건강과 의학의 새로운 흐름 - 담론과 실천 방식에 관한 사회문화적 비평 - (Emerging Currents in Health and Medicine - A Socio-Cultural Critique of Their Discourses and Practices -)

  • We have witnessed several kinds of new discourses and practices in health and medicine since the 1970s, such as popular concerns with alternative or complementary medicine, inordinate attention to the promotion of 'healthy' living, rapid resurrection of traditional medicine and ecological management of health. Four structural and situational factors are discussed to underlie these new trends:(i) as 'crisis' in health care of the 1970s was translated into health care reform of the 1980s backed up by neo-liberal political philosophy, the state responsibility for nation's health is being transferred to the individual ;(ii) it resulted from the limits of biomedical paradigm in dealing with chronic diseases;(iii) medico-scientific knowledge of disease is transformed into the subjective discourses and technologies of health in postmodern society ; and (iv) it is deeply associated with the considerable increase in environmental risk perception of health and disease. There are some inherent countervailing forces in these new discourses and practices. First, while they derive from lifestyle-oriented behavioral change, medicalization of life and death is still consolidated in the new trends. Second, inasmuch as new tides are reliant upon science, they. are likely to be remote from techne that means not the practical application of theoretical knowing but a special form of practical knowing. Third, as new discourses and activities accomplished'in the name of health'increasingly occupy important strategies in forming the self-identity, they serve as moral apparatus which involves prescriptions about how we should live our lives and conduct our bodies, both individually and collectively. Therefore, two points are suggested to consider seriously whether these streams will succeed in improving the‘healthy’living of all the people. Instead of limiting tile perspective to medicine, healing and health care, a new matrix that interweave welfare, ecology and labor along with them is timely needed for enhancing the health for all. In addition, as the World Health Report fm strongly shows, inequality in health heavily depends upon socio-economic development of a society, and it is not the richest countries that have the best health status, but those that have the smallest income differences between rich and poor.

담배가격 인상에 따른 사무직남성근로자들의 흡연행태 변화 (Change of Smoking Behavior by Male White-collar Workers after a Tobacco Price Increase)

  • Objectives: The purpose of this study is to examine the effect of the raise of cigarette prices by KRW 2,000 at the beginning of 2015 on the change in smoking behavior among male office workers, and to analyze the correlation of various factors including their work behaviors and socio-economic factors with their smoking rate. Methods: In this research, a follow-up observation panel was constituted with 420 smokers as targets from among male office workers at a bank located in Daegu, South Korea. A cross-analysis and ANOVA analysis were carried out in order to examine whether changes in smoking status, amount of smoking, stop-smoking motivation, and reasons for smoking cessation failure after the passage of time since the cigarette price hike were statistically significant. The level of statistical significance was P < 0.05. Results: After the cigarette price hike, among the 420 smokers who were the target of the panel the rate of smoking cessation declined at the time-point of the survey to 15.5%, 12.4%, 8.5%, and 5.7% after one month, three months, six months, and 12 months, respectively. As a result of a follow-up observation of 65 smokers who stopped smoking immediately after the price hike, the actual non-smoking rate declined to 15.5%, 8.3%, 4.4%, and 3.1% after one month, three months, six months, and 12 months, respectively. One (1) year after the cigarette price hike, the non-smoking rate among the 420 smokers reached as low as 3.1% (13 persons). The most important reason for the failure of the attempts to quit smoking was stress for more than 60% of the smokers who attempted to stop. Conclusions: It seems that a powerful anti-smoking policy by the state targeting the nation's workers is necessary. For companies, mediation for workers' job stress can become a strategy for the success of non-smoking attempts. The government seems to require a practical policy to reduce the smoking rate by actively carrying out social, economic, and scientific research to come up with a reduction method for the cigarette hazard, an effective price hike policy, and other non-price policies.

광역생태축과 국토환경성평가지도를 활용한 지자체 광역생태네트워크 구축 방안 (Method of Developing the Regional Ecological Network for Local Government using the National Ecological Network and the Environmental Conservation Value Assessment Map)

  • 급속한 경제성장으로 인한 대규모의 개발들은 산림, 농지 등 넓은 면적의 야생 동식물의 서식지의 훼손 및 단절을 유발하였다. 이와 같은 문제를 해결하기 위해 국토의 자연생태계를 하나의 유기체로서 인식하고, 생태적으로 우수한 서식지의 지속적인 관리를 통하여 자연생태계를 보전해야 한다. 특히 지자체에서는 생태적 연결성뿐만 아니라 환경생태적 우수성까지 고려한 광역생태네트워크를 구축하여 지자체 개발관련 계획에 적극 반영해야 한다. 이를 위해 국토의 생태적 연결성을 평가한 광역생태축과 우리나라의 환경생태적 가치를 정량적으로 분석하고 평가하기 위한 평가지도인 국토환경성평가지도의 환경생태적 평가 결과를 활용하여 지자체 광역생태네트워크 구축 방안을 제시하였다. 그리고 경상남도 사례로 지자체 광역생태네트워크를 구축하였으며, 구축 결과 기존에 구축되었던 광역생태축의 핵심지역 $2,986km^2$과 완충지역 $2,940km^2$가 확장되어 핵심지역은 $4,049km^2$, 완충지역은 $3,006km^2$으로 결과가 도출되었다. 개발계획 시 이러한 광역생태네워크의 참조는 지자체 생물다양성 증진과 생태계 보호를 포함한 지자체 통합적 환경관리에 크게 기여할 수 있을 것으로 판단된다.

습지 이용자 생태의식과 시설선호도 연구 - 우포늪을 대상으로 - (A Study on the Wetland User's Eco-consciousness and Preference of Amenities - Focused on Upo Marsh Users -)

  • The researcher noted the fact that wetland users are more and more diversified while people are more conscious of their ecological importance. Wetlands tend to be very sensitive in ecological terms, and therefore, they can hardly accommodate their users' needs indefinitely. With such basic perception in mind, the purpose of this study was to survey wetland users' eco-consciousness, determine their traits, analyze the corelation between their traits and preferences of wetland amenities, and thereby, provide the data useful to planning of an effective wetland management policy. To this end, the researcher sampled nation's largest wetland, Upo Marsh located in Changnyeong for a questionnaire survey. Wetland users' eco-consciousness was measured, using Dunlap's NEP (New Ecological Paradigm) approved by many researchers. Wetland users' preferences of the wetland amenities were measured, centered around 11 amenity types observed commonly at the domestic wetlands. As a result of the survey conducted in October, 2012, a total of 228 effective samples were acquired. Wetland users' eco-consciousness was higher than normal, scoring 3.45 on the 5-point scale consisting of 5 sub-scales. In particular, users were more conscious of 'the possibility of an eco-crisis,' while being less conscious of 'ejection of exemptionalism.' As a result of classifying the users into 3 sub-groups in reference to their eco-consciousness and analyzing their preferences of amenities comparatively, significant differences were found in all 3 sub-areas. In particular, the sub-group most eco-conscious tended to prefer the learning amenities, but the least eco-conscious sub-group tended to prefer the utilities. As a result of the post-hoc test, it was found that most and normal eco-conscious sub-groups were more or less homogeneous, while the least eco-conscious sub-group was significantly different from the former 2 sub-groups in terms of eco-consciousness. As the wetland users were found to be diversified in terms of their eco-consciousness, it is necessary to plan the wetland management policies in consideration of such differences. However, it is perceived that the wetland amenities need to be built to meet the more eco-conscious users.

RCP 시나리오 기반 밀양댐 미래 유입량 산정 및 결과분석 (The Estimation and Analysis of Miryang Dam Inflow based on RCP Scenario)

  • 우리나라는 여름철에 강수량이 집중되고 동고서저형의 지형을 가지고 있다. 이러한 지형과 조건을 가진 나라는 기본적으로 하드웨어적인 대책이 뒷받침되어야 한다. 하지만 점차 그 적지는 감소하고 있고 국가 수자원 정책에 대한 시민 및 언론의 반발은 거세지고 있는 상황에 처해있다. 이러한 상황에서 기후변화는 수자원에 많은 영향을 주고 밀접한 관계를 가지고 있다. 따라서 본 논문에서는 기상청 기후변화 시나리오인 RCP 4.5와 8.5 시나리오를 검 보정한 SWAT모형에 적용하여 밀양다목적댐 유역의 미래 유입량을 산정하고 이에 대한 분석 결과를 바탕으로 댐을 운영하는데 있어 미래 방향을 제시하였다. 기존의 봉우리형 강우패턴에서 겨울철에도 비가 오는 평탄한 형상으로 변화할 것으로 예측되었다. 따라서 기존의 운영룰에서 변화하는 강우 패턴에 맞는 방향으로 댐의 운영룰을 수정하고 이에 대처할 수 있는 방안의 마련이 필요할 것으로 판단된다.

State Indicator of Water Quality for Surface Water and Groundwater in Agriculture

  • Indicators of environmental conditions qualitatively and quantitatively describe the state of the environment and natural resources, and the OECD (Organization for Economic Cooperation and Development) has initiated and suggested AEIs (Agri-Environmental Indicators) to assess trends over time of the effects of agriculture on the environment and the effectiveness and efficiency of agricultural and environmental policy measures since 1990's. This study aims to develop the state indicators of water quality for agricultural water, surface and groundwater, to evaluate the environmental impacts of agricultural activities and policies by qualifying the environmental levels of a nation. Status indicators were calculated according to the agricultural water quality standards of OECD and Korea, and their trends were analyzed over time. Particularly, nitrate ($NO_3{^-}$) status indicators of ground water in 2013 were significantly lower than the ones in 2000. Overall, the water quality indicators of surface water in 2013 were higher than the ones in 2012, except for pH and DO. The groundwater quality indicators in 2013 were lower than those of previous years. The optimal management indicators were calculated to assess agricultural surface water and groundwater quality. The findings of this study indicated that the state indicators could play a significant role to establish policies and procedures for managing and conserving water resources. This study also discussed water pollution caused by agricultural and industrial effluents.

지역간 상대위험도 변동을 고려한 미세먼지 기인 질병부담 및 사회경제적 비용 추정 연구 (Health and Economic Burden Attributable to Particulate Matter in South Korea: Considering Spatial Variation in Relative Risk)

  • Background: Particulate matter (PM) is one of the leading causes of premature death worldwide. Previous studies in South Korea have applied a relative risk calculated from Western populations when estimating the disease burden attributable to PM. However, the relative risk of PM on health outcomes may not be the same across different countries or regions. Objectives: This study aimed to estimate the premature deaths and socioeconomic costs attributable to long-term exposure to PM in South Korea. We considered not only the difference in PM concentration between regions, but also the difference in relative risk. Methods: National monitoring data of PM concentrations was obtained, and missing values were imputed using the AERMOD model and linear regression model. As a surrogate for relative risk, hazard ratios (HRs) of PM for cardiovascular and respiratory mortality were estimated using the National Health Insurance Service-National Sample Cohort. The nation was divided into five areas (metropolitan, central, southern, south-eastern, and Gangwon-do Province regions). The number of PM attributable deaths in 2018 was calculated at the district level. The socioeconomic cost was derived by multiplying the number of deaths and the statistical value of life. Results: The average PM10 concentration for 2014~2018 was 45.2 ㎍/m3. The association between long-term exposure to PM10 and mortality was heterogeneous between areas. When applying area-specific HRs, 23,811 premature deaths from cardiovascular and respiratory disease in 2018 were attributable to PM10 (reference level 20 ㎍/m3). The corresponding socioeconomic cost was about 31 trillion won. These estimated values were higher than that when applying nationwide HRs. Conclusions: This study is the first research to estimate the premature mortality caused by long-term exposure to PM using relative risks derived from the national population. This study will help precisely identify the national and regional health burden attributed to PM and establish the priorities of air quality policy.

우리나라 학교보건사업(學校保健事業) 변천에 관(關)한 연구 (A Study on the Analysis of School Health Program by the Chronological Events in Korea)

  • The purpose of this study was to identify and name clusters of school health program, and to describe some of the characteristics of administratie supports. The literature, materials and public documents were analysed by the chronological events from 1945 to 1989. The result of this study is as follows : 1. A brief summary of the history of school health program was included as an introduction to the analysis of the current programs of school health. Five current school health-program clusters were identified from findings of a study of programs : 1) physical assessment, laboratory examination and health services for the students, 2) health instruction 3) healthful living condition (environmental health), 4) health clinic management, 5) administrative supports. 2. The earliest school- based efforts focused on communicable disease pevention by the ministry of health and social affairs. Annual medical inspection (health assessment) for school children for eyes, ears, nose, and throat were mandated nation-wide in 1951 by physical assessment Act. 3. In 1979, the health instruction of schools to improve the health status of students was improved by health department in the Ministry of Education 4. Experiences in healthful environment were basic components of the school health program. However, without careful planning and supervision these experiences were not contributed to the goal of school health. The formal program of school health environment were initiated in 1979 5. In 1980, the guidelines of school health clinic management were prepared by Ministry of Education such as guidance of essential degrees and facilities in school health clinic. 6. Two patterns of administration of school health programs existed in Korea. In one the school health department operated its own health program and in the other the physical education department operated the health program within the school system. The school health department was established in Ministry of Education from 1979 to 1982. Improved school health programs will be a key element in the comprehensive national child health policy which I will ask the Ministry of Education to develop for the Department.

  • Information Technology is the kernel technology deciding the industrial standard of one nation, and biotechnology will be the main technology of next generation. Based on this fact, a lot of efforts were made to industrialize them. Nano Technology is beginning to position itself as the kernel fusion technology, and its usage and popularity is expanding. Environmental and Energy Technology is a must-have strategic technology considering the increase demand of new energy development, the international environment correspondence, the environment-friendly production, and so forth. Space Technology is the field, which will contribute to raise the domestic component and system technology to the next level. In 2001, new technology research development costs total of 1 trillion 32 billion won in the following fields; 437.82 billion won in IT, 88.457 billion won in BT, 46.799 billion won in NT, 315.682 billion won in ET, and 112 billion won in ST. from component ratio, IT forms 42% which is the most, 31% for ET and in order of BT, ST and NT. ETRI and KISTI are concentrating on IT, KIBB is on BT, KAERI, KIER, KERI and KBSI are focusing on ET, and KIMM, KRISS, KRICT and KORDI is participating together in 4∼5 new technology such as IT, BT, NT and ET. Funds for research development costs in 5 new technology fields of 13 contribution (year) are consisted as follows; The Office for Government Policy Coordination has contributed 131 billion won (13%), 387 billion won (37%) by MOST, 256 billion won (25%) by Ministry of Information and Communication, 67 billion won (6%) by Ministry of Commerce, Industry and Energy, 19% by others and the industrial world. < Strategy for Technology Advancing > o Promotion of comprehensive contributing (year) new technology development research plan project o Increase research efficiency by promoting new technology development project connected with peculiar projects of organization by contribution (year) o Formation of superior research group by technology and introduction of operation system for research accumulation are needed. o Technology demand-oriented assignment deduction and promotion of research development project connected with intermediate long term objective o National will and investment extension of research development costs, training and popularization of professionals, commercialization promotion with efficient control for research plan and result.

기업의 환경, 보건, 및 안전 (EHS) 조직의 직무능력 개선과 환경변화 대응 방안 (Strategies to Improve Functional Competency and Effectively Respond to Environmental Changes of Environment, Health and Safety (EHS) Team in Companies)

  • Our nation has experienced remarkable growth over the half a century. Nonetheless, there is still much room for improvement in the area of Environment, Health and Safety (EHS). In particular, frequency and severity of industrial accidents did not considerably improve compared to the economic and social progress we made. The main objective of this research is to analyze what plans and actions are required for companies to promote industrial safety by 1) fostering functional competencies of EHS staffs and 2) effectively and proactively responding to rapidly changing EHS environment. For this research, EHS staffs from five large companies in Korea were surveyed. Most respondents indicated that one of key expertise required by EHS staffs is capabilities to effectively deal with changes to various domestic and international EHS-related laws and regulations. Furthermore, a predominant number of respondents commented that it is imperative for EHS staffs to have a broad knowledge of business management. As for internal issues that EHS staffs encounter within their organizations, many pointed out that their EHS vision is not sufficiently shared throughout the organization, and that the rules of leadership are critical in solving this issue. On the other hand, the survey respondents also raised an issue of limited EHS-related investments due to slow economy. As a solution, they proposed ways to align EHS organization's performance with the company's performance. Based on this survey, issues and solutions for EHS organizations were identified. Results of this research can benefit companies that plan to newly establish or further expand EHS organization.
