Development of Work-related Musculoskeletal Disorder Questionnaire Using Receiver Operating Characteristic Analysis (Receiver Operating Characteristic 분석법을 이용한 업무관련성 근골격계질환 설문지 개발)

  • Objectives: Receive Operating Characteristic(ROC) curve with the area under the ROC curve(AUC) is one of the most popular indicator to evaluate the criterion validity of the measurement tool. This study was conducted to develop a standardized questionnaire to discriminate workers at high-risk of work-related musculoskeletal disorders using ROC analysis. Methods: The diagnostic results determined by rehabilitation medicine specialists in 370 persons(89 shipyard CAD workers, 113 telephone directory assistant operators, 79 women with occupation, and 89 housewives) were compared with participant's own replies to 'the questionnair on the worker's subjective physical symptoms'(Kwon, 1996). The AUC's from four models with different methods in item selection and weighting were compared with each other. These 4 models were applied to 225 persons, working in an assembly line of motor vehicle, for the purpose of AUC reliability test. Results: In a weighted model with 11 items, the AUC was 0.8155 in the primary study population, and 0.8026 in the secondary study population(p=0.3780). It was superior in the aspects of discriminability, reliability and convenience. A new questionnaire of musculoskeletal disorder could be constructed by this model. Conclusion: A more valid questionnaire with a small number of items and the quantitative weight scores useful for the relative comparisons are the main results of this study. While the absolute reference value applicable to the wide range of populations was not estimated, the basic intent of this study, developing a surveillance fool through quantitative validation of the measures, would serve for the systematic disease prevention activities.

A Study on the Level of Air Pollution and Noise in Pusan Area (부산의 지역별 대기오염과 소음정도에 관한 조사연구)

  • In order to assess the degree of atmospheric pollution and noise and to contribute the health improvement of residents in Pusan, the author measured the levels of CO, $SO_2,\;NO_2$, TSP, HCHO and noise in 8 areas (industrial, residential and park areas) from January to March in 1990. The results were as follows : 1. Sasang industrial area was significantly higher($2.85{\pm}0.84ppm$) in the average concentration of CO than other areas. However, there no areas to affect the human body in terms of CO concentration. 2. In general, industrial area was significantly higher ($0.134{\pm}0.084ppm$) in the average concentration of $SO_2$ than other areas, and it was the lowest ($0.009{\pm}0.005ppm$) in the Namchon-dong area. 3. Industrial ($0.033{\pm}0.009ppm$) and residential area ($0.029{\pm}0.004ppm$) were significantly higher in the average concentration of $NO_2$ than Park area ($0.009{\pm}0.001ppm$). However, there were no areas to affect the human body in terms of $NO_2$ concentration. 4. Sasang industrial area was the highest ($580.4{\pm}415.26{\mu}g/m^3$) at the average concentration of TSP and Hae-Un Dae area was the lowest ($97.22{\pm}37.86{\mu}g/m^3$). But TSP concentration showed the level to affect the human body in most areas. 5. Industrial area was significantly higher ($2.25{\pm}1.15ppb$) in the average concentration of HCHO than residential ($1.13{\pm}0.25ppb$) and park area ($1.33{\pm}0.20ppb$). 6. Industrial area was significantly higher ($77.28{\pm}6.92dB(A)$) in the level of noise than residential ($65.77{\pm}3.76dB(A)$) and park area ($64.65{\pm}5.25dB(A)$). In comparison with regional Standard Noise Level, howeverm the average noise level of residential and park area was higher than that of the Standard. In general, the level of pollution of industrial complex areas was relatively higher than those of residential and park areas. Among the industrial areas, sasang area was worst in most items. Both $SO_2$ and TSP showed the level to affect the human body in most of studied areas. It is necessary to install a new Air Quality Standard for HCHO to screen our environmental pollution.

Comparative analysis of RN-BSN Program in Korea and U. S. A. (간호학사 편입학제도의 교과과정 비교분석)

  • In response of the increasing demand for professional degree in nursing, some university in Korea offers RN-BSN program for R. N. from diploma in nursing. However, RN-BSN program in Korea is in formative period. Therefore, the purpose of this survey study is for the comparative analysis of RN-BSN curriculum in Korea and U.S.A. In this study, subjects consisted of 18 department of nursing in university and 5 RN-BSN programs in Korea and 18 department of nursing in university and 12 RN-BSN programs in U.S.A. For earn the degree of Bachelor of Science in Nursing, the student earns 134 of mean credits in U.S.A., whereas 150.3 of mean credits in Korea. The mean credit for clinical pratice is 30.1 in U.S.A., whereas 23.9 in Korea. Students are assigned to individually planned clinical experiences under the direction of a preceptor in U.S.A. In RN-BSN program, total mean credits through lecture and clinical practice for earn the degree of BSN is 35.5(lecture : 27.7, practice ; 7.8)in U.S.A., whereas,48.1 (lecture;42.1, practice;6.0) in Korea. RN-BSN program can be taken on a full-or-part time basis in U.S.A., whereas didn't in Korea. Especially, emphasis is place on the advanced nursing practicum that focus on the role of the professional nurse in providing health care to individuals, families, and groups in community setting in U.S.A. 27.7 of mean credits was earned through lecture in U.S.A., whereas 42.1 of mean credits in Korea. It means that RN-BSN program in Korea is the lesser development in teaching method and appraisal method than in U.S.A. Students of RN-BSN program in U.S.A. can earns credit through CLEP, NLN achievement test, portfolio review session etc as well as lecture. Therefore, the authors suggests some recommendations for the development of curriculum of RN-BSN program in Korea based on comparative analysis of RN-BSN curricula in U.S.A. and Korea. 1. The curriculum of RN-BSN Program in nursing was required to do some alterations. Nursing care, today, is complex and ever changing. According to change of public need, RN-BSN curriculum intensified primary care program in community setting, geriatric nursing, marketing skill, computer language. 2. The various and new methods of earning credit should be developed. That is, the students will earn credits through the transfer of previous nursing college credits, accredited examination of university, advanced placement examination, portfolio review session, case study, report, self-directed learning and so on. Flexible teaching place should ile offered. 3. Flexible teaching place should be offered. The RN-BSN curriculum should accommodate each RN student's geographical needs and school/work schedule. Therefore, the university should search a variety of teaching places and the RN students can obtain their degrees comfortably throughout the teaching place such as lecture room inside the health care agency and establishment of the branch school in each student's residence area. 4. The RN-BSN program should offer a long distance education to place-bound RN student in many parts of Korea. That is, from the main office of university, the RN-BSN courses are delivered to many areas by Internet, EdNet (satellite telecommunication) and other non-traditional methods. 5. For allowing RN student to take nursing courses, program length should be various, depending upon the student's study/work schedule. That is, the various term systems such as semester, three terms, quarter systems and the student's status like full time or part time should be considered. Therefore, the student can take advantage of the many other educational and professional opportunities, making them available during the school year.

A Study on Seasonal Variations of Food Consumption of Korean Farmers (한국농촌주민(韓國農村住民)의 계절별(季節別) 식품섭취조사연구(食品攝取調査硏究))

  • This is the report of a food consumption survey of 193 members of 30 farm families in three rural villages of Korea. From a total of 188 households of the three villages, namely Wolgok in Kyunggi Do Province, Wachon in Kangwon Do Province, and Sobong in Cholla Namdo Province, 30 households were chosen for the survey. Four consecutive seasonal surveys, beginning in the autumn, November 1974 and finishing with the summer, August 1975, were made and each survey covered three consecutive days. The Precise Weighing Method was used in evaluating the kinds of food and nutrient intakes of the subjects. This method entails the accurate weighing of all foods by the investigator just prior to their consumption by the subjects under investigation. This survey was male to determine the actual food eaten by the members of these farm families at each season of the year, and to note any special differences in the foods consumed. The results obtained are summarized as follows: Food Intake The traditional ordinary diet of Korean farmers consists mainly of rice and other cereals as staple foods, and simple subsidary foods. The nutritional shortcomings of rice as a staple food are evident in the subclinical status of nutritional deficiencies. The mean consumption of cereals and their products was very high and it was more than half of the daily total food intake throughout all the seasons. The consumption of animal food, fruits, and fat by the farmers was very low. Fruit intake during winter and spring was almost nil. One of the special foods in the Korean dietary pattern is Kim-chi, a variety of pickled and fermented vegetables. The mean intake of Kim-chi during autumn and winter was very much higher than the other seasons. Korean farmers have difficulties in balanced year-round supply of food due to seasonality of food production and lack of food preservation. As nutrition problems of Korean farmers are highly influenced by seasonal variations, the expansion of an appropriate food preservation and storage programme should be encouraged. Special consideration should be given to new methods of preservation, the types of foods preserved, and production and consumption of nutritionally valuable foods for the improvement of the nutritional status in rural communities. Nutrient Intake Average adult rates for calorie and protein in the three study areas were 0.80 and 0.87 respectively, with little variations by villages. The adult caloric intake in averaged 2,928 kcal for autumn, 2,662 for winter, 2,673 for spring, and 2,760 for summer. Cereals contributed most of the total calories. In terms of the recommended daily dietary allowances for the Korean given by the Korea FAO Association, the diets in this survey were adequate except in the winter and spring. The protein intake averaged from 76.4 grams in autumn to 83.4 grams in summer and was near the recommended allowance, but a relatively large proportion of the total protein came from cereals and their products. The fat intake was unsatisfactory, ranging from 19.2 grams to 29.3 grams for the four seasons, and the consumption during farming season was higher than other seasons. It is apparent that the diet of the Korean farmers should be supplemented by calcium during autumn and winter. The highest calcium intake, with a mean of 583.5 milligrams, was recorded in summer. The mean intake of iron ranged from a low of 9.2 milligrams in the autumn to a high of 15.0 milligrams in the spring and it was satisfactory for all seasons. Among the vitamin group, the daily average intakes of vitamin A, thiamine, riboflavin, and ascorbic acid were lower than the desired intake throughout all the seasons. The mean vitamin A intake as ${\beta}-carotene$ ranged from a low of 2,807.8 IU to a high of 5,221.0 IU. The thiamine intake ranged from 0.98 milligrams to 1.19 milligrams, while mean riboflavin intake ranged from 0.92 milligrams to 1.13 milligrams. Ascorbic acid intake was 37.3 milligrams, the lowest, and the highest 47.8 milligrams. Niacin intake among Korean farmers was higher than the recommended allowance throughout all the seasons, and the mean intake ranged from 18.3 milligrams to 25.8 milligrams. The main character of the Korean farmers' diet has been found low in quality of protein and high in carbohydrate. Attention should be paid to the low intake of vitamin A, thiamine, riboflavin, and ascorbic acid among Korean farmers.

Evaluation of Social Nicotine Dependence Using the Kano Test for Social Nicotine Dependence (KTSND-K) Questionnaire in Korea (Kano Test for Social Nicotine Dependence(KTSND-K) 설문지를 통한 한국인의 사회적 니코틴의존성의 평가)

  • Background: Smoking is one of the most important leading causes of morbidity and mortality. Smoking habit is recognized as nicotine dependence, which consists of physical and psychosocial dependence. To evaluate social nicotine dependence, the Kano Test for Social Nicotine Dependence (KTSND) working group developed a new questionnaire, which consists of 10 questions with a total score of 30 in Japan. We examined the social nicotine dependence among healthy adults using the new KTSND questionnaire and evaluated validity of the KTSND questionnaire in Korea. Method: We applied Korean KTSND questionnaire version 2 to employees of hospital, university students and people for medical examination and promotion test. Complete data obtained from the 741 responders were analyzed. Result: The mean age of responders was 31.8 years. Among them, males were 57.8%. Current smokers, ex-smokers, and non-smokers were 13.8%, 12.8%, and 73.4% respectively. According to smoking status, the total KTSND scores of current smokers were significantly higher than those of ex-smokers, and of non-smokers ($17.1{\pm}5.4$ versus $14.3{\pm}5.5$, and $12.3{\pm}5.5$, $p{\leq}0.001$). The total KTSND scores of males were higher than those of females, suggesting that males have a propensity for depending nicotine socially much more than females ($14.3{\pm}5.7$ and $11.7{\pm}5.4$ respectively, p<0.001). Eight of ten questions produced significantly different scores among three different smoking groups. When current smokers were sub-classified by heavy smoking index (HSI) that represented physical nicotine dependence, we did not find a significant difference of KTSND score between low HSI group (<4) and high HSI group (${\geq}4$), This finding suggested that the psychosocial dependence might play a different role from physical nicotine dependence in smoking. Most of the non-smokers (62.5%) had an experience of harmful passive smoking especially in public place. Conclusion: Our study suggested that the KTSND questionnaire could be a useful method to evaluate psychosocial aspects of smoking.

Applicability of American and European Spirometry Repeatability Criteria to Korean Adults (한국 성인을 대상으로 한 미국 및 유럽 폐활량 검사 재현성 기준의 유용성)

  • Background: The objective of this study was to evaluate the clinical applicability of the repeatability criteria recommended by the American Thoracic Society/European Respiratory Society (ATS/ERS) spirometry guidelines and to determine which factors affect the repeatability of spirometry in Korean adults. Methods: We reviewed the spirometry data of 4,663 Korean adults from the Korean National Health and Nutritional Examination Survey (KNHANES) Chronic Obstructive Pulmonary Disease Cohort (COPD cohort) and the Community-based Cohort Study VI-Fishing village/Islands (community cohort). We measured the anthropometric factors and differences between the highest and second-highest FVC (dFVC) and $FEV_1$ ($dFEV_1$) from prebronchodilator spirometry. Analyses included the distribution of dFVC and $dFEV_1$, comparison of the values meeting the 1994 ATS repeatability criteria with the values meeting the 2005 ATS/ERS repeatability criteria, and the performance of linear regression for evaluating the influence of subject characteristics and the change of criteria on the spiro-metric variability. Results: About 95% of subjects were able to reproduce FVC and $FEV_1$ within 150 ml. The KNHANES based on the 1994 ATS guidelines showed poorer repeatability than the COPD cohort and community cohort based on the 2005 ATS/ERS guidelines. Demographic and anthropometric factors had little effect on repeatability, explaining only 0.5 to 3%. Conclusion: We conclude that the new spirometry repeatability criteria recommended by the 2005 ATS/ERS guidelines is also applicable to Korean adults. The repeatability of spirometry depends little on individual characteristics when an experienced technician performs testing. Therefore, we suggest that sustained efforts for public awareness of new repeatability criteria, quality control of spirograms, and education of personnel are needed for reliable spirometric results.

A Comparative Analysis of Teukyakmeip and Consignment (특약매입과 콘사인먼트 비교분석)

  • Purpose - The purpose of this study was to compare and contrast the applicability and effectiveness of both teukyakmeip contracts of Korea and consignment contracts of the United State to demonstrate the effectiveness and practicability of teukyakmeip in Korea. These are popular contract agreements between large retailers and their suppliers and vendors. In recent years, teukyakmeip was critically examined and scrutinized by the politicians, the media, and the public of Korea. Consequently, this paper focusesheavily on identifying and analyzing different types of contract agreements between large retailers and their suppliers that currently exist in Korea and compares and contrasts those analyzed contract agreements with teukyakmeip. The article also comparesand contrasts teukyakmeip with the consignment agreements of the United States to identify similarities and differences. Research design, data, and methodology - This study is a descriptive study and has used personal interviews to collect and analyze the data. This study also fits the definition of the case study wherein it is entirely focused on investigating a real-life event: analyzing and examining contract agreements in the distribution industry. Both randomly selected management and vendor representatives from the three major department stores, Lotte, Hyundai, and Shinsegae, in Korea were interviewed between July and September 2013. The analysis of the consignment agreement was conducted based on existing secondary data. Results - Although the evidence of the abuse of teukyakmeip and consignment by large retailers from both countries clearly exists, the findings suggestthat both contract agreements would remain as the most relevant and effective legal contracts between large retailers and their suppliers. Based on the comparisonanalysis of teukyakmeip and consignment, both contracts indicated that suppliers are fully responsible for inventory and inventory management. If sales person is necessary for promoting special product, then suppliers are responsible for providing a sales person and their wages under both contracts. However, American department stores, those located outside urban area, tend to use their own employees to perform special product and sales promotion. The retailersare fully responsible for any interior or floor design or redesign of the retail store to accommodate the products from vendors under consignment; however, both suppliers and retailers share the cost of designing and redesigning the interior to accommodate vendors'products under teukyakmeip. Suppliers are responsible for pricing and supplying the quantity of the products under both agreements. Both contracts allow special sales commission as long as vendors agreed. Vendors use this special commissionto introduce their new products or apply market penetration strategy. Conclusions -The findings of this study showed the changing pattern of contract agreements between large retailers and their suppliers from both countries. Furthermore, this study evidently generated policy implications of teukyakmeip which recently became the major social issue in Korea and attracted many policymakers to gain political points by criticizing the teukyakmeip system and the large retailers. The findings of the study would be valuable to policy makers in making appropriate decisions and to large retailers and vendors in making beneficial agreements. The major implication of this study is that teukyakmeip and consignment agreements include very similar or almost identical characteristics, and they are popular among department stores and suppliers. The issue of abolishing teukyakmeip in Korea needs to be examined cautiously because teukyakmeip is the best one available at the moment, and the study suggests that no one benefits from abolishing this system.

The objective and quantitative analysis of malocclusion : Part 1. Objective malocclusion severity and subjective treatment difficulty (부정교합의 객관적 정량분석: Part 1. 객관적 부정교합 경중도와 주관적인 치료난이도의 상관관계)

  • The evaluation of malocclusion has to be done quantitatively and qualitatively. This will be lead toward an analysis of malocclusion severity as well as treatment difficulty. The method of proper evaluation of malocclusion severity and treatment difficulty is necessary to assess treatment effect and efficiency for the orthodontists and to establish fundamentals for planning and executing the health-related policies in private and public institutions. The purposes of this study as the first part of the objective and quantitative analysis of malocclusion were 1) to measure treatment difficulty based on the opinions of several orthodontists. and 2) to investigate the relationships between objective malocclusion severity and subjective treatment difficulty 100 pairs of dental casts that had various types and severity of malocclusion were selected from the orthodontic departments of Kyurghee University and Samsung Medical Center The objective malocclusion severity was measured with the PAR (Peer Assessment Rating) index and the subjective treatment difficulty was evaluated by 8 experienced orthodontists. The relationships between objective malocclusion severity and subjective treatment difficulty were statistically evaluated. There were significant relationships between objective malocclusion severity and subjective treatment difficulty especially in the measurements of the upper anterior alignment, the buccal occlusion. the overjet, the overbite and the midline discrepancy en the malocclusion components. The results of this study can provide the background knowledge to develop a new occlusal index. which contains both the malocclusion severity and treatment difficulty for Korean orthodontists.

Analysis of Hepatobiliary Disorders from a Nationwide Survey of Discharge Data in Korean Children and Adolescents (전국 퇴원자료조사를 통한 소아청소년 간담도 질환의 분석)

  • Purpose: To update the epidemiologic information of hepatobiliary diseases in pediatric inpatients using cross-sectional survey data throughout the Republic of Korea. Methods: Nationwide cross-sectional survey was obtained from the 85 residency training hospitals in Korea to gather the final diagnosis on discharge. The surveyed periods were from 2004 to 2006. All the reports regarding the diagnosis were based on ICD-10 system. In this study, we focused on hepatobiliary diseases. Results: A total of 826,896 cases with discharge data were collected, of which 4,151 (5.0%) hepatobiliary cases were identified; 2,385 cases (57.4%) of hepatobiliary disease were hepatitis, which was the most common hepatobiliary disease. Other diseases included congenital hepatobiliary diseases (524 cases [12.6%]) and biliary diseases (315 cases [7.6%]). The prevalence of hepatobiliary disease according to age differed. Biliary atresia was the most common hepatobiliary disease in the neonatal period, whereas the prevalence of hepatitis increased in adolescents. The total number of hepatobiliary operations was 416 cases. With the comparison of annual data, there was no definite difference in the total number of hepatobiliary cases. The average duration of hospital stay appeared to decrease gradually. Conclusion: In this study, we have summarized the recent epidemiology of hepatobiliary disorders in Korean children based on discharge data. Hepatobiliary disorders in pediatric inpatient units consisted of diverse disorders with a low prevalence, so multi-center approaches should be considered to enhance the clinical and public health outcomes. To improve this nationwide survey, a new data collecting system should be developed.

A Study on Mother's Feeding Practice in a Urban Apartment Area (일부도시(一部都市) 아파트지역(地域) 어머니의 수유(授乳)에 관(關)한 조사연구(調査硏究))

  • This study was undertaken to observe relationships between patterns of feeding, supplementary-feeding and various maternal, family, and socioeconomic charactereistics in Hae Cheog Apartment area in Cheongdam-dong, Kangnamku, Seoul on July 2 to 12 in 1982. The results were based on a questionnaire from 179 mothers who have the last-born child under two years old. Results were as follows: 1) In socio-demographic characteristics, most of mothers were 25 to 30 years old and 52.0% of index children were under 6 months old. About 56.0% of families were the salaries and 47.0% of them earn over 500,000 won a month. 40.8% of mothers were college graduates and 81.6% of mothers had no occupation. 2) 89.4% of mothers received prenatal care in pregnancy of the index children and mothers who have delivered the child in medical institute were 88.3%. Mothers who recieved education of breast care and feeding technique through prenatal care were 22.4%, 31.8% respectively. 3) In the feeding method, 44.1% of mothers took the breast feeding, 24.0% of them chose the artificial feeding, and 20.7% of them chose the mixed feeding. Mothers who changed the-method from breast feeding to artificial feeding were 10.6% and only 0.6% of mothers changed from artificial feeding to breast feeding. 4) According to the questionnaire, 37 mothers have already finished lactation (no relation with. the beginning of weaning food). In breast feeding, one mother has lactated for $4{\sim}6$ months, one has lactated for $7{\sim}9$ months, four have lactated for $10{\sim}12$ months, and seven have. continued the lactation over 12 months. In artificial and mixed feeding, as the same phenomenon, most of mothers have lactated for more than 12 months. 5) The reasons for feeding method were as follows: In breast feeding, 64.6% of them took the method because they thought the breast milk nutrious, in artificial feeding, 34.9% of them chose it because they had occupation and in the mixed feeding, 67.6% of mothers took the method because of lack of their breast milk. In the case of changing the method from breast feeding to artificial feeding, 42.1% of them answered that they had to change the method because of lack of breast milk. 6) In most of cases, the 4th month was the proper period to begin the weaning food and 32.5% of breast feeding children and 27.6% of artificial feeding children began the weaning food in 4th month. After 4th month, there was no difference between breast feeding and artificial feeding in the beginning of weaning food. 7) In the matter of menstruation, 29.8% of mothers who had breast feeding started their menstruations in 3 months and the rest of them delayed until 12 months. 40% of mothers who had artificial feeding began to menstruate after 2 months and all the rest started within 5 months. 8) The birth interval between the index child and next new child (would-be-born): In breast feeding, the interval of $18{\sim}24$ months had a majority as 50.0%, and in the artificial feeding, the interval of over 24 months marked 66.7% of them. It was analyzed that the birth interval of artificial feeding was wider than that of breast feeding. 9) In the desirable number of children, the mothers who had breast feeding wanted two sons and two daughters as proper children. Those who want two children in disregard of the sex (son or daughter) were 89.3% of breast feeding, and 80.0% of artificial feeding respectively. Mothers who had breast feeding wanted two children rather than one child. 10) In the family planning practice, the rate of practice were 41.9% in breast feeding, and 58.1% in artificial feeding respectively. In the case of breast feeding, the using rate of family planning practice in men was higher than in women.
