미세전류치료기 전극 종류에 따른 효능 비교 (Efficacy of Microcurrent Electrical Neuromuscular Stimulation with Different Types of Stimulating Electrodes)

  • Objectives The purpose of this study was to evaluate the most effective stimulating electrode in Microcurrent electrical neuromuscular stimulation (MENS) for pain relief, and to apply to the treatment of Delayed onset muscle soreness (DOMS). Methods We included 45 participants who met the inclusion criteria. DOMS of both Triceps Surae Muscles were experimentally induced through eccentric contractions. 24-hours after induction of DOMS, who scored more than 40 mm in visual analogue scale (VAS) were randomly assigned into three groups (n=44). In 15 of them, needle electrodes were inserted into BL57, BL56. In 15 of them, surface electrodes were applied on the same points and the others were treated with manual acupuncture. The effects of pain relief were assessed by visual analogue scale (VAS), mechanical pain threshold (MPT), surface electromyography (SEMG). Results In VAS, group comparison had no significant difference after all treatments had done. The difference in VAS between the time before the treatment was started (Day 2) and after all treatments were completed was greater in Acupuncture group than that of needle-electrode group. In MPT, there was no significant difference among groups. Electrical contraction decreased significantly at 2nd before-after treatment comparison in needle-electrode MENS group. But fatigue scores were not significantly different between groups. Conclusions The results suggest that the types of electrodes has no significant effect on microcurrent therapy in DOMS.

A case of canine patent ductus arteriosus showed favorable therapeutic response by surgery combined with needle-acupuncture therapy

  • A 6-month-old male Poodle was referred with chief complaint of dysponea and cough. Cyanosis was not detected. This dog was diagnosed as a case of canine patent ductus arteriosus (PDA) by X-ray and ultrasonography. Diuretics and bronchodialators were administered for 4 weeks. Clinical symptoms were not improved. Thoracotomy was done at right recumbent position. The length of ductus arteriosus was 8 mm and the diameter was 6 mm, respectively. Double ligation was performed in surgery. Continuous cardiac murmur, cough and strong femoral pulsation were disappeared after surgery. The diameters of the pulmonary artery and vein found to normal by X-ray on 10 days after operation. The diameter of the aorta measured to be larger than that of the main pulmonary artery by ultrasonography. The fractional shortening (FS) on 10 days after surgery was 17.32% by ultrasonography. The needle-acupuncture therapy was used in acupoints BL-15 as local point plus PC-6, HT-7 and HT-5 as distant points for 3 days (15 min/once/a day). The FSs were increased to 21.31% and 29.44% at 1 and on 3 days after acupuncture, respectively. The present patient was a case of typical canine PDA that showed favorable therapeutic responses by surgery combined with needle-acupuncture treatment.

Application of Modified Mupit for the Recurrent Vulva Cancer in Brachytherapy

  • 재발한 vulva tumor의 근접 치료에 있어서 정상조직의 장애와 tumor volume내의 dose uniformity는 치료성적에 매우 중요한 요인이다. 이를 개선하기 위하여 modified MUPIT applicator를 제작하여 modified MUPIT applicator의 적용에 대한 유용성을 평가하고자 한다. modified MUPIT applicator는 template, cylinder, interstitial needle로 구성되었으며, tumor volume을 정하기 위하여 CT를 시행하였다. CT image를 이용하여 interstitial needle의 삽입 위치를 확인하고 수술실에서 template 를 치료 부위에 고정을 시키고 cylinder를 vaginal cavity에 삽입한 후 interstitial needle을 tumor volume 내에 삽입 하였다. tumor volume내에서 interstitial needle의 정확한 위치를 확인하기 위하여 CT를 시행하였으며 orthogonal film을 이용하여 computer planning을 실시하였다. daily tumor dose는 600 cGy, BID로 3000 cGy를 조사하였으며 치료 시 rectal dose를 평가하기 위하여 TLD를 이용하여 anal verge를 기준으로 5개 지점에서 rectal dose를 측정하였다. rectal dose는 34.1 cGy, 57.1 cGy, 103.8 cGy, 162.7 cGy, 165.7 cGy로 측정되었으며 EBRT(whole pelvis RT), ICR과 overlap되는 지점은 34.1 cGy, 57.1 cGy로 매우 우수하게 평가되었다. 결론적으로 자체 제작한 modified MUPIT applicator 사용하여 interstitial brachytherapy를 시행함으로써 EBRT로 cover하기 어려운 환자의 tumor volume내에서 irregularity를 효율적으로 극복할 수 있었고 우수한 rectal dose 분포를 통하여 rectal complication의 발생 확률을 현저히 감소시킬 수 있었다.

정혈 지열감도 측정법의 객관화 -기기 개발 및 시험 측정 결과 보고- (Instrumentation of Thermo-sensitivity Test of Well Points, a Pilot Report)

  • Objectives : Although the thermo-sensitivity test in Well points in 12 Meridians(井穴知熱感度測定法) has been used for several decades, it is still performed by a manual way, and lack in objectivity consequently. To make it more objective, accurate and convenient test, we have developed a novel system to test thermo-sensitivity in Well points. Methods : To enhance the blindness of the test, we made a shielded system which give heat to some fingers of subjects in random way. To perform the test in quantitative manner, we developed a fully automated, computer-controlled system composed of heating bulbs and response button array. Results : The developed system showed linearity in heating the finger phantom. It also gave acceptable but interesting features in pilot tests with several young adult subjects. Conclusions : By the developed system, we could improve objectivity and convenience of the thermo-sensitivity test in Well points.

견비통에 침치료를 시행한 무작위대조군연구(RCT)들에 대한 계통적 연구 (A Systematic Review of Randomized Controlled Trials on Acupuncture Treatment for Shoulder Pain)

  • Objectives : To review RCTs on acupuncture treatment for shoulder pain in order to establish a standard acupuncture treatment model in treating shoulder pain. Methods : RCT articles on traditional acupuncture treatment for shoulder pain were searched through online database. Quality of studies were assessed using the FEAS and the modified Jadad score. Results : Eighteen trials of acupuncture for shoulder pain were analyzed. Based on the results of these reviews the following factors might contribute to optimal results from acupuncture treatment. 1) Usage of LI, SI, TE meridians, usage of $LI_{15}$, $TE_{14}$, $GB_{21}$, $LI_{11}$, $LI_4$, $SI_{14}$, $LI_{14}$, $TE_{15}$ acupuncture points. 2) More than four acupuncture points should be used. 3) More than 15 minutes of needle retention time. 4) Needle length-40mm and diameter-0.30mm. 5) More than 9 times treatment 6) More than 5 weeks treatment duration. Conclusions : There was no relation between quality of article and effectiveness of acupuncture. To improve the remedial value, it is necessary to mention De-qi, stimulation of acupuncture and correct variation in diagnosis with the above-mentioned. It is better that clinical trials of acupuncture treatment is designed that type of RCT and double blind. Also when it is set that sham nonpenetrating acupuncture, no treatment group as a control group, and participants don't distinguish wheather acupuncture treatment or not, it will be more meaningful.

안면견갑상완형(Fascioscapulohumeral:FSH) 근이영양증 1례에 대한 증례보고 (A Clinical Study on the case of Fascioscapulohumeral (FSH) Muscular Dystrophy Treated with Traditional Korean Medicine)

  • Objective : There was no report on the treatment of Fascioscapulohumeral (FSH) Muscular Dystrophy by Oriental medicine. But the treatment conducted on the patient admitted to the Sangji Oriental Medicine Hospital from January 9, 2001 to February 23, 2001, a significant treatment result was yielded and would like to suggest treatment plan for the future treatments. Methods : Under the assumption that Korean Bee-Venom Therapy may be affective for treating FSH Muscular Dystrophy, the following points were administered : SI10(노유), SI11(天宗), BL23(腎兪), BL26(關元兪), ST36(足三里), LI4(合谷), Liv3(太衝), SI9(肩貞). CFC(Carthami Flos;紅花 and Cervi Pantotrichum Cornu;鹿茸) herbal extract was treated on the other acupuncture points. Sa-Am(Four needle technique) Acupuncture (tonifying SI5 and ST42, sedating GB41 and ST43) was done every day. For herbal medicine, TaeEumIn ChoWiSeungChung-Tang was given based on the constitutional diagnosis. Results : After 7 weeks of treatment, a remarkable improvement was made for facial muscular movement and muscular strength of the scapular and another regions. Conclusions : 1. Significant improvement in the muscular strength for the case of FSH Muscular Dystrophy was obtained with through Korean Bee-Venom Therapy, Four needle technique, and herbal medication. 2. For progressive muscular dystrophy, it is necessary to practice muscular strength recovery exercise in conjunction with Korean Bee-Venom Therapy. 3. Although this case yielded favorable result, further observation and study must be made to concretely prove the effectiveness of Korean Bee-Venom Therapy for treating muscular dystrophies.

성장장애(成長障碍)의 침구치료(鍼灸治療)에 관(關)한 고찰(考察) (The Study on Acupuncture and Moxibustion Treatment of Delayed Growth)

  • Objectives : The objective of this study was to research delayed growth with acupuncture and moxibustion treatment. Methods : We search the oriental medical literature related to delayed growth, especially loose skull, pigeon chest(龜胸), turtle back(龜背), five kinds of flaccidity(五軟), five kinds of retardations(五遲) and infantile malnutrition(疳證). Results : 1. Loose skull is treated with moxibustion therapy of CV8(神厥) and two points(1.5cm upper and under of CV8) 2. Pigeon chest(龜胸) is treated with moxibustion therapy of GB38(外丘), ST18(乳根) and 6 points around of STl7(乳中). 3. Turtle back(龜背) is treated with moxibustion therapy of BLl3(肺兪), BL15(心兪) and BLl7(膈兪) 4. Acupuncture therapy of five kinds of flaccidity(五軟) was rare. but there are one case to stimulate Hwatahyeopcheok point using plum-blossom needle. 5. Five kinds of retardations(五遲) is treated with moxibustion therapy of BLl5(心兪) and two point of medial malleolus 6. Infantile malnutrition(疳證) is treated with acupuncture therapy of the spleen channel and stomach. channel, therapy using three-edged needle, cutting therapy(LUlO(魚際) and Sabong), moxibustion therapy(LRl3(章門) and BL2l(胃兪), and Ch'una therapy. Conclusion: We expect that acupuncture and moxibustion treatment of delayed growth will be applied practically in clinical medicine due to further study on delayed growth.

수술후 오심구토에 대한 이침요법의 임상적 연구 (Effect of Auricular acupuncture on Postoperative Nausea and Vomiting)

  • 본 논문은 수술후 오심, 구토증에 대한 이침요법의 효과를 연구하기 위하여 시도되었다. 경희대학교 의과대학 부속병원에서 전신마취하에 복식 자궁절제술을 받기로 예정된 100명의 환자를 두 군으로 나누어 이침요법을 하지 않은 대조군과 이침요법을 시행한 실험군을 대상으로 연구를 하였다. 대상환자의 연령, 체중, 신장, 마취시간은 두 군간에 통계적으로 유의성있는 차이는 없었다. 수술후 12시간이내에 한번이라도 오심, 구역 및 구토가 발생된 환자는 대조군이 68%, 실험군이 30%으로 실험군이 대조군에 비하여 유의성있는 효과를 보였다. 이침요법에 의한 부작용은 나타나지 않았다. 이침요법은 복식 자궁절제술 후 오심, 구토를 부작용없이 예방하는데 상당한 효과가 있는 것으로 사려된다.

탈발(脫髮)의 병인병기(病因病機) 및 침구치료(鍼灸治療)에 관한 문헌고찰 (A Philological Study on the Pathology, Acupuncture and Moxibustion of Alopecia)

  • Objectives : The purpose of this study is to analyze literature on oriental medicine to investigate the pathology, acupuncture, and moxibustion therapy applicable to alopecia Methods : We searched for the causes of alopecia in the bibliographic data from 41 sources of literature and the acupunctural treatments for alopecia in the bibliographic data from 49 sources of Chinese and Korean literature and from the journals of the acupunctural treatments for alopecia which have been published since 2000. Results : 1. The main causes of alopecia are blood deficiency, blood heat, static blood, depletion of kidney's water, wind excess, seven emotional damage, dietary damage, etc. 2. The acupuncture points frequently used are $GV_{20}$, $GB_{20}$, $ST_{36}$, $SP_6$, $SP_{10}$, $BL_{17}$, $BL_{18}$, $BL_{23}$, $GV_{23}$, $GV_{14}$, $CV_6$, $ST_8$, $KI_3$, $LR_3$, $HT_7$, $LI_{11}$, EX-$HN_1$, and ouch point. 3. New acupuncture therapies, including herbal acupuncture therapy, ear acupuncture therapy, dermal needle therapy, needle-embedding therapy and scalp acupuncture therapy are frequently used to treat alopecia. Conclusions : The acupuncture points frequently used for treating alopecia have characteristics such as tonifying blood, tonifying the liver and kidney, extinguishing wind, activating blood, clearing heat and draining dampness, so they are highly associated with the causes and the mechanisms of alopecia.

  • 1. Objectives : Millions of people throuout the world are affected by some form of dry eye disorder. I made researches for more effective treatments for dry eye. 2. Methodes : I refered to occidental and oriental medical records. 3. The results were as follows : Common symptoms of dry eye are dryness, burning, irritation, grittiness, itching, fatigue of eye, photophobia, congestion, mattering and tear. Treatments of dry eye are to clear away heat and fire, remove dampness, replenish Um and promote production of body fluid, tonify the blood and replenish Ki. Acupunctre and moxibustion therapy of dry eye are as follows: Very busy points of traditional regular acupuncture are Sangsong, Chanjuk, Sajukkong, Chongmyong, Sabaek, Tongjaryo, Taeyang, Pungji, Tuimup, Paekoe, Un-gyo, Chonjong, Hapkok, Yangbaek, Kansu, Yanggye, Imup, Chok-samni, Taechung, Kwangmyong, Yang-gok, Uihui, Chohae, Haenggan. Less busy points of it are Oyo, Kuhu, Sungup, Konmyong, Konmyongl, Konmyong2, Sangmyong, SanghaChongmyong, Shinmyong, Osang, Shinjong, Yonghyang, Yaemyong, Chon-yu, Chon-ju, Kwallye, Naebi, Noeho, Tuyu, Mokchang, Ponshin, Shinhoe, Yepung, Okchim, Pungbu, Kokchon, Kollyun, Nae-gwan, Tae-nung, Samumgyo, Sokolkong, Shinsu, Um-gyo, Igan, Chongok, Choktaek, Kyonjungsu, Kokchi, Kongchoe, Kyuum, Kihae, Taekolkong, Taedon, Pino, Pisu, Sabong, Samgan, Kokchi, Shinmun, Shinmaek, Shimsu, Yangno, Aengmun, Yolgyol, Oegwan, Wijung, Chang-gan, Chungjo, Chungdo, Chigu, Chium, Chollyo, Tongni, Pungmun, Haryom.
