EFFECT OF POTASSIUM ION ON INTRADENTAL NERVE ACITIVITY (칼륨이온이 치수내 신경활동에 미치는 영향)

  • The intradental nerve activity was recorded from single pulp nerve unit dissected from the inferior alveolar nerve in canine teeth of anesthetized cats. The effects of various test solutions on intradental nerve activity evoked by 4M NaCl applied to the deep dentinal cavities were investigated. 1. Total 15 single pulp nerve units were recorded. Of these 9 were Mi - fibers and 6 were C -fibers. The mean conduction velocity and electrical threshold of $A{\delta}$ - fiber were $6.3{\pm}3.7m/sec$, $1.2{\pm}0.7V$ and those of C - fiber were $1.0{\pm}0.2m/sec$, $2.3{\pm}1.3V$, respectively. The response to cold stimuli of $A{\delta}$ - fiber began immediately and that of C - fiber began after a latency. 2. When applied to deep dentinal cavity, 4M NaCl induced irregular bursts of action potential in 4 $A{\delta}$ - fibers and 3 C - fibers, which continued until the solution was washed away. 3. In the $A{\delta}$ - fiber, histamine failed to induce any nerve acitivity and did not produce an increase in intradental nerve activity evoked by 4M NaCl. However following the application of 1M KCl, the response to 4M NaCl was eliminated. 4. In the C - fiber, histamine generated some nerve activity and produced a significant increase in intradental nerve activity evoked by 4M NaCl, but 5M $CaCl_2$ did not abolish this enhandced response.

The Restorative Effect of Gallic Acid on the Experimental Sciatic Nerve Damage Model

  • Objective : Peripheral nerve injuries occur mostly as a result of mechanical trauma. Due to the microvascular deterioration in peripheral nerve damage, it becomes challenging to remove free oxygen radicals. Gallic acid is a powerful antioxidant with anti-inflammatory effects and a free radical scavenger. The purpose of the study is to show that gallic acid contributes to the restorative effect in mechanical nerve damage, considering its antioxidant and anti-inflammatory effects. Methods : Thirty male Sprague Dawley albino mature rats were included in the study. Ten of them constituted the control group, 10 out of 20 rats for which sciatic nerve damage was caused, constituted the saline group, and 10 formed the gallic acid group. Post-treatment motor functions, histological, immunohistochemical, and biochemical parameters of the rats were evaluated. Results : Compared to the surgery+saline group, lower compound muscle action potential (CMAP) latency, higher CMAP amplitude, and higher inclined plane test values were found in the surgery+gallic acid group. Similarly, a higher nerve growth factor (NGF) percentage, a higher number of axons, and a lower percentage of fibrosis scores were observed in the surgery+gallic acid group. Finally, lower tissue malondialdehyde (MDA) and higher heat shock protein-70 (HSP-70) values were determined in the surgery+gallic acid group. Conclusion : Gallic acid positively affects peripheral nerve injury healing due to its anti-inflammatory and antioxidant effects. It has been thought that gallic acid can be used as a supportive treatment in peripheral nerve damage.

Infraorbital Nerve Function Following Tailoring of Medpor® in Reconstruction of Inferior Orbital Wall Fracture (안저골절재건 시 Medpor®의 맞춤조작에 따른 안와아래신경의 기능)

  • Purpose: In the orbital floor fracture, sensory impairment due to the damage of the infraorbital nerve is one of the most common symptom and complication. In this report, we have the assumption that tailoring of $medpor^{(R)}$ for decompression may have correlations to the damage and regeneration of the nerve. Methods: Among patients who had open reduction for pure orbital floor fracture in our hospital from March 2005 to March 2008, we selected 80 cases. In 40 cases, we inserted tailored $medpor^{(R)}$, and in other 40 cases, non-tailored $medpor^{(R)}$ was inserted. Patient's reports were obtained and analyzed, and the pin-prick test and the 2-point discrimination test on the infraorbital nerve regions were done for testing the sensory impairments. Results: The results show that the patients who adopted sculpture of $medpor^{(R)}$ showed higher tendency of recovery of sensory impairments in the patient's subjective report, static touch sensation, static two point discrimination using. And in postoperative 3 months, there are statistically significant recovery of sensory symptoms, signs and the result of sensory tests. Conclusion: From these results, tailored $medpor^{(R)}$ in reconstruction of orbital wall fracture may improve recovery of sensory impairments for decreasing of compression of infraorbital nerve.

Nerve Injuries after the Operations of Orbital Blow-out Fracture (안와골절 수술 후 발생한 신경손상)

  • Purpose: In accordance with the increasing number of accidents caused by various reasons and recently developed fine diagnostic skills, the incidence of orbital blow-out fracture cases is increasing. As it causes complications, such as diplopia and enophthalmos, surgical reduction is commonly required. This article reports a retrospective series of 5 blow-out fracture cases that had unusual nerve injuries after reduction operations. We represents the clinical experiences about treatment process and follow-up. Methods: From January 2000 to August 2009, we treated total 705 blow-out fracture patients. Among them, there were 5 patients (0.71%) who suffered from postoperative neurologic complications. In all patients, the surgery was performed with open reduction with insertion of $Medpor^{(R)}$. Clinical symptoms and signs were a little different from each other. Results: In case 1, the diagnosis was oculomotor nerve palsy. The diagnosis of the case 2 was superior orbital fissure syndrome, case 3 was abducens nerve palsy, and case 4 was idiopathic supraorbital nerve injury. The last case 5 was diagnosed as optic neuropathy. Most of the causes were extended fracture, especially accompanied with medial and inferomedial orbital blow-out fracture. Extensive dissection and eyeball swelling, and over-retraction by assistants were also one of the causes. Immediately, we performed reexploration procedure to remove hematomas, decompress and check the incarceration. After that, we checked VEP (visual evoked potential), visual field test, electromyogram. With ophthalmologic test and followup CT, we can rule out the orbital apex syndrome. We gave $Salon^{(R)}$ (methylprednisolone, Hanlim pharmaceuticals) 500 mg twice a day for 3 days and let them bed rest. After that, we were tapering the high dose steroid with $Methylon^{(R)}$ (methylprednisolon 4 mg, Kunwha pharmaceuticals) 20 mg three times a day. Usually, it takes 1.2 months to recover from the nerve injury. Conclusion: According to the extent of nerve injury after the surgery of orbital blow-out fracture, the clinical symptoms were different. The most important point is to decide quickly whether the optic nerve injury occurred or not. Therefore, it is necess is to diagnose the nerve injury immediately, perform reexploration for decompression and use corticosteroid adequately. In other words, the early diagnosis and treatment is most important.

Patterning of Sympathetic Nerve Activity in Patients with Inner Ear Dysfunction Examined by Heart Rate Variability (심박변이도(Heart rate variability) 검사를 통한 내이(內耳) 질환 환자들의 교감 신경 활성 패턴 고찰)

  • Objective : To evaluate autonomic nerve balance and sympathetic nerve activity in patients with inner ear dysfunction as examined by heart rate variability(HRV) Research Methods and Procedures : One hundred and twenty three patients(between 15 to 59 years old) who visited Dept. of Oriental medical Opthalmology & Otolaryngology, Kyunghee university and had an examination of HRV test, were selected as subjects of our study. We checked items of HRV test(such as SDNN, RMSSD, LF, HF, TP, norm-LF, norm-HF, LF/HF ratio), and analyzed time and frequency domain differences between three groups classified as tinnitus, hearing loss and vertigo. And we compared HRV items(such as SDNN,RMSSD, TP and LF/HF ratio) with standard levels. Results and Conclusion : Our results showed no significant differences between three groups in HRV items except for mean heart rate. Patients with inner ear dysfunction showed significant lower TP than 2000$(ms^{2})$(p <0.001) and significant higher LF/HF ratio than 1.5(p=0.003). So we suggest that patients with inner ear dysfunctions such as tinnitus, vertigo and hearing loss, have sympathetic hypertonus and inactivity of autonomic nerve regulation.

Effects of Caffeine on Nerve Conduction Velocity (카페인이 신경전도속도에 미치는 영향)

  • This study examined the effect of the nervous-system-stimulating caffeine on the nerve conduction velocity. The purpose of this study is to investigate the effect of caffeine that stimulates the nervous system on nerve conduction test. Although both measurement intervals did not show statistically significant differences when comparing the pre- and post-NCV values within the control and individual experimental groups, it was found that the nerve conductivity in the Axilla-Above Elbow section increased significantly after caffeine intake for the experimental group. Caffeine intake, which has increased the nerve conduction velocity (NCV), was determined to play roles in improving motor skills, muscle strength and nerve performance by temporarily increasing the nerve conduction velocity. Through this study, we learned that caffeine has an influence on the peripheral nervous system as it helps to improve the nerve conduction velocity. upon an appropriate amount of caffeine intake. We hope that these results will help develop treatment and diagnostic methods for patients with nerve dysfunction and myofunctional disorders.

The Cross Sectional Study of Autonomic Nerve Balance And Stress In Patient Who Visted Dept. of Oriental Medical Dermatology (HRV 검사를 통한 피부과 내원환자의 자율신경 균형 및 스트레스 단면 연구)

  • Objective : Etiologically, stress is a important factor to immune system and skin disease. Human body is controled to keep his homeostasis from stress by autonomic nerve system. So we investigated autonomic nerve balance and stress level of patients with skin disease by heart rate variability(HRV) test. Research Methods and Procedures : Fifty two patient(from 15 years old to 35 years old) who visited Dept. of Oriental medical Dermatology, Kyunghee university at first and had an examination by HRV test, were selected as the subject of our study. We checked items of HRV test(such as SDNN, RMSSD, VLF, LF, HF, TP, norm-LF, norm-HF, LF/HF ratio, LnHF and LnHF), and analyzed time and frequency domain by sex, age, onset, color on end of patient's tongue, sweating and sleep condition. Results and Conclusion : Men showed significant higher SDNN, RMSSD, LF, norm-LF, norm-HF, LF/HF ratio, LnLF and LnHF than women(p <0.05). Patients who were attacked by skin disease within 6 months, showed significant higher TP and VLF than over 6 months(p <0.05). Patients with normal color on the end of their tongue showed significant higher TP and VLF than with flare color. We analyzed correlation to color on the end of tongue and RMSSD. Prevalence risk(RMSSD <20ms) in the case of normal color on the end of tongue was 5 times as high as in the case of flare color(p <0.1). And we analyzed correlation to age and LF/HF ratio. Prevalence risk(LF/HF ratio <1) of patients from 15 years old to 25 years old was 1.467 times as high as from 26 years old to 35 years old(p <0.1).

Antioxidant Effects of Berchemia berchemiaefolia in Nerve Pain Models

  • Berchemia berchemiaefolia (BB) are climbing plants or small to medium-sized trees that live in Africa, Asia and America. We performed the present study to investigate whether oral administration of Berchemia berchemiaefolia extract (BBE) protects SD rats from pain. The SD rat experimental groups were divided into four groups. Two of the animal model groups were fed on BBE (200 mg/kg or 100 mg/kg). We performed oral acute toxicity test to determine the optimal oral dose of BBE. To explore if BBE alleviated pain in the SD rat, we undertook the tail flick latency test and formalin test. Additionally, we conducted the anti-oxidative test. The findings of the present study suggest that Berchemia berchemiaefolia extract exhibits strong antioxidant and analgesic activities.

Measurement of Magnetic Flux and Induced Current in Magnetic Stimulation for Urinary Incontinence Treatment (요실금 치료용 자기 자극기의 자속밀도 및 유도전류 측정)

  • A simple method for measuring magnetic flux and induced current in magnetic nerve stimulation for urinary incontinence treatment is proposed. Unlike electric nerve stimulation, direct measurement of the induced current in magnetic nerve stimulation is impossible. Since induced currents stimulate nerves or muscles in magnetic nerve stimulation, measuring induced current is very important in validating stimulation efficacy and securing safety. The magnetic flux measuring system is composed of 6 layers with pick-up coils of 7 by 7 in each layer, and the induced current measuring system is composed of 6 layers with 7 concentric circular coils in each layer. The proposed method can be used in the design or performance test of a magnetic nerve stimulator for many clinical applications such as urinary incontinence treatment, activation of peripheral nerves, and transcranial magnetic stimulation.

Case Report of Korean Medical Treatment on Acute Peroneal Nerve Palsy Patient Caused by Prolonged Immobilization (장시간 부동 자세로 야기된 급성 비골신경 마비 환자의 한방치료 증례보고)

  • Objectives This study was carried out to investigate the clinical effects of Korean medical treatment on acute peroneal nerve palsy caused by prolonged immobilization. Methods One patient with acute peroneal nerve palsy was treated with various korean medical techniques such as acupuncture, moxibustion, cupping and herbal medicine, bee-venom acupuncture for 12 weeks. Manual Muscle Test, Range of motion, Numerical Rating Scale, Ankle-hindfoot scale, Digital infrared thermographing imaging system were used to evaluate treatment effect. Results After treatment, all the scales mentioned above were improved significantly. Conclusions This result showed that Korean medical treatment is effective on acute peroneal nerve palsy caused by prolonged immoblization.
