cessation. In this study, we tested whether a newly combined product Ciga-X, an aerosol for cessation aid, had toxicity in human embryonic lung fibroblast (MRC-9). The inhibitory effect of Ciga-X on cytotoxicity induced by cigarette smoke extract (CSE) or nicotine was examined in MRC-9, and craving for cigarettes and smorkers satisfaction after using Ciga-X was estimated. Ciga-X did not affect cell viability and had no toxicity in MRC-9. Ciga-X significantly inhibited not only CSE-induced cytotoxicity but also nicotine-induced cytotoxicity in MRC-9. One hundred and forty smokers rated the satisfaction for Ciga-X aerosol and craving reduction for cigarettes after using Ciga-X. The percentage of over 5 rating was 71.0% and 50.0% of subjects in satisfaction test for Ciga-X compared to their own brand and in craving reduction for cigarette, respectively. Besides, craving reduction for cigarette was highly correlated with the duration of smoking. Subjects have smoked under 10 years were more reduced in craving for cigarettes after using Ciga-X as compared to over 10 years (p=0.049). These results suggest that Ciga-X may be effective in promoting smoking abstinence with the reduction of CSE- or nicotine-induced human lung fibroblasts cytotoxicity.

청소년 흡연자를 위한 금연건강캠프의 효과 (The Effects of a Smoking Cessations Health Camp among Adolescent Smokers)

  • Purpose: This study was aimed at evaluating the effects of a smoking cessations health camp on levels of expiratory Carbon Monoxide concentration, self-efficacy, smoking knowledge and nicotine dependence among adolescents smokers. Methods: This research uitlized a one group pre-test post-test design. The smoking cessations health camp was developed after considering cognitive and behavioral aspects and was provided for two days and one night. The number of subjects was 31 high school students. Data were analyzed using descriptive analysis and paired t-test. Results: As compared to a pre-test, the average score of expiratory CO concentration and nicotine dependence were significantly lower and the average score of smoking knowledge was significantly higher in a post-test. An increase in the mean score of self-efficacy was not statistically significant. Conclusion: These results suggest that this smoking cessations health camp can provide teen smokers with the insightful antismoking information and an opportunity for anti-smoking practices. Therefore, this smoking cessations health camp can be effective and useful for adolescent smokers in other school settings.

  • Smoking damages nonsmoker's health who have been exposed to passive smoking as well as smoker's own health. Passive smoking can cause serious health damage to particular groups, such as the old aged, children and pregnant women. The purpose of this study is to investigate the relationship between nicotine concentrations in environmental tobacco smoke (ETS) and urinary cotinine concentrations of nonsmokers exposed to ETS, and to provide basic information related to health risk assessment. The results of this study were summarized as follows: 1. When 180 cigarrette were smoked during S hours (high concentrations exposure) in 132 m$^3$chamber, mean concentrations of nicotine in ETS showed 263.52 $\mu\textrm{g}$/m$^3$${\pm}$51.93. When 45 cigarretts were smoked (low concentrations exposure), it was 69.43${\pm}$8.96 $\mu\textrm{g}$/m$^3$. 2. The urinary cotinine concentrations of each times (0, 2.5, 5, 17 and 24 hours) in nonsmokers ranged from 0.27∼12.52 ng/ml in high concentrations exposure and 0.22∼2.28 ng/ml in low concentrations exposure. Mean while the total urinary cotinine concentrations during 24 hours ranged from 11.62∼31.65 ng/ml in high concentrations exposure and 3.45∼5.64 ng/ml in low concentrations exposure. 3. The correlation equation and coefficient between cotinine concentrations in nonsmokers' urine (y) and nicotine concentrations in ETS (x) was y=0.421+0.0171x and 0:875 (p<0.01) respectively, 4. The quantity of nonsmokers' smoking exposure by passive smoking can be assumed as based on the estimation of nicotine concentrations in ETS by measuring cotinine concentrations of nonsmokers' urine.

청소년기 흡연의사결정-유혹 유형과 유형별 흡연 관련 특성 (Types of Smoking Decision Making-Temptation in Adolescents and Related Characteristics)

  • Purpose: This study was done to identify types of smoking decision making-temptation in adolescents and characteristics related to type among student smokers. Method: Data collection was done from March to July. 2006. A survey was administered to 275 students in 13 high schools and 15 middle schools in Seoul, South Korea. To identify types and characteristics smoking decision making-temptation in adolescents, cluster analysis using the K-mean method was employed. Characteristics of the influential variables according to the identified types of adolescent smokers were evaluated using ANOVA. Results: Four types of smoking pattern in adolescents were identified: habitual craving (17.7%), nicotine dependence (35.8%), feeblemindedness (28.4%), and self control (18.1%). The score for nicotine dependency was higher in the habitual craving type than any other type (F=11.79, p=.001), while the score for self efficacy for smoking abstinence was higher in the self control type (F=23.06, p=.000). Conclusions: Findings from this study suggest that effective interventions for smoking cessation in adolescents require not only active implementation of nicotine replacement therapy but also development of individualized approaches for each person targeting change in the social environment that may lead to positive smoking decisional balance.

남성 당뇨병환자의 음주 및 흡연에 관한 연구 (Alcohol Consumption and Cigarette Smoking in Men with Diabetes Mellitus)

  • Purpose: The purpose of the study was to investigate the level of cigarette smoking and alcohol consumption in men with Diabetes Mellitus and to examine the relationships among those variables. Method: The subject consisted of 152 adult men with diabetes mellitus. Q-F methods and FTQ was used to measure the level of alcohol consumption and nicotine dependency. The amount of smoking was measured by the number of cigarette packs used per week. Results: Prevalence of drinking was 63.6% and in the current drinkers, 34.4% were heavy drinkers. The prevalence of cigarette smoking was 36.2% with a mean of 6.03 packs per week. Twenty percent of the smokers were dependent on nicotine. Subjects who had complications or other diseases drank alcohol more than who had not. There was a positive relationship between the level of smoking and nicotine dependency. Conclusion: Alcohol drinking and cigarette smoking is a serious health problem in men with diabetes. It is necessary to have an educational approach for controlling drinking and smoking in diabetes patients.
