여자고등학생의 성형수술 욕구에 영향을 주는 요인 (Factors Influencing Need for Cosmetic Surgery among High School Girls)

  • Purpose: The increase of cosmetic surgery procedures is partly motivated by more positive attitude toward cosmetic surgery as a means of improving physical appearance. In this study factors expected to predict need for cosmetic surgery were examined. Methods: A descriptive design was used with 321 high school girls who completed a questionnaire about sociocultural attitudes toward appearance, body image, self-esteem, and need for cosmetic surgery. Stepwise multiple regression analysis was performed to examine factors affecting need for cosmetic surgery. Results: Bivariate correlations showed that need for cosmetic surgery was positively correlated with internalization and awareness of sociocultural attitudes toward appearance, and appearance orientation in body image and negatively correlated with self-esteem and ideal body mass index (BMI). Multiple regression showed that the predictive variables explained 22.1% in need for cosmetic surgery. The significant factors influencing need for cosmetic surgery were greater internalization and awareness of sociocultural attitudes toward appearance, better appearance evaluation and higher appearance orientation in body image, lower self-esteem, and lower ideal BMI. Conclusion: Overall, results of this research suggest that prevention of media influence and body image disturbances, and promotion of self-esteem are very important to young women.

여대생의 미용성형 경험 유무에 따라 미용성형욕구에 영향을 미치는 요인 (Predictors of Wish to Undergo Cosmetic Surgery according to Experience of Cosmetic Surgery in Female College Students)

  • 본 연구는 여대생을 대상으로 미용성형 비경험군과 미용성형 경험군 각각의 미용성형욕구에 영향을 미치는 주요변수를 검증하기 위한 서술적 조사연구이다. 연구 목적은 미용성형 비경험군과 경험군의 미용성형욕구에 영향을 주는 변수를 검증하여 영향을 미치는 요인에 대한 올바른 이해를 증진시키고, 미용성형을 추구하는 여대생을 상담하고 지도하기 위한 기초자료를 제공하기 위하여 시행되었다. 연구대상자는 254명으로 K도에 소재하고 있는 일개 대학의 여대생으로 편의표출에 의하여 선정되었다. 254명의 여학생 중 성형경험이 없는 비경험군 여학생이 142명(55.9%), 성형경험이 있는 경험군 여학생은 112명(44.1%)이었다. 미용성형 비경험군과 미용성형 경험군의 자아존중감, 신체상(외모지향, 외모평가, 신체만족도), 성형수술태도(성형을 받아들이는 정도, 사회적 동기, 개인내적인 동기) 및 미용성형욕구를 비교한 결과 미용성형 경험군이 비경험군보다 성형을 받아들이는 정도가 높았고, 사회적동기도 더 높았으나, 미용성형욕구는 미용성형 비경험군이 경험군보다 더 높았다. 미용성형 비경험군과 미용성형 경험군 각각의 미용성형욕구에 영향을 미치는 변수를 확인하기 위하여 단계적 회귀분석을 하였다. 미용성형 비경험군을 분석한 결과 성형태도 중 성형을 받아들이는 정도(${\beta}$=.46, p<.001)와 신체이미지 중 신체만족도(${\beta}$=-.23, p<.05)가 영향을 주는 변수이었으며, 미용성형욕구를 23.0% 설명하였다. 미용성형 경험군에서 영향을 미치는 변수는 성형태도 중 성형을 받아들이는 정도(${\beta}$=.47, p<.001)로서 미용성형욕구를 20.0% 설명하였다. 이와 같이 성형 경험 유무에 따라 미용성형 욕구에 영향을 미치는 요인이 다르므로 미용성형을 추구하는 여대생을 상담 시 성형 경험 유무에 따라 다른 접근 방법을 사용하는 것이 효과적일 것임을 시사하고 있다.

소셜커머스를 이용한 미용성형시술의 인식 및 만족도에 관한 연구 (A Study on the Recognition and Satisfaction of Aesthetic Plastic Surgery using Social Commerce)

  • 본 연구는 뷰티 소셜커머스 이용자 530명 중 미용성형시술 쿠폰 구매 경험이 있는 244명을 대상으로 설문조사를 하여 미용성형시술 인식이 만족도 및 재구매에 미치는 영향을 분석하였다. 미용성형 시술 쿠폰 구매 경험은 46.0%로 낮았고 구매하지 않는 이유는 정보부족, 업체에 대한 불신이었고 이용 고객의 불만족 이유는 기대효과이었으며 20대가 정보성에 낮은 만족도를 나타냈다. 미용성형시술 이용 요인 중 편리성, 정보성이 만족도, 재구매에 정(+)의 영향을 미치는 것으로 나타났으며(p<0.001) 편리성을 중요한 요인으로 인식하였다. 따라서 소셜커머스 업체 및 미용 의료기관의 고객 응대 서비스 및 사이트 이용 방법, 객관적인 정보 제공 등 방면의 개선이 필요할 것으로 사료된다.

Facial asymmetry: a case report of localized linear scleroderma patient with muscular strain and spasm

  • Facial asymmetry is found in patients with or without cosmetic facial alterations. Some patients have facial asymmetry that manifests underlying skeletal problems, while others have only limited soft-tissue facial asymmetry. Orthognathic surgery brings about a dermatic change, as soft tissue covers underlying bones. Limited soft-tissue asymmetry, meanwhile, is difficult to correct. The treatment modalities for the creation or restoration of an esthetically pleasing appearance were autogenous fat grafts, cartilage graft, and silicon injections. A young female patient had right-side facial asymmetry. The clinical assessment involved visual inspection of the face and palpation to differentiate soft tissue and bone. Although the extra-oral examination found facial asymmetry with skin atrophy, the radiographic findings revealed no mandibular atrophy or deviation. She was diagnosed as localized scleroderma with muscle spasm. In conclusion, facial asymmetry patients with skeletal asymmetry can be esthetically satisfied by orthognathic surgery; however, facial atrophy patients with skin or subdermal tissue contraction need treatment by cosmetic dermatological surgery and orthodontic correction.

기존의 창상봉합과 Histoacryl Blue®를 이용한 창상봉합의 비교 분석: 전향적 무작위 임상실험 (Comparing Conventional Suture Method Versus Wound Closure Using Tissue Glue(Histoacryl Blue®): a Prospective Randomized Clinical Trial)

  • Tissue adhesives have gained popularity for quicker and painless closure of lacerations. The use of tissue glue is currently popular for the closure of superficial lacerations, especially in children. Histoacryl $Blue^{(R)}$(2-N-butylcyanoacrylate) is a topical wound closure that precludes the need for foreign bodies to close wounds. The purpose of this study was to compare the applications of Histoacryl $Blue^{(R)}$(HAB) and conventional suture, regarding cosmetic outcome. To compare the short term and long term results of various repair methods, we designed the prospective, randomized, blind study. Patients with laceration undergoing repair were randomly allocated to conventional suture, subcutaneous suture plus HAB, and HAB only groups. The exclusion criterions were large wound that require large tension for repair or avulsion wound. An independent, blinded observer assessed cosmetic result at 7-10 days after repair and 3-9 months postoperatively. Physician's satisfaction with wound appearance was recorded on 100 mm Visual Analogue Scale(VAS)(0=worst, 100=best). The difference in VAS score between conventional suture method and subcutaneous suture plus HAB methods were not significant. Tissue glue being easy to use with no complications and still resulting in equivalent cosmetic outcomes has several benefits. Especially in the case of children, the wound closure with Histoacryl $Blue^{(R)}$ could be a good alternative for repair of laceration in emergency room.

부이주에서 진피전환피판술을 활용한 새로운 치료법 (A Dermal Turnover Flap for Treating the Accessory Tragus)

  • Purpose: Accessory tragus is a fairly common congenital malformation and usually located at pretragal area. Surgical removal is a common treatment of accessory tragus irrespective of location and morphology. Most accessory tragi do not have depression site around them, but some do. So in those cases, simple surgical excision was not enough to promote the aesthetic facial appearance. For depression site remodeling, the excess amount of skin and cartilage need to be remained partially instead of total excision. This method can achieve the symmetric contour of pretragal area. The authors excised the epidermis and cartilaginous tissue totally and remained the dermis for reconstruction of the depression site around accessory tragus. The depression site is filled with dermal turnover flap. The purpose of this report is to present new idea to promote cosmetic result in treatment of accessory tragus containing the depression site. Methods: Two patients had a pair of accessory tragi at pretragal area. One was a common featured accessory tragus, but the other was different. Depression site was found around accessory tragus. After epidermis and cartilaginous tissue were removed from it, dermis component was used as turnover flap for reconstruction of depression site. Results: After accessory tragus was removed and depression site was reconstructed, facial contour and cosmetic result was achieved. Complication such as flap necrosis and wound dehiscence was not observed. Conclusion: The accessory tragus has variant morphology and degree of invasive depth. And some has a depression site around them. In those cases, simple surgical removal results in morphological distorsion and do not promote facial symmetry. The authors suggest dermal turnover flap as reconstruction method of the depression site. This method improves both surgical outcome and cosmetic result.

소아 장중첩증에 대한 복강경 수술의 경험 (Laparoscopic Surgery for Intussusception in Children)

  • Intussusception is common cause of intestinal obstruction in children. Most of intussusceptions can be treated with non-operative reduction using air or barium. However, about 10% patients need operative treatment due to failure of reduction, peritonitis, and recurrence after reduction. We introduce our experience of laparoscopic surgery for intussusception. From April 2010 to March 2013, we reviewed 57 children who diagnosed intussusception. Twelve patients underwent an operation. The cause of operation was 7 of failure of air reduction and 5 of recurrence after air reduction. Median age was 21.5 months (range: 5.0~57.7 months) and 11 children (91.7%) underwent successful laparoscopic reduction. Median operating time was 50 minutes (range: 30~20 minutes) and median hospital days was 4.5 days (range: 3~8 days). One patient had a leading point as a heterotopic pancreas and underwent bowel resection through conversion. There was neither intra-operative nor postoperative complication. Laparoscopic reduction for intussusception can bring an excellent cosmetic effect with high success rate.

관골에 발생한 혈관종의 수술적 절제술 (Intraosseous Hemangioma of the Zygoma and Its Management)

  • Purpose: Intraosseous hemangiomas are rare and account for fewer than 1% of all bone tumors. The site that is most commonly involved are the vertebral column and the skull. Within the facial skeleton, hemangiomas can occur in the mandible, maxilla, the nasal bones, and rarely the zygoma. Methods: We report a case of an intraosseous hemangioma of the zygoma in a 49-year-old male. The patient had a slow growing hard mass in the left zygoma, which had been present for 8 years. Other than the cosmetic deformity, the patient experienced no pain and did not have any problem. He had no history of trauma in that area and no ocular symptoms. Preoperative computed tomography showed a trabeculated mass arising from the body of the left zygoma. The mass was surgically removed without having to reconstruct the bone defect by spairing the inner cortex. Results: Histopatholgical examination indicated a cavernous hemangioma. After 4 months of follow up, no functional and cosmetic impairment was identified. The patient was satisfied with the result. Conclusion: An intraosseous hemangioma of the zygoma can be treated with total surgical excision with preservation of the inner cortex, thus eliminating the need for reconstruction of bone defect.

Liposuction in the Treatment of Lipedema: A Longitudinal Study

  • Background Lipedema is a condition consisting of painful bilateral increases in subcutaneous fat and interstitial fluid in the limbs with secondary lymphedema and fibrosis during later stages. Combined decongestive therapy (CDT) is the standard of care in most countries. Since the introduction of tumescent technique, liposuction has been used as a surgical treatment option. The aim of this study was to determine the outcome of liposuction used as treatment for lipedema. Methods Twenty-five patients who received 72 liposuction procedures for the treatment of lipedema completed a standardized questionnaire. Lipedema-associated complaints and the need for CDT were assessed for the preoperative period and during 2 separate postoperative follow-ups using a visual analog scale and a composite CDT score. The mean follow-up times for the first postoperative follow-up and the second postoperative follow-up were 16 months and 37 months, respectively. Results Patients showed significant reductions in spontaneous pain, sensitivity to pressure, feeling of tension, bruising, cosmetic impairment, and general impairment to quality of life from the preoperative period to the first postoperative follow-up, and these results remained consistent until the second postoperative follow-up. A comparison of the preoperative period to the last postoperative follow-up, after 4 patients without full preoperative CDT were excluded from the analysis, indicated that the need for CDT was reduced significantly. An analysis of the different stages of the disease also indicated that better and more sustainable results could be achieved if patients were treated in earlier stages. Conclusions Liposuction is effective in the treatment of lipedema and leads to an improvement in quality of life and a decrease in the need for conservative therapy.

Atrial Septal Defect Closure: Comparison of Vertical Axillary Minithoracotomy and Median Sternotomy

  • Background: This study aims to evaluate whether or not the method of right vertical axillary minithoracotomy (RVAM) is preferable to and as reliable as conventional sternotomy surgery, and also assesses its cosmetic results. Methods: Thirty-three patients (7 males, 26 females) with atrial septal defect were admitted to the Cardiovascular Surgery Clinic of Cukurova University from December 2005 until January 2010. The patients' ages ranged from 3 to 22. Patients who underwent vertical axillary minithracotomy were assigned to group I, and those undergoing conventional sternotomy, to group II. Group I and group II were compared with regard to the preoperative, perioperative and postoperative variables. Group I included 12 females and 4 males with an average age of $16.5{\pm}9.7$. Group II comprised 14 female and 3 male patients with an average age of $18.5{\pm}9.8$ showing similar features and pathologies. The cases were in Class I-II according to the New York Heart Association (NYHA) Classification, and patients with other cardiac and systemic problems were not included in the study. The ratio of the systemic blood flow to the pulmonary blood flow (Qp/Qs) was $1.8{\pm}0.2$. The average pulmonary artery pressure was $35{\pm}10$ mmHg. Following the diagnosis, performing elective surgery was planned. Results: No significant difference was detected in the average time of the patients' extraportal circulation, cross-clamp and surgery (p>0.05). In the early postoperative period of the cases, the duration of mechanical ventilator support, the drainage volume in the first 24 hours, and the hospitalization time in the intensive care unit were similar (p>0.05). Postoperative pains were evaluated together with narcotic analgesics taken intravenously or orally. While 7 cases (43.7%) in group I needed postoperative analgesics, 12 cases (70.6%) in group II needed them. No mortality or major morbidity has occurred in the patients. The incision style and sizes in all of the patients undergoing RVAM were preserved as they were at the beginning. Furthermore, the patients of group I were mobilized more quickly than the patients of group II. The patients of group I were quite pleased with the psychological and cosmetic results. No residual defects have been found in the early postoperative period and after the end of the follow-up periods. All of the patients achieved functional capacity per NYHA. No deformation of breast growth has been detected during 18 months of follow-up for the group I patients, who underwent RVAM. Conclusion: To conclude, the repair of atrial septal defect by RVAM, apart from the limited working zone for the surgeon in these pathologies as compared to sternotomymay be considered in terms of the outcomes, and early and late complications. And this has accounted for less need of analgesics and better cosmetic results in recent years.
