A way of life perceived from the Chinese poem of Nam Hyo-On - Focusing on wound and healing - (남효온(南孝溫)의 한시(漢詩)를 통해 본 삶의 방식 - 상처와 치유를 중심으로 -)

  • This study aims to examine the life of Chugang Nam Hyo-On (1454~1492) from his Chinese poem and to figure out how he overcame and healed his agony and wound. Nam Hyo-On was one of the Saengyuksin who submitted Sureungbokuiso and spent his whole life with alcohol and wandering. Thus, he is known to be a hermit writer or stranger. He had to suffer poverty during his life thanks to his only one choice. He was weak by nature but also feeble mentally as he always worried about death. In his 30s, he regarded himself as an aged man and always worried about death due to his frequent illness. Sometimes, he used to dream of being forever young. He composed self-elegy poem that is about his death, which is the ever first self-elegy poem in our history. He sent it in his letter to his master Kim Jong Jik (1431-1492) in 1489. His agony and motivation of writing such poem can be conjectured from his master's reply and his self-elegy poem. Nam Hyo-On tried to overcome and heal his real discontent and wound from his literary works. And it seems appropriate to regard him that he attempted to get compensation of his limitation and discontent from literature. We sometimes heal our wound by writings (or literature) and by reflecting through such writings. Nam Hyo-On reminded of his beloved person through his poet writing and healed his fear of wound of death.

Nanotechnology in the Surface Treatment of Titanium Implant. (임상가를 위한 특집 2 - 티타늄 임플란트 표면처리에서의 나노테크놀로지)

  • Tissue engineering has been enhanced by advance in biomaterial nature, surface structure and design. In this paper, I report specifically vertically aligned titania ($TiO_2$) nanotube surface structuring for optimization of titanium implants utilizing nanotechnology. The formation, mechanism, characteristics of titania nanotubes are explained and emerging critical role in tissue engineering and regenerative medicine is reviewed. The main focus of this paper is on the unique 3 dimensional tubular shaped nanostructure of titania and its effects on creating epochal impacts on cell behavior. Particularly, I discuss how different cells cultured on titania nanotube are adhered, proliferated, differentiated and showed phenotypic functionality compared to those cultured on flat titanium. As a matter of fact, the presence of titania nanotube surface structuring on titanium for dental applications had an important effect improving the proliferation and mineralization of osteoblasts in vitro, and enhancing the bone bonding strength with rabbit tibia over conventional titanium implants in vivo. The nano-features of titania nanotubular structure are expected to be advantageous in regulating many positive cell and tissue responses for various tissue engineering and regenerative medicine applications.

A Herbological study on the plants of Rhamnaceae in Korea. (한국산(韓國産) 서이목(鼠李目) 식물에 관한 본초학적(本草學的) 연구(硏究))

  • Objectives : The plans can be used for medicinal purposes among Rhamnales in korea and examined their effects and distributions. Methods : The examined herbalogical books and research papers which published at home and abroad. Results : 1. There are up to 10 genera and 35 species in the Rhamnales in Korea, and among them medicinal plants are 9 genera, 17 species, some 45% in total. 2. Rhamnella genus is the main kind that has 9 species among 35 species in the Rhamnales, of which medicinal plants are 4 species. 3. The cortex in the Rhamnales is the main part which is used medicinally. And the number of the species in the Rhamnales which are used medicinally is 16. 4. According to the nature and flavor of medicinal plants in the Rhamnales, they were classified into balance 33 species, and warm 13; sweet taste 28 and bitter taste 24 in the order. 5. According to meridian propism of medicinal plants in the Rhamnales, they were classified into heart meridian 8 species, stomach meridian 7, and liver meridian 6 in the order. 6. According to the properties and principal curative action, they were classified into drugs for detoxicant 30 species. drugs for antifebrile 23, and drugs for drain damp 13 in the order. 7. The number of toxic species in the Rhamnales was examined to be 6 species. Conclusion : There were totaled to 10 genera and 35 species in Rhamnales in Korea and among them medicinal plants are 9 genera, 17species, some 45% in total.

Characterization of Growth Hormone Secretion to Growth Hormone releasing Peptide-2 in Domestic Animals - A Review

  • Growth hormone-releasing peptide-2 (GHRP-2, also named KP102) is a new hexapeptide of a series of synthetic growth hormone-releasing peptides (GHRPs) which stimulates the secretion of growth hormone (GH) in vitro and in vivo in several species including calf, sheep and pig. The GH-releasing activity of GHRP-2 is two to three times more effective than that of the original GHRP-6, and GHRP-1 in the rats and humans. To date, GHRP-2 seems to be the most potent member of the family of GHRPs. Since the GHRPs are short peptides (5-7 amino acid residues), they are synthesized easily and are not as readily degraded in plasma as GHreleasing hormone (GHRH). These features ameliorate their potential on domestic animals because of their chemical nature the GHRPs are efficacious when administered i.v. orally or orally. However, studies in cow, pig and sheep do not indicate such a close relationship between GHRH, somatostatin (SS) and GH, calling into question the general applicability of the human and rat models. Perhaps there is an important role for an endogenous GHRP in the regulation of GH secretion in domestic animals. This review provides an overview on the current knowledge of physiological role of GHRP-2 in domestic animals.

  • Non-thermal atmospheric-pressure plasma, also named cold plasma, is defined as a partly ionized gas. Therefore, it cannot be equated with plasma from blood; it is not biological in nature. Non-thermal atmospheric-pressure plasma is a new innovative approach in medicine not only for the treatment of wounds, but with a wide-range of other applications, as e.g. topical treatment of other skin diseases with microbial involvement or treatment of cancer diseases. This review emphasizes plasma effects on wound healing. Non-thermal atmospheric-pressure plasma can support wound healing by its antiseptic effects, by stimulation of proliferation and migration of wound relating skin cells, by activation or inhibition of integrin receptors on the cell surface or by its pro-angiogenic effect. We summarize the effects of plasma on eukaryotic cells, especially on keratinocytes in terms of viability, proliferation, DNA, adhesion molecules and angiogenesis together with the role of reactive oxygen species and other components of plasma. The outcome of first clinical trials regarding wound healing is pointed out.

A Study of Juhakhae(周學海)'s Three Eum and Three Yang[三陰三陽] Field Theory (주학해(周學海)의 삼음삼양(三陰三陽) 분야설(分野說)에 관한 고찰)

  • Three Eum and Three Yang[三陰三陽] is a way of studying the changing of nature from a Eum(陰) and Yang(陽) point of view, Juhakhae(周學海) divides this into two dimensions. The first is the concept of dividing the exterior part of the body excluding the organ into 6 divisions as Eumyang(陰陽) of front and rear characteristic. The second is the concept of dividing the systematic bio-activity of the central organ into another 6 dimensions. The former concept is illustrated in the body as a certain meridian controlling a certain field of the body, while the latter is presented as a certain organ performing an activity of Gi(氣). Three Eum and Three Yang[三陰三陽] is not based on fixed substances. but it is a thought process model of observing the structural-functional changes of the body in disease state. Therefore, by understanding Three Eum and Three Yang[三陰三陽] of field and activity of Gi(氣) respectively and comprehensively putting these into practice, we will be able to approach the essence of the disease state in a more precise manner.

Mechanisms of immune tolerance to allergens in children

  • Because the prevalence of allergic diseases has significantly increased in recent years, understanding the causes and mechanisms of these disorders is of high importance, and intense investigations are ongoing. Current knowledge pinpoints immune tolerance mechanisms as indispensable for healthy immune response to allergens in daily life. It is evident that development and maintenance of allergen-specific T cell tolerance is of vital importance for a healthy immune response to allergens. Such tolerance can be gained spontaneously by dose-dependent exposures to allergens in nature or by allergen-specific immunotherapy. Allergen-specific immunotherapy induces regulatory T cells with the capacity to secrete interleukin-10 and transforming growth factor-${\beta}$, limits activation of effector cells of allergic inflammation (such as mast cells and basophils), and switches antibody isotype from IgE to the noninflammatory type IgG4. Although allergen-specific immunotherapy is the only method of tolerance induction in allergic individuals, several factors, such as long duration of treatment, compliance problems, and life-threatening side effects, have limited widespread applicability of this immunomodulatory treatment. To overcome these limitations, current research focuses on the introduction of allergens in more efficient and safer ways. Defining the endotypes and phenotypes of allergic diseases might provide the ability to select ideal patients, and novel biomarkers might ensure new custom-tailored therapy modalities.

CONSERVATIVE CARE OF NONUNION OWING TO OSTEOMYELITIS ASSOCIATED WITH FRACTURE OF MANDIBLE;REPORT OF 3 CASES. (하악골절부 골수염에 의한 비유합의 보전적 처치;증례보고)

  • Failure to use effective methods of reduction, fixation, and immobilization may lead to nonunion with osteomyelitis, owing to the compound nature of most fractures of the mandible. Nonunion results in fibrous pseudoarthrosis at the fracture site with instability that, once formed, does not improve spontaneously. Once the nonunion with osteomyelitis secondary to fractures has become established, intermaxillary fixation and drainage of infected tissue should be instituted as early as possible, because the fixation & drainage enhances the patient comfort and hinders ingress of microorganisms & debris by movement of bone fragments. The authors treated three cases of nonunion with osteomyelitis by intermaxillary fixation, incision & persistent drainage on the previous fistula site and endodontic drainage of infected teeth in the fracture site of mandible. The localization & sequestration of the infected bone around the fracture was better performed persistently by natural homeostatic mechanism in $8{\sim}10$ weeks and the bony union was then attained without bone grafting.

Experiences of Admission for Critically Ill Patients in ICU (중환자실 환자의 입원 경험)

  • Purpose: The purpose of this study was to understand the meanings and nature of ICU admission experienced among patients with critical illness. The present study adopted a hermeneutic phenomenological method which was developed by van Manen. Methods: The participants for this study were 6 men and 3 women, who were over the age of 20 with ICU admission period more than 3 days. Data were collected by using in-depth interviews and observations from March, 2007 to September, 2007. The contents of the interviews were tape-recorded with the consent of the subject. Results: The essential themes that fit into the context of the 4 existential grounds of body, time, space and other people were as follows: a body that cannot react the way it wants, a wave of fear and insecurity everywhere, a struggle to survive, coming out from death's door, loss of time path, a long and continued waiting until escaping, more of machinery room than a patient's room, existence of life and death, an abyss of suffering seen thru another patient, taken care of by a doctor, trust and distrust, family, the ultimate safe zone. Conclusion: Critically ill patients in ICU experienced feelings of discomfort, unsafety, and insecurity. The result of this study can give nurses some insight into these experiences and help promote empathetic care.

Neutron dosimetry with a pair of TLDs for the Elekta Precise medical linac and the evaluation of optimum moderator thickness for the conversion of fast to thermal neutrons

  • Introduction: In this study, TLD 600 and TLD 700 pairs were used to measure the neutron dose of Elekta Precise medical linac. To this end, the optimum moderate thickness for the conversion of fast to thermal neutrons were evaluated. Materials and methods: 241Am-Be and 252Cf sources were simulated to calculate the optimum thicknesses of the moderator for the conversion of maximum fast neutrons (FN) into thermal neutrons (TN). Pair TLDs were used to measure F&TN doses for three different field sizes at four depths of the medical linac. Results: The maximum thickness of the moderator was optimized at 6 cm. The measurement results demonstrated that the TN dose increased with the expansion of field size and depth. The FN dose, which was converted TN, exhibits behaviors comparable to the TN due to its nature. Conclusion: This study presents the optimum thickness for the moderator to convert FN into TN and measure F&TN using TLDs.
