주사침 말단의 형상과 위치, 세척액 주입속도가 치근단에 작용하는 압력에 미치는 영향 (Effect of needle tip design and position, and irrigant flow rate on apical pressure)

  • 본 연구에서는 근관 내의 주사침 말단의 위치, 근관 세척액의 주입 속도, 주사침 말단의 형상이 근관 세척 시 발생하는 치근단 압력에 미치는 영향을 평가하고자 하였다. 5개의 사람 하악 소구치에 근관 와동 형성 후 #35(0.06 taper) 니켈-티타늄 회전식 기구로 근관을 형성하였다. 주사침 말단의 형상에 따라 구분되는 notched, side-vented, flat 3가지 종류의 주사침을 근단 협착부로부터 치관 방향으로 1, 3, 5 mm 거리가 되는 지점(주사침 말단의 위치)에 위치시켰다. 각 군에 대하여 세척액 주입 속도는 0.05, 0.1, 0.2, 0.3 ml/s로 변화시키면서 치근단 압력을 측정하였다. 나머지 조건이 동일한 경우 주사침 말단의 위치가 감소할수록, 세척액 주입 속도가 증가할수록 치근단 압력은 유의하게 증가하였다(p<0.05). 주사침 말단의 위치와 세척액 주입 속도가 동일한 조건 하에서 side-vented 주사침이 가장 낮은 치근단 압력을 보였고, notched, flat 주사침 순서로 치근단 압력이 유의하게 증가하였다(p<0.05). 주사침 말단의 위치가 1 mm인 군 또는 세척액 주입 속도가 0.1 ml/s 이상인 군에서는 나머지 조건에 관계없이 모든 경우에서 중심정맥압(5.88mmHg)보다 높은 치근단 압력을 나타냈다. Flat 주사침은 세척액 주입 속도와 주사침 말단의 위치에 따른 치근단 압력의 급격한 증가로 인해 임상에서 사용이 추천되지 않으며, 안전하고 효율적인 근관 세척을 수행하기 위해서는 근단 협착부로부터 치관 방향으로 3 mm 떨어진 지점에 주사침 말단을 위치시키고 0.05 ml/s 이하의 세척액 주입 속도로 근관 세척액을 적용해야 할 것이다.

The effect of needle tip position on the analgesic efficacy of pulsed radiofrequency treatment in patients with chronic lumbar radicular pain: a retrospective observational study

  • Background: Pulsed radiofrequency (PRF) is a treatment modality that alleviates radicular pain by intermittently applying high-frequency currents adjacent to the dorsal root ganglion. There has been no comparative study on analgesic effect according to the position of the needle tip in PRF treatment. The objective of this study is to evaluate the clinical outcomes of PRF according to the needle tip position. Methods: Patients were classified into 2 groups (group IP [group inside of pedicle] and group OP [group outside of pedicle]) based on needle tip position in the anteroposterior view of fluoroscopy. In the anteroposterior view, the needle tip was advanced medially further than the lateral aspect of the corresponding pedicle in group IP; however, in group OP, the needle tip was not advanced. The treatment outcomes and pain scores were evaluated at 4, 8, and 12 weeks after applying PRF. Results: At 4, 8, and 12 weeks, there were no significant differences between the successful response rate and numerical rating scale score ratio. Conclusions: The analgesic efficacy of PRF treatment did not differ with the needle tip position.

주사전자현미경(SEM)을 이용한 일회용침 끝의 미세 형태에 대한 관찰(2) (Investigation of Micromorphological Characteristics of Acupuncture Needle Tip Using SEM in Korea(2))

  • Objective: There have not been so many study about the quality of acupuncture needle tip. In order to ensure safe acupuncture treatment, the quality of needle tip is essential. Therefore we investigated the current condition of the tip of the acupuncture needles in Korea. Methods: We have selected the needles that made by 8 companies in Korea, and selected 50 pieces from 1000 pieces each company by randomized methods. and observed the tip of each needle using a scanning electron microscope at ${\times}800$ magnification. Results & Discussion: We found that needles had several defects such as scratch marks on the surface, metallic scuff, lumps and irregularities of the needle tips, stubbed or malformed tips, tips of point off-center, peeled off coated tips. There was much difference on the quality of needle among the manufacturer, and some needles seem to need to do- thorough quality control. It is necessary to intensify quality management and a concentrated control on manufacturing process of acupuncture needles.

  • Objectives : In order to ensure safe Herbal Acupuncture treatment, the quality of needle tip is much important. Therefore we investigated micromorphological characteristics of Herbal Acupuncture needle tip according to the number of injection. Methods : We have selected the 26 Gage syringes & 29 Gage syringes that using currently in Herbal Acupuncture treatment. And we evaluated the change of shape of Herbal Acupuncture needle tip after several times of injection by Inverted System Microscope(x 200). Results & Conclusion : We found at Herbal Acupuncture needle tips had several defects such as hooked, scuffed, lump and irregular, stubbed or malformed shape in spite of new syringes. And we found at Herbal Acupuncture needle tips had several defects that is harmful to body such as mild pain, possibility of damage of tissue after several times of injection. Therefore it is necessary to pay attention to use Herbal Acupuncture needle tips.

약침용 불량 Needle의 미세 형태 관찰 (Observation of Fine Structures of Defective Herbal Acupuncture Needles)

  • Objectives : This study was conducted to observe the tip of needles which induce poor needle sensation upon injection of herbal acupuncture. Methods : Among the syringes used for herbal acupuncture, needles which cause poor sensation were collected and evaluated by SEM (scanning electron microscope) for changes in structure, existence of impure matters, physical damage, and etc. Results : 1. Various forms contributed to poor needle sensation. Bent needle body, damaged or crooked needle tip, and blunted tip 2. Poor needles could be made during the manufacturing process, during insertion of the needle into the vial(except for the lubricants), and repeated procedures with one needle at different regions. Conclusion : Poor needles induce unnecessary pain and discomfort not only to the patients but also to administrators during injection. Herbal acupuncture is a treatment technique injecting herbal extract on the meridian points, and strict quality check must be enforced to prevent occurrence of poor needles.

Effect of needle type on intravascular injection in transforaminal epidural injection: a meta-analysis

  • Background: Lumbosacral transforaminal epidural injection (TFEI) is an effective treatment for spinal disease. However, TFEI may have several types of complications, some of which can be attributed to intravascular injection. We reviewed studies to compare the intravascular injection rate among different needle types. Methods: We searched the literature for articles on the intravascular injection rate among different needle types used in TFEI. The search was performed using PubMed, MEDLINE, the Cochrane Library, EMBASE, and Web of Science. Results: A total of six studies comprising 2359 patients were identified. Compared with the Quincke needle, the Whitacre needle reduced the intravascular injection rate (OR = 0.57, 95% CI = [0.44-0.73], P < 0.001). However, compared with the Quincke needle, the Chiba needle did not reduce the intravascular injection rate (OR = 0.80, 95% CI = [0.44-1.45], P = 0.46). In one study, the intravascular injection rate using a blunt-tip needle was lower than that using a sharp needle. In another study, the Whitacre and the blunt-tip needle have similar intravascular injection rates, while, the catheter-extension needle showed a reduced intravascular injection rate. Conclusions: This meta-analysis showed that the Whitacre needle reduced the intravascular injection rate as compared with the Quincke needle, but failed to establish that the Chiba needle can decrease the intravascular injection rate in TFEI. Moreover, the blunt-tip needle can reduce the intravascular injection rate compared with the Quincke needle, and the catheter-extension needle can reduce the intravascular injection rate compared with the Whitacre and the blunt-tip needle.

유럽에서 유통되는 일회용침 끝의 미세 형태에 대한 관찰 (Investigation of Micromorphological Characteristics of Acupuncture Needle Tip Sold in Europe)

  • Objectives : In order to ensure safe acupuncture treatment, the quality of the needle tip is essential. But, there have not been so many studies about the quality of the acupuncture needle tip. For this reason we have been already reported about the quality of acupuncture needle tip in Korea using scanning electron microscope(SEM) in 2002, 2003. In order to compare than other society, we investigated the current condition of the tips of the acupuncture needles sold in Europe. Methods : We obtained the needles made by 7 companies, which are sold currently in Europe, and selected 50 pieces out of 100 pieces from each company by randomized methods. And then we observed the tip of each needle using a scanning electron microscope at ${\times}800$ magnification. Results and Discussion : We found that needles had several defects such as scratch marks on the surface, metallic scuff, lumps and irregularities of the needle tips, stubbed or malformed tips, tips of point off-center, peeled off coated tips, same as Korean needles. There was much difference on the quality of needles among the manufacturers, and some needles seem to need thorough quality control. Allowing for the high price, the quality of some needle in Europe generally are better than that of Korean needles. But some Korean needles hold a top position than European ones in quality. We want a good industrial standard to be made in acupuncture manufacturing fields in the near future, because the safety is not less valuable than the efficacy in medicine.

  • Background and Purpose: There has been little study about the quality of acupuncture needle tips. In order to ensure safe acupuncture treatment, high quality of needle tips is essential. Therefore, we investigated the current condition of the tips of the acupuncture needles on the market. Methods: We selected needles made by 6 companies in Korea, one company each in Japan, the USA and China, and observed the tip of each needle using a scanning electron microscope. Results and Discussion: We found that needles had several defects such as scratch marks on the surface, metallic scuff, lumps and irregularities of the needle tips, stubbed or malformed tips, tips of point off-center, and peeled off coated tips. There were more faulty ones than good ones. Further, better-designed study with a bigger sample size is warranted.

  • In this paper, we investigated the effects of gap length and tip radius influenced in breakdown of mineral based insulation oil Electrode system was needle-plane geometry It is to model conductive extrusions in oil filled electrical power apparatus. The tip radius of needle electrode was 5, 10, 20 and 25${\mu}{\textrm}{m}$, respectively. We measured breakdown voltage for each of tip radius with increasing electrode gap, 2mm to 12mm. It was calculated electrical breakdown strength at tip using Mason\`s equation from breakdown voltage As gap lenght increased. breakdown strength increased linearly. But, as tip radius of needle increased, breakdown strength decreased exponentially. It can be explained by tole phenomenon that electron is easily injected, as tip radius increases, and effective work function decreases. When appling DC voltage. breakdown 7tr7ilgtll was higher wheal polarity of needle was negative than positive. It is because of the space charge effect ill accordance with the influence of liquid motion.

  • In this paper, we investigated the influence of gap length and tip radius on breakdown of mineral based insulation oil. Applied voltages were DC and AC voltage. Electrode system was needle-plane structure. The tip radius of needle electrode was 5, 10, 20 and 25${\mu}m$, respectively. We measured breakdown voltage for each of tip radius with increasing electrode gap, 2mm to 12mm. Electric breakdown strength at tip was calculated using Mason's equation contained geometric figure. As gap length increased, breakdown strength increased linearly. But, as tip radius of needle increased, breakdown strength decreased exponentially. It can be explained by the phenomenon that electron is easily injected, as tip radius increases, and effective work function decreases. When appling DC voltage, breakdown strength was higher when polarity of needle was negative than positive. It is because of the space charge effect in accordance with the influence of liquid motion.
