Computerized Tomographic Study on the Anatomic Variation of the Paranasal Sinus (전산화단층사진을 이용한 부비동의 해부학적 변이에 관한 연구)

  • Objectives : To evaluate the anatomic variations of the paranasal sinuses on computed tomographs. Materials and Methods : The author examined the CT images of the paranasal sinuses retrospectively in 500 patients who visited Chonbuk National University Hospital between January 1996 and December 1997. Results : The highest incidence of anatomic variation of the paranasal sinuses in bilateral structures was agger nasi cel1(73.2%), followed by concha bullosa(31.1%), Onodi cell(24.0%), Haller ce1109.8%), maxillary sinus septum(3.0%), paradoxical middle turbinate(2.5%), pneumatized uncinate process(2.0%), and bent uncinate process. The highest incidence of anatomic variation in midline structures was nasal septum deviation(53.2%), followed by nasal septum aerated(29.4%), bulla galli(24.7%) asymmetric intersphenoid septum(22.3%), and nasal septum spur(13.8%). The correlation between anatomic variation and paranasal sinusitis was not found. Conclusions : The results of this study will aid in the diagnosis and treatment of paranasal sinus diseases, especially in the treatment planning before functional endoscopic surgery.

Agreement between cone-beam computed tomography and functional endoscopic sinus surgery for detection of pathologies and anatomical variations of the paranasal sinuses in chronic rhinosinusitis patients: A prospective study

  • Purpose: The diagnosis of chronic rhinosinusitis requires a comprehensive knowledge of the signs and symptoms of the disease and an accurate radiographic assessment. Computed tomography (CT) is the superior imaging modality for diagnosis of chronic rhinosinusitis. However, considering the lower dose and higher resolution of cone-beam computed tomography (CBCT) compared to CT, this study aimed to assess the agreement between the findings of CBCT and functional endoscopic sinus surgery (FESS). Materials and Methods: This descriptive prospective study evaluated 49 patients with treatment-resistant chronic rhinosinusitis who were candidates for FESS. Preoperative CBCT scans were obtained before patients underwent FESS. The agreement between the CBCT findings and those of FESS was determined using the kappa correlation coefficient. The frequency of anatomical variations of the paranasal sinuses was also evaluated on CBCT scans. Results: Significant agreement existed between pathological findings on CBCT scans and those of FESS, such that the kappa correlation coefficient was 1 for mucosal thickening, 0.644 for nasal deviation, 0.750 for concha bullosa, 0.918 for nasal polyp, 0.935 for ostiomeatal complex (OMC) obstruction, and 0.552 for infundibulum thickening. Furthermore, 95.9% of patients had 1 or more and 79.6% had 2 or more anatomical variations, of which nasal deviation was the most common (67.3%). Conclusion: Considering the significant agreement between the findings of CBCT and FESS for the detection of pathological changes in the paranasal sinuses, CBCT can be used prior to FESS to detect chronic rhinosinusitis and to assess anatomical variations of the OMC.

Magnetic resonance imaging study of incidental findings in the paranasal sinuses and ostiomeatal complex

  • Purpose: This study aimed to assess incidental abnormal findings in the paranasal sinuses and anatomical variations of the ostiomeatal complex (OMC) on magnetic resonance imaging (MRI) scans. Materials and Methods: MRI scans of 616 patients (mean age, 44.0±19.4 years) were evaluated. Prior to obtaining the MRI scans, a checklist of patients' clinical symptoms was filled out after obtaining their consent. The Lund-Mackay classification was used to assess the paranasal sinuses and OMC. The prevalence of abnormal findings and their associations with patients' age, sex, and subjective symptoms were analyzed by the chi-square test, independent-sample t-test, and analysis of variance. The level of significance was set at 0.05. Results: Abnormal findings in the paranasal sinuses were detected in 32.0% of patients, with a significantly higher prevalence in males (P<0.05), but no significant association with age (P>0.05). Epithelial thickening and retention cyst were the most common abnormal findings in the paranasal sinuses. According to the Lund-Mackay classification, 93% of the study population had normal sinuses (score<4). Concha bullosa and paradoxical concha were detected in 15.3% and 3.4%, respectively, with no significant association with the presence of septal deviation or Lund-Mackay classification (P>0.05). Conclusion: Considering the relatively high prevalence of abnormal findings in the paranasal sinuses, it appears that clinical symptoms alone are not sufficient to diagnose sinusitis. A more accurate strategy would be to assess radiographic images of the paranasal sinuses and use a classification system. Sinusitis should be suspected in patients receiving a high score in this classification.

Diagnostic imaging of nasal malignant melanoma in a dog (개 비강에서 발생한 악성 흑색종의 영상 진단 증례)

  • A neutered male, 8-year-old, Pekingese, weighing 4.3 kg with a history of anorexia, sneezing, nasal discharge, and epistaxis for one month was referred. Soft tissue swelling around the nasal bone and small defects of the hard palate with a tiny round dark red mass were found on physical examination. The laboratory tests represented mild leukocytosis. On skull radiographs, soft tissue swelling and osteolytic change of the incisor bone, nasal bone, and maxilla were found. On computed tomography scan images, there was soft tissue attenuating opacity with calcified spots in the bilateral nasal cavities and frontal sinuses. Loss of nasal turbinate pattern and nasal septum was found. And destruction of the insicor bone, nasal bone, maxilla, hard palate, perpendicular palatine bone, and cribriform plate were identified. Nasal malignant melanoma was confirmed by nasal biopsy.

Two Cured Cases of Nasal Polyposis Combiend with Chronic Sinusitis by Allergy Therapy (알레르기 치료를 이용한 비용증을 동반한 부비동염의 치험 2례)

  • Nasal polyps were apparently common in many parts of the world and treated for nearly three thousand years. Nasal polyps are round, smooth, soft, semi-translucent, yellow or pale glistening structures, usually attached to the nasal or sinus mucosa by a relatively narrow stalk or pedicle. The incidence of nasal polyps is increased in patients with atopic diseases; it varies from 15% to 25% and now increased using allergy therapy for nasal polyposis treatment. Sinusitis is an inflammation of the mucous membranes of the sinuses. Many agents can cause an inflammatory response, including organisms such as bacteria and viruses, physical and chemical trauma, and antigen antibody reactions. The role of antigen antibody interactions (allergy) in simusitis is not completely understood ; however, patients with allergic rhinitis and nasal polyps have a high incidence of sinusitis. Recently authors have experienced two cured cases of nasal polyposis combined with chronic sinusitis by allergy therapy, that cases were treated only allergy thereapy after Caldwell Luc operation with ethmoidectomy and polypectomy. At now cases were not recur of nasal polyps and nasal symptoms. So the cases were reported with a brief review of literature.

Multilple Pulmonary Nodules in A Patient with Sinusitis, Proteinuria and Hematuria (부비동염 환자에 병발한 다발성 폐결절)

  • A 49-year old man who had been treated for five months at a private clinic due to chronic paranasal sinusitis was admitted to our hospital because of recently aggravated nasal stuffiness, headache, and cough. The X-ray film of paranasal sinuses and facial CT scan showed marked mucosal thickening of the nasal cavity and paranasal sinuses. The plain chest film and chest CT scan showed multiple, vatiable sized, pulmonary nodules in both lungs. The level of c-ANCA was elevated and urinalysis revealed proteinuria and hematuria. Percutaneous lung and kidney biopsies were performed for confirmative diagnosis. Histologic examination of the lung nodule demonstrated extensive necrosis and poorly-formed granulomatous inflammation. The histologic finding of the kidney showed focal necrotizing glomerulonephritis. A diagnosis of Wegener's granulomatosis involving the paranasal sinuses, lung and kidney was made, and treatment was successfully performed with cyclophosphamide and prednisone.

  • Sinonasal undifferentiated carcinoma(SNUC) is a distinct, relatively rare neoplasm arising in the nasal cavity and paranasal sinuses composed of undifferentiated epithelial cells and clinically characterized by a fulminant course. We report a case of SNUC in a 56-year old man who have had bilateral neck masses since one month ago before coming to our hospital. The paranasal computed tomography showed soft mass density in the left maxillary sinus and the nasal cavity with bone destruction in the anterior medial and the inferior maxillary sinus wall. This mass was extruded into the left orbital wall. Biopsy of the nasal mass and fine needle aspiration(FNA) of the neck mass were done. FNA revealed medium-sized neoplastic cells forming clusters or individually dispersed. Nuclei were round to oval, slightly to moderately pleomorphic, and hyperchromatic. Chromatin was finely granular, but occasionally was coarsely granular. Nucleoli varied from large to inconspicuous and the cytoplasm was scanty.

  • Renal cell carcinoma is the third most common metastatic tumor to the bone and soft tissues of the head and neck. The common sites of metastatic renal cell carcinoma in head and neck region are nasal cavity, paranasal sinuses, oral mucosa, gingiva, tongue, palate, lip as the favored site. The present paper deals with one patient with metachronous oral tongue, nasal cavity and suspicious brain metastases after 2 years of renal cell carcinoma nephrectomy. Also, the patient had history of total thyroidectomy for thyroid follicular carcinoma. Total excision of nasal cavity and tongue mass were performed. Therapeutic aspects are briefly reviewed in literature.

The Role of Radiotherapy in Squamous Cell Carcinonoma of Nasal Cavity - Case Report of 2 - (비강 편평상피암의 방사선치료 역할 및 증례보고)

  • Tumors of the nasal cavity are rare and are usually classified and reported jointly with tumors of the paranasal sinuses. Neverthless their frequency, diagnostic problems, management, and prognosis differ considerably from the latter and justify their study as a separate entity in order to define clearly and evaluate their characteristics. Males outnumbered females in a ratio of 2 to 1. Regional lymph node metases is infrequent; only 10% of the patients presented nodes on admission. Radiation therapy is more favorable method of treatment for nasal cavity tumor secondary to good cosmetic results and high local control rate. The tumor doses are in the range of 5,000 to 6,000 rads in 5 to 6 weeks and an additional dose of 500 to 1,000 rads were given through reduced field to residual disease area. The results of two cases of nasal cavity tumor treated by radiation alone were reasonably good, and review of literature indicated that the radiation alone can give high local control rate and good cosmetics. Therefore, at present time, we recomend radiation therapy for the nasal cavity tumor.

Two Cases of the Inverted Papilloma of the Nose and Sinuses (비강 및 부비동의 반전성유두종 2례)

  • Inverted papilloma of the nose and paranasal sinuses has been classified as a true neoplasm by most authors. Although these tumors are histologically benign, they tend to be act clinically in a malignant fashion. They are certainly characterized by the high recurrence after inadequate removal. The authors report 2 cases of the inverted papilloma which had been diagnosed as a nasal polyp for which an intranasal operation was performed. The polyp recurred and the total mass was removed at our E.N.T. department and proved to be an inverted papillcma by histologic examination.
